Early detection measures and triage procedures for suicide ideation in chronic pain patients.

There is a dearth of writings about early detection of potential suicide patients in chronic pain centers. Early detection measures used at the Vanderbilt Pain Control Center include a Symptom Checklist-90, with questions about depressive symptomatology and "Thoughts of Ending Your Life"; medical and psychological interviews; monitoring of changes in emotional disturbance; and, if warranted, administration of the Scale of Suicidal Ideation. Three case studies are presented that indicate that the results of an assessment measure should be tempered with clinical judgment. Suicidal behavior, including suicidal ideation, is a medical emergency; therefore, there is great need for early detection and triage measures.

Early detection of radioactive fallout by gamma spectrometry.

Radioactive fallout should be detected as early as possible. A new and efficient method for detection of low-level irradiation from manmade radioactivity is developed. Radiation abnormalities are detectable down to air kerma rates of 0.5 to 1.0 nGy h(-1) for 137Cs and even lower for 131I. For multi-gamma energy radioactivity the detection level is 2.6-3.5 nGy h(-1). A standard NaI detector and a 512-channel analyser are used together with noise adjusted singular value decomposition (NASVD). Statistical noise is removed and the measured spectra are reproduced using spectral components produced by NASVD. Stripping is not used and false alarms due to washout of atmospheric radon progeny are almost eliminated. Detection levels and the criteria for setting warning and alarm levels are discussed. The method may also be useful in other situations, for example where low-level signals from radioactive sources need to be detected.

Age-related vision loss in the older adult: the role of the nurse practitioner in prevention and early detection.

Vision loss is a common problem in older adults and may result in functional impairment and accidental injury. The four most common causes of vision loss in this age group are age-related macular degeneration, cataracts, glaucoma, and diabetic retinopathy. The nurse practitioner has an important role in the prevention and early detection of these conditions. This article reviews scientific evidence on risk factors and preventative interventions for the causes of age-related vision loss. It also describes the important role of the nurse practitioner in assessment of eye health, so that conditions that may be damaging to vision may be detected early and monitored in the primary care setting.

Short version of the Guideline: Early Detection of Breast Cancer in Germany. An evidence-, consensus-, and outcome-based guideline according to the German Association of the Scientific Medical Societies (AWMF) and the German Agency for Quality in Medicine (AeZQ).

The goal of the Guideline "Early Detection of Breast Cancer in Germany" is to assist physicians, healthy women, and patients in the decision-making process in favour of appropriate health care regarding early detection and diagnosis of breast cancer. The principle of early detection of breast cancer embraces the detection of non-invasive stages of breast cancer (UICC stage 0, carcinoma in situ), reducing the frequency of invasive breast cancer development, as well as the identification of breast cancer at an early stage (UICC stage I) having a chance of cure of more than 90%, as shown by a large number of trials. The Guideline summarized in the following paper is a precondition to establishing a nation-wide, comprehensive, quality-assurance program for the early detection and diagnosis of breast cancer. The resulting consequence should be a timely mortality reduction of breast cancer. The cure of early stage disease will additionally be achieved by less intensive treatment methods while largely maintaining the quality of life of breast cancer patients. Implementing the Guideline offers the possibility of a significant improvement in women's health care.

Early detection of oral cancer - Is dielectrophoresis the answer?

The early detection of oral squamous cell carcinoma by non-invasive methods has the potential to hasten diagnosis and thus lessen the morbidity associated with tumour therapy. Dielectrophoresis (DEP) can non-invasively determine electrophysiological parameters such as conductivity and permittivity of cellular cytoplasm and membrane. The present study demonstrates that DEP can be utilised to characterise H357 and UP cells and reveals that there are significant differences in these parameters between malignant and more normal epithelial cell lines. The present results suggest that DEP has potential for the early detection of cancerous from non-cancerous cells in a clinical setting.

Ventilatory defect in coal workers with simple pneumoconiosis: early detection of functional abnormalities.

Airway obstruction is a prominent feature in coal workers' pneumoconiosis (CWP). However, many patients with CWP have even demonstrated a normal forced vital capacity (FVC) and forced expiratory volume in 1s (FEV1). The purpose of this study was to evaluate the ventilatory defect by spirometry and search for parameters, other than FVC and FEV1, suitable for early detection of pulmonary impairment in CWP. A sample of 227 coal miners was selected from the medical clinics of two teaching hospitals. Maximal expiratory flow volume measurement and determination of functional residual capacity (FRC) and residual volume (RV) were carried out with an automated plethysmograph. The prevalence of airway obstruction (FEV1/FVC < 70%) in this sample of miners was 52.9% (120/227). There was a progression of functional impairment with the transition from category 0 to categories 2 and 3, no matter what the miners smoking habits. All of the 107 non-obstructed miners had a normal FVC and FEV1. However, the mean values for FEF25-75% (mean forced expiratory flow during the middle half of FVC) and Vmax50 (maximal expiratory flow rate at 50% FVC) were abnormally low, and RV was already elevated, in those non-obstructed subjects with category 1 simple pneumoconiosis. A borderline abnormally elevated FRC in the miners with radiological category 3 of CWP was also noted. We conclude that the Vmax50, FEF25-75%, and RV appeared to be the discriminative indices for detecting early ventilatory defect in non-obstructed patients with simple CWP. Further studies is still needed to clarify the cause of small airway dysfunction.

