A prospective randomized trial comparing the silver-impregnated collagen cuff with the bedside tunneled subclavian catheter.

A prospective study was undertaken to compare the silver-impregnated collagen cuff (Vitacuff) with the bedside tunneled catheter. Fifty patients were randomly assigned to three groups: group I received triple-lumen catheters with Vitacuff application and a semiocclusive dressing material; group II received triple-lumen tunneled catheters with a semiocclusive dressing; and group III received triple-lumen tunneled catheters with collodion as a dressing material. In patients suspected of having central venous catheter sepsis, blood cultures were obtained through the catheter, the catheter was removed, and the tip was cultured semiquantitatively. Central venous catheter sepsis was defined as a positive catheter-tip culture and blood culture for the same organism. No catheter-related sepsis was seen in either the Vitacuff or the tunneled catheters with collodion dressing. In the tunneled catheters with semiocclusive dressing, there was one case of catheter-related sepsis and one case of insertion-site infection. There was also one insertion-site infection in the Vitacuff group, but there was no statistical difference in infection rates between the three groups.

Ichthyosis: etiology, diagnosis, and management.

The ichthyoses are a heterogeneous group of disorders with both inherited and acquired forms. Clinical presentation, pattern of inheritance, and laboratory evaluation may establish a precise diagnosis, which can assist in prognosis and genetic counseling. Congenital autosomal recessive ichthyosis (CARI) usually presents at birth, often as a collodion baby. CARI can progress into any one of a spectrum of disorders. Lamellar ichthyosis is characterized by dark, plate (armor)-like scale. This disease is often caused by mutations in the gene encoding the enzyme transglutaminase 1. Congenital ichthyosiform erythroderma is another phenotype within CARI, marked by generalized redness and fine white scale. Epidermolytic hyperkeratosis is an autosomal dominant disorder characterized by hyperkeratosis and blistering, and at least six clinical phenotypes have been described. It may be due to mutations in the gene encoding the intermediate filament proteins keratin 1 and 10. Ichthyosis vulgaris is the most common ichthyosis, and is inherited in an autosomal dominant pattern. Involvement is generally mild and may vary greatly with climate and humidity. X-linked ichthyosis, due to a defect in the enzyme steroid sulfatase, affects males with generalized scaling that usually begins soon after birth. There may be associated corneal opacities that do not affect vision. Sjögren-Larsson syndrome is an autosomal recessive ichthyosis associated with progressive spastic paralysis and mental retardation. This condition is caused by mutations in the gene for fatty aldehyde dehydrogenase. Refsum's disease, due to accumulation of phytanic acid, results in ichthyosis and progressive neurologic dysfunction. The erythrokeratodermas are characterized by hyperkeratosis and localized erythema. Erythrokeratodermia variabilis is autosomal dominant and characterized by generalized or localized hyperkeratosis and migratory red patches. Mutations in the genes encoding the gap junction proteins, connexins, underlie this disorder. Netherton's syndrome is an autosomal recessive disorder characterized by ichthyosis, a hair shaft abnormality and atopy. The ichthyosis may present at birth with erythroderma or in some cases a collodion presentation. However, a frequent characteristic skin manifestation is ichthyosis linearis circumflexa. Netherton's syndrome has been found to be due to an abnormality in a serum protease inhibitor. Acquired ichthyosis can have a variety of underlying causes including neoplastic, infectious, drugs, endocrine, metabolic, autoimmune, malabsorptive states, and hereditary. Topical, and in more severe cases, systemic, therapy are useful in managing this array of disorders of cornification.

Enhancement of clearances by activated charcoal in an in vitro model of peritoneal dialysis.

The use of sorbents in dialysate to increase solute clearances in continuous ambulatory peritoneal dialysis (CAPD) was investigated. With in vitro simulations of CAPD, the kinetics of irreversible binding of creatinine to activated charcoal were assessed. Cellulose dialyzer fibers were submerged in two liters of dialysate for 3-8 hour exchanges. Perfusate was pumped single pass through the fibers. Commercial dialysates with 1.5% and 4.25% dextrose as an osmotic agent were controls. Experimental exchanges contained either large or small particles of activated charcoal. Unencapsulated and collodion encapsulated large particles were also studied. From the perfusate side, creatinine, clearance and mass transfer were determined; dialysate/perfusate ratios (D/P) of free creatinine concentrations were assessed. We found that incorporation of small unencapsulated particles of activated charcoal would double both clearance and mass transfer of creatinine. It also maintained D/P values less than 0.6 even up to 8 hours. Small particles absorbed more than 10 times more creatinine per gram than large particles. Significant differences between encapsulated and unencapsulated large particles were not found. In summary, activated charcoal might double creatinine removal per exchange in CAPD. Animal studies of collodion encapsulated small particles and other sorbent-enzyme systems in CAPD dialysate solutions seem warranted.

