[Lung abscess due to Fusobacterium nucleatum (author's transl)].

A patient with epilepsy, 45 years of age, developed a lung abscess due to Fusobacterium nucleatum which was successfully treated with intravenous lincomycin. Quantitative anaerobic cultures of washed sputum proved to be useful in detecting the causative organism.

Yield of percutaneous needle lung aspiration in lung abscess.

STUDY OBJECTIVE: To evaluate the accuracy of PLA with a thin needle in the bacteriologic diagnosis of patients with lung abscess and in demonstrating possible coexistence of an underlying lung carcinoma, and the influence of this technique in the treatment and outcome of these patients. DESIGN: Case series. SETTING: Tertiary university referral center. PATIENTS: Consecutive sample of 50 patients with clinical picture suggestive of pulmonary infection and single or multiple cavitation of at least 1 cm in diameter on chest x-ray films, and lack of clinical suspicion of active pulmonary tuberculosis. One patient was excluded from the study after demonstration of tuberculosis by PLA. INTERVENTIONS: Lung aspirates were obtained under fluoroscopic guidance by introduction of a 22-gauge disposable spinal needle within the abscess cavity and were immediately transported to the bacteriology laboratory and pathology department for processing. All patients were initially treated with clindamycin. Tobramycin was added in all those patients with hospital-acquired infection, lack of foulness of sputum, and those who were initially severely ill. Definite treatment was based on the results of bacteriologic cultures. MEASUREMENTS AND MAIN RESULTS: Cultures of LAs were positive in 82 percent (40/49) of patients. In 20 cases the isolates were monobacterial (13 aerobic bacteria and seven anaerobic). In the remaining 20 cases, cultures grew more than one kind of bacteria (four exclusively aerobic, five exclusively anaerobic, and 11 mixed), with an average of 3.25 types of bacteria per case. Anaerobes were found as a single bacteria or associated with other aerobic bacteria in only 58 percent (23/40). The results of LA cultures led to change in the initial antibiotic trial in 23 patients (47 percent). Of ten cases in which bronchogenic carcinoma was demonstrated, cytologic study of LA was done in nine, and eight had positive cytologic yield. Pneumothorax occurred in seven cases (14 percent) as the sole complication. CONCLUSIONS: (1) Percutaneous lung aspiration had a high diagnostic yield and accuracy in our series, with a relatively low incidence of complications. (2) Anaerobic bacteria were less frequently implicated in our cases than previously reported. This finding led to significant changes in the initial empiric antibiotic treatment.

[Pasteurella multocida lung abscess. Study of a case with demonstration of a cat as vector].

We report a case of a lung abscess due to Pasteurella multocida, isolated from bronchial secretions by fiberoptic bronchoscopy. Antibodies against Pasteurella multocida were elevated in the patient, and absent in controls (including the patient's wife). The identity of the strain isolated in the patient and that in his cat was proved by molecular method using pulsed field gel electrophoresis.

Pleuritis secondary to pneumonia or lung abscessation in 90 horses.

Of 122 horses with pleural effusion, 90 (73.8%) had pleuritis secondary to pneumonia or lung abscessation. Fifty-one horses died or were euthanatized. The highest prevalence was in Thoroughbred and Standardbred racehorses. Eleven (12.2%) horses were postsurgical patients and 22 (24.4%) horses had been transported over 500 miles. There was no relationship between final outcome and the age, sex, breed, hematologic values, or laboratory findings pertaining to pleural fluid except for the bacterial isolation of Escherichia coli from the pleural fluid, as this was more frequently associated with death. Follow-up on 38 of the 39 horses that survived showed that 18 (46.2%) recovered and were able to return to performance equal to that prior to their illness. Ten (25.6%) were returned for breeding or pleasure use, with no attempt made to return them to racing. Follow-up was not available for 5 horses, 4 horses had just recently been discharged from the hospital, and 2 horses are racing poorer than prior to their illness.

Fatal Selenomonas sputigena septicemia probably originating from lung abscess.

A case of fatal septicemia with Selenomonas sputigena in an immunocompromised patient is reported. The patient had a lung abscess from which the septicemia is believed to have originated. In contrast to the only other case reported in the literature, the isolate from our patient was characterized by very slow and difficult growth.

Noncatheter-associated vegetative phlebitis of the superior vena cava presenting with multiple lung abscesses.

