Treatment of hirsutism and acne in women with two combinations of cyproterone acetate and ethinylestradiol.

175 women of reproductive age, with hirsutism of differing degrees and different pathogenetic causes (ovarian, adrenal, iatrogenic) or idiopathic, and acne were treated with two different combinations of Cyprotrone acetate and ethinyl estradiol (SH 8.1041 and SH B209AB). 90 patients were given SH 8.1041 and 10 were given SH B209AB. 75 received both preparations. The total number of treatment cycles was 1534. Clinical, hormonal and biochemical assessments were made before, during and after treatment. The degrees of hirsutism and acne, and of seborrhea and hair loss when present, were scored by means of a modified version of the Ferriman and Gallway criteria. SH 8.1041 brought about a significant improvement in the majority of the patients. SH B209AB was generally used as maintenance therapy for hirsutism and severe acne. It was the initial treatment of choice in patients with milder acne. Reduction of hirsutism was usually apparent after the fourth cycle of therapy and acne regressed after the first month. Both combinations were well-tolerated biochemically. In a few patients on SH 8.1042, slight and transient increases in BSP, SGOT, SGPT and bilirubin were observed, but cessation of treatment was not necessary. Some patients on SH 8.1041 complained of transient frigidity, mild depression, breast discomfort and nausea.

[Why women consult sexologists: an approach for practicing physicians].

Female sexual dysfunctions are less known than male ones because women are more discrete and introverted. Female genitals are mostly internal and women live sex more cerebrally and sentimentally than men. Still half the patients my current sexological consultation are women. They seek help due for lack of desire, frigidity, coital or clitoridal anorgasmy, dyspareuny, vaginism and problems related to pregnancy or the post-partum period. This article describes how to construct a precise clinical etiology following a "5 circles" scheme, which will help to determine the most adequate therapeutic approach.

[Responses of microbes in rhizospheric soil of Abies faxoniana to elevated atmospheric CO2 concentration and temperature].

With independent and top-enclosed chamber system, this paper studied the responses of culturable bacteria, fungi, and actinomycetes in rhizospheric soil of Abies faxoniana sapling to elevated atmospheric CO2 concentration (ambient +350( +/- 25) [micromol x mol(-1), EC), temperature (ambient + 2. 2 (+/- 0.5) degrees C; ET), and their combination (ECT) under high-frigid conditions of West Sichuan Province. The results showed that in comparing with the control, treatments EC and ET increased the number of rhizospheric bacteria by 35%, 164% and 312%, and 30%, 115% and 209% in June, August and October, respectively, but had little effects on the numbers of rhizospheric actinomycetes and fungi. In treatment ECT, the numbers of rhizospheric actinomycetes and fungi increased by 49%, 50% and 96% ,and 151%, 57% and 48% in June, August and October, respectively, while that of rhizospheric bacteria had little variation. EC, ET and ECT had significant effects on the total number of rhizospheric microbes, with the R/S being 1.93,1.27 and 1.46, respectively, but had little effects on non-rhizospheric microbes.

[Two cases of food-dependent exercise-induced anaphylaxis difficult to evoke symptoms by provocation test].

We report two cases of food-dependent exercise-induced anaphylaxis (FDEIA), which were hardly induced by provocation test in the hospital. Case 1: A 28-years-old Japanese female suffers repeated episodes of sternutation, nasal discharge and edema of eyelids after wheat ingestion of wheat followed by exercise. Case 2: A 14-years-old Japanese male suffers repeated episodes of wheal formation on whole body and dyspnea after lunch containing apple followed by exercise. Both of them had never developed symptoms by either ingestion or exercise alone. Provocation tests were performed on admission by combinations of the ingestion of suspected foods, exercise, and aspirin, but no symptoms were reproduced by any combination of them. After discharge, case 1 reproduced symptoms during exercise after the ingestion of wheat under prostration and cold climate. Case 2 reproduced symptoms during exercise after ingestion of apple when he suffered from common cold. Warm and comfortable condition in admission may make it harder to evoke symptoms by the provocation test. Frigidity, cold, prostration, and stress should be reckoned with in the provocation test to improve the accuracy of diagnosis for FDEIA.

[Sexual violence as a sociomedical problem].

