High ideals & perverse consequences: risk adjustments, encounter data & Medicare+Choice.

Why should you take on the risks and complications of developing a PSO? Is it worth it? Will it mean assuming undue risk? The answer to this question gets complicated. This article will introduce more uncertainties than you had before. But, you need to have your eyes wide open. It's not just a question of your capability as a health system or a medical group. It's a question of HCFA's ability to administer the program. Even without knowing the capabilities of HCFA, most organizations would say that they don't trust the government to follow through with any program. In this article by Bruce Fried, we get a rare look at the inner thoughts and gyrations of government, as they attempt to risk-adjust Medicare payments to PSOs and HMOs. In ordinary language, that means, if you happen to have a very sick population of seniors, you will be paid more to take care of them. But, read this and weep.

[Radiation-induced morphea].

Although the aetiology of morphea often remains unknown, various precipitating causes have been identified. Morphea is a rare development following irradiation. We observed a circumscribed scleroderma 14 years after x-ray-radiotherapy following surgery of breast cancer. Furthermore multiple weeping papules occurred within the radiation area. A relapse of the breast cancer was ruled out by histological examination which displayed a transepidermal elimination of acid mucin. The irradiation had obviously induced different reactions of the dermal fibroblasts (radiodermatitis, morphea, mucin production).

[Pharmacological treatment of neurotic depressions].

Neurotic depression covers all forms triggered by exogenous factors, often in conjuction with an underlying personality disposition, and includes reactive, exhaustion and neurotic depression. A feeling of sadness and loss of affection is generally present, together with anxiety provoked by insecurity and apprehension. The complexity of the picture makes treatment of the overall situation difficult with one drug only. For this reason, a series of 25 patients aged 31 to 67 yr with syndromes classifiable as neurotic depression were treated with 2 to 6 daily administrations of an association of 25 nortryptiline and 1 mg trifluoperazine. Analysis of the behaviour of a number of target symptoms showed the effectiveness of the association. There were virtually no side-effects. Success was obtained in the management of anxiety, loss of appetite, depression, irritability, weeping, failure to adapt, and remissiveness, especially in the first week of treatment.

An apparently gluten-induced photosensitivity in horses.

Primary photosensitization was observed in 3 Appaloosa mares. The skin lesions were diffuse erythema followed by edema and subsequently weeping and finally dry gangrene and ulceration. The severe photosensitivity dermatitis was apparently induced by gluten ingestion. Resolution of lesions occurred after withdrawal of the suspected dairy concentrate feed and prevention of exposure to sunlight. Neither the ponies nor donkey, which were not fed with the suspected concentrate, exhibited similar skin lesions or other clinical abnormalities. Gluten metabolites may contain photodynamic agents that cause photosensitization in horses.

Mood disorders in multiple sclerosis: diagnosis and treatment.

Emotional disturbances are common in MS and consist of disturbances of mood and disturbances of affect. The important mood disorders are major depressive disorder, dysthymic disorder, bipolar disorder, panic disorder, and generalized anxiety disorder. Their relationship to MS is multi-factorial and complex, and the extent to which they are direct consequences of the disease process or psychological reactions to it remains unclear. Whatever their cause, however, the symptoms of mood disorders in people with MS are no different from the symptoms of mood disorders in people without MS, and respond just as well to standard treatments. The disorders of affect are euphoria, pathological laughing and weeping, and other frontal lobe syndromes. These disorders result from demyelination, are some of the most characteristic symptoms of MS, and have the same implications for treatment as do other aspects of the disease. Mood and affective disturbances can cause enormous pain and suffering and lead to significant disruption of family, work, and social life. Physicians who can identify, diagnose, treat, and manage mood and affective disturbances effectively and who can help their patients and family members acknowledge these difficulties, talk about them, and accept psychiatric consultation and treatment can have a dramatic impact on the quality of their lives. This paper outlines the symptoms and diagnostic criteria for mood disorders and affective disturbances, reviews current treatment options, summarizes data from epidemiologic and pathophysiological studies, and suggests areas for future research.

