Oval lipid corneal opacities in beagles: ultrastructure of normal beagle cornea.

The ultrastructure of normal Beagle cornea was similar to the human cornea, except the Beagle lacked an anterior limiting lamina (Bowman's layer) and banding in the anterior portion of the posterior limiting lamina (Descemet's membrane). The epithelium was an average of 19 cells thick with 3 basic types: squamous, polygonal, and basal. The subepithelial stroma with a mean thickness of 9 microns was hypocellular, and the bundles of collagen were randomly oriented. The majority of the stroma consisted of closely packed, parallel collagen lamellae interspersed with keratocytes and occasional nerve fibers. The posterior limiting lamina was homogeneous and thick with increasing age. The endothelial cells formed a monolayer and had apical tight junctions.

The cornea and disorders of lipid metabolism.

Disorders of lipid metabolism, either hyperlipidemia or hypolipidemia, are associated with the formation of corneal opacities. Corneal arcus, the most commonly encountered peripheral corneal opacity, is frequently associated with abnormal serum lipid levels, but may occur without any predisposing factors. Reports also have linked corneal arcus with alcoholism, diabetes mellitus and atherosclerotic heart disease. Unilateral arcus is a rare entity that is associated with carotid artery disease or ocular hypotony. Diffuse corneal opacities associated with hypolipidemic disorders such as LCAT deficiency, fish eye disease and Tangier disease, may be the initial manifestation of these disorders and puts the ophthalmologist in a position to make an early diagnosis. Corneal arcus, along with a central corneal opacity, is seen in Schnyder's crystalline stromal distrophy. The association of the disorder with a dyslipidemia remains controversial. A review of lipid metabolism, corneal arcus and several disorders of lipid metabolism that affect the cornea are presented.

Ocular findings in mannosidosis.

Three patients with typical features of mannosidosis and deficiency of alpha-mannosidase activity, who were examined ophthalmologically, had similar lenticular opacities. Corneal opacities were absent. Chamber angle and striking ophthalmoscopic anomalies occurred in two young patients who had normal electroretinograms. Two patients had strabismus. Conjunctival biopsy specimens morphologically confirmed the lysosomal nature of this disorder.

Protective effect of a synthetic polynucleotide complex (poly I: C) on ocular lesions produced by trachoma agent in rabbits.

Live trachoma agent (TE55) injected into the anterior chamber of the rabbit eye produced the following agent-specific ocular lesions: corneal opacity, corneal neovascularization, and iritis. Three intravenous injections of 1 mg of a potent interferon (IF) inducer, polyriboinosinic acid-polyribocytidylic acid complex (poly I:C), markedly suppressed the ocular lesions produced by the trachoma agent but failed to inhibit the growth of the agent in the ocular tissues. Furthermore, a single intravenous injection of 1 mg of polyriboinosinic acid or of polyribocytidylic acid was also protective, although each failed to induce any detectable circulating IF. These results indicate that a factor or factors other than IF may play a role in the suppression of the ocular lesions produced by trachoma agent in poly I:C-injected rabbits.

Effect of metalloproteinase inhibitor on corneal cytokine expression after alkali injury.

PURPOSE: Interleukin (IL)-1alpha and IL-6 levels in the cornea are greatly elevated during the early stages after an alkali burn in mice. The authors investigated the effect of synthetic inhibitor of matrix metalloproteinase (SIMP) on the expression of inflammatory cytokines in alkali-burned murine corneas and evaluated the clinical appearance of the eyes. METHODS: After 0.5N NaOH-alkali burns to 400 corneas of ICR mice, 200 received 400 microg/ml of SIMP topically 4 times a day while 200 corneas were similarly treated with vehicle only. At days 4, 7 and 14 after injury, each cornea was assigned a clinical score for corneal opacity, corneal epithelial defect, hyphema and cataract. Extracts of injured corneas in each group were then assayed for cytokine production using ELISA systems for IL-1alpha, IL-1beta, IL-6 and tumor necrosis factor-alpha (TNF-alpha). RESULTS: The levels of IL-1alpha, IL-1beta and IL-6 were significantly lower in the SIMP-treated group than in the vehicle-treated group 7 days after the burn. However, levels of these cytokines were similar in the SIMP and non-SIMP groups at days 4 and 14. Levels of TNF-alpha did not differ between both groups at any postinjury time. In the SIMP-treated corneas, there was less opacification and hyphema formation and epithelial regeneration was faster. CONCLUSIONS: Topical application of SIMP in alkali-burned murine corneas reduced the expression of IL-1alpha, IL-1beta, and IL-6 and lessened the severity of the injury.

[Safety evaluation of NK 631. Antigenicity, effect on delated hypersensitivity, irritative effect on eye mucous membrane and mutangenicity of pepleomycin (NK 631) (author's transl)].