A review is made on data, described in the literature, in respect to frequency and significance of sexual abuse. In spite of the scarcity of data and discretion, with which the problem is surrounded, it could be determined that in some countries around 10 to 25% of women at mature age run a risk of sexual abuse. The forms are described, in which this violation of sexual freedom of the personality are manifested: abuse, combined abuse and "passive" form of sexual abuse (s refusal of sexual contact in family partners). Data are given on the characteristic of the abuser and his victims. Sexual abuse is connected with individual constitutional factors: character, biorhythms of sexual needs, physiological states leading to frigidity or impotence as well as socially determined factors: education, usage of alcohol, crimogenic environment, ect. In the end a review is made on the measures, which are taken in various countries in the struggle with sexual abuse: legal protection, institutions for recovery of victims of abuse, "hot telephone lines" social measures, etc.

[A pilot study of sexual problems in chronic schizophrenia: a report on 51 cases].

In order to explore the sexual problems of chronic schizophrenics, 51 cases were investigated. The result showed that 42 cases (82.3%) had various kinds of sexual problems. The problems were erectile impotence, frigidity, premature ejaculation, no ejaculation, vaginismus and pain on coitus. Only 4 cases had increased sexual desire. The authors found that the main sexual problems of chronic schizophrenics were inhibited sexual desire and excitement. The authors suggested that the sexual problems of chronic schizophrenics were related to their conditions in the body, rapport with their wives or husbands and the severity of affect, thought and volition disturbances.

The pathology of self-mutilation and destructive acts: a forensic study and review.

Self-destructive behavior of man and its consequences may be presented in various forms, including self-mutilation, injury, and malingering, and with various manifestations, including the Munchausen syndrome, polysurgery, purposeful accidents, impotence, and frigidity. The general subject of focal suicide has been a relatively unknown and unrecognized entity for different reasons, including the fact that most physicians are trained to approach a case in a manner which treats the patient-physician relationship as one of honor and faith regarding the authenticity of the complaints of the patient. The maturity of years and experience may be required before the physician becomes aware of the motivation of patients' behavior. It is hoped that this article will provide new insight into areas which may be unfamiliar to the forensic practitioner and clinician but which might enable them to learn about the background of an unexpected death or the possibilities of motivation in civil suits alleging professional negligence or malpractice.

[Clinical and histological studies on vaginal reconstruction after radical hysterectomy].

The extensive resection of vagina has a prime importance to obtain the remission in patients with invasive carcinoma of cervix. Maintenance of optimal length, configuration and physiologic function of vagina has been neglected because of malignant nature of lesions. Thus, sexual frigidity coupled with dyspareunia was usually a serious problem faced by postoperative patients. In order to resolve this problem we devised a new attempt to obtain sufficient regeneration of vagina in postoperative patients. The protection procedure for a shortened vagina consists of enlargement of the vaginal vault and promotion of epithelial regeneration. Vaginal length in 145 patients who had received the protection procedure as well as the radical hysterectomy was 7.0 +/- 0.1 cm, being not different from the one in 187 patients with simple hysterectomy. However, significantly shortened length was observed in 21 patients hysterectomized radically who had not the protection. The influence of preserved ovarian functions into the regeneration process of vagina was investigated. The mean length was 7.3 cm in 75 patients with ovarian conservation and 6.3 cm in 7 patients with bilateral oophorectomy. The latter was significantly shorter than the former. However, administration of synthetic estrogens alone was incomplete for the full reconstruction of remnant vagina. Adjuvant radiotherapy was not an obstacle in obtaining the optimal vaginal length. The mean length of irradiated group was 7.4 cm, being not different from the one of the nonirradiated group. Regeneration process of vagina was investigated histologically. New vaginal lumen consisted originally of the remnant vagina and granulation tissues obliterating dead spaces in the pelvic floor. Then, epithelia stained with iodine followed to cover the granulation. Enlargement of such areas directed into the deep part of vault was evident with the passage of time following the operation. As a result, epithelial covering of the whole reconstructed vagina was completed within 2 months after the operation. Those results indicated the clinical availability of the protection procedure in radically hysterectomized patients. Then, ovaries conserved either by orthotopic manner or transposition plays some role for the reconstruction of remnant vagina.

[Schizoid versus hysterical personality structure. I: Symptoms and diseases].

