[Tumor cells on the inside and tumor cells on the outside. A contribution to the diagnosis and elucidation of the pathogenesis of mammary tumors in dogs].

This review discusses some recent experience with intra- and extracellular components of tumours as markers for tumour diagnosis. Intermediate filaments are cytoskeletal proteins of either epithelial or mesenchymal cells. Antibodies raised against human intermediate filament proteins cross-react with their canine counterparts. A study of the presence and distribution of intermediate filaments in normal mamma and mammary tumours of dogs showed that they do not contribute to a more adequate classification of the tumours. However, the presence of vimentin in epithelial tumour cells may be a marker of malignancy. Proteoglycans are extracellular matrix proteins, containing long chains of glycosaminoglycans (GAGs). Analysis of the presence of GAGs in canine mammary tumours showed that accumulation of chondroitin sulfate, frequently of an abnormal type, was a predominant finding in all tumour types, but not in normal mamma, and was not related to biological behaviour. Although demonstration of the before-mentioned tumour components does not help tumour diagnosis, it may contribute to the elucidation of the development of such tumours as the complex and mixed adenomas of the mamma of which cartilage and bone are important constituents of unknown origin.

Neuroendocrine carcinoid tumours of the breast: a variant of carcinoma with neuroendocrine differentiation.

Carcinoid tumours most frequently develop in the gastrointestinal tract but have been described in many organs of the body. In 1977 the first cases were reported in the mamma, followed by descriptions of argyrophilic carcinoid-like, neuroendocrine mammary tumours by many investigators who performed immunohistochemical and ultrastructural examinations. The existence of true carcinoids in the mamma is still a controversial issue. Eight mammary neoplasms with monomorphous cytonuclear features, five of the small cell carcinoid-like variety and three composed of larger cells, were examined by immunohistochemical and ultrastructural examination. We believe this kind of tumours are ductal or lobular carcinomas with focal or more extensive neuroendocrine features and are the result of a dual differentiation of neoplastic precursor stem cells along epithelial and endocrine lines. Consequently, we consider that treatment of such cases should not be different from that of the ordinary type of mammary carcinomas.

Decreased histamine content and metabolism in mammary cancer tissue from C3H mice.

Histamine (HA) level and its metabolism in adenocarcinoma mammae, spontaneously growing cancer in C3H mice, were examined in relation to the type of tumor, intensity of tumor vascularization and the presence or absence of a secretory function. Histamine concentration being in mammary gland one of the highest among mammalian organs (418 nmol/g) was decreased by 90% in tumor (34 nmol/g). Similarly, histidine decarboxylase (HDC) activity dropped from approximately 7.6 pmol/min/g in normal gland to an undetectable level in adenocarcinoma mammae. Of the two main HA degrading enzymes, namely, diamine oxidase and histamine N-methyl-transferase (HMT), only HMT could be detected in mammary gland, either healthy or neoplastic, and its activity was about 5-fold lower in tumor than in the control tissue (1 nmol/g/min vs. 5.2 nmol/g/min). The pattern of changes in histaminergic parameters evoked by the tumor was similar irrespective of the morphological type it represented, characteristics of its vascular bed or whether or not it showed secretory activity.

[Mammography/stereotactically controlled vacuum excisional biopsy. Interventional methods in breast diagnosis].

As well as in the complementary mamma diagnosis (clinical examination, mammography, sonography) including the dynamic MRI and the mammography screening projects the interventional methods like mammographically/stereotactically guided vacuumbiopsy/excisional biopsy are established methods to avoid unnecessary open operations and they are integrated in the clinical routine. By using strict criterias it is a way to improve the specificity and the negative predictive value in the mamma diagnosis. In addition the interventional methods requires less costs, less time and reduces the morbidity in comparison to an open operation.

Computed tomography in suspected local breast cancer recurrence.

