Influence of dentinal regions on bond strengths of different adhesive systems.

This in vitro study assessed comparatively the shear bond strengths of three composite resins, 3M Valux Plus (3MVP), Herculite (H), Clearfil AP-X (CAP-X), a polyacid modified composite resin Dyract (D), and a resin modified glass-ionomer materials Vitremer (V), to cervical and buccal dentine regions of extracted human molar teeth. Four different bonding systems, 3M ScotchBond Multipurpose (SB), Clearfil Liner Bond 2 (LB2), Opti Bond (OB), and Prime & Bond 2.1 (PB 2.1) were used with the manufacturer's respective composite and compomer materials. One hundred freshly extracted mandibular molar teeth were selected for this study. Flat buccal dentine surfaces were created on 50 teeth and cylindrical rods of the five materials were bonded to the dentine surfaces. For assessment of cervical bond strengths, the materials were bonded to mesial and distal enamel bordered occlusal dentinal surfaces of the remaining 50 teeth. The five groups of restorative procedures were applied as follows; Group 1: SB + 3MVP, Group 2: LB2 + CAP-X, Group 3: OB + H, Group 4: PB2.1 + D, Group 5: Vitremer primer (VP) VP + V. Each restorative procedure thus had 20 specimens (10 buccal + 10 cervical). After 24 h of water storage (37 degrees C), the specimens were tested on a Universal Testing machine in shear with a cross head speed of 0.5 mm min-1. The bond strength values were calculated in MPa and the results were evaluated statistically using Kruskal-Wallis one-way/anova and Mann-Whitney U-tests. It was found that the bond strengths of SB + 3MVP, LB2 + CAP-X and VP + V to buccal dentine surfaces were significantly stronger (P < 0.05) than those to the occluso-cervical dentine floors. When the bond strengths to the occluso-cervical dentine and buccal dentine surface were compared, there was no significant difference between the materials (P > 0.05). Vitremer was found the least successful adhesive material in terms of shear bond strength on both buccal and occluso-cervical dentine surfaces.

Induction and maintenance of ganglionic long-term potentiation require activation of 5-hydroxytryptamine (5-HT3) receptors.

1. An extracellular recording technique was used to study the effects of 5-hydroxytryptamine (5-HT, serotonin) on the tetanus-induced long-term potentiation (LTP) of the nicotinic pathway of transmission in the superior cervical ganglion (SCG) of the rat. The postganglionic compound action potential (CAP), made submaximal by treatment with hexamethonium (O.4 mM), was used as an index of transmission in the ganglion. 2. Serotonin (10 microM) markedly enhanced the magnitude of LTP without affecting the post-tetanic potentiation (PTP). The serotonin (2-30 microM) concentration-response curve for LTP was bell shaped as no enhancement was seen with 30 microM serotonin. This may largely be due to activation of a 5-HT1 receptor subtype and not to desensitization. 3. When superfused before tetanus, the 5-HT1A receptor agonist 8-hydroxydipropylamino-tetralin (8-OH-DPAT, 5 microM) prevented the expression of LTP without affecting PTP. 4. Pretreatment of ganglia with the 5-HT2 receptor agonist R-(+)-dimethoxy-4-iodoamphetamine (R-(+)-DOI, 1 microM) enhanced the tetanus-induced LTP. Similar treatment with the 5-HT2 receptor antagonist ketanserin (3 microM) had no significant effect on LTP. 5. Pretreatment of ganglia with the 5-HT3 receptor agonist 1-m-(chlorophenyl) biguanide (m-CPGB, 1 microM), markedly increased (300%) the tetanus-induced LTP. Similar pretreatment with the 5-HT3 receptor antagonist 3-tropanyl-3,5-dichlorobenzoate (MDL 72222, 0.5 microM) completely prevented the expression of LTP. Fully expressed LTP was reversibly blocked by MDL 72222 when applied during the maintenance phase of LTP. 6. Tetanic stimulation of monoamine-depleted ganglia (from reserpine-pretreated rats, 3 mg kg-1 for 24 h) failed to induced LTP. 7. In monoamine-depleted ganglia, tetanus preceded by superfusion with m-CPBG readily induced LTP. MDL 72222 completely blocked this LTP. However in these ganglia tetanus failed to induced LTP when m-CPBG was given 2 min (during PTP) or 1 h after tetanus. 8. Tetanic stimulation of monoamine-depleted ganglia in the presence of R-(+)-DOI failed to induced LTP. 9. We conclude that tetanus-induced LTP of the SCG of the rat requires activation of 5-HT3 receptors both for induction and maintenance.