This part examines the symptoms and diseases of neurotics with either a schizoid (38) or a hysterical (70) personality structure, comprising 5% and 10% respectively of the first admissions in 7 years. Compared to the hysterical structure, schizoids more frequently show the following characteristics: they are men and have as their secondary component a mixed character structure, an earlier age of onset, a longer duration of illness but earlier referral for psychotherapy. They suffer mainly from psychic symptoms--depressive states and contact difficulties as the indicating signs. Further complaints are defective concentration, feelings of apprehension and difficulty in working, also bulimia and the inability to breathe deeply. They were mostly born between 1940 and 1959. Their relatives suffered from psychiatric disorders. Compared to the schizoids, hysterically structured individuals are almost exclusively women, their second most important structural component being obsessive-compulsive. They develop their illness later, have it for a shorter time and are referred for psychotherapy later. Their indicating signs manifest themselves more often somatically, less frequently as anxiety states or phobias. The symptoms occur simultaneously in several organ systems, particularly the respiratory tract, the gastrointestinal tract and the musculoskeletal system. Common symptoms are dyspnea and/or hyperventilation tetany, nausea, weight gain, frigidity and fluor as well as crying fits. They formerly suffered from gynecological disorders and had to undergo operations. They were mainly born between 1920 and 1939. Their relatives had cardiac disorders and their symptoms were identical or similar to those of their father.

Respiratory heat loss in exercise-induced asthma. Measurement and clinical application.

The theoretical considerations of conditioning inspired air and the application of the respiratory heat loss (RHL) formula are discussed. An on-line method for measuring RHL is described together with the apparatus for generating frigid dry and warm humid air. Exercise-induced asthma (EIA) was studied using these methods. Thirteen asthmatic and 6 normal children and adolescents participated in the study. Each subject undertook two submaximal exercise tests consisting of 6 minutes' ergometric cycling against a fixed load. One test was done while breathing cold dry air (mean temperature -22 degrees C and 0% relative humidity) and the other while breathing warm humid air (mean temperature 36 degrees C and 100% relative humidity). All the other exercise parameters (e.g. heart rate, minute ventilation, oxygen uptake) were carefully matched between the two tests. In the cold dry air tests with a mean RHL of 1,43 kcal/min, all asthmatic subjects developed EIA with a mean fall in forced expiratory volume in the 1st second (FEV1) of 48% from baseline. In the warm humid air tests with negligible RHL (0,02 kcal/min) none of the asthmatics developed EIA (mean fall in FEV1 5%). The difference between the two tests was highly significant (P less than 0,001). Neither air condition caused bronchospasm in the normal subjects. A dose-response relationship was obtained between the degree of RHL and corresponding fall in FEV1.

Receptivity of female rats.

Receptivity of female rats has been investigated under chronic experimental conditions. A receptivity index has been worked out for quantitative evaluation. It proved suitable for disclosing stimulatory (2.5 mg noretynodrel, oestradiol propionate) as well as inhibitory (etynodiol diacetate) effects. 2.5 mg noretynodrel has a biphasic effect; causing an inhibition and later an increase in receptivity. Receptivity of untreated female rats is low and can be increased by various steroids; the best treatment was 50 micrograms oestradiol monopropionate administered twice weekly, but this resulted in an inverse mating reaction, with frequent copulations in every phase of the cycle. In untreated females the number of copulations varied widely. Previous deliveries did not influence receptivity and frigidity was also observed.

[Couple heterosexual dysfunction in relation to homosexual latency (author's transl)].

Clinical experience of couple and sexological consultations indicates that some symptoms find in the heterosexual couple are the expression of a nevrotic solution of latent homosexual tendencies. This means that homosexual tendencies are repressed, or actualized through an imaginary elaboration or a symbolic one, by staying unconscious or manifesting through acting in a state of obnubilation. These different situations are illustrated by sexual dysfunctions (some forms of impotence and frigidity), hostile rejection and homosexual panic, jalousy, triolism and alcoholic aspects. This brief analysis is concluded by some reflexions from the clinical, social, couple and individual points of view.

Psychosomatic factors in patients with hyperprolactinemia and/or galactorrhea.

A biographic and clinical investigation of 101 patients with hyperprolactinemia and/or galactorrhea is reported. Fifty-one patients were reared without their fathers and 18 with an alcoholic, violent one. These situations were uncommon in the control population, and the differences were statistically significant. There was a high frequency of complaints of obesity, headaches, frigidity, lightheadedness, and fullness of the abdomen, limbs, or face. There was a significant temporal correlation of external events in the natural history with onset or worsening of the symptoms. It is concluded that exposure during childhood to an environment characterized by an absent or alcoholic, violent father conditions some women to develop hyperprolactinemia and/or galactorrhea later in life as a response to specific environmental changes. These conclusions apply similarly to patients with prolactinoma, idiopathic hyperprolactinemia, and euprolactinemic galactorrhea, suggesting a close relationship among the 3 entities.