PURPOSE: The purpose of this prospective study was to test computed tomography (CT) in the evaluation of local breast cancer recurrence. MATERIALS AND METHODS: Our study population included 103 women referred for mamma CT examination for reasons of suspicious findings in a conservatively treated breast. In these patients, both clinical and conventional imaging studies (mammography, ultrasonography) features of local recurrences were non specific or suspicious. The patients underwent breast CT before and 90 s after intra-venous contrast medium administration. Criterion for cancer recurrence was detection of a lesion with an enhancement of 20 UH or more. The final diagnosis was based on operative histopathological findings or follow-up for over two years. RESULTS: Local recurrence was confirmed on histology in 52 patients. CT identified 47 breast recurrent cancers. False-positive contrast enhancement was seen in five patients. CT showed 90% sensitivity, 90% specificity and 90% accuracy. CONCLUSION: Although mamma CT examination proved to have a high diagnostic efficacy in evaluating breast cancer recurrence, routine computed tomography is unnecessary for most patients.

Very low density lipoprotein receptor from mammary gland and mammary epithelial cell lines binds and mediates endocytosis of M(r) 40,000 receptor associated protein.

We here report that the M(r) 40,000 receptor associated protein (RAP), previously found to bind to alpha 2-macroglobulin receptor/low density lipoprotein receptor related protein (alpha 2MR/LRP) and glycoprotein 330 (gp330), binds to an M(r) 105,000 membrane protein from bovine mammary gland, human mamma tumors and mammary epithelial cell lines. We have purified this protein from bovine and human sources. N-terminal amino acid sequencing and immunoblotting analyses showed that the protein was identical or closely related to very low density lipoprotein receptor (VLDL-R). Experiments with the human mamma carcinoma cell line MCF-7 showed that this receptor was able to mediate an efficient endocytosis of RAP. These novel findings strongly suggest that RAP functions as a modulator of ligand binding to VLDL-R, similarly to alpha 2MR/LRP and gp330.

In vivo exposure of rats to a weak alternating magnetic field increases ornithine decarboxylase activity in the mammary gland by a similar extent as the carcinogen DMBA.

Magnetic field (MF) exposure has been discussed in the process of tumor promotion as indicated by epidemiologic data as well as laboratory studies. However, the precise mechanisms of tumor promoting effects of MFs are unknown. Tumor promotion is often accompanied by an increase in the activity of the enzyme ornithine decarboxylase (ODC), i.e. a key enzyme in the biosynthesis of polyamines, which have roles in cell proliferation and control of gene expression. In the present work, we studied if exposure of female rats to a 50-Hz MF with a flux density of 50 microT influences ODC activity in different tissues, including the mamma. Rats were exposed for a period of 6 weeks either with or without oral administration of the chemical carcinogen DMBA and all data were compared with those from sham-exposed controls. Magnetic field exposure resulted in an approximate doubling of ODC in mammary tissue. A significant ODC increase was also seen in the spleen, but not in the liver, small intestine, bone marrow, and ear skin. The ODC increase produced by MF exposure in the mammae was of similar magnitude as that observed after treatment with DMBA. Combined treatment with MF and DMBA was not more effective in increasing ODC than treatment with DMBA alone, except for liver tissue. The present results on in vivo increases of ODC by MF exposure strengthen the hypothesis that weak 50-Hz MFs affect ODC activity and may thus function as a tumor-promoting or co-promoting agent.

Lutein in selected Canadian crops and agri-food processing by-products and purification by high-speed counter-current chromatography.

This study mainly focused on lutein content in several selected crops grown in southern Ontario, Canada. Marigold flower, a good rotation crop for the control of nematodes in tobacco fields was found to contain 0.77% lutein (after saponification, on dry basis). A high-speed counter-current chromatography (HSCCC) method using a two-phase solvent system consisting of hexane-ethanol-water (6:4.5:1.5, v/v/v) was developed for the purification of lutein from the saponification mixture of marigold flower extract. The purity of lutein prepared using this HSCCC method was 97%. Free lutein was found to be the predominant form in three squash varieties, and it was mostly found in the peel rather than the commonly consumed flesh. Sweet Mamma, Buttercup and Pepper squash varieties contained 25.4, 18.4 and 30.1mg/100g fresh weigh (FW) of lutein in the peels, respectively. These concentrations were significantly higher than that in spinach and kale (3.7 and 12.3 mg/100 g FW). beta-Carotene was found most in the peel of Sweet Mamma squash at 13.6 mg/100g FW, whereas it was below 2mg/100g FW in all other samples. Cooking increased extractable free lutein by 22-65% in squash peels. Lutein in Yukon Gold potato was at ca. 0.4 mg/100 g FW. Certain Yukon Gold was also found to contain violaxanthin (0.35 mg/100 g FW). Structures of lutein, beta-carotene and violaxanthin were identified by LC-atmospheric pressure chemical ionization MS in positive ion mode, and by comparing the retention time and UV-vis spectral data with standards. Results from this study suggest the selected crops and agri-food industrial processing by-products of these can be a good source of free lutein.

