Delivery of laboratory data with World Wide Web technology.

We have developed an experimental World Wide Web (WWW) based system to deliver laboratory results to clinicians in our Veterinary Medical Teaching Hospital. Laboratory results are generated by the clinical pathology section of our Veterinary Medical Diagnostic Laboratory and stored in a legacy information system. This system does not interface directly to the hospital information system, and it cannot be accessed directly by clinicians. Our "meta" system first parses routine print reports and then instantiates the data into a modern, open-architecture relational database using a data model constructed with currently accepted international standards for data representation and communication. The system does not affect either of the existing legacy systems. Location-independent delivery of patient data is via a secure WWW based system which maximizes usability and allows "value-added" graphic representations. The data can be viewed with any web browser. Future extensibility and intra- and inter-institutional compatibility served as key design criteria. The system is in the process of being evaluated using accepted methods of assessment of information technologies.

Determination of the critical O2 delivery from experimental data: sensitivity to error.

Normally, metabolic need determines tissue O2 consumption (VO2). In states of reduced supply, VO2 declines sharply below a critical level of O2 delivery (QO2 = blood flow X arterial O2 content). Although several investigators have measured a critical O2 delivery in whole animals or in isolated tissues, there is no general agreement over how to determine the critical point from a collection of real data. In this study, we compare three algorithms for finding the critical O2 delivery from a set of experimental data. We also present a technique for estimating the effect of experimental error on the precision of these algorithms. Using 16 data sets collected in normal dogs, we compare single-line, dual-line, and polynomial regression algorithms for identifying the critical O2 delivery. The dual-line and polynomial regression techniques fit the data better (mean residual square deviation 0.024 and 0.031, respectively) than the single-regression line approach (0.110). To investigate the influence of experimental error on the derived critical QO2, we used a Monte Carlo technique, repeatedly perturbing the experimental data to simulate experimental error. We then calculated the variance of the critical QO2 frequency distribution obtained when the three algorithms were applied to the perturbed data. By this analysis, the dual-line regression technique was less sensitive to experimental error than the polynomial technique.

Baseline data on the delivery of clinical preventive services provided by nurse practitioners.

The National Alliance of Nurse Practitioners (NANP) was one of five primary care provider groups that participated in a national government-sponsored survey on clinical preventive services (CPSs). The survey was based on 17 of the Healthy People 2000 national health objectives related to CPSs. This survey was sent to 2000 randomly selected nurse practitioners (NPs) to obtain information concerning the percentage of their patients who "routinely" receive the specified assessment and intervention services. Data from 892 completed and returned survey instruments indicated that NPs already exceed the Healthy People 2000 objective targets in some important clinical preventive assessment and intervention areas. They are close to the targets in a number of other CPS areas. In some areas, however, NP provision of preventive services was reported to be below the Healthy People 2000 targets. This data has far-reaching implications for NP practice, basic and continuing education, and curriculum reform.

Deliveries by family physicians in Connecticut: results of a practice-based research network.

OBJECTIVE: To obtain descriptive data on deliveries performed by family physicians in Connecticut during 1 year. DESIGN: All family physicians in Connecticut who provided obstetric care were personally contacted to enlist their participation in a research network. All such physicians agreed to participate and were instructed to mail a 15-item delivery data card to the author following each delivery that they participated in during calendar year 1996. MAIN OUTCOME MEASURES: Method of delivery, use of obstetric intervention (eg, oxytocin administration, episiotomy, and epidural anesthesia), and obstetric consultation rates. RESULTS: Thirty-two physicians, representing 9.0% of the family practice workforce in Connecticut, provided obstetric care. These 32 physicians, who practice in only 7 of the 31 acute care hospitals in the state, delivered 478 neonates during 1996. Most deliveries were by the spontaneous vaginal route, with forceps and vacuum used in 2.1% and 5.0% of vaginal deliveries, respectively. The primary cesarean section rate of these family physicians was low at 5.6% compared with a statewide rate for all providers (physicians and certified nurse midwives) of 12.4% (odds ratio, 0.42; 95% confidence interval, 0.28-0.63). Rates of certain obstetric interventions, such as oxytocin administration, epidural anesthesia, and episiotomy, varied greatly by hospital. Obstetric consultation rates also varied greatly among hospitals, ranging from 12.8% to 49.6%. CONCLUSIONS: Family physician involvement in maternity care in Connecticut is low. This study confirms a low rate of instrument-assisted and cesarean delivery in births attended by family physicians. Use of obstetric interventions and obstetric consultation varies greatly within the state.

[Obstetrical data of 11,874 deliveries at a university clinic of a developing country].

A report a given shout data of 11,874 deliveries and 12,338 childs which was born in 1987 in Central Hospital of Maputo PR of Mozambique. The perinatal mortality smouted to 65% and the maternal mortality 253%. Totally registrated a frequency of 15.2% of cesarean sections and observed in 30 cases a rupture of the uterus. Deliveries have been terminated in 2.8% by vaccum extraction and only in 0.1% by forceps. We observed a twin delivery in 458 cases or in 3.8% and a triplet delivery in 6 time. These data have been discussed in detail.

