[Transvaginal follicular aspiration in follicle phase in the management of anovulation associated with polycystic ovary syndrome].

OBJECTIVE: To investigate the follicle development and pregnancy in clomiphene citrate-resistant patients with polycystic ovary syndrome (PCOS) after transvaginal follicular aspiration and gonadotropin administration. METHODS: Seventeen clomiphene citrate-resistant patients with PCOS failed to gonadotropin treatment associated with ovarian hyperstimulation syndrome (OHSS). They are anovulatory infertility with normal tubal and husbands' sperm condition. In this treatment regimen, all patients was administered with gonadotropin from day 5 of menstrual cycle (or withdrawal bleeding) and were asked to undergo ultrasound-guided transvaginal follicular aspirations for 5 days later. Only one or two follicles for every ovary were remained and gonadotropin administration was continued. A follicle development, ovulation rate, pregnancy rate and sex hormone were monitored. RESULTS: Fifteen of the 17 patients experienced dominant follicles development and ovulation. 9 of them with one (52.9%), 4 with two (23.5%) and 2 with three (11.8%) dominant follicles ovulated and pregnancy occurred in 7 patients (41.2%). All of pregnancy are singleton. CONCLUSION: A single follicle development and singleton pregnancy can be realized in clomiphene citrate-resistant PCOS patients by combined ultrasound-guided transvaginal follicular asperation with gonadotropin administration.

[Nature of the estrous cycle, incidence of anovulation, chromosome gametopathies and fetal death in female rats with 2,4-dichlorophenoxyacetic acid (2,4-D) before pregnancy].

The effect of herbicide--2,4-D, injected before pregnancy begins, on the main parameters of the reproductive function of the female rats has demonstrated that chronic administration in doses 1 mg/kg and 12 mg/kg per day for 2 months results in certain disturbances in the estrus cycle, manifesting as prolongation of the diestrus phase and in changes of estrus and meta-estrus++ duration, as well as in an essential increase in rates of anovulatory cycles. Single administration of the preparation during the preovulatory period (50 mg/kg) produces certain disturbances in the chromosomal complex, manifesting as appearance of numerical chromosomal aberrations. When the administration is acute (10 mg/kg and 50 mg/kg) an essential increase of the embryonal death takes place, in the first case--at the expense of ++pre-implantational, and in the second--both at the expense of pre- and ++post-implantational death. At chronic administration increasing rate of the intrauterine death takes place mainly at the expense of ++post-implantational death of embryos.

Normoprolactinemic anovulation nonresponsive to clomiphene citrate: ovulation induction with bromocriptine.

Ovulation under bromocriptine was studied in 14 women with normoprolactinemic amenorrhea (5 primary, 9 secondary), unresponsive to clomiphene citrate (CC). On bromocriptine alone, ovulation occurred in 4 (28.6%). In the same subjects, bromocriptine was subsequently associated with CC. Seven patients ovulated (50.0%), including 3 that had responded to bromocriptine alone. Ovulation occurred once or twice in 6 of the 9 cases of secondary amenorrhea (66.6%). In several occasions, when ovulation induction failed, luteinized unruptured follicles were found under ultrasonographic monitoring. Four patients who had a negative response to progestin challenge did not ovulate with the treatment. Women with plasma prolactin in the upper normal range had a greater probability of achieving ovulation induction.

Clinical and endocrine response to pulsatile intravenous gonadotropins in refractory anovulation.

We investigated the endocrine response to an experimental protocol in which human menopausal gonadotropin was delivered in pulses every 90-120 minutes by an automated pump. Thirty clomiphene citrate- and gonadotropin-releasing hormone-unresponsive anovulatory women were studied over 107 treatment cycles. In response to episodic intravenous (IV) delivery, pulses of FSH and LH were demonstrable in the circulation. In four World Health Organization (WHO) group I anovulatory women, ovulation occurred in all cycles (N = 8); the pregnancy rate per cycle was 63% and the cumulative pregnancy rate 100%. In 26 WHO group II patients, 42.4% of treatment cycles were in 12 women previously refractory to the intramuscular route of administration; the rate for ovulation was 86% (total of 99 cycles), the pregnancy rate per ovulatory cycle was 14%, and the cumulative pregnancy rate 56%. A mild phlebitis occurred at the site of the IV catheter in 24% of treatment cycles. Intravenous delivery of gonadotropins in small pulses had a dose-sparing effect and was an effective method of treating anovulatory infertile patients refractory to other conventional methods of ovulation induction.

Anovulation and plasma hormone concentrations after administration of dexamethasone during the middle of the luteal phase in sows undergoing estrous cycles.

The effect of glucocorticoids on early follicular growth in sows undergoing normal estrous cycles was evaluated by administration of dexamethasone during the middle of the luteal phase. Plasma specimens were obtained for measurement of luteinizing hormone (LH), follicle-stimulating hormone (FSH), progesterone, and estradiol-17 beta concentrations. Fifteen sows were used. Control sows (n = 5) were given physiologic saline solution twice daily from day 9 to day 14 of the estrous cycle. Sows of the second group (n = 5) were given dexamethasone (30 micrograms/kg of body weight, IM) similarly, and those of the third group (n = 5) were given dexamethasone plus gonadotropin-releasing hormone (GnRH; 50 micrograms at 6-hour intervals, IV). Plasma specimens, obtained twice daily from day 8 through day 26, indicated that progesterone production and luteal regression were not inhibited by any of the 3 treatment regimens. Although preovulatory plasma estradiol concentration increased in control sows, such was not observed in the sows treated with dexamethasone or dexamethasone plus GnRH (P less than 0.01). Ovulation, with formation of corpora lutea, occurred in gilts given saline solution. Dexamethasone administration resulted in persistence of 19 to 41 follicles/ovary (2 to 4 mm in diameter), and dexamethasone-plus-GnRH treatment resulted in 6 to 18 follicles/ovary (5 to 6 mm in diameter). Plasma was obtained at 15-minute intervals for 12 hours to compare the effect of treatment on hormone concentrations on day 12 of the estrous cycle with the values on day 8.(ABSTRACT TRUNCATED AT 250 WORDS)