Boost irradiation of breast carcinoma: teletherapy vs. brachytherapy.

BACKGROUND: The results of adjuvant radiotherapy including a boost dose after breast-conserving surgery of mamma carcinoma were retrospectively analysed to relate local tumor control, survival, and cosmetic results to the boost technique. MATERIAL AND METHODS: The study included 229 female patients who were treated in the period 1986--1997. Group A consisted of patients where the primary irradiation was hyperfractionated (two fractions per day) and the boost was applied by reduced portals, also at two fractions per day. In group B the 'boost' was applied interstitially intraoperatively and the 'primary' irradiation followed at one fraction per day. The cosmetic results and the late changes to the mamma were the subjects of follow-up examinations and were assessed using the EORTC score. RESULTS: 129 patients formed group A, median follow-up 4.2 years, and 100 patients with median follow-up of 9.4 years formed group B. 59% of group A and 60% of group B were in stage pT1, 38% and 39% were in pT2, and 25% and 39% were in stage N1 or N2 (no significant differences). Tumors were mostly poorly to moderately differentiated. The upper outer quadrant was most afflicted. Local recurrence occurred in two and five cases, lymph-node recurrence in two cases each, while there were ten and 12 cases of distant metastases. There were no deaths in group A and in group B six with obvious distant metastases and eight with other causes of death. The cosmetic results and late side-effects (induration, teleangiectasis, ulcers) were significantly worse in the interstitial group B. Multivariate analysis revealed that only the total applied dose significantly affected the severity of late radiation side-effects. The cosmetic results worsened with time, the tendency for lymph edemas in the irradiated side increased. Induration decreased continuously after pure teletherapy but increased continuously after interstitial therapy. These trends did not change in either group. CONCLUSIONS: Both therapy schemes resulted in successful local tumor control with good cosmetic results and few side effects, but the interstitial boost therapy was clearly less favourable. Longer-term follow-up is required to compare the late side-effects even further.

Effects of tibolone on the breast.

OBJECTIVE: to evaluate the effect of hormone replacement therapy and tibolone on the breast. STUDY DESIGN: prospective, controlled, randomized study. SETTING: Outpatient Menopause Clinic of the Second University of Naples. PARTICIPANTS: forty four women in spontaneous menopause without any risk factor for breast cancer were randomly allocated to three groups: 15 patients (group A) were treated with transdermal oestrogens 50 microg, 2 patches/week for 3 weeks per month, plus acetate nomegestrolo per os 5 mg/die for 12 days per cycle, 17 patients (group B) were treated with tibolone 2.5 mg/die. Twelve patients not given any medication represented the control group (group C). METHODS: at the time of recruitment and after at least 12 months of therapy the patients were subjected to a questionnaire aimed at quantifying the slight, moderate or severe presence of the tension/mastodynia symptoms and to a mammographic test to assess the parenchymal pattern according to a quantitative method: type 1 (less than 25% of mammary gland covered by dense tissue), type 2 (from 25% to 75% of total glandular area covered by dense tissue), type 3 (more than 75% of mammary parenchyma covered by dense tissue). Statistical analysis was carried out by means of Fisher's exact test. RESULTS: after at least 12 months of treatment in Group A 5 out of 15 patients (33%) showed a trend of increase in mammographic density not statistically significant (P=0.22) when compared with group B in which one patient showed a swift from type 1 to type 2 and another from type 2 to type 3. The analysis of tension/mastodynia symptoms revealed a significantly difference between the two groups (P=0.02): in group A mastodynia appeared in three previously asymptomatic women and increased in five women, with a total increase in the symptomatology in 8 out of 15 patients (53.3%), in group B only in one case (5%) mastodynia turned from slight to moderate. CONCLUSION: in postmenopausal women oestroprogestogenic replacement therapy may be associated with an increase in mammographic density and with the onset or increase in mastodynia. On the contrary tibolone does not seem to affect normostructured mammas and may be considered a first-rate replacement therapy in case of mammas showing particular density or benign mastopathies.