Congenital ichthyosiform erythroderma: particulate staining pattern of TGK.

A case of late onset non-bullous congenital ichthyosiform erythroderma (CIE) was studied. This patient was not born as a collodion baby and did not have skin abnormalities until 9-10 years of age. She gradually developed erythroderma and fine scales, callosities of her feet, and a mild ectropion. Since recent work has revealed that in the majority of CIE patients, transglutaminase (TGK) is distributed in the cytoplasm of granular cells and horny cells (11), TGK was studied in our case. It was found that TGK was distributed along the cell periphery of horny cells and also in the cytoplasm of granular cells. In the control skins, TGK was stained along the cell periphery of horny cells and granular cells. The marginal band formation was normal. Involucrine and loricrin, the building materials of the marginal band whose-cross-linking is mediated by TGK, were normally stained in the upper epidermis. Cytoplasmic TGK of granular cells and normal development of the marginal band may serve as a helpful diagnostic marker of CIE, particularly because the often confusing collodion baby of lamellar ichthyosis may lack TGK staining and the marginal band altogether.

Bathing-suit ichthyosis. A peculiar phenotype of lamellar ichthyosis in South African blacks.

Autosomal recessive congenital ichthyosis is a heterogeneous group of disorders of cornification showing a wide spectrum of clinical phenotypes. Here, thirteen cases of a so far unreported distinct type of autosomal recessive congenital ichthyosis are described. All the skin lesions were present at birth and ten were then compatible with collodion baby. Shedding of the collodion led to an ichthyotic condition with a very characteristic distribution of the lesions which involved the trunk, the most proximal parts of the upper limbs, scalp and neck but spared the central face and extremities. The term "bathing-suit" ichthyosis is suggested for this unusual phenotype of lamellar ichthyosis.

[Sjogren-Larsson syndrome. Apropos of a case].

Sjögren-Larsson syndrome is a rare genodermatosis described by Sjögren and Larsson in 1957 (28) and characterized by ichthyosis, spastic di- or tetraplegia and oligophrenia. In the case reported here a female child, born to consanguineous parents, presented at birth as a "collodion baby". Progressively, she developed an ichthyosiform and erythrodermic state and at the same time, after an episode of seizures, spasticity of the lower limbs set in. From then on, her acquisitions were very slow. Symptomatic treatment with retinoids (etretinate, 13-cisretinoid, etretinate again) was attempted repeatedly, but it only resulted in temporary improvement of the ichthyosiform state. Overall evaluation at the age of 6 years showed: i) persistence of the ichthyosiform and erythrodermic state; ii) growth retardation (-2 in stature); iii) spasticity of the lower limbs with difficulty in walking, and beginning of speech. Routine examinations gave normal results. Serum linoleic acid level was within normal limits. Electroencephalography displayed immaturity tracings without signs of epilepsy. Radiography of the skull was normal, and in particular there was no thickening of the sphenoid bone. Histological examination of the skin, outside or during treatment with retinoids, showed ortho- and parakeratosic hyperkeratosis, absence of granular layer, moderate hyperacanthosis, vacuolization of numerous acanthocytes in the uppermost layers and inflammatory infiltrates in the superficial dermis. All these abnormalities were confirmed by electron microscopy. Previously published series are reviewed. In most cases the patient is a "collodion baby" at birth and progressively develops ichthyosis, spasticity, psychomotor and growth retardation.(ABSTRACT TRUNCATED AT 250 WORDS)

[In vitro method for estimation of enteral absorption of clanobutin and other drugs].

Two models have been developed for the determination of in vitro absorption of 4-[4-chloro-N-(4-methoxyphenyl)-benzamidol]-butyric acid (clanobutin) and other agents. Permeability coefficients PM are obtained on artificial phospholipid collodion membranes which permit calculation of absorption coefficients k1 for anticipated human enteral absorption with consideration of physiological parameters. At a pH of 7--8 (small intestine) in particular, k1 shows good agreement with in vitro absorption determined in swine intestine and absorption constant ka derived from pharmacological studies in man. The kinetic absorption model simulates gastro-intestinal transport of the substance as well as local changes in pH. It allows calculation of anticipated human gastric (pH 3) and intestinal (pH 7.5) absorption rates with k1 for different gastric filling volumes. The calculated invasion curves are held against invasion curves calculated from pharmacokinetic studies in man. The comparison shows that all invasion curves calculated according to the kinetic absorption model for gastric filling volumes of 125--1000 ml are within the 95% confidence range; very good agreement exists above all with filling volumes of 125--250 ml. The prognostic value of permeation studies on artificial phospholipid collodion membranes and of the use of the kinetic absorption model in drug design is demonstrated by corresponding studies with chenodesoxycholic acid, indometacin, and clofibrinic acid.

