[Therapeutic effect of helium-neon laser on microcirculation of nasal mucosa in children with acute and chronic maxillary sinusitis as measured by conjunctival biomicroscopy].

Using biomicroscopy of the bulbar conjunctiva, 65 patients, aged 6 to 15 years, with catarrhal and purulent forms of acute and chronic maxillary sinusitis were examined. At the repair stage the patients were exposed to laser therapy. The results give evidence that laser therapy produced a positive effect on microcirculation and reduced the potential of relapses. Low-energy helium-neon laser affected mostly vessel permeability (decrease of perivascular edema) and blood rheology (red blood cell aggregation). In terms of circulation changes, laser therapy was more beneficial in the case of acute maxillary sinusitis. Circulation lesions, that persisted after laser therapy in some patients with chronic maxillary sinusitis, can be attributed to stable morphological changes in the wall architectonics of microvessels of the nasal mucosa.

[Effects of helium-neon laser energy on the tissues of the middle ear in the presence of biological fluids and drug solutions].

By means of biophotometry the coefficients of absorption and penetration depth of helium-neon laser radiation with a wavelength of 632.8 nm were measured. Measurements were made in the inflammatory mucous membrane of the tympanic cavity, mucous membrane of the tympanic-cavity infiltrated with the saline solution, cortical bone of the mastoid, cartilage of the auricle, blood, serum, exudate, 0.02% furacillin, 3% boric acid, 0.9% and 10% NaCl. These observations were used to develop an efficient method of laser therapy of the inflammatory and wound surface of the middle ear biological tissue, which formed the basis of a new procedure of the laser treatment of "closed" cavities of the middle ear in an early postoperative period after debridement or hearing repair surgery of suppurative otitis media which were filled with a transparent antiseptic solution.

[Effects of helium-neon laser on physico-chemical properties of the bile].

The influence of helium-neon laser radiation on bile physico-chemical characteristics in healthy subjects and in patients with the physico-chemical stage of gallstone disease was studied in vitro. This type of laser was found to induce positive therapeutic effects, such as: correction of hydrogen ion concentrations, surface tension and viscosity decrease and prolonged bile nucleation in patients with gallstone disease.

[The healing of chronic stomach ulcers by irradiating them with a helium-neon laser in rats subjected to vagotomy].

Helium-neon laser (HNL) irradiation of chronic gastric ulcers (GU) in rats subjected to vagotomy promotes their healing. This is linked with the vagotomy-induced decrease in the number of parietal and chief cells. Irradiation of the gastric mucous membrane with HNL accelerates the differentiation of the chief and parietal cells without increasing their relative volume in the fundal glands. The number of mucocytes in these glands increases, which facilitates intensification of the protective factors of the gastric mucosa. It was established that in complex use of vagotomy and HNL in chronic GU their positive properties are potentiated.

A randomized double-blind clinical trial on the effectiveness of helium-neon laser in the prevention of pain, swelling and trismus after removal of impacted third molars.

The effect of helium-neon (He-Ne) laser on the prevention of pain, swelling and trismus following the removal of an impacted third molar was studied in 100 patients randomly allocated to receive He-Ne laser, ibuprofen or placebo in a prospective double-blind parallel clinical trial. Trismus was significantly reduced in the He-Ne laser and ibuprofen treatment groups. Pain was significantly less in the ibuprofen group with regard to He-Ne laser and placebo groups. Swelling was the same in the three treatment groups.

[An electron microscopic and radioautographic study of bronchoalveolar lavage in chronic inflammation of the lungs under the effects of helium-neon laser].

136 samples of bronchoalveolar lavage (BAL) from 45 men-patients with chronic inflammatory processes of lung were studied in course of endobronchial laser influence. It was shown, that considerable quantitative and qualitative changes in cell composition of BAL-fluid were resulted from helium-neon laser influence, these are the percentage increase of macrophage amount and the decrease of neutrophil numbers. By use of radioautographic analysis the stimulation of metabolic and proliferative reactions (on the level of RNA and DNA syntheses) of alveolar macrophages under laser influence was found. On the basis of morphological changes of cells in respiratory compartment of the lung it was concluded that clinical effect of laser treatment of chronic inflammatory processes in the lung was conditioned by stimulation of regenerative-plastic reactions.

[The red light of the helium-neon laser reactivates superoxide dismutase].