[Enhanced liposomal transfection through the application of sex steroids in gynecological cancer cells].

OBJECTIVE: Liposomal transfection in gene therapeutic application against gynecological malignoma does not reach satisfying efficacy. A desirable goal would be the specific intensification of transfection in these kind of cells. Steroids have successfully been used in other systems to increase liposomal transfection and hopefully there might be a specific impact of sexual steroids in cells from high sex steroid receptor expressing malignoma, like some mamma- and endometrium cancer. MATERIAL AND METHODS: The mamma carcinoma cell line T-47D was transfected with the transfection agent DOTAP and cyclodextrin solubilized steroids and cholesterol were co-applied. The efficiency of transfection was followed by luciferase activity resulting from the transfected reporter gene. RESULTS: Like cholesterol, which is already established as transfection co-agent, also the steroids progesterone, estrogen, testosterone and hydrocortisone provoked a clear increase in transfection efficiency shown in a dose dependent manner. CONCLUSIONS: These results indicate the usefulness of steroids as additives for liposomal transfection procedures in gene therapeutic application. As sexual steroid receptors migrate into the nucleus of a cell after binding its specific ligand a targeted enhancement of transfection is supposable in malignoma overexpressing steroid receptors. There is evidence that plasmid DNA can be co-transported with nuclear proteins into the nucleus.

[Three-dimensional ultrasound study (3-D sonography) of the female breast].

Three-dimensional sonography of the mamma with a Voluson annular array transducer (10 MHz) (Kretztechnik, Austria) is a new method applicable in differential diagnosis of mamma foci. 50 patients (19 of them with breast cancer) were thus pre-surgically examined. Both sectional and stereoscopic representations were made use of. The suspected diagnoses and their correspondences with post-surgical findings were compared to the correspondences obtained through 2D-sonography. 3D-sonography produced 4 cases of the incorrectly positive diagnosis breast cancer, 2D-sonography 2 cases of incorrectly negative diagnoses. Other advantages of 3D-sonography over 2D-sonography include: better judgement of the conditional of focal environs (infiltration), existence and form of intracystic structures and of multifocal disease, short duration of examination, possible re-diagnosing of stored data.

[Cytokine production by gynecologic cancers].

Aggressive polychemotherapy and ultraradical surgery had had only small benefits for patients with gynaecological cancer in recent years. That is why we measured cytokines in these patients to evaluate a possible future immune therapy. To investigate the influence of gynaecological cancer on in vitro and in vivo cytokine production of peripheral mononuclear cells (PBMC) from 27 patients with mamma carcinoma and 29 patients with cervical cancer, we evaluated the production of interferon-alpha (IFN-alpha), interferon-gamma (IFN-gamma) and tumour necrosis factor-alpha (TNF-alpha). We compared the immune function of these patients versus 20 patients who had had routine hysterectomies performed for non-malignant reasons and 20 healthy female controls. Blood for cytokine production was collected prior to surgery, for IFN-alpha-production two weeks after surgery and for all cytokines three months after surgery. Measurement of TNF-alpha-production did not differ significantly in all investigated groups. IFN-gamma-production was reduced by 50% in patients with mamma carcinoma in three months - possibly caused by chemotherapy and radiation. The amount of IFN-alpha production in cancer patients was dramatically reduced before primary therapy started. The low levels of IFN-alpha persisted for three months compared to the IFN-alpha production in non-cancer patients and healthy controls. The follow-up of patients with breast cancer undergoing chemotherapy showed a suppressive effect on immunological function in IFN-alpha. Our results demonstrate, that patients with breast cancer and cervical cancer showed a significantly reduced capacity to product IFN-alpha.(ABSTRACT TRUNCATED AT 250 WORDS)

[Acne comedonica following radiation therapy].

