Intracellular proliferation of Legionella pneumophila in Hartmannella vermiformis in aquatic biofilms grown on plasticized polyvinyl chloride.

The need for protozoa for the proliferation of Legionella pneumophila in aquatic habitats is still not fully understood and is even questioned by some investigators. This study shows the in vivo growth of L. pneumophila in protozoa in aquatic biofilms developing at high concentrations on plasticized polyvinyl chloride in a batch system with autoclaved tap water. The inoculum, a mixed microbial community including indigenous L. pneumophila originating from a tap water system, was added in an unfiltered as well as filtered (cellulose nitrate, 3.0-microm pore size) state. Both the attached and suspended biomasses were examined for their total amounts of ATP, for culturable L. pneumophila, and for their concentrations of protozoa. L. pneumophila grew to high numbers (6.3 log CFU/cm2) only in flasks with an unfiltered inoculum. Filtration obviously removed the growth-supporting factor, but it did not affect biofilm formation, as determined by measuring ATP. Cultivation, direct counting, and 18S ribosomal DNA-targeted PCR with subsequent sequencing revealed the presence of Hartmannella vermiformis in all flasks in which L. pneumophila multiplied and also when cycloheximide had been added. Fluorescent in situ hybridization clearly demonstrated the intracellular growth of L. pneumophila in trophozoites of H. vermiformis, with 25.9% +/- 10.5% of the trophozoites containing L. pneumophila on day 10 and >90% containing L. pneumophila on day 14. Calculations confirmed that intracellular growth was most likely the only way for L. pneumophila to proliferate within the biofilm. Higher biofilm concentrations, measured as amounts of ATP, gave higher L. pneumophila concentrations, and therefore the growth of L. pneumophila within engineered water systems can be limited by controlling biofilm formation.

Ethylene chlorohydrin determination in ethylene oxide sterilized polyvinyl chloride tubing.

A convenient rapid gas-chromatographic method is described for the quantitative determination of ethylene chlorohydrin. The method reported herein extracts the ethylene chlorohydrin with water. The method is simple and offers advantages since no elaborate and expensive gas extraction apparatus is required. The ethylene chlorohydrin levels in five different sterilized polyvinyl chloride samples were found to vary between 5 and 25 ppm. The effect of resterilization with ethylene oxide was investigated and was found to produce less ethylene chlorohydrin.

Testicular cancer and occupational exposures with a focus on xenoestrogens in polyvinyl chloride plastics.

In a case-control study of 148 cases of testicular cancer and 314 healthy controls information was collected on lifetime working histories and specific exposures. The main finding was a six-fold increase in the risk for seminoma, one type of testicular cancer, among plastic workers exposed to polyvinyl chloride (PVC). No overrisks were observed for other plastics than PVC. There may be an aetiological link to xenoestrogens, i.e. chemicals in the environment with oestrogenic properties. Phthalates are used in PVC as plasticizer and have oestrogenic properties that could promote the growth of endocrine sensitive tumour cells. However, this "endocrine disruptors" hypothesis must be further evaluated in experimental models with regard to the causative mechanisms of the oestrogenic tumour promotion.

Advantages of a new polyvinyl chloride double-lumen tube in thoracic surgery.

Double-lumen endobronchial tubes offer many advantages during thoracic operations. However, technical problems with tube placement and potentially life-threatening complications have discouraged widespread use of standard double-lumen tubes. Some of these problems may be reduced with a new polyvinyl chloride (PVC) double-lumen tube. A total of 214 intubations were undertaken in 204 patients using one of three endobronchial tubes. The cases of these patients were reviewed to determine differences in the complications associated with the Carlens, Robertshaw, and PVC tubes. Complications included unsuccessful or difficult intubation, tube dislodgment, unsatisfactory lung deflation, tube malposition, and hypoxemia. In 8 of 16 intubations with the Carlens tube and in 14 of 62 intubations with the Robertshaw tube, there were complications. In all, 22 of 78 intubations (28%) using conventional double-lumen tubes were complicated compared with 5 of 136 (4%) using the PVC tube. The technical problems and risks of endobronchial intubation were reduced significantly with the PVC double-lumen tube.

Allergic contact dermatitis from a pyridine derivative in polyvinyl chloride leather.

Antimicrobial coating of household products has gained wide acceptance in Japan in the past several years. Pyridine derivatives, used as antifungal or antibacterial agents in many common products, are known to cause contact dermatitis. We present a case of severe contact dermatitis caused by a pyridine derivative used as an antifungal agent in the polyvinyl chloride (PVC) leather of a chair. An open patch test was performed with each ingredient of the PVC leather. Other products were previously eliminated from consideration based on a series of negative patch tests. The PVC leather obtained from the patient's chair gave a ++ reaction with evident blistering, according to the International Contact Dermatitis Research Group standard. Fifteen ingredients of the PVC leather were open patch tested; a positive reaction was found with 2,3,5,6-tetrachloro-4 (methylsulphonyl) pyridine (1% in petrolatum). Clinicians should be aware that antifungal or antibacterial agents may be increasingly incorporated into common household products and should be suspected in cases of contact dermatitis.

[Material labeling of soft plastic toys and plasticizers in polyvinyl chloride products].

We investigated the labeling and content of plasticizers of soft plastic toys. First, we investigated material labeling in 96 products purchased in fiscal years 2000 and 2001. Among these products, 43% of those purchased in fiscal 2000 and 68% of those purchased in fiscal 2001 were labeled. We then investigated the kinds and amounts of plasticizers in 73 soft polyvinyl chloride (PVC) toys. Three kinds of phthalates and six other kinds of plasticizers were detected in the soft PVC toys. Diisononyl phthalate, di(2-ethylhexyl) phthalate, diisononyl adipate, and O-acetyl tributyl citrate were detected at high frequency, and in large amounts. The average total content of all plasticizers was 280 mg/g for the products purchased in fiscal 2000 and 227 mg/g for those in fiscal 2001. In fiscal 2001, antioxidants, 4-nonylphenol, and bisphenol A were investigated in addition to plasticizers.