Rocker is a new variant of the voltage-dependent calcium channel gene Cacna1a.

Rocker (gene symbol rkr), a new neurological mutant phenotype, was found in descendents of a chemically mutagenized male mouse. Mutant mice display an ataxic, unstable gait accompanied by an intention tremor, typical of cerebellar dysfunction. These mice are fertile and appear to have a normal life span. Segregation analysis reveals rocker to be an autosomal recessive trait. The overall cytoarchitecture of the young adult brain appears normal, including its gross cerebellar morphology. Golgi-Cox staining, however, reveals dendritic abnormalities in the mature cerebellar cortex characterized by a reduction of branching in the Purkinje cell dendritic arbor and a "weeping willow" appearance of the secondary branches. Using simple sequence length polymorphism markers, the rocker locus was mapped to mouse chromosome 8 within 2 centimorgans of the calcium channel alpha1a subunit (Cacna1a, formerly known as tottering) locus. Complementation tests with the leaner mutant allele (Cacna1a(la)) produced mutant animals, thus identifying rocker as a new allele of Cacna1a (Cacna1a(rkr)). Sequence analysis of the cDNA revealed rocker to be a point mutation resulting in an amino acid exchange: T1310K between transmembrane regions 5 and 6 in the third homologous domain. Important distinctions between rocker and the previously characterized alleles of this locus include the absence of aberrant tyrosine hydroxylase expression in Purkinje cells and the separation of the absence seizures (spike/wave type discharges) from the paroxysmal dyskinesia phenotype. Overall these findings point to an important dissociation between the seizure phenotypes and the abnormalities in catecholamine metabolism, and they emphasize the value of allelic series in the study of gene function.

Spontaneous biliary peritonitis in acalculous cholecystitis: fact or misdiagnosis?

It is often speculated that an inflamed gallbladder weeps bile to produce bile peritonitis. This may be so, but more likely the problem is a peritoneal effusion in a jaundiced patient which thus resembles bile. So-called "spontaneous or idiopathic biliary peritonitis" in acute acalculous cholecystitis without a proven cause is a further example of this very rare condition. Spontaneous perforations of the extrahepatic biliary ductal system associated with acalculous cholecystitis are uncommon albeit reported in adults. Most patients present with an acute abdomen and are operated upon urgently without diagnostic iter. A recent experience with such a case prompted a thorough review of 27 similar cases previously reported.

Embryogenic cell suspensions from different explants and cultivars of Eragrostis curvula (Schrad.) Nees.

The aim of this work was the establishment of embryogenic calli and cell suspensions from different explants and cultivars of weeping lovegrass, Eragrostis curvula (Schrad.) Nees, to be used as targets for biolistic transformation. Calli were initiated from immature inflorescences, seeds, embryos, leaf bases and root tips. Modified MS medium (Murashige and Skoog, 1962) was used for calli induction and proliferation. Cell suspensions were established and maintained in AAF medium (Wang et al., 1993). Morphogenic calli, embryogenic cell suspensions of moderate growth rate--consisting mainly of compact proembryogenic cell clusters- and green plants were obtained from all the explants and cultivars assayed, except root tips. Both, explant and genotype were very important factors to be considered in order to obtain a morphogenic response and to establish cell suspensions from this grass. The statistical analysis detected interaction between both factors, explants and genotypes. Immature inflourescences were the best source of explant and Kromdraai was the cultivar that showed the best morphogenic response (expressed as the percentage of calli/explant and the percentage of calli with green spots--every green spot developed into green plants-) with inflourescences, embryos and leaf bases. For Morpa and Don Pablo embryos as explants were less responsive than seeds and leaf bases. There were no differences in leaf bases for all the three cultivars analysed.

[Death according to old-Israeli beliefs].