Quality assurance activities of the College of American Pathologists.

Since its inception, the College of American Pathologists (CAP) has played a fundamental and pivotal role in the development and execution of quality assurance programs for laboratories. Within the realm of anatomic pathology, operational programs include those in surgical pathology, immunohistochemistry and cytopathology. The emphasis of prior cytopathology programs on cervical cytology has now been expanded to include body fluids and fine needle aspiration material. CAP's role in the expansion of quality assurance programs in cytology may be enhanced in the future by intersociety cooperation with established cytology organizations and will also be influenced by and closely linked to the expansion of its quality assurance programs in surgical pathology. As for the Papanicolaou smear, it can no longer be regarded as the "Cinderella of cytology"; it is in fact the present-day cynosure in the laboratory. In recognition of this, CAP has undertaken efforts to help heighten public awareness about the value of Papanicolaou smear testing and is encouraging women to become more informed about the process involved in the examination of their smears.

Variation in the human cervical neural canal.

BACKGROUND CONTEXT: The dimensions of the cervical spinal canal can impact the likelihood of an individual suffering longtime effects from a spinal neck injury as well as influence recovery time. Most studies have used radiographic studies to compare differences in the neural canal, but few have examined skeletal populations to determine variation in the neural canal dimensions without the presence of soft tissue. PURPOSE: To analyze variation seen in the cervical neural canal (anterior-posterior and transverse diameters) with respect to sex and ancestry and to define cervical canal narrowing in the sample. STUDY DESIGN: Observational. METHODS: Measurements of the anterior-posterior (sagittal) (CAP) and transverse (CTR) diameters were taken from 321 individual skeletons. Comparisons were made between males and females and individuals belonging to different ancestral (racial) groups. RESULTS: CAP was narrowest at the C4 level for African-Americans and at C6 for Caucasians. CTR was narrowest at the C2/C3 level for all groups. Statistical analyses indicated that significant differences in cervical canal dimensions are due first to sexual dimorphism and then to ancestry. CONCLUSIONS: Significant variation in cervical canal dimensions precludes usage of universal definitions to determine spinal stenosis in individuals; definitions should be according to sex and ancestry.

Immunohistochemical localization of periostin in tooth and its surrounding tissues in mouse mandibles during development.

Previous reports have shown expression of immunoreactivity for periostin, originally identified as osteoblast-specific factor-2, in the periosteum and periodontal ligament. However, the developmental changes in its expression and the detailed immunolocalization have remained veiled. The present study was undertaken to examine the spatiotemporal expression of this protein in teeth and their associated tissues of mice during development at light and electron microscopic levels. In tooth germs at cap stage, periostin immunoreactivity was recognizable in the interface between inner enamel epithelium and preodontoblasts as well as in the mesenchymal tissues around cervical loop. Dental follicles around tooth germs at bell stage localized periostin immunopositivity in addition to the immunopositive areas observed in cap-staged tooth germs, although the functional significance of periostin has remained unclear in tooth development. Furthermore, periostin immunoreactivity was also found in the alveolar bone surface. In the incisors of both 7- and 21-day-old mice, immunoreaction for periostin was discernible in the lingual periodontal ligament and labial fibrous tissue adjacent to the papillary layer. After postnatal day 7, immunoreaction for periostin came to be restricted to the fibrous bundles in the periodontal ligament in accordance with the organization of the periodontal fibers, indicating its localization matched the morphogenesis of the periodontal ligament. Immunoelectron microscopic observation of the mature periodontal ligament verified the localization of periostin between the cytoplasmic processes of periodontal fibroblasts and cementoblasts and the adjacent collagen fibrils. Our findings suggest that periostin is involved at the sites of the cell-to-matrix interaction, serving as adhesive equipment for bearing mechanical forces, including occlusal force and tooth eruption.

Space motion sickness: phenomenology, countermeasures, and mechanisms.

