[Experimental study of the pulmonary form of plague, tularemia and pseudotuberculosis].

There was revealed a regular reduction of plague, tularemia and pseudotuberculosis bacteria count in the lungs of guinea pigs the first 12 hours after aerosol infection. Generalization of the infectious process and associated septicemia occurred in pulmonary plague and pulmonary tularemia on the 1st-2nd day, and in pulmonary pseudotuberculosis - on the 4th-5th day. Limits of accumulation of the causative agents in the organs and the blood at various stages of the infectious process were established.

[Possible atypical course of tularemia (persistence) in the common vole Microtus arvalis Pall].

The possibility of the atypical course of tularemia with the prolonged persistence of Francisella tularensis in common voles (M. arvalis), the twin species of East European voles (M. rossiaemeridionalis), was studied. Experiments were made on 33 animals grown in the laboratory. F. tularensis strain 165 was used. The animals were infected by feeding them according to the previously developed scheme. 7 out of 33 voles showed the atypical course of tularemia: in 3 voles the disease took a prolonged course with bacteriuria and death on days 25-34; 3 other voles with bacteriuria registered before days 33, 66 and 172 (the term of observation) survived. The surviving animals were killed on day 183, and the presence of bacteria in their organs and seroconversion were established. One vole excreted no bacteria with urine and had no bacteria in its organs (the animal was examined on day 156), but in its blood specific antibodies were detected. To determine bacteriuria, the immunofluorescence test was used together with biological assays. Thus, M. arvalis, like M. rossiaemeridionalis studied earlier, can harbor F. tularensis at the period between epizootics. When voles of the former species penetrate stacks of straw and hayricks, conditions appear for the transfer of the infection to the latter species, M. rossiaemeridionalis. Therefore, in the foci of the meadow-field type each of these two species of voles may be not only of epizootic, but also of epidemic importance.

[Tularemia study in Khmel'nitski Province].

The differentiated approach to carrying out the vaccinal prophylaxis of tularemia is necessary in view of the determination of the types of the natural foci of this infection and their epizootic manifestation, as well as taking into account concrete local conditions and social factors. At the same time, the level of zooparasitological investigations must be maintained for the timely determination of the activity and spread of the natural foci of tularemia.

[Mechanism of the changes in the animal reactivity to tularemia in mixed infection].

Experiments were conducted on albino rats infected with listeria or salmonelloses, and then with tularemia; differences were revealed in the duration of manifestation of nonspecific resistance associated with peculiarities of pathogenesis and immunogenesis of the background infections. One of the significant factors causing an increase of albino rats resistance to tularemia in mixed infection was activation of the immunomorphological reaction promoting accelerated development of specific immunity reactions to this infection.

[Activation of natural tularemia foci of the field-meadow and steppe types on the territory of Tula Province 1977-1978].

Natural tularemia foci of the meadow and steppe type are extremely stable and become active in those years when the most favourable living conditions for rodents appear. For the first time during the last 30 years a great increase in the number of common voles, accompanied by widely spread epizooty covering the whole territory of the Tula region, was observed. House mice, common field mice, harvest mice and black rats were also involved in this epizooty and 235 tularemia patients with all clinical forms of the disease were registered, the pulmonary form of the disease being prevalent. The cases of the disease were observed among both urban and rural population. In spite of a high morbidity rate, no cases of group infection were registered in domestic conditions and among agricultural workers due to the existence of the numerous immune layer among the population. The formation of this layer resulted from planned vaccinal prophylaxis covering, on the average, 86.3% of the rural population of the region.

[Detection of the tularemia zoonosis in the territory of the Kartlian Plain].

The natural focus of tularemia was found to cover the whole territory of the Kartlian plain. Epizooty occurred mainly among common voles with the involvement of insectivorous voles. Hard ticks, gamasids and fleas infected with Francisella tularensis were detected. Rodents highly sensitive to tularemia can be affected by this infection. Water rats were not involved into the enzootic process. Further studies are necessary for the final solution of this problem.

[Occurrence of natural tularemia foci in Kamchatka].

The focus of tularemia in the Ust-Kamchatsky region belongs probably to the flood-plain and swamp type with muskrats and tundra voles being the main carriers. In autumn 1980 for the first time three cultures were isolated in this region: 1 from a tundra vole and 2 from muskrats. The presence of antibodies to tularemia antigen in Norway rats was revealed in the passive hemagglutination test.

[Experience with a bacteriologic and serologic study of a tularemia epizootic in rodents in a natural focus of the meadow and field type].

The autumn-winter (1977-1978) tularemia epizootic in small murine rodents was revealed and studied at the natural focus of the meadow and field type in the south of the Moscow Region. The efficacy of the serologic method (the antibody neutralization test) of studying the organs of the caught rodents and the bodies of dead rodents was found to be greater than that of the traditional bacteriologic methods (26.6% and 9.6%, respectively). The serologic study of 908 specimens of avian excrements collected during the period from autumn to spring (1977-1978) revealed that tularemia antigen could be constantly detected, starting from October. The serologic method was effective when used both for the early and retrospective detection of the infective agent and allowed to characterize the epizootic process in greater detail.

[The role of lemmings in natural tularemia foci of arctic regions].

The presence of Taimyr of independent natural tularemia foci of tundra type was revealed on the basis of isolation of tularemia causative agent from lemmings and the objects of external environment and detection of a specific antigen in the pellets of birds-myophages and mummified cadavers of animals. Possible ways of circulation of the microbe in the lemming population are discussed.

