Comparison of the ICD-10-classification with the ICD-9- and the DSM-III-classification of mental disorders.

The diagnostic concepts in ICD-10 show better congruence with those in DSM-III than with those in ICD-9. The diagnostic definitions used by ICD-10 and DSM-III for the majority of diagnoses turn out to be similar; the definitions of anxiety disorders and schizoaffective disorders are, however, still at variance. Consequently, the empirical results from the WHO field trial in German-speaking countries reveal a high degree of overlap between the two systems; the degree of overlap between ICD-9 and ICD-10 was lower, but still surprisingly high overall. These empirical results support the validity of ICD-10.

Lymphoid lesions of the thyroid: review in light of the revised European-American lymphoma classification and upcoming World Health Organization classification.

Thyroid lymphomas are rare diseases and almost always rise in the background of chronic lymphocytic thyroiditis (Hashimoto's thyroiditis). Large cell lymphoma is an aggressive disease and usually is not a significant diagnostic challenge from the pathological point of view. Small cell lymphoma, however, can sometimes be difficult to distinguish from chronic lymphocytic thyroiditis. In the past, most thyroid lymphomas were considered to be of follicle center cell origin. Today, after the introduction of the mucosa-associated lymphoid tissue (MALT) and the extranodal lymphoid tissue (ELT) concepts, most of the lymphomas in the extranodal sites are thought to originate from the marginal zone of the lymphoid follicles. The distinction between the different types of lymphomas has significant impact on the patient's prognosis, treatment, and follow-up. It is imperative that clinicians (endocrinologists and surgeons) and pathologists are aware of these types of lymphomas in order for the most appropriate diagnostic procedures to be selected, specific staging principles to be applied, and appropriate disease-specific treatment to be implemented. It is also important that terms such as "lesion of uncertain malignant potential" as defined by ancillary studies be understood.

New high blood pressure guidelines create new at-risk classification: changes in blood pressure classification by JNC 7.

High blood pressure has become increasingly prevalent and is an important risk factor for cardiovascular disease. The Seventh Report of the Joint National Committee on the Prevention, Detection, Evaluation, and Treatment of High Blood Pressure (JNC 7) has redefined normal blood pressure as less than 120/80 mm Hg and created a new blood pressure category called "prehypertension" for those with a systolic BP of 120 to 139 mm Hg or a diastolic BP of 80 to 89 mm Hg. This new blood pressure category was created to identify adults considered to be at risk for developing hypertension and to alert both patients and healthcare providers of the importance of adopting lifestyle changes. Recognition of prehypertension provides important opportunities to prevent hypertension and cardiovascular disease.