In Palestine the Neanderthal men and fossil contemporary men treated the dead kinsmen in a special way which can be observed in his intentional burials. They are the evidence of the belief in life after death that probably was imagined as a continuation of earthly life. In the Bible thanatology issues do not assume the crucial importance; the descriptions of dying and posthumous continuance of a man are brief and subject to a certain development. Death is not glorified, the dead cult is forbidden, the corps are treated in a ritual context as impure objects, contaminating people and environment. Death was not considered as boundary, temporary phenomen but as a gradual outflow of life. Only in the later fragments of the Old Testament there appears the concept that in death there takes place the ultimate separation between two constituent elements of a man i.e. between body that turns into dust and soul that goes back to God. The Bible distinguishes various types of death and its causes; but it is God who is the ultimate master of life and death. The other life is considered to be a manifestation of God's blessing. The Holy Scriptures pays special attention to the words spoken by the dying man, occasionally ascribing a prophetic sense to them. Although the funeral ceremony was not ostentatious it was most of the time excessive. The funeral was organized immediately after death, usually after 8 hours and the same day if it was possible. The corpse was wrapped up in linen and buried in hewn in the rock caves or terrestrial graves; bodies were neither burnt nor mummified. "Weeping" and mourning for the deceased lasted quite short - 7 days, seldom longer. Any forms of communicating with the soul of the dead person were severely forbidden and prosecuted; they were considered as discordant with the Jahweh cult. Some of the texts seem to reveal personification of death; it is never considered, however, as a being or power independent of God.

Attitude to Death↗

Continuing care of the terminally ill.

Terminal care has specific goals for the attending medical team. In truth, most dying patients do not expect miracles. What they ask for is assurance that they will not be abandoned or discarded because they have an incurable condition. Truth can be gentle and merciful, and not necessarily without hope. Grieving is normal for patients, families, nurses and doctors. Weeping with sad friends is not evidence of weakness, rather it displays great empathy, and can be therapeutic for patients and doctors together.

Brain abscess as a rare complication in a hemodialysed patient.

BACKGROUND: Infections remain among the most common morbid events and are an important cause of death in end stage renal disease. They have reduced immune response and increased hazard of infections due to repeated puncture of an arterio-venous fistula, formation of haematoma at the site of cannulation and central vein catheterisation. CASE REPORT: We report a case of brain abscess in chronically haemodialysed patient admitted to our department due to haedache, vomiting, accelerated hypertension and fever. The clinical examination revealed narroving of the right palpebral slit, weeping and right oral angle hanging loose. He had mild microcytic anaemia and high level of g-globulin. Ophtalmologic examination showed normal oculi fundi. The computed tomography revealed heterogenous mass marginally enhanced with contrast agent in the right frontal cerebral lobe. The right fronto-temporal craniotomy was performed and the right frontal lobe abscess was found and totally excised. The postoperative course was uneventful besides of seizures which were effectively treated with carbamazepine. After bilateral nephrectomy the patient undervent succesfull kidney transplantation and is in good condition without any neurological defect. A probable cause of his brain abscess was peridontal abscess recognized 3 month earlier or bilateral vesicoureteral refluxes. CONCLUSIONS: 1. Uremic patients have a reduced immunocapacity and are a high risk group for infections of various etiology. 2. Prompt eradication of all sources of infection is essential in hemodialysed patients.

Should potassium permanganate be used in wound care?

Potassium permanganate is a mild antiseptic with astringent properties. It is used in dermatology to treat weeping skin conditions. Potassium permanganate tablets are commonly used in clinical practice. The 400 mg (1:1,000) tablets are diluted in four litres of water to give a dilution of 1:10,000 (0.01%) (British Medical Association and Royal Pharmaceutical Society of Great Britain, 2003).

[Phenologic observations of the Rabka health resort in 1990].