We present a patient with a large vegetation in the superior vena cava (SVC) presenting with multiple staphylococcal lung abscesses. We believe that this is the first reported case of vegetative phlebitis of the SVC without a history of central venous catheter line. Our case emphasizes the need for careful imaging SVC using transesophageal echocardiography in bacteremic patients even in the absence of a central catheter.

[A case of lung abscess with elevated serum levers of sialyl Lewis X-i (SLX) and CA 19-9].

A case of lung abscess of the felt lower lobe in a 19-year-old woman with elevated serum levels of Sialyl Lewis X-i (SLX) and CA 19-9 is reported. Completing the lobectomy, serum SLX level returned to the normal range within a week postoperative day. Serum CA 19-9 level also decreased at half life of 2 weeks to the normal range within 6 weeks postoperative day. Laboratory examination demonstrated high levels of these antigens in abscess fluid. Histologically, the abscess was revealed to be associated with a markedly dilated bronchus with hyperplastic bronchial glands, and there was no evidence of malignancy. Immunohistochemical examinations using monoclonal antibodies against human SLX and CA 19-9 showed highly positive reaction with those antigens in both goblet cells in bronchial epithelia and the mucous cells in bronchial glands.

Lung abscess due to Corynebacterium equi. Report of the first case in a patient with acquired immune deficiency syndrome.

A lung abscess and persistent bacteremia due to Corynebacterium equi are described in a bisexual man with the acquired immune deficiency syndrome (AIDS). Eleven of the 12 previously reported cases have occurred in immunocompromised humans. The occurrence of this infection in a patient with AIDS has not been previously described. Development of resistance to beta-lactam antibiotics was noted following initial therapy. Because this organism resembles nonpathogenic organisms, it may easily be overlooked despite its ability to cause serious infection and persist even with aggressive antimicrobial and surgical therapy.

Lung abscess caused by Clostridium ramosum.

A 49-year-old male presented with weight loss and fever. Further evaluation revealed lung abscesses and Clostridium ramosum bacteremia. This is an unusual cause of infection. There have only been seven reports of infections caused by C. ramosum.

Postanginal sepsis syndrome with metastatic lung abscess in a young man.

A postanginal Sepsis Syndrome with metastatic lung abscess caused by Fusobacterium necrophorumin a 25-year-old previously healthy man is described. The incomplete and ineffective antibiotic treatment at onset of angina ended progressively in septicaemia and metastatic infections in a 3-week time period. The early parenteral use of Metronidazole based only on the clinical picture, the Gram stain findings and the strict anaerobic feature of the blood isolate in parallel with the long-term antibiotic treatment were possibly the main reasons for the good outcome of this serious infection.

Lung abscess (6 cm) of unexplained origin in a 36-year-old woman.

A 36-year-old woman developed a lung abscess following an episode of influenza. Repeated blood cultures were negative. In necrotic material obtained by bronchoscopy no pathogenic micro-organisms were found. Penicillin-clavulanate, cefodizim and metronidazole were without success. The combination therapy with imipenem-cilastatin together with clindamycin improved clinical symptoms, normalized pathological laboratory parameters and completely resolved the lung cavity.

Primary lung abscess caused by multidrug-nonsusceptible Streptococcus pneumoniae in a child.

We report a 21-month-old boy with primary lung abscess caused by multidrug-nonsusceptible Streptococcus pneumoniae. A computed tomography-guided aspirate obtained on admission grew penicillin-, ceftriaxone-, erythromycin- and clindamycin-nonsusceptible S. pneumoniae. The patient was treated for 10 days with parental rifampin and cefotaxime and then for 4 weeks with oral cefpodoxime and recovered without complications.

Lung abscesses in children: diagnostic and therapeutic needle aspiration.

Three children aged 17 months to 17 years developed right-sided peripheral lung abscesses. Clinical signs were fever and cough. Laboratory cultures were negative, and the patients did not respond to appropriate antibiotic coverage. Under fluoroscopic guidance, purulent material was removed from the abscesses by needle aspiration. The patients became afebrile within 24 hours; none suffered complications of bleeding or pneumothorax. Cultures of the aspirate were positive for microorganisms sensitive to the prescribed treatments. A simple aspiration technique is described and proposed as useful for selected patients when surgical drainage is recommended. There was no morbidity in our cases, and recovery from a typically prolonged course was shortened by the procedure.
