[Adrenocorticotropic and somatotropic functions of the pituitary gland in juvenile thyrotoxicosis].

The adrenocorticotropic (ACT) activity of the hypophysis (according to the blood 11-HCS level) was studied in 27 female patients suffering from thyrotoxicosis, aged from 16 to 24 years (19 with moderately severe and 8 with severe form) and in 7 healthy girls. The ST activity of the hypophysis was ascertained in 11 patients by the blood STH content before and after the insulin hypoglycemia. The metopyron test was also carried out in 6 patients. The ACT function of the hypophysis proved to be increased, and the ST function was normal in the patients with thyrotoxicosis at rest and on fasting stomach. The value of the corticotropic and somatotropic response to hypoglycemia was similar in sick and healthy persons. In the patients with a pronounced elevation of the 11-HCS and STH content under the effect of hypoglycemia the metopyron response was determined by the initial adrenal cortex function.

[Serum neuraminic acid level in patients with cerebrovascular disorders].

The authors determined the level of neuraminic acid in the serum of 50 patients with encephalomalacia and 25 patients with cerebral haemorrhages on the 1st, 3d and 7th days of the disease. Twenty healthy subjects served as controls. A statistically significant rise in the concentration of this substance was observed in both groups of patients throughout the whole period of observations.

[Increase in the sensitivity threshold of the hypothalamo-hypophyseal complex to the inhibiting action of dexamethasone in the process of aging and in cervical cancer].

The basal level of the androgenic and glucocorticoid hormones excretion and the level of blood glucocorticoids were studied in 237 females (52--control group, 185--cervical cancer patients) depending on the age and the ovarian function state. Also, the threshold of sensitivity of the hypothalamo-hypophyseal function complex was estimated by a dexametazone test. Healthy females were shown with progressive ageing to have a relative preponderance of glucocorticoid hormones over androgenous ones, which is also observed in cervical cancer irrespective of age group. With ageing in healthy females the threshold of the hypothalamo-hypophyseal complex sensitivity is raised, whereas in cervical cancer patients an increased threshold of the sensitivity is observed in younger age groups and is more manifest. The increased threshold of the hypothalamo-hypophyseal complex sensitivity seems to be one of the mechanisms responsible for the occurrence of such unfavourable hormonal shift (a relative predominance of glucocorticoid hormones over androgenic ones), which in its turn play a significant part in tumors origination and development.

[Hormonal balance in ovarian tumors].

As a result of examining the urine excretion of 17-OCS, 17-KS, estrogens and the concentration of protein bound iodine and butanolextractable iodine in blood of patients with benign and malignant ovarian tumors, there were found some deviations from the normal hormonal balance in all these patients. These deviations were manifested in a delayed transformation of cortisol to cortisone, in a shifted androgeno-estrogenic balance toward estrogen active fractions, and in the reduced production of thyroid hormones. In ovarian cancer, however, these changes were manifested to a greater extent.

Septum and adrenocortical function.

Dorsal septum is an intermediary centre in between the hippocampus and hypothalamus and it has got inhibitory influence over the hypothalamo-pituitary-adrenal axis. Stimulation of the dorsal septum manifested with fall in adrenocortical output whereas its lesion has opposite response. Ventral septum is a facilitatory area regarding activation of the hypothalamo-pituitary-adrenal axis. It is a balancing centre lying in between the higher central nervous system structures and the hypothalamus. Stimulation and lesion of the ventral septum led to rise and fall in adrenal venous 17-OHCS output respectively. Adrenocortical response to stress of burn was not blocked or inhibited following lesion of the dorsal as well as ventall septum.

[Certain pathogenetic mechanisms of development of atherosclerosis in dogs, induced without the use of cholesterol].

Chronic investigations (up to 3 years) conducted in dogs have evidenced that periodic disturbances of vascular permeability produced by using dicoumarin with a long-term blocking of the thyroid function greatly aggravate pathological manifestations induced by the action of 6-methylthiouracil. They result in the development in the animals of endogenous hypercholesterinemia, elevated content in the blood of beta-lipoproteins and in a periodic rise of the arterial pressure. These changes were particularly spectacular at the final stage of the experiment in animals with marked manifestations of atherosclerosis and were accompanied by reduced stand-by potentialities of the adrenal gland, disruption of metabolic processes in the myocardium along with the slowed down resorption of NaI131 from the heart muscle. The sensitivity of the heart to adrenalin was down too.

Comparative study of anti-endotoxic potency of dexamethasone based on its different ester types.

Difference in the anti-endotoxic potency in mice between dexamethasone 21-disodium phosphate (DM-P) and 21-sodium sulfate (DM-S) was investigated. Changes in the plasma levels of free 17-hydroxycorticoids (17-OHCS), further, were defined after intravenous administration of 2 mg/kg dexamethasone in elective surgical patients for elucidation of the metabolic basis of the different anti-endotoxic potencies of the two ester types of dexamethasone. Young male mice of DDN strain were pretreated with 0.2 mg DM-P or DM-S. Following the pretreatment, 0.5 mg of the endotoxin was administered at varying intervals of 0--24 hours, and survival rates were determined after 72 hours. In the DM-P group the mice exhibited a significant resistance to the endotoxin within two hours after the pretreatment, but in the DM-S group, the mice exhibited no anti-endotoxic potency. When different doses of dexamethasone (from 3.2 to 0.00625 mg) were administered with 0.5 mg of the endotoxin simultaneously, the mice in the DM-P group showed improvements in survival rates that tended to be dose related, but the mice in DM-S group showed no improvement in survival rates. Plasma levels of free 17-OHCS in elective surgical patients, further, were remarkably elevated after administration of DM-P, 2 mg/kg, but they showed no elevation after administration of DM-S, 2 mg/kg. Thus, it is clear that the in vivo metabolism of the two esters is different. DM-P easily liberates its free form, whereas DM-S does not. From this evidence, it was concluded that there is a close correlation between the anti-endotoxic potency of dexamethasone and its plasma level of free form, and that glucocorticoids can demonstrate their anti-endotoxic action when they are present at pharmacologically high levels in the bloodstream. There is considerable difference in biological action, moreover, between the two water-soluble esters of dexamethasone. More attention should be paid to this difference when investigating the treatment of shock with glucocorticoids.

Partial 11- and 21-hydroylase deficiencies in hirsute women.

Thirty-one women with idiopathic hirsutism were evaluated for partial 11- and 21-hydroxylase adrenocortical enzyme deficencies. Twenty-four hour urine collections for 17-ketosteroids, 17-hydroxycorticoids, tetrahydro compound S (THS), and -pregnanetriol were obtained basally and following a continuous 24 hours infusion of alpha 1-24 ACTH (cosyntropin). The results were compared to those in eight normal, nonhirsute women studied under identical conditions. Normal control subjects and 18 of 31 hirsute female patients (Group I) showed similar small increments in the excretion of THS and pregnanetriol following the infusion of cosyntropin. Thirteen hirsute women (Group II) showed cosyntropin-stimulated increments in either THS and/or pregnanetriol that were significantly greater than the mean response of the control group. The cosyntropin-stimulated increments in 17-ketosteroid excertion and basal sebum production rates were also significantly greater in Group II. The results are consistent with partial II - and/or 21-adrenocortical hydroxylase deficiencies in some hirsute women whose condition would have previously been designated as "idiopathic." Thus, prolonged ACTH stimulation testing may identify patients who might benefit from glucocorticoid suppression therapy.