A summary of the incidence of Space Motion Sickness (SMS) in 27 Soviet cosmonauts who flew on missions varying from 2-185 d in the Salyut-6/Soyuz vehicle complex is presented. A questionnaire indicated that 88% (24) of the cosmonauts developed some type of "illusionary sensations" while 44% (12) presented some degree of SMS. The SMS countermeasures used in flight included an antihistaminic drug, pneumatic cuffs applied to the thigh region, application of lower body negative pressure, a head cap that restricted head movement while simultaneously providing force stimulus to the cervical antigravity muscles, and finally the use of an insole counterpressure device that added pressure to the sole of the foot.

Cervical laminar exostosis in multiple hereditary osteochondromatosis: anterior stabilization and fusion technique for preventing instability.

Multiple hereditary osteochondromatosis is a genetically transmitted disorder consisting of multiple projections of bone capped by cartilage, which are called exostoses. Spinal cord compression due to expansion of a laminar osteochondroma is rare but well recognized. Surgical decompression usually improves the patient's neurological status but, in cervical exostosis, post-laminectomy kyphosis and instability problems, especially in the high-risk adolescent group, form the most significant potential difficulties in the postoperative period. We report a case of cervical laminar exostosis that was treated by anterior stabilization and fusion and discuss the benefits of this technique.

[Current status of combination chemotherapy].

The current status of clinical investigations of combination chemotherapies was reviewed. A randomized trial comparing low-vs high-dose cisplatin conducted in cases of testicular cancer indicated that a high-dose regime (120 mg/m2 D1) was superior to a low-dose regime (15 mg/m2 D1-5) while similar studies conducted in head & neck cancer and cervical cancer showed no significant differences. PVB or BEP in testicular cancer and CAP in ovarian cancer appear to have on established clinical role, but other combinations containing CDDP for use on various tumors are in the stage of phase II trial. Alternating non-cross combination is an attractive direction in clinical investigation but no such regimes tested in the past has shown any significant superiority over the results obtained from standard combinations.

Abolition of vagino-cervical stimulation-induced analgesia by capsaicin administered to neonatal, but not adult rats.

We have previously reported that vagino-cervical mechanical stimulation (VS or probing) produces analgesia in rats. Neonatal treatment with capsaicin (CAP) has been shown to reduce the concentrations of several neuropeptides in dorsal root ganglia, spinal cord, and autonomic ganglia, via a neurotoxic effect. In the present study, we report that CAP administered in the neonatal period abolishes the analgesic effect of probing in adulthood. In addition, we report that the ability of VS to potentiate the lordosis response to manual stimulation of the flanks is abolished by neonatal CAP treatment. By contrast, rats treated as adults with CAP show the typical VS-produced effects of analgesia and potentiation of the lordosis response. Our results suggest that neonatal, but not adult, CAP treatment depletes a neuropeptide(s) that mediates the analgesia and lordosis-inducing effects of VS.

Effect of gamma-aminobutyric acid on synaptic transmission and long-term potentiation in rat superior cervical ganglion.

The effect of gamma-aminobutyric acid (GABA) on synaptic transmission in rat superior cervical ganglion (SCG) was assessed in vitro by extracellular recording. Postganglionic compound action potentials (CAPs) triggered by preganglionic stimulation were blocked in a reversible and concentration-dependent fashion by short, 60 s long, superfusion with GABA (IC50 = 39.3 microM), with the GABAA agonist muscimol (IC50 = 8.7 microM) or with the GABAB agonist baclofen (IC50 = 145 microM). Responses to GABA and muscimol, but not to baclofen, exhibited desensitization after 5 min long superfusions with the drugs. In a long-term potentiation (LTP) paradigm, the degree of potentiation found 30 min after a tetanic train of stimuli (20 Hz for 20 s) was strongly inhibited by GABA (100-250 microM), when superfused at the time of tetanic stimulus or shortly thereafter. The effect of GABA on SCG LTP was mimicked by muscimol but not by baclofen. The results are compatible with the view that GABA exerts overall inhibitory effects in rat SCG, including transmission blockade of single impulses (through activation of GABAA and GABAB receptors) and impairment of activity-dependent potentiation of nicotinic transmission (through activation of GABAA receptors).

Cell proliferation in teeth reconstructed from dispersed cells of embryonic tooth germs in a three-dimensional scaffold.

