[The role of helminthiasis in the etiology of acute appendicitis].

279 vermiform processes removed for different form of appendicitis were studied hystologically with obligatory analysis of appendix content. High rate of helminth intestation (27.2%) in vermiform processes removed in acute appendicitis surgery confirms an important role of small helminths (Enterobius, Trichocephalis and their larvae) in acute appendicitis development. The incidence of acute appendicitis and helminths interstation in children is higher than in adults. All the patients with acute appendicitis need helminthological examination and etiotropic treatment because appendectomy eliminates the complication but not the cause of intestinal disease.

Allergy, total IgE and eosinophils in East and West -- serious effects of different degrees of helminthiasis and smoking.

Allergies are rarer in East Germany than in West Germany, although elevated IgE-levels and blood eosinophil counts can be found in the East. The aim of this study is to control the known results in Slowakia. The total-IgE and the blood eosinophil counts as well as stool samples of 311 plant-workers in Slovakia and 522 in West Germany were checked. In 1999/2000 21% of the plant workers examined in Slovakia suffered from an allergy compared with 32 % of the workers in West Germany. In contrast the total IgE in the East (Slovakia) was 166 IU/ml compared with 99 IU/ml in West Germany. Worms were detected in the stools in 7 % of those examined in the East and in only 1 % of those in the West. The percentage distribution of eosinophils in the blood was on average 7.5% in the East and 2.7% in the West. 39% of the workers in the East were smokers compared with 20% in the West. Nevertheless among non-smoking workers volunteers without parasites, the total IgE and eosinophil count were higher in those from the East than in those from the West. For Slowakia we can show for the first time that allergies are less frequent compared with West-Germany. The total IgE and the blood eosinophils are associated with higher smoking habits and more frequent helminthic diseases.

[Prevalence of intestinal helminthiasis in primary school children in Santa Fé city].

The aims of this work were: 1.--to determine the intestinal helminth prevalence in children who live and attend primary schools in Santa Fe city. 2.--to characterize the socio-cultural and sanitary aspects of the children studied and consequently to group them in three supposed strata (hypo, meso and hyperendemic). The study was carried out during 1998. Considering the following variables: kind of streets, sewers and water supplies, parents' educational levels, satisfied or unsatisfied basic needs, the city was divided in three strata, according to the intestinal helminthic prevalence expected: 1--hypoendemic (< 25%), 2--mesoendemic (25-75%) and 3--hyperendemic (> 75%). Schools with children of both sexes, and ages between 5 and 13 who belonged to each one of the supposed strata, were identified. A direct parasitic examination and a Kato Katz technique were performed on a stool specimen of each selected child. An epidemiological survey was answered by the children's parents. The prevalence of helminths and geohelminths, with 95% confidence intervals, were respectively: total sample (n = 163). 36.2% and 34.4%; stratum 1 (n1 = 60) 0% and 0%; stratum 2 (n2 = 70) 41% (29.9-52.9) y 38.6% (27.2-50.0) and stratum 3 (n3 = 33) 91% (81.1-100) and 87.9% (76.6-99.2). The above-mentioned variables, related to the socio-cultural and sanitary aspects, allowed to identify three well defined strata: hypoendemic, mesoendemic and hyperendemic, according to the different helminth prevalence detected among pupils who attend primary level school in Santa Fe city.

[Modern therapy of intestinal helminthiasis (author's transl)].

The modern chemotherapy of intestinal worm diseases is essentially based on niclosamide, thiabendazole and mebendazole. Thus an effective therapy with few side effects has become possible for these helminthiases. In infections with Taenia, Ascaris, Trichuris, Ankylostoma, Strongyloides, Trichostrongylus and Oxyuris, as the most frequently imported helminths, an elimination rate of over 90% of the parasites can be attained. Infections with nematode larvae as with Larva migrans and trichinoses are more problematic therapeutically because the anthelmintics are directed principally against the adult worms.

[The treatment of intestinal helminthiasis with mebendazole. 294 treated patients].

294 patients with slight intestinal helminthic infections have been treated with mebendazole, a broad-spectrum antihelminthic of high effectiveness and remarkably low toxicity. The high cure rates (91-95%) obtained in cases of trichuriasis, ascariasis and ankylostomiasis are in agreement with the results as published by other investigators. For helminthiases, the present patient sample was too limited to permit accurate evaluation. Nevertheless, mebendazole may be considered the drug of choice for treatment of trichuriasis in particular and for intestinal helminthic polyparasitism in general.