Early detection of renal dysfunction: practical recommendations.

Test programmes for early detection of renal dysfunction are urgently needed. They should be adapted to the population under investigation, whether the general population or an occupationally or medically exposed population. At present, there is no clinically relevant definition of renal dysfunction on the basis of pathological test results. Due to the complex function and structure of the kidney, measurement of a single parameter is not sufficient for early detection of renal dysfunction. However, any prophylactic and prospective protocol should take into account the sensitivity and specificity of the applied tests, the amount of work involved and the possible positive effects for the population at risk.

Biomarkers for early detection of colon cancer.

There is an increasing demand for biomarkers in colon cancer for risk assessment, early detection, prognosis, and surrogate end points. A number of biomarkers have been identified for early detection of colon cancer, although the risk factors have not been identified extensively. The major advances in understanding colorectal cancer include the identification and the involvement of APC, p53, and Ki-ras in the development and progression of the disease, the identification of the aberrant crypt foci as an early preinvasive lesion, and its relation to the development of cancer. Detecting malignant neoplasms in the early stages offers clinical advantages; therefore, the National Cancer Institute has established an Early Detection Research NETWORK: The emphasis of the network is on translational research and collaboration among scientists.

Detection of alcohol misuse using a routine test panel: the early detection of alcohol consumption (EDAC) test.

AIMS: This study describes the derivation and validation of the early detection of alcohol consumption (EDAC) test, which uses linear discriminant function (LDF) analysis for the identification of alcohol misuse. This form of LDF aims to predict a categorical dependent variable (alcohol misuse) on the basis of several independent, predictor variables (routine laboratory tests). METHODS: EDAC was developed to classify individuals as heavy or light drinkers using a database of 1599 subjects recruited from 25 sites in the USA. The predictor variables for the LDF were 36 routine chemistry and haematology analytes. RESULTS: The EDAC model produced 80.7% sensitivity and 84.4% specificity, with an overall correct classification rate of 82.5%. Using a stepwise method, 20 of the 36 routine tests used in the LDF were selected as the optimal predictor variables. The top three variables with the highest contribution in the stepwise EDAC model were: bilirubin ratio (direct to total), aspartate aminotransferase and albumin. CONCLUSIONS: This study shows that LDF analysis in conjunction with new, user-friendly computer packages is a practical and cost-effective laboratory tool for detecting excessive drinking using blood constituents ordered routinely in a variety of clinical settings. Diagnostic performance can be adjusted to achieve higher specificity rates.

Development and validation of a serum peptidomic signature for early detection of asymptomatic ovarian cancer: A multi-center prospective study.

Early detection of asymptomatic ovarian cancer (asym-OC) remains a critical challenge, the failure of which underlies its high mortality. Performing serum peptidomic profiling of 843 participants in the cohort SOCFCP, we distill 1,081 initial features into a 7-marker panel for asym-OC detection via a biology-informed machine-learning (ML)-based feature selection strategy. Three markers significantly revert toward non-OC levels after surgery. Integrating the panel with age, CA125, and HE4, we develop and externally validate (n = 159) a LightGBM model, ProMS+. For early-stage OC detection, ProMS+ shows a specificity of 92.6% at 95.0% sensitivity, outperforming CA125 (44.7%), HE4 (11.2%), and Risk of Ovarian Malignancy Algorithm (ROMA) (24.0%), with an area under the curve (AUC) of 0.993. In a simulated high-risk population (n = 100,000; OC prevalence = 1%), ProMS+ yields a high AUC (0.983) and a higher positive predictive value than CA125, HE4, and Age + CA125 + HE4 combined model (0.201 vs. 0.027, 0.090, and 0.064). ProMS+ offers a promising, non-invasive, and interpretable approach for the early detection of asym-OC.

[Indices allowing early detection of chronic pulmonary emphysema].

