Power Doppler sonography: evaluation of hepatocellular carcinoma after treatment with transarterial embolization or percutaneous ethanol injection therapy.

OBJECTIVE: The aim of this study was to compare tumor detectability by assessing the vascularity on power and color Doppler sonography and CT after transarterial embolization or percutaneous ethanol injection therapy or both in hepatocellular carcinoma. SUBJECTS AND METHODS: Forty-seven nodules of hepatocellular carcinoma (size, 28 +/- 7 mm [mean +/- standard deviation]; range, 20-40 mm) in 38 patients were treated with transarterial embolization (n = 6), percutaneous ethanol injection therapy (n = 23), and transarterial embolization plus percutaneous ethanol injection therapy (n = 9). Power Doppler sonography, color Doppler sonography, and CT were performed before and 2 weeks, 3 months, and 6 months after the treatments. The existence of hepatocellular carcinoma was confirmed by positive findings for color signals on both Doppler sonography techniques and for tumor stains on CT. All the tumors were determined to be malignant by microscopic examination of biopsy specimens. RESULTS: Before the treatments, power Doppler sonography (100%) and CT (100%) were significantly more effective than color Doppler sonography (61.7%) (p < 0.001, for both). Six months after the treatments, the sensitivity of power Doppler sonography (87.5%) was significantly better than that of color Doppler sonography (12.5%) but was not significant in comparison with CT (66.6%). However, power Doppler sonography detected color signals in two of three tumors in which iodized oil was accumulated and no tumor stain appeared on CT, and the two lesions detected with power Doppler sonography were carcinomas. CONCLUSION: Power Doppler sonography can be considered the most sensitive technique in assessing the viability of hepatocellular carcinoma treated with transarterial embolization or percutaneous ethanol injection therapy or both.

Regional correlation by color-coded tissue Doppler to quantify improvements in mechanical left ventricular synchrony after biventricular pacing therapy.

Cardiac resynchronization therapy (CRT) can improve cardiac function in patients with heart failure and left bundle branch block. To test a new synchrony index derived from mitral annular velocity by color tissue Doppler, 19 subjects were studied: 9 patients with heart failure and left bundle branch block at baseline and at 1, 3 and 6 months after CRT and 10 normal controls. The synchrony index in patients with heart failure was less than that in controls at baseline (r = 0.60 +/- 0.13 vs 0.94 +/- 0.02; p <0.01), but improved at 6 months after CRT (r = 0.77 +/- 0.09; p <0.05 vs baseline).

Analysis of the inter- and intravillous placental blood circulation in high-risk pregnancies before and after the hemodilution therapy using the colour-angio-mode.

OBJECTIVE: The aim of the study was to examine the efficiency of a hemodilution (HD) therapy on the inter- and intravillous placental blood circulation in high-risk pregnancies by applying the colour-angio-mode (CAM). PATIENTS AND METHODS: 24 patients were examined who had fetal growth retardation and/or pathological Doppler results. In all cases the hematocrit was >36%. The patients were given 500 ml Ringer solution and 500 ml HAES 10% intravenously per day; maternal heart rate and blood pressure were measured continuously. Prior to and after the therapy the same area of the placenta was examined using CAM; histograms demonstrating the distribution of the intensity values and summation values of the color pixels were calculated. RESULTS: In 14 patients the fetal outcome was good. The histograms showed in 11/14 cases an increase of low intensity values indicating an improvement of the placental circulation. The summation value of the color pixels as measured by CAM was significantly increased after the therapy in these cases (p = 0.022). In 10 patients the fetal outcome was unfavorable. The histograms showed a decrease of the intensity values in 6/10 cases. There was no significant change in the summation values of the color pixels after the therapy (p = 0.139). CONCLUSIONS: CAM sonography demonstrated for the first time a local effect of the HD therapy within the placenta. In the positive case the placental blood circulation will improve and the pregnancy develops favourably (responder). In the negative case there is no change of the placental blood circulation and pregnancy must be terminated due to a pathological fetal heart rate tracing, fetal growth failure or hypertensive disorders (nonresponder).

Cranial arteriovenous malformations in neonates: color Doppler imaging with angiographic correlation.

Five neonates with cranial arteriovenous malformations were studied with color Doppler sonography. Excellent correlation was found between sonography and subsequent angiography. In three patients with vein of Galen aneurysms, sonography showed a cystic structure with rapid or swirling flow. Careful adjustment of the color Doppler system was required to demonstrate flow in another patient with a dural arteriovenous malformation. An arteriovenous fistula in a fifth patient appeared as an area of increased flow. Arterial feeders and major draining veins were visualized in all five patients. Color Doppler imaging also was used to assess the effect of embolic or operative therapy in three of the patients. We conclude that color Doppler sonography is able reliably to characterize flow patterns in neonatal cranial arteriovenous malformations. Color Doppler imaging also is helpful in assessing flow after embolic or surgical therapy.

Palliative treatment of hepatocellular carcinoma with percutaneous ethanol injection using tumor's feeding artery occlusion under the ultrasonic color Doppler guidance.

We evaluate the efficacy of PEIT in patients with HCC using duplex color Doppler US. The study included 27 HCC patients admitted to the University Hospital Centre Zagreb, between 1993 and 1997. PEIT was performed for ablation of tumor supplying vessels in HCCs of < 5 cm in diameter, and as a palliative measure for tumor feeding vessel obliteration in larger tumors. The efficacy of PEIT was evaluated with duplex color Doppler US, and controlled by dynamic CT scan (16 patients) or selective angiography of hepatic artery (11 patients). All patients had well vascularized tumors before PEIT, and after therapy 25 of them showed absent or minimal tumor vascularization. Recanalization of the tumor feeding vessel was detected with Doppler US within 9 months after therapy. Study results suggested that duplex color Doppler US should be the method of choice in the evaluation of PEIT as well as in the follow-up of HCC patients after PEIT.