Exposures to polyvinyl chloride, methyl ketone and other chemicals. The pulmonary and non-pulmonary effect.

As part of the continuing assessment of the health impact of exposures in the emerging industries of Nigeria, a study was conducted to determine the relative impact of exposures encountered in four operations of a shoe factory. The health impact assessment consisted of spirometric lung function evaluations and environmental measurement for polyvinyl chloride (1.6 +/- 5 ppm). The study showed that there were differences among exposure subgroups with respect to pulmonary, neurological and dermal toxicities and that these differences were dictated by the types of exposure encountered. Pulmonary toxicity was most severe in the vinyl chloride-exposed subgroup. Neurological impact was most severe in the leather and methylethyl ketone-exposed subgroup and dermal toxicity most severe in the subgroup exposed to plasticizers and stabilizers. There existed substantial deficits in lung function (forced expiratory volume, forced vital capacity FEV1, FVC) among the subgroups relative to normal, non-industrially exposed Nigerians of similar age and height. The deficits in lung function, particularly in FVC, paralleled the variations in the prevalence of restrictive lung disease, which for the whole study group was 56.5 cases per 1000 person-years. The vinyl chloride-exposed subgroup had the highest prevalence of restrictive lung disease, 92.6 cases per 100 person-years. Step-wise multiple regression suggested that 27% of the deficit in FEV1 was explainable by the number of exposures. The use of person-years as a denominator gives a better estimate of risk than the total number of subjects as it incorporates both the number exposed and the total exposure experience.(ABSTRACT TRUNCATED AT 250 WORDS)

[Exposure of workers to electric and magnetic fields from radiofrequency dielectric heaters to process polyvinyl chloride material].

Exposure of workers to electric and magnetic fields from radiofrequency dielectric heaters (RF heaters) to process polyvinyl chloride material was surveyed. Measurements of electric and magnetic field strengths were made in 10 workers operating 7 RF heaters at 3 plants in Japan. Six of the RF heaters are of the sewing machine type and the other is of the shuttle tray type. In all the RF heaters surveyed the nominal frequency of the radiofrequency generator was 40 MHz, and therefore electric and magnetic field from these are assumed to oscillate at the same frequency. The power output ranged from 0.83 W to 1.8 W for the sewing machine type and was 2.4 W for this shuttle tray type. Measurements were made at about 5 cm from the surface of the hand, eye, chest, waist, knee and foot of each worker. The meter readings were converted to equivalent plane wave power density and corrected for duty cycle (0.073 to 0.27). It was found that all the workers surveyed were exposed to electric and magnetic field strengths greater than the TLV recommended by ACGIH (1 mW/cm2). For the hand, eye, chest, waist, knee and foot of the worker, 95%, 63%, 32%, 47% and 36% and 27% of the measured field strengths exceeded the TLV, respectively. Especially for the sewing machine type RF heaters, electric fields at the hand and eye were extremely strong. Thus, for the hand, 100%, 75% and 38% of the measured electric field strengths exceeded the TLV, ten times the TLV, and one hundred times the TLV, respectively, and for the eye, 88% and 25% exceeded the TLV and ten times the TLV, respectively. No correlation was observed between power output of the RF heaters and measured electric and magnetic field strengths at any anatomical site of the worker.

Occupational exposure to polyvinyl chloride as a risk factor for testicular cancer evaluated in a case-control study.

Occupational exposures were assessed in a case-control study on testicular cancer using self-administered questionnaires. In total, answers were obtained for 148 (91%) cases and 315 (87%) controls. Of the cases, 101 had seminoma and 47 had embryonal testicular cancer. An increased odds ratio (OR) was found for exposure to polyvinyl chloride (PVC) yielding an OR of 6.6 (95% confidence interval, 1.4-32). The risk increased further if cases with self-reported cryptorchidism or orchitis were excluded. Six of the 7 exposed cases had seminoma. Exposure to other types of plastics did not significantly increase the risk of testicular cancer.

The effect of Perasafe and sodium dichloroisocyanurate (NaDCC) against spores of Clostridium difficile and Bacillus atrophaeus on stainless steel and polyvinyl chloride surfaces.

Clostridium difficile is an important cause of nosocomial diarrhoea. The aim of this study was to evaluate the potential for Perasafe, a recently introduced biocide, to contribute to control of C. difficile spores in the patient environment, in comparison with the chlorine-releasing agent sodium dichloroisocyanurate (NaDCC). These agents were evaluated against a water control, in a surface test on stainless steel and polyvinyl chloride (PVC) floor covering, materials commonly found in the hospital environment. The organisms studied were a toxigenic clinical isolate of C. difficile, and Bacillus atrophaeus (formerly B. subtilis var niger). The data indicate that in our in vitro system, Perasafe was significantly more active than NaDCC (1000 ppm available chlorine) against C. difficile spores dried on stainless steel surfaces, and against B. atrophaeus on PVC floor covering material, achieving mean log10 reduction factors in viable counts of 6 and 5.5, respectively, at 10 min exposures. Perasafe appeared to be less lethal in 10 min exposures to C. difficile spores fixed on PVC floor covering material. In general, 1000 ppm chlorine generated from NaDCC showed lower log10 reduction factors in viable counts at 10 min, ranging from 0.7 to 1.5, than Perasafe which ranged from 2.7 to 6.0. The potential efficacy of Perasafe in reducing the density of C. difficile spores in the patient environment in hospitals, nursing homes or other long-stay facilities should be evaluated in field studies.
