Multi-hospital information network improves care for uninsured.

When people come to a hospital's emergency department for treatment, they don't bring their medical records with them. That means physicians and nurses either have to spend valuable time looking for information on the patients, or they have to run diagnostic tests, which may duplicate tests that have already been done somewhere else. Four health care organizations in New Haven, CT, have developed a solution to improve electronic access to health records and reduce the expense of duplicate tests.

Information Networks in the Arterial Wall.

The main task of the arterial system is to secure an adequate supply of oxygen to organs. This fact implies the integration of multiple signals in the vascular wall. This review deals with the exchange of information between and among smooth muscle and endothelial cells through gap junctions in the vessel walls of arteries and arterioles.

A web interface for multimedia electronic patient record: consensual validation of the Aquitaine Health Information Network prototypes.

The PromptPR patient record originates from the research and development project DILEMMA (1992-94) and has been undertaken in the context of the research project PROMPT (4th Framework Health Telematics). Here we present the work undertaken with the doctors of the project RISA (Réseau d'Information de Santé en Aquitaine) in order to adapt the patient record to their various practical medical conditions. Effectively, there are implications for general practitioners, the private sector and hospital personnel. The richness of the workgroup and the adoption of a participative methodology, guided by the demands of the end-users allowed the creation of an electronic patient record (EPR) sufficiently simple and generic as to serve as a common base for both general practitioners and hospital medical staff. Independent of this system, the multimedia interface presented is based on frames, which enables visualization of the EPR either vertically or horizontally, with access to both the multimedia and external documentation of the patient. This web interface is the result of a consensus of opinion within the group.

[Surgical specialty department in the world wide web. Tribute to contemporary life style or information network?].

An increasing number of surgical departments is creating its own home-page in the World Wide Web (WWW). We investigated 184 presentations concerning their quality in content and technical realization. Our research revealed that most presentations regard patients as their main target group in order to meet the patients' demand for information and transparency. However, universities are especially concentrating on scientific development and neglecting the patients. Only a few presentations contain means for web-based training. In general, technical realization seems to be much better than the content itself. As patients tend to appear as customers who will choose the hospital they feel comfortable with, every clinical department that is a competitor should create a communication and information base for their customers. Thus, they will be able to keep in touch, present their treatment types and to enable patients to compare a multitude of surgical departments. The usefulness for colleagues or cooperating institutions must be improved. The WWW has become an important center of information in surgery.

Building an interoperable regional health information network today with IHE integration profiles.

One of the key challenges of architecting electronic record sharing solutions that are scalable and can provide acceptable performance is how to create a longitudinal record for a patient when the desired data will be stored in several distributed point-of-service systems. This paper will review the design and standards selection process that has been made by Integrating the Healthcare Enterprise, a multi-national collaborative of care providers and developers that analyzed a variety of approaches. In particular, the requirements, issues, and alternative solutions for scalable, standards-based data locating services supporting regional and national health information exchange solutions will be discussed. Guidance will be offered to architects of regional health information organizations to take advantage of this experience and leverage the IHE Technical Framework, its testing processes, and tools to accelerate their projects and facilitate the interfacing of EHR systems serving different care settings from different vendors or developers. The implementation experience in 2005-2006 of the IHE Integration Profile specifications supporting an interoperable RHIO solution among various EHR systems from more than 30 vendors will be analyzed, with key lessons summarized.

RheumaNet--a novel Internet-based rheumatology information network in Germany.

Knowledge transfer in medicine has traditionally been achieved through personal communication, the medical literature, conferences and continuous medical education. However, many of these methods of communication have the problem of currentness, and the latest knowledge is mostly transferred with a certain delay. In particular, chronic disease management is frequently burdened by missing, incomplete or delayed communication. We present the implementation of a novel, Internet-based information system (RheumaNet) to facilitate rapid communication and information transfer between the 21 German Multipurpose Arthritis Centres (MACs). Medical information, agendas, urgent notifications, guidelines, etc., can be exchanged easily via electronic mail, file transfer protocol or the World Wide Web. RheumaNet allows interactive on-line communication as a common platform for joint projects or remote consultation. It supports a set of 'core documentation' that has been used by all German MACs for several years. RheumaNet was used for the organization, on-line presentation and evaluation of the 1997 annual meeting of the German Rheumatology Society and is currently appointed to establish its public representation in the 'Web'. An on-line version of the 'Quality Assurance Textbook' of German rheumatology is achieved with database-derived 'dynamic' Web pages, allowing easy access to any information within it. Advances in the future include the development of on-line patient information bulletin boards, on-line questionnaires, the coordination of rehabilitation and educational tools for medical students, and professional training.