To establish criteria allowing early detection of pathologically significant alterations in pulmonary emphysema caused by smoking, pulmonary-function tests and high-resolution computed tomography were done in 104 subjects categorized into three groups: nonsmoking healthy adults, smokers with a normal FEV1%, and smokers with a low FEV1% (cross-sectional analysis). Fifty-six of the 104 patients underwent pulmonary-function testing and high-resdution computed fomography once per year for 3 years (longitudinal analysis). Cross-sectional and longitudinal analyses showed that abnormalities in functional residual capacity, in single-breath diffusing capacity for carbon monoxide, and in the average tomographic density of sections in the lower lung fields obtained after a deep inspiration could be used to predict whether the disease would reach an advanced stage, even if the patients had no significant symptoms at the time of testing. Relative areas of low-attenuation regions, which were alleged to directly reflect the size of emphysematous areas, appear not to be useful for early detection of pathological emphysema.

[Economic evaluation of "early detection and treatment of esophageal cancer"].

BACKGROUND & OBJECTIVE: In China, esophageal cancer is the forth death cause of malignant cancers. The survival rate of esophageal cancer patients is less than 10%. Early detection and treatment of esophageal cancer is essential to improve the survival rate of these patients. This study was to evaluate the economic and social benefits of the project "early detection and treatment of esophageal cancer" through cost-benefit analysis, so as to provide evidence to extend this project. METHODS: Interview with structured questionnaires was conducted by stratified sampling among 136 esophageal cancer patients in 3 hospitals at Linzhou City, and a sampling survey was conducted among 53 patients at their homes afterwards. The 2 surveys measured the expense of "traditional treatment of esophageal cancer". Based on an intervention study of "early detection and treatment of esophageal cancer" among 936 persons at Linzhou City, cost-benefit analysis and the evaluation of economic burden of disease among esophageal cancer patients were carried out. RESULTS: The expense of "traditional treatment of esophageal cancer" was 7,183 Yuan per patient. Regarding the 936 persons who were recruited to the intervention study, the cost was 148,246 Yuan, and the benefit was 589,006 Yuan. The difference between benefit and cost of the project was 440,760 Yuan. The benefit-cost ratio was 4:1. By the project, the ratio of the expense in treating disease to the total family income in 1 year was reduced from 1.300 to 0.125. CONCLUSION: The cost of "early detection and treatment of esophageal cancer" is low, while the benefit is high, and the project can also lighten the economic burden of esophageal cancer patients.

[Usefulness of biological tumor markers for early detection of neoplastic changes in the bladder of persons exposed to aromatic amines].

Industrial exposure to aromatic amines may result in bladder cancer, especially in persons characterized by a slow acetylation phenotype. What is particularly important in neoplastics therapies is an early detection of neoplastic changes. Prophylactic and prognostic examination suggest that determination of neoplastic markers may contribute to an early detection of these changes. From the results gathered it emerges that carcinoembryonal antigen (CEA) and tissue polypeptide antigen (TPA) are most useful for an early detection of bladder cancer. It seems reasonable to examine the most seriously exposed ones using methods for CEA and TPA determination. Such an examination performed twice or three times in this group should help to early detect hyperplasia of urinary bladder epithelium.

Coronary artery disease: new insights into the pathophysiology, prevalence and early detection of a monster menace.

Coronary artery disease (CAD) is the leading cause of morbidity and mortality in the United States and industrialized countries. In the undeveloped world a similar epidemic is brewing. A new pathophysiologic paradigm has emerged, which assigns the mediators of inflammation a much larger role in the disease process. This paradigm has helped explain the unpredictable nature of many adverse consequences of CAD. The long latent phase of the disease and often sudden initial presentation make efforts at early detection extremely important. Considerable work has been devoted to identify as well as influence predisposing risk factors for developing arteriosclerosis. Novel markers of inflammation, like C-reactive protein, have been identified and compared to traditional risk factors. In addition, new imaging modalities introduce the possibility of screening for sub-clinical disease. Electron-beam and spiral CT scanners, as well as other techniques, are emerging as powerful tools to detect early disease presence and allow intervention to take place before major clinical events occur. Advances in our understanding of the pathophysiology and our ability to image the stages of silent disease will go hand in hand to revolutionize our approach to prevention and treatment of this deadly disease.

[Short version of the guideline "Early Detection of Breast Cancer in Germany"].

The aim of this evidence-, consensus- and outcome-based good clinical practice guideline is to help physicians, and women in making appropriate healthcare decisions about the early detection of breast cancer. The principle of early detection of breast cancer comprises the detection and diagnosis of pre-malignant breast tumours (UICC stage 0, carcinoma in situ) with a possible cancer risk reduction and the detection and diagnosis of breast cancer at an early stage (UICC stage 1) with a scientifically proven 90% chance of cure. By establishing a nation-wide comprehensive quality assurance program for the early detection of breast cancer this guideline lays the foundation for a timely reduction of breast cancer mortality and achievement of cure with less impairment of patient's quality of life. It would appear that this guideline makes major improvements in women's healthcare feasible.
