Transesophageal two-dimensional echocardiography in the diagnosis of cor triatriatum in the adult.

Two adult patients with left ventricular inflow obstruction are presented. Conventional two-dimensional echocardiography had failed to yield a definite diagnosis, whereas transesophageal two-dimensional echocardiography clearly documented a membraneous echo structure within the left atrium, diagnostic of cor triatriatum. On the basis of the transesophageal echocardiographic findings, left heart catheterization and angiocardiography were not performed and both patients successfully underwent cardiac surgery.

Cor triatriatum masked by primary pulmonary hypertension.

In a 17-month-old boy with clinical signs of right heart failure, the diagnosis of primary pulmonary hypertension was made, based on cardiac catheterization findings with high pulmonary arterial pressure and normal pulmonary wedge pressure although two-dimensional echocardiography demonstrated a left atrial membrane. Postmortem examination confirmed a left atrial membrane (cor triatriatum), and the microscopic examination revealed the histological pattern of primary pulmonary hypertension. Due to reduced pulmonary blood flow, pulmonary venous obstruction was masked and could not be assessed by measuring pulmonary wedge pressure.

Isolated pulmonary vein stenosis in complex congenital heart disease, simulating cor triatriatum by cardiac catheterization and transoesophageal echocardiography.

A case of localized stenosis of the lower left pulmonary vein, associated with perimembranous ventricular septal defect (VSD) and secundum atrial defect (ASD), a borderline sized mitral valve apparatus and left ventricle, is described. This final diagnosis was made at surgery, as the localized stenosis of the left pulmonary vein was misinterpreted as partial cor triatriatum by cardiac catheterization, cineangiocardiography and extensive echocardiographic studies. Surgical correction of all the defects was successful, but pulmonary vascular resistance was still increased 2 years after surgery.

Acute pulmonary edema in the post partum and cor triatriatum sinistrum.

A case is described of an apparently healthy young woman in her first pregnancy who presented with acute pulmonary edema in the early postpartum period in the context of mild pregnancy-induced hypertension. After quick improvement in her condition, a Doppler followed by a transesophageal study revealed a left atrial septation with a small atrial septal defect secundum type and moderate mitral regurgitation strongly suggestive of a cor triatriatum sinistrum.

Cor triatriatum dextrum simulating right ventricular myxoma and pulmonary stenosis.

A case of enormous Chiari's network in a 7-year-old girl is described. It formed a parachute or spinnaker-like sack which moved freely into the pulmonary artery causing mild pulmonic stenosis. The preoperative diagnosis was believed to be myxoma of the right ventricle. The real condition was recognized during the operation. This is the seventh reported case of correction of so-called cor triatriatum dextrum.