Tissue engineering can now reproduce tooth from postnatal tooth cells. However, crown formation is not accurately reconstituted, even when the complex structure of the enamel dentin is reproduced. Here, we showed that a tissue-engineered (TE) tooth, exhibiting morphogenesis according to regular crown-cusp pattern formation, was produced by embryonic tooth germ cells in a three-dimensional scaffold. Heterogeneous cells dissociated from embryonic day 14 (E14) mice tooth germs were seeded on a scaffold and implanted under a kidney capsule in adult mice. The developmental process of the implants was examined for up to 14 d. At 5 d, the cells had formed initial tooth germ, followed by enamel-covered dentin tissue formed symmetrically. To study the developmental process, we examined the growth pattern using 5-bromo-2'-deoxyuridine (BrdU)-labeling analysis. The initial cell-proliferation patterns of the TE teeth were similar to that at the cap and early bell stages in natural teeth. This was particularly true in the cervical loop, which showed a similar distribution pattern of BrdU-positive cells in TE- and natural teeth. These results suggested that even when embryonic tooth germs are dissociated, the single cells can reconstitute tooth, and that enamel organ morphogenesis proceeds as in natural teeth.

The effect of cervical sympathectomy on cochlear electrophysiology.

The purpose of this study was to examine the electrophysiologic responses to sound from guinea pig cochleas, 3--6 days after a unilateral cervical sympathectomy. After recovery from surgery the guinea pigs developed a Horner's syndrome on the sympathectomized side. Some days later their bullas were opened and electrodes were placed bilaterally on the round windows. Most of the sympathectomized cochleas showed signs of decreased sensitivity to sound. They had a smaller dynamic range for the click- and tone-burst evoked compound action potential (CAP) compared with the non-sympathectomized cochleas. The threshold sound levels for the CAPs and the sound levels to produce 1 muV of cochlear microphonic potentials were unaffected. In other animals with chronic implanted electrodes, the same electrical responses were measured with only light sedation before and after sympathectomy and showed the same results. In additional animals the crossed olivocochlear bundle (COCB) was electrically stimulated and a similar inhibition of the CAP was registered on both the intact and the sympathectomized side. The results suggest that the sympathetic nerves to the cochlea, coming from the ipsilateral cervical ganglion and ending near the habenula perforata, may be important for the sound perception as they influence the CAP.

Cyclic AMP antagonizes adenosine-induced inhibition of ganglionic transmission.

The mechanism of adenosine-induced inhibition of ganglionic transmission was investigated in the isolated superior cervical ganglion (SCG) of the rat. The inhibitory effect of adenosine on the postganglionic compound action potential (CAP) was antagonized by pretreatment of ganglia with forskolin, isoproterenol (IPNE), arginine vasopressin (AVP), or papaverine, all of which are known to increase tissue cAMP level by different mechanisms. Furthermore, pretreatment of ganglia with the adenylate cyclase inhibitor SQ 22, 536, or the phosphodiesterase activator imidazole reversed the effects of IPNE and forskolin. Pretreatment with 8-bromo-cAMP, resulted in a marked antagonism of the adenosine-induced inhibition. By themselves, none of these drugs had any significant effect on the CAP. Adenosine slightly but significantly decreased the basal level of cAMP in untreated ganglia. Formation of cAMP induced by IPNE was markedly reduced by adenosine. This was largely reversed in the presence of the adenosine A1 receptor antagonist 8-cyclopentyl-1, 3-dipropylxanthine (DPCPX) but not the A2 receptor antagonist 3, 7-dimethyl-1-propargylxanthine (DPMX). We conclude that the inhibition of ganglionic transmission by adenosine involves reduction of cAMP formation through activation of A1 receptors.

Effects of inorganic potassium channel blockers on calcium requirement of transmission in a sympathetic ganglion.

