The health effects of wastewater on the prevalence of ascariasis among the children of the discharge zone of El Jadida, Morocco.

Proximity to wastewater constitutes an infection hazard by ascariasis. This is supported by the results of an epidemiologic study of a group of children living near an area of wastewater effluents. The subjects of this study were schoolchildren from Sidi Daoui, a neighborhood located in the area of the main effluent of the city of El Jadida, Morocco. For a comparative approach, a control group was selected from Sidi Moussa, a neighborhood far from the discharge area. The incidence of ascariasis was 18.1% in the study group (Sidi Daoui) and 1% in the control group (Sidi Moussa). It was found that 71% of the exposed children were suffering from acute parasitic infections. The risk attributable to wastewater in transmitting ascariasis to examined children in Sidi Daoui was about 17%. Boys, particularly those aged 7 - 10, appeared to be the most vulnerable to contracting ascariasis. This study demonstrates the relationship between ascariasis in children and their proximity to wastewater effluents.

Assessment of the health hazards associated with wastewater reuse: transmission of geohelminthic infections (Marrakech, Morocco).

To address the problem of water scarcity, wastewater has long been used for agricultural purposes in Marrakech (Morocco) and its benefits have long been recognized. However, the benefits of urban wastewater reuse may be limited by its potential health hazards associated essentially to the danger of transmission of pathogenic organisms to human beings. The current study aimed to evaluate the risks associated with raw wastewater reuse for agricultural purposes, and the incidence of geohelminthic infections among the children of the wastewater spreading area of Marrakech as compared to a control group. Faecal samples from 610 individuals were examined for Ascaris spp. ova and Trichuris spp. ova (323 children for the exposed group and 287 for the control group). The study duration was two months and a half, and was carried out in the summer period. A questionnaire-interview with children and parents was used to collect data on possible demographic, hygiene and behavioural-contact factors such as gender, age, family size, source of water supply, parental occupation. The investigations resulted in the detection of Ascaris spp. ova in 43 samples indicating a point-prevalence of 13.3% in the exposed group versus 1.7% for the control group. Trichuris spp. ova was detected in 43 faecal samples indicating a point-prevalence of 13.3% in the exposed group, and 3.8% in the control group. The Chi-square test showed that the differences observed between the exposed and control group are statistically significant (p < 0.01). Males are more infected with ascariasis and trichuriasis than females and children of 2-8 years of age are heavily infected as compared to 9 - 14-year-old children. Based upon the findings of this study, it is concluded that wastewater reuse for agricultural purposes presents an increased risk of infection in the exposed group.

Nutrient intake and food consumption among pregnant women from an agricultural region of Morocco.

This study aimed to assess dietary intake and nutrient adequacy in pregnant women selected from an agricultural region of Morocco. On a sample of 172 pregnant women from rural and urban area, data on socio-demographic characteristics and dietary habits by a quantified food frequency were collected using questionnaires. Mean daily intakes of energy and nutrients were estimated and compared to reference nutrient intakes (RNI). The results show that the mean daily intakes of energy (3110.9 kcal in urban; 2707.5 kcal in rural) and some nutrients were adequate. However, iron, folate, zinc and calcium intakes were inadequate for the majority of women and more markedly in rural area. Education level appeared to affect nutrient intakes while no differences were registered across pregnancy stages or according to household size. The study suggests that an education program should be conducted to improve dietary intakes in order to prevent risks for pregnancy outcome in this population.

The pattern of reproductive life in a Berber population of Morocco.

Reproductive patterns were studied from data collected in 1,450 Berber households in the province of Marrakesh, Morocco in 1984. Women aged 45-49 years had a mean of 8.9 pregnancies to achieve 5.7 living children. Social influences on fertility rates show the importance of tradition, particularly through time-dependent variables such as age at marriage, waiting time to first birth, interbirth intervals, and duration of breastfeeding. Birth control does not appear to affect the tempo of fertility; rather, its main use is to bring the reproductive period to a close. The comparison of two subsamples of women separated by a 25-year interval indicates an actual acceleration of the tempo of fertility by the reduction of waiting time to first birth and of interbirth intervals. The supposed ongoing process of demographic transition is not clearly observed in this population.

Are husbands a barrier to women's family planning use? The case of Morocco.