[Basic study of myocardial coagulation by intraoperative laser ablation: in the presence and absence of blood].

To investigate whether the efficacy of laser ablation therapy is affected by the color of the target organ, we irradiated 9 canine hearts through the epicardium with Nd-YAG laser (200 J, 400 J), and compared coagulated myocardial volume in the beating heart (reddish) with that in the stopped heart (whitish) infused with 0 degrees C saline (cardioplegic model) via a coronary artery. The coagulated myocardial volume of the beating heart was significantly larger than that of the arrested heart, indicating the YAG laser energy is absorbed by red myocardium to greater extent than it is by white myocardium. In conclusion, during intraoperative laser ablation, the presence or absence of blood in cardiac tissue is an important factor for safe irradiation.

[Influence of Eqb 761 on the function of the retina in children and adolescent with long lasting diabetes mellitus--preliminary report].

PURPOSE: Early detection of patological function of the retina, before anatomical changes, plays very important role in monitoring of visual complications in patients with diabetes mellitus. The aim of the study was the evaluation of anatomical and functional changes in visual organ in children and adolescents with long lasting diabetes mellitus type 1 and taking Egb 761 (Tanakan Beaufour Ipsen) as an adjuvant. MATERIALS AND METHODS: Group consists of 15 patients, age between 11 and 19 years, with diabetes mellitus lasting 6-12 years. All patients had full ophthalmologic examination and color vision tests (Panel D 15 saturated and desaturated). The examination was done 3 times, every 3 months. Egb 761 was given: 1 tablet - 3 times a day, during 3 months. RESULTS: No diabetic retinopathy was found. The results of color vision test were better after therapy (25% of pathological results) and 3 months later (only 4 % of patients). CONCLUSIONS: 1. Egb 761 seems to be good adjuvant in patient with long lasting diabetes mellitus. 2. Color vision tests are sensitive tests of the retinal function and are easy to perform.

[Diagnosis of deep pelvic and leg vein thromboses by imaging procedures].

The gold standard for the diagnosis of deep venous thrombosis (pelvis and lower limb) is still phlebography--sometimes supplemented by DSA to clarify findings in the pelvic region. The indication for phlebography can, however, be markedly reduced by noninvasive procedures and restricted to the clarification of uncertain findings or the confirmation of the diagnosis for subsequent invasive therapy. Ultrasound techniques, particularly color-coded duplex sonography, presently appear to take priority among the noninvasive procedures.

Extensive bullae formation in acute iliofemoral venous thrombosis.

A 58-year-old man presented with an acutely painful swollen left leg which was cyanotic and crowded with cutaneous bullae. After confirmation with color duplex ultrasound, intravenous streptokinase therapy was commenced with a bolus of 250,000 i.u. followed by maintenance infusion of 100,000 i.u. hourly for 72 hours. Treatment was changed to heparin for 14 days and then oral warfarin for another 6 months. Effacement of bullae accompanied subsidence of swelling of the leg. This case highlights the potential contribution of fluid retention in acute deep venous thrombosis to extensive bullae formation. This has not been previously reported in the literature.

Power Doppler sonography: clinical applications.

OBJECTIVE: Color Doppler imaging (CD) has had a great impact on ultrasonography (US). This technique depicts local flow by encoding an estimate of the mean Doppler frequency shift at a particular position in color. However, the choice of the mean frequency shift as the parameter for representing flow in color Doppler is somewhat arbitrary. Power Doppler ultrasound is a technique that encodes the power in the Doppler signal in color. This parameter is fundamentally different from the mean frequency shift. The frequency is determined by the velocity of the red blood cells, while the power depends on the amount of blood present. Providing an image of a different property of blood flow, power Doppler has shown several key advantages over colour Doppler, including higher sensitivity to flow, better edge definition and depiction of continuity of flow. In this paper we review the results of power Doppler clinical studies. MATERIALS AND METHODS: All relevant information available in the literature on the potential clinical applications of this technique was revised to give a detailed survey. RESULTS: The increased flow sensitivity and better vascular detailing of power Doppler have been used to detect flow presence and characteristics in vessels that are poorly imaged with conventional color Doppler. The improved depiction of tissue vasculature has shown potential advantages, especially in some areas, such as the cortex of native kidneys and renal allografts, the prepuberal testis, the infant hip and the bowel wall, in which color Doppler is not sensitive enough to detect clinically important, slow and poor flow in small vessels. In inflammatory conditions, power Doppler was valuable in depicting increased flow in vessels that are dilated because of inflammatory response. In this field, advantages have been reported in acute cholecystitis and in inflammatory states of musculoskeletal tissues. The higher sensitivity to slow flow and the improved detailing of the course of tortuous and irregular vessels made power Doppler a promising technique to image intratumoral vessels and, thereby, to ameliorate the accuracy of color Doppler in predicting the likelihood of benign versus malignant nature of nodules. Specific flow patterns, missed at color Doppler studies, have been indicated with power Doppler in some tumors of the liver and breast. In different settings, power Doppler also permitted to monitor serial blood flow changes after therapy and to display them as color intensity, allowing the observer to distinguish flow changes. CONCLUSION: Although the actual role of power Doppler in changing patient management has not been assessed yet, this technique can depict flow which was previously undetectable, and thus permits an easier and more confident diagnosis in body regions where the ultrasound signal is weak because blood vessels are small.