Important factors decisive of the health resort's climatic characteristics are the results of aero-phenologic studies. These results take in account the different pollinating seasons of various plants, grasses and weeds. Quantification and qualification of air pollination using the volume method with a Burkhard analytical device. Basing on results the authors have found increasing pollination string from March. At his time the main pollens come from birch, hazel, alder, weeping willow. The concentration of these pollens decrease in April, completely diminishing in May. Grass pollens increase in mid-May, peaking in June to early August. Pine pollens are found in May and June. A characteristic feature of the phenologic calendar is pollination of grass and pines in the same period (late May and June). The results of this are seen in children treated at the Mother and Child Institute Branch in Rabka, who demonstrate increased signs of pollinosis. The results of this study should modify treatment and change prophylaxis in children treated in Rabka.

[Preliminary clinical study on krypton laser photodynamic therapy for PWS].

OBJECTIVES: In order to explore an ideal PWS treatment, which could destroy the capillary of lesion without any damage to skin. Based on principle of improving elective therapeutic efficacy of photodynamic therapy, new Krypton laser PDT was introduced to this preliminary clinical trial. MATERIALS AND METHODS: The absorptive spectrum was measured on photosensitizer PsD-007. The therapeutic laser dosage and result and side effect were surveyed. Twenty cases of PWS were randomly selected by means of self-control. 413 nm wavelength laser PDT was carried on those patients. Lesion responses were investigated at different phases after treatment. More attention was paid to the result and side effect. Biopsy was taken from one case. RESULTS: PsD-007 has the soret peak at 408 nm. There was purplish reaction within a week, which disappeared spontaneously within two months. Lesion blanch with neither weeping soakage nor scar formation. All cases got dramatically blanch of lesions. Histological finding revealed red blood cells leaking through broken vessels, while dermal remained normal. CONCLUSION: 413 nm Krypton laser PDT had target elective effect, which had satisfactory result. It suggested good potential plication.

Multimodal imaging in the congenital pulmonary lymphangiectasia-congenital chylothorax-hydrops fetalis continuum.

We report on three infants with congenital chylothorax (CC) and congenital pulmonary lymphangiectasia (CPL). CPL appears to be a characteristic pathological finding in CC. Through the use of lymphoscintigraphy and computed tomography, this study suggests that CC and CPL are strongly correlated entities and that the dysplasia of the lymphatic system results in a pulmonary lymphatic obstruction sequence. The initial microscopic dilatation of the lymph channels may lead to progressive weeping of lymphatics and, consequently, to pleural effusion. Non-Immune Hydrops Fetalis (NIHF) may be the final consequence of impaired systemic venous return and may help to explain pleural-pulmonary involvement in this generalized lymph-vessel malformation syndrome.

Hair-thread tourniquet syndrome. A case report.

The authors report the case of a 3-month-old boy with hair-thread tourniquet syndrome, seen just distal to the MP joint on the right fourth toe. The patient was brought to the hospital with a history of irritability and weeping. On examination there was swelling, ecchymosis and a hair fibre was found wrapped around his right fourth toe. After the hair fibre was removed there was a fast healing period and no signs of tissue necrosis were seen.

[A discussion on influence of philosophical idea of Guan Zi on TCM].

Guan Zi has substantial content and deep idea, being paid great attention among the various ancient schools of thought. After discussing the influence of its philosophical idea on TCM, it can be concluded that Guan Zi raised Yin Yan concept to a natural law earlier after Lao Zi first philosophized it. Meanwhile discussed the relationship between Yin and Yang systematically such as respecting Yang, Yang can control Yin, and relativity of Yin and Yang. It first setup Jing -Qi Theory, discussing materiality of the Nature and life from the sense of ontology. But it confused Jing Shen and Jing Qi, and could not weep the primitive color of Xing-Shen Monism. It emphasized on self-cultivation and embodied Xu (voidness), Yi (unity), Jing (tranquility) and Yin (cause) on preserving health.