[Hypnosis in obstetrics and gynecology].

Since the basic problem in the use of hypnosis in physiopathological obstetric and gynaecological conditions is the choice of the right method for the right indications, the symptoms and clinical conditions which may benefit from this treatment are discussed. The following gynaecological indications are suggested: dysmenorrhoea and algomenorrhoea; hypogastric plexalgia; frigidity; dyspareunia and vaginismus; psychogenetic amenorrhoea; postoperative psychoses and depressions; gynaecological somatisations of psychotic states. In the obstetric field the main indication are: hiperemesis gravidarum; antenatal training and assistance in labour.

[Occurrence of deviant tendencies in phalloplethysmographic and vulvoplethysmographic examinations in sexually dysfunctional patients (case reports)].

The author demonstrates on four case-histories the presence of sexual deviant trends as the cause of sexual dysfunctions in conventional heterosexual relations in men and women and the irreplaceable part played by vulvo- or phalloplethysmography used for their detection. In a man treated for some time on account of impaired erectile potency and in a woman who visited the surgery with her husband and presented complaints diagnosed as secondary frigidity and anorgasmy, PPG or VPG examination revealed a homosexual orientation. Both these patients knew of their homosexual orientation but did not tell anybody. The mentioned male patient intentionally dissimulated homosexuality, the woman attempted in vain to achieve heterosexual adaptation. In two young men with primary disorders of erectile potency PPG examination revealed sadomasochistic tendencies. In the first one the impaired erectility created the impression of "debutant's failure" when starting a partnership, in the second one it seemed to be due to adverse psychosocial factors. During an interview following the examination both described spontaneously and in detail erotic dreams and masturbation fantasies with sadomasochistic contents. They were not aware of the association of their specific sexual motivation structure. It was typical for all four patients that they never disclosed previously their deviant sexual tendencies and only the specific situation during the PPG or VPG examination, which they had voluntarily after previous instruction, they were motivated to confess their problems.

[Gynecologic diseases in the diabetic woman].

Gynecologic affections such as diabetic vulvitis, vulvo-vaginal mycosis, some dysfunctions of the ovary, recurrent abortion, secondary frigidity and sometimes carcinoma of the endometrium, may be the first signs of a disorder of carbohydrate metabolism. Contraception is particularly indicated in diabetic women. The methods are discussed.

[Mass-hysteria with Koro-symptoms in Thailand].

Koro, a psychogenic anxiety syndrome interfering with genital body image and sexual functioning, has hitherto been described as occurring mainly in isolated cases of South Chinese males. The present communication reports an epidemic outbreak in November 1976 in Northeastern Thailand where within a few days at least 200 patients, most of them Thai and two-thirds males, were treated at local hospitals. Main presenting symptoms were acute anxiety, in some cases leading to fainting, (subjective) shrinking of the penis and impotency in men, shrinking and/or itching of the external genitals and frigidity in women; further complaints included initial nausea and dizziness, abdominal pains, headaches, facial numbness. All patients recovered after brief symptomatic intervention. Popular opinion and news media echoed the patients' paranoid projection of viewing the epidemic as caused by Vietnamese food and tobacco poisoning in a hideous assault against the sexual vitality and general health of the Thai people, in the context of a specific socio-cultural and politico-historical situation. It appears that an adequate interpretation of Koro and of analogous hysterical symptom formation would have to go beyond the hitherto applied psychoanalytic models by considering the specific sociodynamic factors involved in the pathogenesis of such phenomena.

The family practice resident as sexual counselor.

Little research has been done on family practice residents and their sexual counseling attitudes, knowledge, and abilities. This study sought answers to five broad questions about family practice residents' perceptions of sexual counseling. Subjects were 132 residents and 21 faculty members from eight family practice training units in Minnesota, which were affiliated with the University of Minnesota. Residents regard sexual counseling as important and say they desire more training to deal with sexual problems encountered in family practice. They tend to raise the subject of sex with patients not routinely but only if there appears to be a psychosocial problem. Respondents indicate a lack of ability as well as discomfort with several areas of sexuality, notably frigidity and homosexuality. Family practice residents need to develop their skills in specific areas of sexual counseling. While these findings are most applicable to the eight units involved, the diversity in respondents' backgrounds and differences between units suggest that the results may be relevant to other residency programs.