Little is known about men's role in the adoption of family planning. Recent studies suggest that men may be a barrier to women's use of family planning. However, it is not clear whether husbands represent a true or a perceived barrier. Using the 1992 Morocco Demographic and Health Survey data, this study examines (1) whether women and men report concordant fertility desires, discussions, and contraceptive use; (2) the accuracy of women's perceptions of their husbands' fertility desires; and (3) whether husbands are a barrier to women's family planning use. The results demonstrate that, controlling for women's own fertility desires, husbands' true fertility desires are associated with family planning use. Likewise, women who perceive their husbands to want fewer children than they want are more likely to use family planning. Future fertility and family planning programs need to include men to reduce their role as both perceived and true barriers to family planning use.

Evaluation of annual committed effective doses to members of the public in Morocco due to 238U and 232Th in various food materials.

Uranium (238U) and thorium (232Th) concentrations were measured in different foods widely consumed in Morocco by using CR-39 and LR-115 type II solid state nuclear track detectors (SSNTDs). Annual committed effective doses due to 238U and 232Th intakes from the ingestion of the studied food materials were evaluated for different age groups of individuals, using the ICRP ingestion dose coefficients. The influence of the 238U and 232Th intakes and ages of individuals on the committed effective dose was investigated. Total annual intakes of 238U and 232Th for a typical food basket for adult members of the Moroccan population were estimated to be 451 +/- 27 Bq y(-1) and 359 +/- 20 Bq y(-1), corresponding to committed effective doses of (20 +/- 1) x 10(-6) Sv y(-1) and (83 +/- 5) x 10(-6) Sv y(-1), respectively.

Prevalence and characteristics of nim genes encoding 5-nitroimidazole resistance among Bacteroides strains isolated in Morocco.

We report here an evaluation of the dissemination of nim genes, encoding 5-nitroimidazoles resistance, among Bacteroides clinical strains isolated in Morocco. This study was done using a PCR method. Among 60 strains studied, nine contain a copy of a nim gene. The sequence determination of these genes showed that they are homologous to three nim genes previously characterized in strains isolated in France: nimB (five genes), nimC (three genes), and nimA (one gene). Although the nimA and nimC genes were previously identified on plasmids pIP417 and pIP419, respectively, we found here that they have a chromosomal location. The MICs of three 5-nitroimidazole antibiotics (metronidazole, ornidazole, and tinidazole) of the nim gene-containing strains were very low (0.5-2 microg/ml), indicating that the nim genes were not efficiently expressed in these clinical isolates.

Morphological and isoenzymatic differentiation of sympatric populations of Phlebotomus perniciosus and Phlebotomus longicuspis (Diptera: Psychodidae) in northern Morocco.

The phlebotomine sandfly Phlebotomus perniciosus Newstead is a vector of Leishmania infantum Nicolle in the western Mediterranean basin. In northern Morocco it is often caught in sympatry with a closely related species, P. longicuspis Nitzulescu. In the locality of Chefchaouene where females of both species were morphologically distinguishable, none of the collected males exhibited forked copulatory valves, which is considered a specific character for P. perniciosus. They 1st were identified as P. longicuspis. Isoelectrofocusing of 7 enzyme systems was used to compare these specimens with the sympatric females. Two groups of males were separated and assigned to each species by their isoenzyme patterns associated with morphological differences in the shape of the aedeagus and in the number of coxite hairs. Based on our results, a reexamination of the distribution of both species is necessary especially in many locations where P. longicuspis has been identified only from males.

Entomological investigations in Chichaoua: an emerging epidemic focus of cutaneous leishmaniasis in Morocco.

Cutaneous leishmaniasis due to Leishmania tropica Wright seems to be an emerging disease in Chichaoua, a province located in southwestern Morocco. In this study, sand flies (Diptera: Psychodidae) were collected from 12 stations. Sticky traps were placed in domestic, peridomestic, and sylvatic sites. In total, 3,787 specimens consisting of 10 species (seven Phlebotomus and three Sergentomiya) were identified. Phlebotomus perniciosus Newstead, the predominant species, was abundant, especially in mountainous areas. Phlebotomus sergenti Parrot (12%) was found in all studied villages where it was associated with domestic and peridomestic habitats. On the basis of its abundance, distribution, and notable anthropophily, P. sergenti, a proven vector of L. tropica elsewhere, is considered the cutaneous leishmaniasis vector in this emerging focus.

Distribution of phlebotomine sandflies, of the subgenus Larroussius, in Morocco.