The effects of the alkali metal ions cesium (Cs+) and rubidium (Rb+) and alkaline earth metal ions barium (Ba2+) and strontium (Sr2+) on ganglionic transmission in various calcium (Ca2+) concentrations were investigated in rat isolated superior cervical ganglia. Cesium (1 and 2 mM) moderately supported transmission in low Ca2+ and potentiated the compound action potential (CAP) at high Ca2+ concentration. Higher concentrations of Cs+ caused depression of CAP especially at lower Ca2+ concentration. Cesium (1-4 mM) induced large spontaneous rhythmic spikes or burst of spikes. At 4 mM, Rb+ potentiated CAP at all Ca2+ concentrations. As with Cs+, higher concentrations of Rb+ inhibited CAP except at large Ca2+ concentrations. Barium (1-6 mM) effectively enhanced transmission at all Ca2+ concentrations. Strontium enhanced transmission only at very low Ca2+ concentrations. No enhancement was seen with Sr2+ in the presence of normal or higher Ca2+. The results indicate that Cs+ and Rb+ may antagonize Ca2+ effects at release sites while allowing more Ca2+ influx into the nerve terminal as a result of K+ channel blockade. Barium and to a lesser extent Sr2+ may substitute for Ca2+ in the process of transmitter release in the superior cervical ganglia of rats.

A new goniometer.

A simple new goniometer has been developed for measurement of the mobility of the cervical spine. It consists of a headgear with three rigid scales calibrated in degrees, mounted on a skull cap with straps around the chin, and it measures flexion/extension, lateral flexion and horizontal rotation. Its construction and use are described. Three observers each tested the same 10 subjects to determine inter-observer error. Two observers then each tested 10 different subjects on two occasions at intervals of 10 to 30 minutes to test intra-observer error. Statistical analysis of the inter-observer error data by analysis of variance, and the intra-observer error data by calculation of means and standard errors of the differences between the means, showed that acceptably reproducible results could be obtained with this new goniometer, and it is also shown that an experienced observer can obtain more consistently reproducible results. Reference is made to the successful use of this technique in a comparative study (published elsewhere) of two drugs in degenerative joint disease of the cervical spine.

The tunneled supraclavicular island flap: an optimized technique for head and neck reconstruction.

Reconstructive procedures in the head and neck region use a wide range of flaps for defect closure. The methods range from local, mostly myocutaneous flaps and skin grafts to free microsurgical flaps. To ensure a satisfactory functional and aesthetic result, good texture and color of the flap are always essential. Moreover, the donor-site defect needs to be reduced, with no resulting functional or aesthetic impairment. We have found that the shoulder is a region providing an optimum skin texture match to the neck and face. In cadaver dissection, a vascular pedicle extending from the transversal cervical artery with two accompanying veins was found to vascularize a defined region around the shoulder cap. In line with these findings, the previously described fasciocutaneous island flap, nourished by the supraclavicular artery, was developed further and used purely as a subcutaneously tunneled island flap. The tunneling maneuver significantly improves the donor site by reducing scarring. The flap is characterized by a long subcutaneous pedicle of up to 20 cm. The pivot point is in the supraclavicular region and allows the flap to be used in the upper chest, neck, chin, and cheek. In this article, we introduce the anatomic features and present clinical cases underlining the surgical possibilities of the flap in reconstructive procedures with expanded indications.

Thoracic duct cannulation via a transjugular vascular endoscope (lymphoscope) in the dog.

We describe the feasibility of transjugular endovascular visualization and cannulation of the canine cervical thoracic duct using a flexible cholangioscope modified for lymphoscopy. The lymphoscope is composed of a prismatic side-view optic with a catheter guide channel attached by a metal cap to the terminal end. The technique has clinical applicability to the study of neoplastic diseases, examination and depletion of lymphocyte subpopulations in immunologic disorders, and perhaps the overcoming of increased resistance to lymph flow at the cervical lympho-venous junction in edematous conditions characterized by excessive lymph formation.

Cultural differences in a family planning clinic.

An analysis of the use of contraception in a multi-racial family planning clinic in relation to age, parity and ethnicity (Asian or non-Asian) was carried out in the London Borough of Newham. Whether follow-up appointments were given and kept and the taking of cervical smears were noted. Asian patients favoured the sheath (44%) and the IUD (31%); the Pill was less used at 20% with the cap little used at 3%. Non-Asian patients favoured the Pill (50%), and IUD (22%) with the sheath being used by 15% and the cap by 13%. Parity increased with age in both groups. The 16-20-year-old Asian patients chose the Pill (54%) in preference to the sheath (27%) whereas the older Asian patients chose the sheath in preference to all other methods. When follow-up appointments were given the failure rate was equal in both groups. 29% of the Asian patients had a cervical smear taken as against 45% of the non-Asian patients.