[Comparative study of the basic immunological reactions for assessing tularemia immunity in inoculated persons or those who have had the disease].

In the comparative study of immunological tests used for the determination of immunity to tularemia in vaccinees the greatest number of persons with the positive reaction was detected by means of the tularine skin test. Former tularemia patients retained allergic skin reaction to this test irrespective of the time of the disease.

[Experimental latent tularemia in common voles].

Common voles (Microtus subarvalis) were infected with tularemia by feeding them with the corpses of the animals which had been infected with Francisella tularensis strain having somewhat decreased virulence. This resulted in nonlethal infection in 14 out of 433 voles used in the experiment. A considerable part of the animals having had the infection developed bacterial carriership (11 out of 13 animals) with bacteriuria (8 out of 11 animals) lasting up to 2 months. The persistence of Francisella tularensis in the body of the animals having had the infection could last as long as 6-11 months. The possible epizootological importance of the latent course of tularemia infection in common voles is discussed.

Tularemia in Canada with a focus on Saskatchewan.

Although rare among humans in Canada, tularemia is often endemic in wildlife. The inhabitants of rural areas are especially likely to be exposed to the causative bacterium, Francisella tularensis, through trapping or through the bites of arthropods. Muskrats have replaced rabbits as the principal source of infection, as illustrated by a familial outbreak of oropharyngeal tularemia in Saskatchewan. In humans the disease has six distinct forms and can be asymptomatic, but it generally comes to medical attention as fever, persistent ulcers and enlarged lymph nodes. Serologic tests will confirm the diagnosis.

[Natural focus of tularemia on the Kerchen peninsula (Crimea)].

The study confirming the existence of the steppe-type natural focus of tularemia on the Kerch peninsula has been carried out. For the first time the cultures of Francisella tularensis have been isolated. Voles and house mice play the main role in the circulation of the infection. The parasitic system comprises ticks (Ixodidae and Nyalomma), as well as some species of fleas. In carrying out erizootological studies for detecting tularemia in the Crimea the use of low temperature (0 degrees C) for the preservation of specimens and preparations in recommended.

[Half a century since the discovery of tularemia in the USSR].

The all-round and detailed study of tularemia carried out in the USSR for 50 years (1926-1976) offered a possibility to lay scientific foundation and to carry out an effective control of this infection in the whole country, providing a sharp and stable reduction of the incidence of this infection in man. By a number of sections and by practical results tularemia investigations in the USSR excelled foreign data.

[Tonsillo-pharyngeal form of tularemia].

Clinical, laboratory and serologic features of tonsillopharyngeal form of tularemia in 14 patients were analyzed in the study. The disease was ignored under the diagnosis of typical tonsillopharyngitis followed by cervical lymphadenopathy. Routine laboratory analyses results were non-specific and didn't reveal the disease etiology, while microagglutination test (MAT) of F. tularensis in the second week of disease was positive in 86% of patients. Skin tularin test was positive in all the tested patients during the first week of the disease. The best results in the treatment were achieved by administration of streptomycin and doxycycline. Well timed therapy prevented the lymph nodes colliquation, disease recurrence and infection generalization. The authors point out the significance of this clinical form of tularemia in the differential diagnosis of tonsillopharyngitis and cervical lymphodenopathy.

[Protective properties of doxycycline, rifampicin and sisomycin in experimental pulmonary tularemia of albino mice].

The protective effects of doxycycline, rifampicin and sisomicin were compared in a multifactor experiment with animal infection induced by finally dispersed aerosol of a virulent strain of the European variant of the tularemia causative agent and the optimal regimens for the antibiotic use were determined. By the values of the protective effect rifampicin and sisomicin were shown to provide a high percentage of the animal survival: more than 80 and up to 50-70 per cent of the animals survived when the aerosol infective doses were 100 and 10,000 LD50, respectively. A characteristic feature of doxycycline was its activity only in the infection induced by low doses of the biological agent. The optimal course of the antibiotic therapy in inhalation tularemia was 6 to 8 days. The interval of 1 to 3 days between the infection and the therapy start had no significant effect on the protective efficacies of the antibiotics.

[Preliminary report of an epidemic tularemia outbreak in Valladolid].

We make here a preliminary report of the first 65 cases of tularemia diagnosed at our hospital pertaining to an epidemic outbreak occurred in the area served by the hospital. Diagnosis was made by the presence of a consistent clinical picture and the following laboratory criteria: antibody titre to. Francisella tularensis (tube agglutination) higher than or equal to 1/160, seroconversion or recovery of the microorganism from biological material. The mean age of our patients was 53.8 +/- 13.5 years and female sex predominated (45 women). Fever, lymph node enlargement, cutaneous ulcers, asthenia, and weight loss were the most common symptoms. The presentation clinical forms included: ulceroganglionar (69%), typhoidal (14%), ganglionar (12.5%), pneumonic (1.5%), oculoganglionar (1.5%) and atypical (1.5%). The analytical data did not show significant changes with the exception of erythrocyte sedimentation rate. Streptomycin was the antibiotic of choice, followed by ciprofloxacin and gentamicin. The failure rate was 22%, and ciprofloxacin was used for retreatment in twelve occasions and ofloxacin in 1 occasion with a good response. Tularemia is an infective disease which can become endemic in Spain and that should be considered by clinicians, particularly when unilateral enlarged lymph nodes, prolonged febrile syndromes, pharyngitis with negative culture, poor response to beta-lactams and atypical pneumonia are present.