Adult sandflies (Diptera: Psychodidae) of the genus Phlebotomus and of the subgenus Larroussius were collected with sticky traps from thirty-four localities in central and south-western Morocco. Four Larroussius species were present in this area, Phlebotomus perniciosus Newstead, being the most widespread and abundant species particularly in the mountainous area, was sympatric with the closely related species Phlebotomus longicuspis Nitzulescu. Only one male of P. perniciosus displayed the bifid copulatory valves characteristic of the species. All specimens showed copulatory valves curved at their apex. The number of coxite hairs is a taxonomical tool that differentiates this morph of P. perniciosus from P. longicuspis. Phlebotomus ariasi Tonnoir was collected from six mountainous stations. Phlebotomus mariae Rioux et al. was scarce; only five males were collected and were often associated with P. ariasi.

Prevalence of Theileria in the tick Hyalomma detritum detritum in the Doukkala region, Morocco.

The overall prevalence of Theileria species, mainly, if not exclusively, T.annulata, in 901 Hyalomma detritum detritum collected from cattle in the Doukkala region of Morocco over a period of 2 years was 21.5%. The quantity of infection (number of sporoblasts per infected tick) followed the negative binomial distribution with between one and 250 sporoblasts per infected tick. Infected ticks were found in eight of fourteen areas examined whilst T.annulata was present in all fourteen. There were significant differences in both the prevalence and the quantity of infection between ticks collected from different farms, and between nymphs collected in the autumn from these farms, and moulted in the laboratory, and adults collected in the following summer. The prevalence, but not the quantity, of infection was higher in female than in male ticks. No correlations were established between infection of engorged nymphs and the breed, sex and Theileria piroplasm parasitaemia of the host animal. However, calves infected a greater proportion of nymphs than adult cattle and the heavier the infestation of nymphs on an animal, up to a plateau, the higher the prevalence of infection in those nymphs. There were no differences in infection between ticks moulted at 24 degrees C and 37 degrees C, after the engorged nymphs had been stored at 18 degrees C to simulate over-wintering.

Chloroplast DNA phylogeography of the argan tree of Morocco.

Polymorphisms in the chloroplast genome of the argan tree (Sapotaceae), an endemic species of south-western Morocco, have been detected by restriction site studies of PCR-amplified fragments. A total of 12 chloroplast DNA (cpDNA) and two mitochondrial DNA (mtDNA) fragments were amplified and digested with a single restriction enzyme (HinfI). Polymorphisms were identified in six of the cpDNA fragments, whereas no mtDNA polymorphisms were detected in a survey of 95 individuals from 19 populations encompassing most of the natural range of the species. The cpDNA polymorphisms allowed the identification of 11 haplotypes. Two lineages, one in the south-east and the other in the north-west, divide the range of the argan tree into two distinct areas. The level of genetic differentiation measured at the haplotype level (GSTc = 0.60) (i.e. with unordered haplotypes) was smaller than when phylogenetic relationships were taken into account (NSTc = 0.71-0.74) (ordered haplotypes), indicating that population history must be considered in the study of the geographical distribution of cpDNA lineages in this species. If contrasted with the level of nuclear genetic differentiation measured in a previous study with isozymes (GSTn = 0.25), the results indicate a relatively high level of gene flow by seeds, or conversely a relatively low level of gene flow by pollen, as compared with other tree species. Goats and camels could have played an important role in disseminating the fruits of this tree.

Phenotypic variation in Galerida larks in Morocco: the role of history and natural selection.

The relative influences of history, natural selection and hybridization in shaping phenotypic variation in closely related taxa is a crucial issue in current evolutionary biology. In this study, we used as a model two sibling but paradoxically highly variable species of larks (Galerida theklae and Galerida cristata) of Morocco to separate the impacts of these evolutionary forces. In the former species, variation is manifested mainly in colouration, while in the latter, variation also encompasses bill size and shape. Mitochondrial and nuclear DNA sequencing were used to identify the historical relationships among the subspecies and species. According to our analyses, G. cristata and G. theklae diverged about 3.7 million years ago (Ma), and we found no evidence for a role of hybridization in maintaining their similarity. In G. theklae, there was no further subdivision, while in G. cristata two major mtDNA groups were identified (divergence approximately 1.1 Ma). These two lineages are parapatric and regroup, respectively, the three short-billed subspecies [G. (cristata) cristata] and the two long-billed subspecies [G. (cristata) randonii]. Patterns of morphological variation were then contrasted to this pattern of neutral relationships: we found that G. (c.) cristata was morphologically more similar to G. theklae than to G. (c.) randonii. Overall, these results point towards the prominent role of (i) natural selection and/or phenotypic plasticity in adapting the plumage to local conditions and (ii) natural selection in combination with historical isolation in driving the divergence in size and bill morphology in the crested larks.

