[Irradiation planning with computer tomograph and treatment planning equipment (author's transl)].

Using individual body cross-sections for purposes of irradiation (obtained by computed tomography with the Siemens-Somatom) in combination with a system for treatment planning (TPS, Phillips), a marked improvement of irradiation planning is achieved. Thus, an advance in the quality of radiation therapy may be expected. Some inconvenience with this combination of instruments, due to the necessity of enlarging the CT cross-sections up to life-sized cross-sections suitable for the TPS, is unavoidable so far.

Strategic planning: health plan perspective.

The managed care industry is one of the most dynamic industries in the health care business. The development of new products, formation of alliances, changes in legislation and other types of changes are regular occurrences. This kind of dynamic environment makes it more important than ever to use strategic planning to guide management decisions.

User preferences for contraceptive methods in India, Korea, the Philippines, and Turkey. World Health Organization Task Force on Psychosocial Research in Family Planning and Task Force on Service Research in Family Planning.

Research was conducted to determine the patterns of contraceptive choice in four developing countries and to investigate the association between method selection and demographic/socioeconomic variables, future family planning intentions, and users' perceptions of methods and preference for personnel. After listening to a balanced presentation of information on the methods, each woman indicated her choice and was then interviewed to determine her reasons for selecting that method. The pattern of contraceptive selection differed from the previous pattern of contraceptive use in each clinic, and this suggests that the presentation of balanced information combined with the provision of a free choice of methods is a useful approach to the assessment of the demand.

[A practical nursing guide for the planning and evaluation of the care of the patient with domiciliary parenteral nutrition. The planning of patient care with DPN].

We present a proposal for the planning of nursing care for the patient with home parenteral nutrition (HPN). In this proposal the problems, goals, and actions to be taken by the nurse are defined. In this working guide, it is explained what the diverse aspects are that the nurse taking care of a patient with HPN needs to know, as well as the care to be given and the evaluation of the same, in order to insure that the patient with HPN is given a good preparation in his self-care, giving the patient an acceptable degree of independence and quality of life, as well as a lower risk of complications.

[1995 Berlin social structure atlas--a planning instrument for health and social planning within the scope of administrative reform in Berlin].

Social unequality and social differences of a population can be defined as social structure. The objective of this study is the construction of indices with which an analysis of space-related social disparities from their spacious dimensions can be made. Each dimension has to be described with a multitude of space-related social indicators first, and then identified and calculated with a multivariate statistic procedure (Factor Analysis). With the existing analysis basic dates are provided. This information about the social situation of Berlin districts as well as the resulting political plannings shall contribute to the reduction of existing differences, in particular to the reduction of social disadvantages. In general, decisions regarding allocation on the private as well as on the public sector should be supported by the social structure analysis.

Three-dimensional display in planning radiation therapy: a clinical perspective. Photon Treatment Planning Collaborative Working Group.

We describe the experience with 3-D display capabilities of four institutions engaged in a comparative three-dimensional treatment planning study. A high degree of interactivity was found to be desirable, which puts great demands on the hardware. Displays which were found particularly helpful included: display of multiple 2-D sections (transverse, sagittal, coronal and/or oblique) of one or more imaging studies; superimposition of outlines of structures of interest; superimposition of dose distributions displayed as isodose lines or as a color-wash overlay; beam's eye view displays of: structures displayed as contour-stacks or as shaded surfaces, dose distributions on the surface of structures, interactively viewed isodose surface displays, and digitally reconstructed radiographs. Dose-volume histograms provided valuable summarizations of dose data.

Utilization of a commercial prostate brachytherapy planning system for 3D treatment planning of ophthalmic tumors.

The 3D brachytherapy transperineal ultrasound implant (B3DTUI) program by the Multimedia Medical System has been adopted for eye plaque treatment planning. The post op CT digitization option was selected to enter the actual seed coordinates present in the eye plaque. Although the B3DTUI algorithm is designated to calculate a permanent dose values, the described procedure allows determination of the dose distribution as the dose rate values. The dose computation program utilizes updated source quantities recommended by the AAPM TG-43 formalism, such as air kerma strength, dose rate constant, radial dose function, and anisortopy factor function. As a result, more accurate dose distribution in the target is obtained than by using traditional dose computation formalism.

Learning to plan? A critical fiction about the facilitation of professional and practice development plans in primary care.

A shift from continuing medical education towards professional and organisational development policies, coupled with the introduction of accountability frameworks (clinical governance), has generated interest in professional and practice development plans (PPDPs) in general practice. The problems of implementing this change in an independent contractor-based service remain unexplored and the aims of this study were to focus on the facilitator's experience of the issues that hampered or fostered development in general practice. Facilitators of PPDPs were asked to document their experience of supporting 12 practices in an all Wales feasibility study. In order to maintain organisational anonymity while reporting accurate accounts of the obstacles encountered, a method known as critical fiction was employed. This method allowed the authors to write detailed reflective accounts that were then fictionalised. The culture of general practice reflects the development of an independent contractor service that has developed into partnerships that employ some professionals (practice nurses, managers and administrative staff) and collaborate with others in variable arrangements (community nurses, health visitors, midwives and others). Developing organisation-wide systems in so-called 'primary health care teams' is a difficult exercise, given the ethos of autonomous decision-making processes and the lack of experience of 'whole systems' approaches in primary care. The potential for multiprofessional synergy and the evidence that systematic changes lead to sustained health care improvements are well established. But the implementation issues of these concepts have not been addressed. Existing educational policies are based in uniprofessional paradigms and the protected time requirements and funding streams required for PPDPs have not been clarified.

IMRT head and neck treatment planning with a commercially available Monte Carlo based planning system.

The PEREGRINE Monte Carlo dose-calculation system (North American Scientific, Cranberry Township, PA) is the first commercially available Monte Carlo dose-calculation code intended specifically for intensity modulated radiotherapy (IMRT) treatment planning and quality assurance. In order to assess the impact of Monte Carlo based dose calculations for IMRT clinical cases, dose distributions for 11 head and neck patients were evaluated using both PEREGRINE and the CORVUS (North American Scientific, Cranberry Township, PA) finite size pencil beam (FSPB) algorithm with equivalent path-length (EPL) inhomogeneity correction. For the target volumes, PEREGRINE calculations predict, on average, a less than 2% difference in the calculated mean and maximum doses to the gross tumour volume (GTV) and clinical target volume (CTV). An average 16% +/- 4% and 12% +/- 2% reduction in the volume covered by the prescription isodose line was observed for the GTV and CTV, respectively. Overall, no significant differences were noted in the doses to the mandible and spinal cord. For the parotid glands, PEREGRINE predicted a 6% +/- 1% increase in the volume of tissue receiving a dose greater than 25 Gy and an increase of 4% +/- 1% in the mean dose. Similar results were noted for the brainstem where PEREGRINE predicted a 6% +/- 2% increase in the mean dose. The observed differences between the PEREGRINE and CORVUS calculated dose distributions are attributed to secondary electron fluence perturbations, which are not modelled by the EPL correction, issues of organ outlining, particularly in the vicinity of air cavities, and differences in dose reporting (dose to water versus dose to tissue type).

Material requirements planning: a better way to plan material.

MRP systems can benefit hospitals in their management of material. Systems provide the means to schedule surgical procedures, calculate material requirements, release orders, plan future capacity requirements, and release and track work orders. MRP can be a powerful tool if properly implemented. All it takes is individuals dedicated to maintaining the discipline and data integrity required to make MRP successful.
