The spectrum of beta-thalassaemia in Burma.

The molecular defects causing beta-thalassaemia (beta-thal) have been analyzed in 63 unrelated Burmese patients. The patients include 49 with Hb E/beta-thal, 13 with beta-thal major and 1 with Hb S/beta-thal. Using synthetic oligonucleotide probes and aided by the polymerase chain reaction 64/76 (84%) of the alleles have been characterized. To date 6 mutations have been identified. The most common mutation is the splicing defect at IVS-1 nt 1 which accounts for 32% of the alleles. Complete characterization of these alleles should aid the initiation of a prenatal diagnosis programme for beta-thalassaemia in the Burmese population.

A profile of ascariasis morbidity in Rangoon Children's Hospital, Burma.

The Rangoon Children's Hospital (RCH) inpatient summary returns from hospital wards from 1981 to 1983 and the surgical operation registers from 1983 to 1984 were studied and analysed in relation to ascariasis morbidity. The majority (57.7%) of clinical manifestations of ascariasis responsible for hospital admission were due to intestinal obstruction, the next most common manifestation was intestinal colic (33.0%). Mean ages for the two conditions were 5.9 years and 7.6 years, respectively. Hospital admissions due to ascariasis varied greatly by season and were significantly higher in the months of May, June and July. Ascariasis was responsible for about 3% of admissions to the hospital. Of the total ascariasis cases, 61.6% were treated in the surgical wards. Moreover, 57.6% of all the acute abdominal cases in the surgical wards were clinically diagnosed to be due to Ascaris lumbricoides infection. In 1983 and 1984, 43 cases of abdominal surgery were performed due to adult Ascaris worms, representing 16.7% of all the acute abdominal conditions operated. Of these 43 cases, intestinal obstruction constituted 65.1%, followed by appendicitis at 20.9%. The mean age of those undergoing surgery for Ascaris-induced abdominal complications was 6.8 years. The validity of some of the clinical data and the public health significance of this study are briefly discussed. The findings are also compared with those of other hospital-based studies.

The efficacy of artemether (qinghaosu) in Plasmodium falciparum and P. vivax in Burma.

Seven sulfadoxine-pyrimethamine resistant Plasmodium falciparum infected patients and 2 patients with mixed (P. falciparum and P. vivax) infection were given Artemether. The results showed that Artemether is effective in sulfadoxine-pyrimethamine resistant malaria. The mean fever clearance time and parasite clearance time of patients treated with Artemether is shorter than those treated with chloroquine, quinine or sulfadoxine/pyrimethamine. In one patient with mixed infection all parasites disappeared but P. vivax reappeared on day 11 and on day 14 after two consecutive courses. In another P. vivax reappeared on day 21.

Observations on the flight activities of Culex pipiens fatigans in Rangoon, Burma.

Knowledge of the behaviour of a vector is often of value in devising suitable methods for assessing control or eradication measures. Flight behaviour is particularly important, since it is during this activity of a mosquito that trapping is possible. Trapping data have proved useful in the assessment of the action of insecticides on the vectors of malaria, and there is no reason why this should not also be the case with the vectors of filariasis. In this paper the difficulties of trapping Culex pipiens fatigans by conventional means are stressed. Moreover, since flight is a dynamic process, the interpretation of flight activities from data collected by traps, which are static, is difficult. Nevetheless, the results presented, however inadequate, show that adult C. p. fatigans move about more, both by day and at night, than is usually suspected and that the subject would repay closer study.
