[A case of lightning resulting in ear burn].

The authors described a rare case of 13 years old girl burnt by lightning. The burns of the skin of neck, perforation and destruction of the handle of malleus were stated. No internal ear injury was observed.

Lightning injury of the tympanic membrane.

Lightning injury to the ear is known, but specific reports are lacking. Four patients with tympanic membrane perforations who were managed surgically are reported. Their presentations, evaluations, intraoperative findings, and outcomes are discussed as they relate to the proposed pathogenic mechanisms. The authors' standard wide exposure tympanoplasty approach with two layer tympanic membrane repair is described. The added steps in performing this procedure may be necessary to ensure a good result in this unique group of patients.

Thunder and Lightning and Rain: a Latino/Hispanic diabetes media awareness campaign.

The prevalence rates of diabetes in communities of color are higher than in Caucasian populations. Social marketing can be an effective approach to educating communities and encouraging visits to health care providers. This article describes Thunder and Lightning and Rain, a diabetes media awareness campaign implemented in a 5-county area in central Washington State with a large Latino/Hispanic population. The Washington State Department of Health's Diabetes Prevention and Control Program, along with national and community partners and focus groups, used a social marketing model to reach those with uncontrolled diabetes. A telephone survey-based evaluation, conducted in Spanish, provided data on demographics, media access, calls to a toll-free information line, provider visits, and recall of the campaign's central message: "Control your diabetes. For Life."

Ball lightning as a force-free magnetic knot

The stability of fireballs in a recent model of ball lightning is studied. It is shown that the balls shine while relaxing in an almost quiescent expansion, and that three effects contribute to their stability: (i) the formation in each one during a process of Taylor relaxation of a force-free magnetic field, a concept introduced in 1954 in order to explain the existence of large magnetic fields and currents in stable configurations of astrophysical plasmas; (ii) the so called Alfven conditions in magnetohydrodynamics; and (iii) the approximate conservation of the helicity integral. The force-free fields that appear are termed "knots" because their magnetic lines are closed and linked.

Tabetic lightning pains: high-dosage intravenous penicillin versus carbamazepine therapy.

The relief of tabetic lightning pains (LP) by carbamazepine (CBZ) was questioned as in some instances penicillin was given simultaneously. Only 1 out of 6 cases became pain-free on high-dosage intravenous penicillin, while CBZ rapidly abolished LP in those unresponsive to penicillin. Gastric crises were similarly abated by CBZ in 1 case under a placebo-controlled study. Inflammatory and degenerative changes in the posterior nerve root and ganglia may be the underlying pathologic basis that explains these two different responses to penicillin.

Lightning streaks of Moore: a cause of recurrent stereotypic visual disturbance.

Lightning streaks of Moore are recurrent stereotypic vertical visual phenomena that must be distinguished from similar symptoms due to retinal ischemia, optic disk abnormalities, or mechanical retinal or optic nerve stimulation. These brief flashes are oriented vertically and are perceived in the temporal visual field after eye movement. Senescent posterior vitreous changes induce collapse and detachment with persistent vitreoretinal adhesions through which mechanical forces exert traction on the macula and retina, inducing photopsias. Clinical recognition of these symptoms avoids unnecessary diagnostic interventions.

[Microwave-assisted extraction technology of skin-lightning agent in Rhizoma Typhonii].

The microwave-assisted extraction (MAE) technology of skin-lightning agent in Rhizoma Typhonii was studied and the relative inhibitory activity of the extract against tyrosinase was used as evaluating criterion. It was concluded that the optimal condition was two doses of microwave radiation (450 W) for 7 min with a mass ratio of water to material of 10: 1. The dry extract of Rhizoma Typhonii with 20.9% yield and 47.1% relative inhibitory activity against tyrosinase was obtained. Compared with direct-heating extraction, MAE can shorten extracting duration, and improve relative inhibitory activity of the extract against tyrosinase.

Recurrent Guillain-Barré syndrome: lightning does strike twice.

Recurrent Guillain-Barré syndrome following a long asymptomatic interval is very rare. Forty-nine cases are reviewed from the literature, their common features are noted and conclusions are drawn. Thus it can be seen that, with Guillain-Barré syndrome, lightning can indeed strike twice.