HISTORY AND CLINICAL FINDINGS: A 50-year-old female patient underwent extirpation of a moderately differentiated, partially tubular ductal carcinoma of the left mamma and dissection of the left axilla followed by postoperative radiation therapy (total dose 58/48 gy). 2 weeks after the radiation therapy was stopped, treatment with goserelin subcutaneously was started. 6 weeks later numerous open and closed comedons appeared which were sharply confined to the radiation areas of left mamma and sternum. DIAGNOSIS, TREATMENT AND COURSE: Because of the clinical diagnosis of acne comedonica following radiation therapy a local treatment with benzoyl-peroxide gel 5% and erythromycin cream 2% was performed, which led to improvement of the skin lesions within a few weeks and total restitution within 3 months. During one year no comedons have occurred although the local therapeutics are not applied constantly anymore. CONCLUSION: Acne comedonica in the radiation area is a rarely known side effect of radiation therapy. Although the skin lesions cause no pain or itching they can affect the patient due to cosmetic changes. Local treatment with comedolytic acne therapeutics is effective.

[Diagnostic value of increased lactate dehydrogenase activity in vaginal secretions for the early detection of diseases requiring treatment of the female genitals and breast].

The Diagnostic Validity of Enhanced Lactate Dehydrogenase Activity in the Vaginal Secretion for Early Diagnosis of Diseases of the Female Genitals and the Mamma in Need of Treatment. An impregnated hygiene tampon (Gynaegnost) was used, which by a colour reaction permits the determination of augmented activity of lactate dehydrogenase (LDH) in vaginal secretion. The diagnostic validity of the results of the findings by this method in the detection of diseases of the female genitals and the mamma needing treatment was examined in two clinical diagnostic studies. The first study including 486 patients chiefly suffering from manifest carcinoma was aimed at solving a deliberately limited question. In patients with carcinoma stages I to IV of the most important genital locations augmented LDH-activity (correct positive results) was found in about 90%. A second study comprised 1955 women entering into outpatient gynaecological services for preventive examination; the definition of this series was difficult. Three categories of diagnostic groups can be determined by the results of this study. In the first category with the most important subgroup "routine examination without specification of a pathological finding" less than 20% "false" positive test results by the indicator were observed. A second category with inflammatory diseases (colpitis, endometritis) showed positive results in about one-third of the examinations. The third category comprised a minor number of malignant tumors roughly with a percentage of positive results corresponding to that of the first study. Augmented LDH activity in vaginal secretion is a diagnostic criterion which detects not only cancer; it is similar to the test of occult blood in faeces.(ABSTRACT TRUNCATED AT 250 WORDS)

[The primary non-Hodgkin lymphoma of the mammary gland].

Basing on an observation made by the authors, the article describes the diagnostic and therapeutic criteria in primary non-Hodgkin lymphoma of the mamma. Despite high malignancy, especially of the lymphoblastic-centroblastic lymphoma and of the immunoblastic lymphoma (Stein, 1974), permanent healing can be achieved via early surgical treatment and subsequent radiotherapy. In the case under discussion, the same tumour reappeared in the contralateral breast after an interval of five years. Since it was possible to exclude a generalisation, it must be assumed that the patient in question had a primary non-Hodgkin lymphoma of the immunoblastic type in both mammae after a period of five years.

[Demonstration of Wegener's granulomatosis in the mammogram].

Communication about two patients with Wegener's granulomatosis of the mamma. In both cases the rare disease was detected on the occasion of a mammographic examination. The results of the mammography could cause misinterpretations, e.g. it might be mistaken for an advanced mamma carcinoma. Up to now, the manifestation of a Wegener's disease of the female breast was neither mentioned in collective statistics of more than 324 cases nor it could be found in examining the publications of additional 231 cases. Both patients showed an unusual course of the disease with a slow progression as well as an unimportant affection of the respiratory tract. The regression of the tumor like lesions in the breast was remarkable.
