Coming of age in a heterosexist world: the development of gay and lesbian adolescents.

The general developmental struggles of gay and lesbian adolescents are described as delineated in recent research. Three developmental areas were selected as a focus: the consolidation of sexual identity and the effects of both parental and peer relationships on gay adolescents' development. Weaknesses are noted in current research and theory, and suggestions are offered which could facilitate the development of both homosexual and heterosexual youth.

Child sexual abuse and forensic psychiatry: evolving and controversial issues.

Child sexual abuse has received growing attention in recent years, and the topic continues to spark controversy among mental health and legal professionals as well as in the popular media. This paper will review the concept of child sexual abuse, cover relevant definitions, address the clinician's role, and then will address the principal evolving and controversial areas. These areas include psychic damages, false allegations, improper investigatory techniques, use of anatomical dolls, admissibility of expert testimony, hearsay testimony, and the competency of minors to testify.

Some changes observed in a clinical case.

The author presents clinical material from a 36-year-old female patient with depressive and hysterical traits, whose personal history shows her psychosexual development to be at a pregenital level. It is intended to demonstrate the type of clinical interpretations that enabled a change towards an oedipal transference in order to reach a psychoanalytic approach. Some of the psychoanalytic interpretations during the treatment process are described, related to several behavioural changes and a discrete structural change.

Freud's Jocasta and Sophocles' Jocasta: clinical implications of the difference.

Evidence is presented that the Jocasta Sophocles depicted in Oedipus Rex is significantly different from the Jocasta of Freud's synopsis of the play. Sophocles' Jocasta prefers her son to her husband and this preference results in parricide and incest. The clinical implications of this difference are discussed in relation to a group of patients whose mothers resembled Sophocles' Jocasta. The analysis of one such patient is briefly presented. All these patients had difficulties at work and in their psychosexual development. It is argued that these difficulties resulted from the mother's attitude interfering with the dissolution of the Oedipus complex and that quite severe pathology does not necessarily mean pre-oedipal conflicts.

[Clinico-pathogenetic characteristics of exhibitionism].

The author relates the results of ++clinico-psychopathological and sexological studies carried out in 61 men with the signs of exhibitionism. With the nosological qualification of the mental status of exhibitionists being diverse, the four uniform ++clinico-pathogenetic groups were distinguished: with the hypothalamic and temporal syndromes, ++non-processual autism syndromes, and schizophrenic defect. The involvement of biological factors in the genesis of pathological inclination is demonstrated to be of uncertain degree, depending on the classifying with the groups indicated. Heterogeneity dysontogenesis of the psychosexual development was discovered with the aid of sexological examination. At the same time the author proves the common character of the pathogenetic components of exhibitionism, lying in the unity of the constitutional basis for their individual characteristics.

Adult sequelae of child sexual abuse.

At present there is no consensus of opinion about the short-term or long-term effects of child sexual abuse, although there is a growing body of literature on the subject. Definitions of what constitutes child sexual abuse vary enormously and, even where agreed, include a wide range of behaviours. Most clinical reports are retrospective in nature and based on self-report. Many authors agree that effects can be classified into four general areas, however, emotional/psychological; sexual adjustment; interpersonal relationships; and social functioning. There are considerable implications for treatment of those who have been abused and for the protection of their children.

[Biological and experimental markers of sexual aggression].

Forty men who committed sexual offences with a varying extent of aggressiveness were examined by a series of psychological tests, phalloplethysmography and electroencephalography. The plasma testosterone level was assessed and in indicated cases the karyotype. Frequently in the delinquents an abnormal course of psychosexual development was revealed, disorders of sex life, the presence of sexual deviations and atypical EEG findings. The extent of aggression of the offence correlated significantly with younger age of the delinquent and the result of the phalloplethysomographic examination. Prediction of future aggressive behaviour of a subject is difficult even when modern examination methods are used.

[Psychology of youth in adolescence].

Adolescence is a phase of human development characterized by major psychophysiological transformations and a high vulnerability and/or potential for disturbance. It can be divided into 5 subphases. For the assessment of the psychopathology and the resources it is important not only to evaluate the individual structure and psychodynamics but also to include the interpersonal relationships (family, peer group) in the diagnostic process.

[The non-scrotal testis].

In this article the classification of the nonscrotal testis is elucidated with a description of all forms of incomplete testicular descent. A great deal of attention is paid to the process of reaching the proper diagnosis, outlining the procedure for physical examination. The risk of infertility, the rather rare occurrence of testicular tumor, and a disturbance of the psychosexual development, are reasons why the undescended testis should be treated. How the undescended testis should be treated, hormonal or surgical, depends entirely on the position of the testis. LHRH nasal spray may induce descent in testes that have emerged from the inguinal canal and can be manipulated to at least the scrotal entrance. Surgical treatment is indicated for all undescended testes that have not emerged from the inguinal canal as well as all ectopic testes, and for those testes that failed to descend with hormonal therapy. The treatment protocol for all types of nonscrotal testes is discussed and schematically presented.

[The 1st lipstick or the fear of femininity in the parents of adolescents].

Adolescence of their daughter disturbs parent's psychical organization. For the father, more or less deep anal regression results from feminine and passive fantasies ("If she does not come back, I'll kill her"). Mother, in interaction with her husband's anxiety is getting depressed ("If she does not come back, I'll kill myself").

[Sexual disorders in women with slowly progressing schizophrenia].

The article deals with sexual disorders in the form of lack of libido, decreased libido, anorgasmy, decreased orgasmic response, loss of the emotional element of libido, predominance of the rational element of sexual libido, manifestations of pseudovaginismus and unmotivated revulsion toward sexual intercourse in women (aged 20-47 years) with slowly progressive schizophrenia and a 2-15 year history of the endogenic process. Definite correlations between psychopathological and sexual pathological manifestations have been ascertained. Sexual disorders were of a secondary systemic nature and were undetected if the endogenic process began after the completion of all stages of normal psychosexual development.

Israeli adolescents and military service: encounters.

Problems of mental health during combat activities are well documented in Israel (Dasberg, 1976). Much less attention has been paid to the problems of the young soldier who enters army life in Israel. This paper delineates some areas of friction and vulnerability between the worlds of the youth and the military. These encounters are systematized into groups, creating the "Binary Model." This model, based on the authors' experience as clinicians in the I.D.F. Central Mental Health Clinic, helps in locating and treating young soldiers in crisis situations.

[The pedophilic criminal].

After a review of the literature dealing with pedophilia, the results of an analysis of 100 forensic psychiatric reports dealing with pedophile criminals are described. They show that, except for a few homosexual pedophiles, pedophilia is a pseudoperversion originating from different developmental conditions and, in individual cases, verifiable personality traits. The authors discuss problems involved in the forensic-psychiatric assessment of these delinquents. Attention is drawn to the necessity of purposeful, coordinated further education in this respect to enable the existing considerable discrepancies between forensic-psychiatric evaluation of these and other sexual deviants to be overcome.