Staphylococcal Protein A column: correlation of mitogenicity of perfused plasma with clinical response.

Eleven patients with advanced breast cancer and four with astrocytoma were treated with plasma perfused over columns containing staphylococcal Protein A (SPA). Doses of 5 to 20 mg of SPA were bound to collodion charcoal particles, and this treatment resulted in partial remissions in one patient with astrocytoma and in two patients with breast cancer. Remission duration was 6 wk to 6 mo. Resolution of lymphadenopathy and a decrease in carcinoembryonic antigen were noted in an additional two breast cancer patients. Systemic reactions to infused plasma consisted of fever, chills, and rigors. In brain cancer patients, increased intracranial pressure was also noted. A mitogenic substance was generated in plasma of 11 patients after it was perfused over the SPA charcoal matrix. The mitogenic material induced lymphoproliferation comparable to concanavalin A and required the presence of SPA on the collodion charcoal but was not due to leakage of SPA from the column during plasma perfusion. Of considerable significance was that only patients whose column perfused plasma contained this mitogenic activity exhibited systemic reactions, and five of these patients obtained antitumor responses. This striking correlation implies that the mitogenic factor is an active component of SPA therapy. The ability to demonstrate mitogenicity in column perfused plasma might also be useful for selecting patients amenable to SPA therapy. These findings attest to the therapeutic value of this mode of treatment and provide an initial definition of a mediator of SPA antitumor activity.

In vitro adsorption spectrum of plasma amino acids by coated charcoal hemoperfusion.

In order to clarify whether coated charcoal hemoperfusion is capable of normalizing amino acid disturbances in hepatic coma, in vitro adsorption and in vitro hemoperfusion studies were carried out. We have found that collodion-coated activated charcoal beads preferentially removed much more aromatic acids (AAA) than branched chain amino acids (BCAA). In the in vitro adsorption experiment with 50 microM amino acid standards aqueous solution, 99% of AAAs were removed by charcoal while only 50 to 81% of BCAAs were removed. As the concentration of amino acids in solution was doubled from microM to 100 microM, BCAA removal was halved while about 90% of AAA was still being removed. In vitro hemoperfusion with heparinized blood from hepatic failure rats, the clearance and the removal of AAAs were significantly greater than those of BCAAs. Consequently, the molar ratio of BCAA over AAA was markedly improved from the initial 1.09 to 3.87 after 60 min of hemoperfusion. Thus, we have demonstrated the preferential adsorption of aromatic amino acids by collodion-coated charcoal beads. The correction of BCAA/AAA molar ratio is also demonstrated.

[Contributions to the liberation of drugs from suspension ointments. Part 12: On the suitability of some synthetic hydrophobic membranes for liberation studies (author's transl)].

The release rates determined for various salicylic acid and prednisolone ointments on using different membranes in the in vitro model according to Horsch and Kögel [20] were compared with one another in regard to possible membrane-specific differences in liberation kinetics and in validity. The results obtained with salicylic acid ointments by the lipophil collodion-lipid membranes and silicone membranes developed by Fürst and coworkers [14] were comparable with those yielded by the hydrophilic cellulose membrane: release rates of comparable order of magnitude; same order of ranking of the bases used; plots of the cumulated percentages of release against the root of time providing no evidence of differences in kinetics. In contrast, the permeability of a further hydrophobic membranes (silicone, polyester) was markedly lower. As to the prednisolone ointments, the collodion-lipid membranes yielded comparable release rates and orders of ranking when the drug was incorporated into hydrophilic bases, whereas the silicone membrane was considerably inferior in permeability.

Analysis of the cornified cell envelope in lamellar ichthyosis.

BACKGROUND: Loricrin and involucrin are major precursor proteins to the cornified cell envelope expressed late in epidermal differentiation. Involucrin expression starts in the upper spinous layers in normal human epidermis and precedes loricrin expression, which is restricted to the granular layer. Subsequently, both proteins become cross-linked by the activity of transglutaminases TGK/E as major components of the cornified cell envelope by N epsilon-(gamma-glutamyl)lysine isopeptide bonds. In this study, three cases of lamellar ichthyosis were analyzed by immunohistologic study with antibodies to loricrin, involucrin, filaggrin, and transglutaminase TGK. OBSERVATIONS: A high expression of loricrin and involucrin with a peculiar and abnormal cytoplasmic staining concurred with a diminished cytoplasmic staining of transglutaminase TGK as assessed by antibodies B.C.1 and K.D.3. This pattern was absent in a collodion baby at birth but present 2 weeks later before a phenotype of lamellar ichthyosis appeared clinically. CONCLUSIONS: The results suggest that in our cases of lamellar ichthyosis, (1) disturbed membrane anchorage of transglutaminase TGK could alter loricrin and involucrin cross-linkage and the formation of the cornified cell envelope and that (2) immunohistologic study might serve as an early diagnostic and prognostic tool in the treatment of collodion babies.