The effect of low-energy helium-neon laser (HNL) on enzymatic activity, absorbtion spectra and electron paramagnetic resonance (EPR) signals of superoxide dismutase (SOD) from bovine erythrocytes in acid medium were investigated. It was found that incubation during 2 hours at pH 5.9 led to eventually complete inactivation of the enzyme. The subsequent illumination of inactivated SOD by HNL brought about the enzyme reactivation. Both absorption and EPR-spectra were changed after incubation at pH 5.9. These changes may be attributed to protonation of histidine residue in the enzyme active site. After laser irradiation both absorption and EPR spectra were restored to those typical of native enzyme at pH 8.2. In a model system, copper-histidine complex, absorption maximum was shifted from 632-633 nm at pH 5.8 to 639-640 nm at pH 8.5-9.0. The similar long-wave length shift of the maximum was observed after illumination by HNL at pH 5.8. It may be postulated that the photoreactivation of SOD consists essentially in deprotonation of His-61 residue in the enzyme active site and subsequent recovery of imidazole bridge between copper and zinc which had been destroyed at low pH. Since many other enzymes possess similar copper-histidine structures in their active sites, one may expect diverse effects of red (laser) light on the enzyme activity.

[Lipid peroxidation as an indicator of the effectiveness of treating patients with chronic ischemic heart disease using a helium-neon laser].

A therapeutic response to clinical application of Helium-Neon laser irradiation in combination with antianginal drugs was evaluated for chronic sufferers with coronary heart disease (CHD). The largest scope of information on the efficacy of the laser therapy has been provided by the parameters of lipid peroxidation. Individually adjusted doses of the irradiation allowed achievement of the best treatment results.

Chicago sky blue and a helium neon laser abolish endothelium dependent relaxation in vivo in the microcirculation.

Chicago sky blue, also known as Niagara sky blue, is a vital dye that can successfully be used as an intravascular energy absorbing target for the light from a helium-neon (HeNe) laser. The result of this light/dye interaction is endothelium damage which can be controlled by adjusting the duration of the laser exposure and the amount of dye injected intravenously. The endothelial damage probably is the result of the heat generated by the dyes absorption of energy at the interface between plasma and endothelium. The most minimal damage resulted in selective loss of the dilation normally produced by acetylcholine and bradykinin, two endothelium dependent dilators. The dilation produced by sodium nitroprusside, a dilator acting directly on vascular smooth muscle, was preserved. More severe injury (i.e. more prolonged exposure to light and/or more dye, resulted in local platelet aggregation at the site of laser impact.

[Effect of helium-neon laser rays on the processes of postradiation recovery in the skeletal muscles of old rats].

The present experiments were conducted to determine the stimulant effect of helium-neon laser on the postradiation recovery in irradiated uninjured skeletal muscle of rats aged 2-2.5. This was indicated by a restored ability of the muscle for posttraumatic regeneration. The both hind rat legs were exposed to local irradiation of 20 Gy and following laser therapy (8-9 procedures at 3 min each, impulsive or continuous one). Then both musculus gastrocnemius were cut across 30 days after irradiation. It was shown that laser therapy employed before injury of the irradiated muscle accelerated fibrin resorption and improved connective tissue, but slightly stimulated muscular tissue. Impulse laser therapy was more favourable for state of skin and healing of the skin wound after irradiation.

[Clinico-morphological analysis of the results of endobronchial use of helium-neon laser in the treatment of chronic pneumonia in children].

In treatment of children with chronic pneumonia by 3-5 bronchoscopic cleansing procedures with endobronchial irradiation of the mucosa with a helium-neon laser beam the highest effect is produced by a dose of 15 mWt/cm2 and 2 minute exposure. Regression is stabler and occurs more rapidly in patients with moderate deformity of the bronchial tree. The mechanisms of the therapeutic effect of the method is explained by normalization of microcirculation, intensification of epithelial proliferation and biosynthetic activity which create favorable conditions for induction of reparative processes in the bronchial mucosa.

[Comparative volume of membrane microflora and morphology of the epitheliocytes during irradiation of chronic stomach ulcer with helium-neon laser in association with vagotomy].

Effect of complex influence of subdiaphragmal vagotomy (VT) and experimental chronic gastric ulcers (GU) thrice-repeated radiation with helium-neon laser (HNL) on mucosal morphology and relative membrane microflora (MM) volume has been studied with the methods of transmission electron and light microscopy in 77 Wistar rats. Observed MM relative volume increase in GU and VT decreases with GU HNL radiation. This is due to fundal glands epitheliocytes "disregenerative" alterations decrease.

[Effect of helium-neon laser on the microcirculation in blood vessels of the rat pia mater].

