The intracerebral 9L rat brain tumor was resected from 38 rats with the use of standard microsurgical techniques, the carbon dioxide laser, or the argon laser. The extent of resection was evaluated by histological examination, and the effect on survival of the three methods was compared. All three methods achieved histologically verifiable gross total resection, and tumor recurrence after resection by all three methods occurred at the same rate.

A 68-year-old male was hospitalized because of headache, nausea, and disturbance of consciousness. Neurological examination on admission disclosed somnolence, disorientation, marked neck stiffness, papilledema, and quadriparesis. Computed tomography (CT) scanning demonstrated a round mass with marked contrast enhancement in the right sylvian fissure and small contrast-enhanced masses in the interpeduncular, quadrigeminal and ambient cisterns. CT also showed marked peritumoral edema, a midline shift, and hydrocephalus. The patient's consciousness level and respiration deteriorated 3 days after admission and a craniotomy was performed. The tumor, which was well demarcated, firmly attached to the sphenoidal ridge, and grossly appeared to be a meningioma, was totally removed. Histologically, the tumor had two well defined components, glioblastoma and fibrosarcoma. The patient underwent ventriculoperitoneal shunting, chemotherapy, and radiotherapy after surgery, but the primary tumor soon recurred, with scalp metastasis, and he died 5 months postoperatively. Autopsy revealed metastases to the liver, spleen, and spinal cord. The histogenesis of this mixed tumor and the mechanism of extracranial metastasis are discussed, and the literature is reviewed.
The authors have investigated various factors involved in the photoradiation treatment of 9L glioma cells. The cells were grown in tissue culture and exposed to light from a laser source that allowed accurate quantitation of the light energy. Cell death was determined following treatment using the trypan blue exclusion test. It was shown that the treatment is very wavelength-dependent following the absorption spectrum of hematoporphyrin derivative (HPD). The absorption peaks in the lower part of the spectrum are more efficient than those of higher wavelengths. Photoradiation therapy is more effective the higher the concentration of HPD. Intensity of light is a very important factor in calculating the total dose of light necessary for this treatment.

UNLABELLED: A 60-year-old woman was admitted to our clinic on September 29, 1983 because of a five months history of personality change and progressive left sided motor weakness. Neurological examination on admission revealed left hemiparesis and papilledema. She was severely demented. Skull X-rays were normal. CT scan showed a large right parietoccipital mass which was markedly enhanced by contrast medium. Right internal carotid angiogram showed abnormal vascular shadow, early draining veins and tumor stain in parietoccipital region. Right external carotid angiogram showed that the tumor was partly fed by the middle meningeal artery in "sunburst" pattern. The tumor was resected subtotally through right parietoccipital craniotomy on October 4, 1983. At operation, well demarcated, reddish-grey tumor attached to the dura was found. Several branches of the middle meningeal artery were penetrated into the tumor through the attachment. The tumor grossly appeared to be a meningioma in its superficial part, but demarcation was found being obscured in removing the deeper part of the tumor. PATHOLOGICAL FINDINGS: The specimens obtained from the superficial part of the tumor showed admixture of two distinctive neoplastic tissues. One was malignant mesenchymal features. Fibrosarcomatous nature was obvious, characterized by cells with moderately chromatic, fusiform nuclei arranged in parallel rows and eosinophilic fibers deeply stained in silver preparation for reticulin. The other was gliomatous tissue forming islands surrounded by the sarcomatous tissues. Both tissues were histologically malignant, as evidenced by mitotic figure, high cellularity, atypical features and variability. In deeper part, sarcomatous findings was less noticed and features of glioblastoma multiforme more prominent, characterized by marked astrocytic anaplasia, endothelial proliferation and extensive perinecrotic pseudopalisading.(ABSTRACT TRUNCATED AT 250 WORDS)
A 70 year old woman underwent an anterior rightsided temporal lobectomy for a mixed glioblastoma-fibrosarcoma. Six months later, she presented with rightsided zygomatic and maxillary swellings, associated with rightsided ptosis, chemosis, and nerve palsies of the III, IV, VI, and VII cranial nerves. A computerized scan (CT) of the head revealed tumor erosion of the skull base of the right middle fossa with extension into the maxillary sinus. This was confirmed at surgery. Pathologic examination revealed a predominantly small cell anaplastic glioblastoma associated with sarcoma elements, similar to the original intracranial tumor. This case documents a rare and atypical behavior of malignant glial tumors.