Membrane permeability for inorganic phosphate ion.

The need for a more efficient removal of inorganic phosphate ions from the blood of uremic patients has led to the quest for blood-compatible semipermeable membranes with a high permeability for phosphate ions. Commercial hemodialysis membranes, various hydrogel-type membranes and a number of collodionbased charged membranes were compared. Positively charged collodion membranes were found most promising for future application in hemodialysis or hemoperfusion systems.

The replica technique used to study superficial corneal epithelium in vivo.

We used 2.15 to 2.3% collodion in amyl acetate to form a replica of the corneal surface epithelium in vivo in seven rabbit eyes and ten human eyes. When it was peeled off the replica removed superficial epithelial cells in patches. The corneal epithelium regenerated within one week. Repeated applications at weekly intervals for three months produced no harmful long-term effects in five rabbit corneas as evaluated with slit-lamp biomicroscopy, histological methods, and vital staining of the endothelium. In ten human eyes, the technique was harmless and chemically cauterized the dendritic herpetic ulcers. We studied the replicas and cells by conventional histological methods and electron microscopy. The preservation of cells was fairly good even for ultrastructural study.

Chemistry in a microenvironment of low pH, generated with the aid of an immobilized proteinase.

alpha-Chymotrypsin, when immobilized in a collodion membrane, exhibits high activity and remarkable stability. When the immobilized proteinase is exposed to 15 mM ethyl N-acetyl-L-tyrosinate in dilute pH 8.5 buffer it generates a microenvironment which, indicator studies suggest, has an effective pH of approximately 4. The presence of this locally highly acidic region produces a marked increase in the rate of hydrolysis of BzPheal = Ala dissolved in the buffer solution (BzPheal = Ala is the acylhydrazide obtained from the reaction between N-benzoyl-L-phenylalaninal and N-acetyl-L-alanine hydrazide). The observed rate is 10-times greater than in comparable control experiments incorporating a concentrated buffer solution, in which a pH-gradient does not form. The enhanced hydrolysis rate is quantitatively explained if it is attributed to the approximately 20 microliters of pH 4 solution within the membrane. Other experimental data are also consistent with this hypothesis.

The problem of dehydration and healing of burn wounds.

In an experiment on rats, a partial thickness burn wound 20 mm in diameter was produced on the dorsum. Half the wound area was left untreated, the other half painted with elastic collodion. Examination on the 10th day following the intervention confirmed that prevention of dehydration can save a good proportion of thermally damaged tissue. It was also shown that prevention of dehydration results in a marked delay in wound healing.

Studies with composite membranes. Preparation and measurement impedances.

Simple and composite membranes have been prepared from 2% collodion solutions containing different amounts of polystyrenesulfonic acid (PSSA). Various membrane parameters such as water content, electrolyte uptake, exchange capacity, and permselectivity of these membranes have been determined. The resistance and capacitance of simple membranes have been measured as functions of both external electrolyte concentration and internal fixed charge density. The impedance characteristics of composite membranes also have been determined and discussed in terms of the resistance and capacitance characteristics of simple membranes from which the composite structures have been formed.

Studies with composite membranes. 3. Measurement of water permeability.

The permeability of tritiated water (THO) across simple and layer-type composite membranes of collodion containing different amounts of polystyrenesulfonic acid has been measured and corrected for the effects of aqueous stationary layers present at the membrane-solution interfaces. It was found that the water permeabilities in the two opposite directions across the composite membranes were different, whereas they were the same across simple membranes. The theoretical permeability value for the composite membrane (formed by putting one simple membrane on top of another simple membrane of increasing charge density and gently pressing them together), calculated from the values due to simple membranes, was found to be always greater than the two measured values. It was shown that the aqueous layers trapped between membranes were not responsible for the low measured values. The factor causing this was ascribed to the mechanism which produced rectification of water flow in the composite membranes. Establishment of the THO concentration profile in the layered membranes showed that accumulation and depletion of THO in the membrane phase when the THO was flowing from the high charge density side to the low charge density side and vice versa, respectively, were responsible for the unequal flows observed across the composite membrane in the two directions.

Determination of the distribution of catalyst activity across a permeable membrane containing an immobilized enzyme. Indeterminacy of a functional approach to a structural problem.

Porous membranes were fabricated from collodion and impregnated with papain, inhomogeneously through the thickness of the membrane. These membranes were placed between reservoirs containing N-alpha-benzoyl arginineamide, a substrate for the enzyme papain. The progress of the reaction was monitored by sampling the reservoirs on each side for ammonia, a reaction product. From these data the diffusion coefficient, enzyme activity, and distribution of enzyme activity of the membrane were estimated. The limitations of this approach are discussed in the context of the analysis of biological transport systems.