The sensitivity of rat's microvessels of microcirculatory bed of brain's pia matter was studied by helium-neon laser radiation. Changes in the reactivity by the microscopy findings were studied on the 7, 30, 45, 60, 90th days of animals postnatal development. Laser radiation was used during 30 min. Local changes in all components of the microcirculatory bed were fixed. It is mentioned that in adult rat the expressiveness of arteriole's local widening reaction in the radiated place reduces, but the degree of the reduction is below or over the radiated place, on the contrary, increases. The arteriole diameter in 30-day old rats increases to 59% in comparison to the initial level, and 90th day old animals--to 16%. The degree of reaction's expressiveness depends upon the microvessel diameter and the animal's age.

[Treatment of trigeminal neuralgia, ganglioneuritis of the pterygopalatine ganglion and other types of prosopalgias by helium-neon laser irradiation of the pterygopalatine ganglion].

The authors describe a method of contact helium-neon laser irradiation of the pterygopalatine node through lateral wall of the nasal cavity. The technique was employed in patients with trigeminal neuralgia, ganglioneuritis of pterygopalatine node, other forms of prosopalgia. Altogether 78 patients were treated. The initial course relieved pain in 56 patients. In 44 patients the remission has been persisting for 2 years.

[Relation between the radioprotective effect of helium-neon laser radiation on bacterial cells and the time period between 2 types of irradiation].

The effect of helium-neon laser radiation (lambda = 633 nm) given to E. coli K-12 cells of various genotypes 4 h following X- or alpha-irradiation was shown to increase the number of viable cells. The irreversible ingradient of a radiation injury to cells remained invariable during the first 60 min after irradiation its values being minimal.

Injury of brain microvessels with a helium-neon laser and Evans blue can elicit local platelet aggregation without endothelial denudation.

A helium-neon laser was used to produce endothelial lesions and local platelet aggregation in brain microvessels (pial arterioles and venules) sensitized by intravascular Evans blue. Electron microscopy showed that the aggregates formed in the absence of endothelial denudation or exposure of basal lamina. Initial lesions consisted only of endothelial lucencies, vacuoles, and distended rough or smooth endoplasmic reticulum. Aggregates consisted not only of degranulating platelets, rounded platelets, and platelets with pseudopods, but also of masses of discoid platelets. Red blood cells were uninjured and did not form the nidus of these collections. The possibility that aggregation occurred as a result of direct injury to the platelet, and independently of the endothelial damage, is discussed and considered unlikely. Rather, the data are consonant with the hypothesis that in the microcirculation, endothelial injury can produce aggregation prior to exposure of basal lamina.

[Effect of helium-neon laser radiation on restoration of the structure of the microcirculatory bed and neurocytes of the small intestine following experimental ischemia].

Influence of helium-neon laser lg-75 rays on the microcirculatory bed and neurocytes of the small intestine after its experimental ischemia has been studied. When a normal small intestine is radiated, dilatation of the luminal diameter is observed in all links of the microcirculatory bed (MCB) and also hypertrophy of neurocytes, when phenomena of distrophic processes are absent. In 30 days after 3-hours' ischemia of the intestinal loop and its successive radiation, spasm of arterial and dilatation of the venous link of MCB is registered; they normalize by the 45th day. In the control, after ischemia (without radiation) in 45 days venous plethora of the vessels in the intermuscular plexus of the intestinal wall is kept. In the nervous elements of the muscular-intestinal plexus at early stages of the experiment against the background of ischemia reactive and distrophic changes appear. By the 30th day after radiation, the volume of neurocytic bodies increases, processes grow out, nuclear-cytoplasmic index increases. Nonspecific character of the laser rays is supposed; their effect is realised via regional microvascular and nervous formations.

[Electron microscopic and radioautographic study of the bronchi in chronic inflammation as affected by helium-neon laser treatment].

Structural-metabolic changes were studied in 98 biopsies of large bronchial mucosa from 39 male patients with chronic suppurative lung diseases. It was shown (by the level of DNA and RNA synthesis) that the proliferative and metabolic processes were induced by endobronchial treatment of the damaged epithelium with helium-neon laser which passing through a number of transitional stage recovers its structure and differentiation to ciliary and goblet cells with normal ultrastructure. In lamina propria hyperemia, intensive leukodiapedesis, leukocyte infiltrates and granulations develop and proliferative and metabolic activity of endotheliocytes and interstitial cells increases, which results in the formation of delicate fibrous connective tissue. Simultaneous reorganization of the epithelium and underlying connective tissue is interpreted in terms of parenchymatous-stromal interactions.