Poisoning of sheep by Astragalus lusitanicus in Morocco: field and experimental studies.

This study demonstrated that Astragalus lusitanicus found in Morocco is toxic to sheep. The clinical syndrome shares some features with locoism. Poisoning may be either acute or chronic, depending on the total amount of the plant that has been ingested. Neurological disturbance was the main clinical manifestation of the syndrome. The signs persisted in poisoned animals after cessation of plant consumption. This suggests the occurrence of a persistent biochemical defect in the neuron. The relationship between vacuolation in neurons and the onset of the signs needs to be clarified. Future biochemical and electron microscopic studies should advance the knowledge of the pathogenesis of this toxic neuronal degeneration.

Ovine lentiviral infection (maedi/visna) in Morocco: a serologic and postmortem survey.

In an attempt to estimate the importance of maedi/visna virus infection in Moroccan sheep and to assess the relationship, if any, between seroreactivity and pulmonary lesions, an abattoir-based serologic survey was undertaken over a period of 2 years. A total of 1000 blood samples and related lungs were collected from sheep of different breeds and ages slaughtered at three major municipal abattoirs in Morocco (Casablanca, Kénitra and Rabat). The agar gel immunodiffusion test showed that the seropositivity for all samples was 24.8%. At post-mortem examination, 107 out of the 248 (43.1%) sheep scored seropositive had gross lesions of maedi; typical microscopic lesions were observed in 125 (50.4%) seropositive sheep, including the 107 with gross lesions. Of the 752 seronegative animals, only 13 (1.7%) sheep had macroscopic maedi-like lesions. Seventeen (2.2%) of these seronegative cases, including those with gross lesions, had histologic pulmonary changes consistent with maedi. No other maedi/visna-associated lesions were found during this study; however, coexisting secondary bacterial bronchopneumonia was frequently encountered. The association between serologic and pathologic findings is reported and discussed.

First report of paratuberculosis (Johne's disease) in sheep in Morocco.

Paratuberculosis (Johne's disease) was suspected in a flock of approximately 1000 sheep after weight losses and scouring had increased in adult animals despite repeated anthelmintic treatment. A total of 10 ewes showing clinical symptoms were submitted to laboratory examination. Gross pathological and histological examination of tissue samples rendered results compatible with Johne's disease in all 10 ewes. The diagnosis could only be confirmed by cultural isolation and identification of Mycobacterium paratuberculosis in the faeces of two ewes. This is considered the first documented isolation of Mycobacterium paratuberculosis in sheep in Morocco.

Periodontal treatment needs in populations of high- and low-fluoride areas of Morocco.

The purpose of this study was to determine the periodontal status and treatment needs, using the CPITN index, in a population aged 7 to 60 years residing in the fluorosis area of Khouribga and the non-fluorosis area of Beni-Mellal, Morocco. A total of 2378 subjects was studied. In the youngest age groups, more subjects and sextants from Khouribga than from Beni-Mellal were free of periodontal diseases. Conversely, in Beni-Mellal, subjects in the youngest age groups presented a higher mean number of sextants with calculus than those from Khouribga. In combining the 2 areas, 1.3% of the 2319 dentate subjects examined needed no treatment and 98.7% needed at least oral hygiene instructions (TN1). The present study, based on the CPITN index, demonstrated in both areas a high prevalence of gingivitis, and a relatively low prevalence of deep pocketing, occurring on the average in less than half a sextant in the total populations.

The impact of the family planning supply environment on contraceptive intentions and use in Morocco.

Although the extent to which organized family planning programs influence reproductive preferences remains a subject of debate, most observers would grant that such programs play a key role in helping individuals to realize their contraceptive and reproductive intentions. However, few prior studies have quantified the magnitude of this facilitating or enabling effect of family planning services, given existing demand for contraception. This study takes advantage of panel survey data and linked information on the supply environment for family planning services in Morocco in order to bridge this research gap. In the analysis, contraceptive use during the 1992-95 period is related to contraceptive intentions in 1992; individual-, household-, and community-level determinants of contraceptive behavior; and family planning supply factors. Estimation procedures are used that control for unobserved joint determinants of contraceptive intentions and use. Evidence of a significant enabling or facilitating role of family planning services is found, and the results also suggest that family planning program factors influence contraceptive intentions in important ways.