Triplet and higher-order births: what is the optimal delivery route?

Data concerning 16 triplet and higher-order deliveries (resulting in a total of 56 infants) are reviewed. The vaginal delivery rate was 81%. Maternal morbidity was more serious after abdominal delivery. Prematurity (less than 36 weeks gestation) rate amounted to 68%. Overall perinatal and neonatal mortalities for infants born after 28 weeks gestation and weighing at least 1000 g were 7% and 3%, respectively. We doubt that neonatal outcome could have been markedly improved by performing more cesareans. The importance of antenatal care is stressed.

Racial and ethnic composition of volunteer cord blood donors: comparison with volunteer unrelated marrow donors.

BACKGROUND: Umbilical cord blood is an alternative peripheral blood progenitor cell source for patients who need transplantation. A presumed advantage of cord blood is the ability to increase minority recruitment. STUDY DESIGN AND METHODS: The racial composition of five member cord blood banks of the National Marrow Donor Program (NMDP) was compared, representing 9020 cord blood donors with NMDP marrow donors from comparable geographic areas, representing 417,676 donors. Cord blood and marrow donors self-reported racial designations on questionnaires. Donor statistics were compared with baseline racial data of deliveries from participating hospitals for cord blood donors and with geographic census data for marrow donors. RESULTS: The California, Florida, and Massachusetts cord blood banks recruited a lower percentage of minorities than the corresponding marrow donor centers. In New York and Colorado, minority recruitment was equivalent. In California, Florida, Massachusetts, and New York, the cord blood banks recruited a lower percentage of minorities than those delivering at the respective hospitals. The cord blood banks in California, Colorado, Florida, and Massachusetts recruited a lower percentage of minorities compared with delivery data than the corresponding marrow donor centers compared with census population (p < 0.001). In New York, the percentages were similar. CONCLUSION: The problem of insufficient minority recruitment of cord blood has not yet been solved. Better strategies are needed to recruit minority donors.

Relationships between length of stay and hospital characteristics under the case-payment system in Taiwan: using data for vaginal delivery patients.

BACKGROUND: Case payment has been implemented since the beginning of Taiwan's National Health Insurance Program in 1995. This study selected patients undergoing a vaginal delivery to explore the relationships between maternal length of stay (LOS) and hospital characteristics under the case-payment system in Taiwan. METHODS: The National Health Insurance Research Database of 1999 from Taiwan's National Health Research Institutes was used in this study. In total, 5456 patients who underwent a vaginal delivery in 1999 meeting the selection criteria were drawn from the database. A multiple regression analysis was performed in which LOS was regressed against the variables of hospital level, hospital location, hospital ownership, and teaching status. RESULTS: The regression model indicated that hospital level, hospital ownership, and hospital location were significantly related to LOS after adjustment for patient age, principal procedure, and the presence of a secondary diagnosis. The LOS for patients undergoing a vaginal delivery in private hospitals was shorter than those in public and non-profit proprietary hospitals. Patients admitted to medical centers or regional hospitals were more likely to have a longer mean LOS in comparison with their counterparts admitted to district hospitals. The LOS for patients hospitalized in northern Taiwan tended to be significantly longer on average than those in central and southern Taiwan. CONCLUSIONS: This study demonstrates that wide variations in LOS exist among hospitals in Taiwan under the case-payment system. It is recommended that the Bureau of the National Health Insurance develop a national system to monitor certain hospitals that have an unusually short LOS.

Biochemical profile of African American women during three trimesters of pregnancy and at delivery.

The data presented are the results from a prospective observational study which was conducted to investigate the effects of nutrition and other related factors on the outcome of pregnancy in nulliparous African American women 16-35 years old. Fasting blood samples were collected from the women during the first, second and third trimesters of pregnancy. At delivery, both maternal and cord samples were collected. Biochemical variables such as, serum folate, vitamin B12, ascorbic acid, vitamin E, ferritin, selected minerals as well as complete blood count (CBC) and red cell folate were analyzed in the blood samples. The concentrations of hematocrit, hemoglobin, white blood cells, red blood cells and vitamin B12 were below the reference non-pregnant ranges throughout gestation. Maternal concentrations of folate and vitamin E increased sequentially with increased gestational age. Serum ferritin, during the third trimester, declined to 58% of the first trimester concentration. Maternal levels of ferritin at delivery were one third of the values found in the infant (cord) sample. Cord levels of folate, ascorbic acid and vitamin B12 were higher than the concentrations in the maternal delivery samples. The data suggest that among this group of pregnant women, major physiological changes, such as plasma volume expansion which alters blood chemistry and maternal to fetal transfer of nutrients, were similar to the findings of other investigators. In this population however, the findings for serum and whole blood folate are contrary to those reported by other researchers, and the sequential increase in the maternal concentration of the vitamin during pregnancy could be attributed to the use of vitamin supplements.
