Controversies in ankle fracture treatment. Indications for fixation of stable Weber type B fractures and indications for syndesmosis stabilization.

Although the treatment of ankle fractures is often straightforward, several controversies remain. This article reviews the need for open reduction and internal fixation of the displaced supination-external rotation fracture, and contrasts the studies that mandate anatomic reduction with clinical results. The many issues surrounding syndesmosis fixation are also reviewed, including the need for fixation of distal fractures and the timing of screw fixation.

[Change in indicators of peroxide resistance in erythrocytes and lipid metabolism indicators in blood plasma during development of ischemic insult].

The authors have displayed that ischemic insults are accompanied by the significant intensification of free radical peroxidation processes both in erytrocyte membranes and in blood plasma. The changes mentioned are characterized by the simultaneous decrease of erytrocites resistance to peroxide hemolysis, as well as by the parallel lowering of alpha-tocopherol concentration in blood plasma.

[Local anesthesia with and without vasoconstrictors: indications, contra-indications and complications in relation to the health of the patient].

Patient's fear of pain is one of the major reasons of not routinely seeking dental care. Local anesthetics are commonly used in dentistry and dental practitioners must be familiar with the effects and side-effects of these substances in medically compromised patients. This article is a review of these side-effects and the resulting contraindications. A frequently asked question is when to combine a vasoconstrictor to a local anesthetics. The purpose would be to reduce the anxiety and avoid any pain during dental procedures. Indeed, pain produces much more endogenous epinephrine than the small amount of epinephrine combined to the local anesthetics.
