[Observations of the course of patients with hyperkinesis].

This article reports observations made on thirty-six patients suffering from oral dyskinesia (tardive dyskinesia). In thirty cases, dyskinesia was observed to occur in the course of neuroleptic therapy. Occurrence of dyskinesia was dependent upon the use of a higher dosage rather than upon the duration of medication. Persons showing dyskinesia were not always subjects with previous cerebral lesions or patients of advanced age. Of patients with schizoform diseases, those were usually affected with oral hyperkinesia who had either partly or completely lost the process symptomatology, but who were unable to live a life satisfying them in every respect.

[Basic concepts on hyperkinesis in children].

With the purpose of clarifying the meaning and clinical manifestations of the syndrome, the discussion begins within the following framework: there is an identifiable set of signs and symptoms; the syndrome is multifactorial and exists in degrees; it deals with a diagnosticable condition in which organic factors seem to be essential, and the syndrome is characterized by alterations of different types. Within the setting of psychiatric theories of child pathology which concluded that all psychological problems had essentially the same etiology and, therefore, treatment, dominant in the first half of the century, the author traces the relevant clinical work carried on during this period which caused attention to be turned toward organic factors and the posterior identification of similar signs and symptoms. Later research from diverse fields broadened knowledge and at present, two positions are discernible: one of acceptance and the other negating reality to the organic hypothesis because the existence of the syndrome's is made to depend on demonstrable structural brain damage. position which obliges the problem to depart from etiological biases.

[Pathogenesis and treatment of tic-type hyperkinesis in children].

Results of clinical and electrophysiological examinations of 52 children with tic-type hyperkineses are presented. Nine of them had generalized tic (Gilles de la Tourette's disease), and 43 had local forms. All the children were examined electroencephalographically; 26 children had, in addition, an electronystagmographic examination. Changes pointing to organic affections of the brain were revealed. The authors recommend that anticonvulsants be used for tic therapy. In children with generalized tic the anticonvulsants may be combined with haloperidol.

Hyperkinesis treated by vestibular stimulation: an exploratory study.

In crossover design with alternate assignment to order, 18 hyperkinetic children were treated with eyes-open rotational vestibular stimulation and with control contact, each eight times over 4-week periods. On behavioral ratings by teachers, they scored better after eight sessions of rotational stimulation than after eight control sessions. This treatment advantage was restricted to the ten children younger than age 10 without undersocialized-aggressive features. Few side effects occurred. Ratings by parents, but not by teachers. showed significant (p less than 0.05) placebo effect from the control situation. These findings suggest that vestibular stimulation be explored as a treatment modality for young children. The importance of concomitant visual stimulation needs to be investigated.

Compensatory hyperkinesis of the ventricular walls in acute myocardial ischaemia.

After ligation of the anterior descending coronary artery in dogs, conductance of the left circumflex branch and contractility of the ventricular muscular areas supplied by the left descending coronary artery and its circumflex branch were studied over 60 min. After ligation a considerable increase in contractility of the myocardial areas of the posterior wall with intact blood supply was found and a further increase parallel with a considerably enhanced flow in the circumflex branch. At the same time the contractile power of the anterior ventricular myocardium with a poor blood supply greatly decreased. A close correlation was found between the increase in flow in the circumflex branch and the increase in contractility of the intact areas. The tissue catecholamine level showed a marked increase in the myocardial areas with impaired blood supply.

[Circulatory hyperkinesis of cirrhosis: etiopathogenesis, clinical and therapeutic implications].

A mysterious complication of cirrhosis is the progressive development of a hyperdynamic circulatory state characterized by high cardiac output and low peripheral and splanchnic arterial resistance. This particular hemodynamic state plays a major role in the progress of portal hypertension and sodium renal retention observed in cirrhosis. In this review, we try to better define the clinical impact and the etiopathogenic mechanism of the hyperdynamic state of cirrhosis.