1. Whether NK 631 is antigenic to guinea pigs and rabbits was studied by the methods of active and passive anaphylactic shock tests, Schultz-Dale reaction, passive cutaneous anaphylaxis, Ouchterlony, tanned red cell haemagglutination test and test according to the U.S. Appraisal of the Safety of Chemicals in Foods, Drugs and Cosmetics. However, none of the tests proved NK 631 to be antigenic. 2. The immunosuppressive effect of NK 631 was studied by delayed hypersensitivity to picryl chloride in normal and L-1210 tumor bearing mice. Therapeutic dosis of NK 631 was no immunosuppressed but toxic dosis of NK 631 was slightly decreased in ear thickness of delayed hypersensitivity. 3. The acute irritative effect of NK 631 and of bleomycin was studied by single instillation to the rabbit eye mucous membrane with 0.1 ml of either of 10, 33 and 100 mg/ml solution of the drugs in physiological saline. The irritative effect of NK 631 on the eye mucous membrane at each concentration was slightly severe than that of bleomycin at the same concentration. However, the manifestations were only mild to moderate dilatation of the conjunctival and nictating membrane blood vessels and eye mucous, and recovered or were mitigated 48 hours after the instillation. No severe changes such as corneal opacity, corneal desquamation, swelling and deaquamation of the conjunctival and nictating membrane were observed. The histopathological examination revealed no striking changes. 4. Mutagenicity of NK 631 and of bleomycin on Salmonella typhimurium strain TA 100 and TA 98 was studied. It was definitely shown that neither NK 631 nor bleomycin exerted any mutagenic action on either test strains.

Linkage of IL-6 with neutrophil chemoattractant expression in virus-induced ocular inflammation.

PURPOSE: Herpes simplex virus (HSV)-1 infection of the murine cornea is known to stimulate a vigorous interleukin (IL)-6 response, but whether this pleiotropic cytokine is an essential participant in corneal inflammation is unclear. This study was designed to compare the early inflammatory response in IL-6 gene-deficient mice to that in wild-type hosts. METHODS: Gene knockout and wild-type mice (C57BL/6 background) were infected intracorneally with HSV-1 (strain RE) and observed through clinical examination and immunohistochemistry for the development of corneal opacity. Virus corneal titers were determined by standard plaque assay on Vero cells. Cytokine and chemokine levels in corneal lysates were measured with commercial ELISA kits. RESULTS: Corneal opacity in IL-6(-/-) mice was substantially diminished in comparison with IL-6(+/+) hosts 24 to 48 hours after intracorneal viral infection, and corneal levels of (MIP)-2 and MIP-1alpha were significantly reduced. Local administration of IL-6 at the time of infection restored corneal opacity and chemokine levels to that of wild-type hosts. Antibody neutralization of endogenous IL-6 in IL-6(+/+) animals reduced corneal opacity scores and MIP-2 levels to that of IL-6(-/-) mice. Ex vivo studies with excised corneal buttons revealed that uninfected IL-6(-/-) corneas injected with IL-6 produced MIP-2 and MIP-1alpha at levels comparable to that seen in IL-6(+/+) hosts. CONCLUSIONS: Collectively, these results suggest that IL-6 promotes corneal inflammation by acting in an autocrine-paracrine fashion to induce resident corneal cells to make MIP-2 and MIP-1alpha, which in turn recruit neutrophils to the virus infection site.

Acute corneal necrosis after excimer laser keratectomy for hyperopia.

OBJECTIVE: To describe a new, rare clinical complication after routine excimer laser photorefractive keratectomy to correct hyperopia. DESIGN: Case report with clinicopathologic correlation. MAIN OUTCOME MEASURES: Four weeks after treatment with excimer laser, a perforating keratoplasty was performed for persistent corneal opacities. The corneal button was examined using light and electron microscopy. Special immunohistochemical stains were used to detect apoptosis. RESULTS: The patient developed corneal opacities, endothelial precipitates, and a fibrinous exudate in the anterior chamber after the laser treatment. The changes did not respond to therapy directed against bacteria, fungi, and Acanthamoeba. All examinations and special stains were negative for micro-organisms. By light microscopy, an anterior zone of corneal necrosis was present with a moderate amount of acute inflammatory cells. At the interface between necrotic and viable corneal stroma, keratocytes with typical features of apoptosis were detected by immunohistochemistry and electron microscopy. CONCLUSION: This is the first full histopathologic report of a case of acute corneal necrosis with signs of apoptosis after excimer laser therapy of the cornea. Surgeons should be aware of this rare but potentially severe complication.

[Corneal ring opacity (Ascher ring)--a case report].

PATIENT: A 63-year-old man without complaints presented with a bilateral symmetric opacity of the cornea between the mid and the outer periphery which was classified as Ascher's ring. CONCLUSIONS: Ascher's ring is a very rare entity. Characteristic slit lamp features allow its differentiation from other anular corneal opacities. Corneal or systemic alterations of the lipid metabolism can be discussed. Heredity can be suspected but could not be proved as yet. The visual prognosis is excellent. Therapy is not necessary.

Ring stromal opacity in lens-corneal adhesion.

Severe congenital corneal clouding may be associated with abnormal anterior segment development (dysgenesis). Often, congenital corneal opacities preclude visualization of intraocular anatomy. In this report, five eyes of four patients with congenital corneal clouding and lens-corneal adhesion (LCA) are described. All had a ring- or doughnut-shaped haze within the stroma surrounding a central clear corneal zone delineating the adhesion. Recognition of LCA is pivotal in planning surgical therapy for eyes with congenital corneal clouding. As a clue to recognition of this anomaly, the ring-shaped stromal opacity may aid in diagnosis of this phenomenon and in avoidance of surgical complications.

[Various forms of opacities of granular corneal dystrophy].

Opacities in granular corneal dystrophy are examined as units as overall opacity patients. Unit opacities are individual spots and can be qualitatively divided into three basic groups: drop-shaped, crumb-shaped and ring-shaped. The typical overall opacity pattern is ray or disk-shaped. The different opacities enable the disease to be classified into early, middle and late stages. Recurrences within grafts show the same characteristics.

An objective method for the evaluation of eye irritation in vivo.

It is generally agreed that intra- and interlaboratory variations in the evaluation of eye irritants are mainly due to the subjective judgement of eye lesions. The scoring of the palpebral conjunctivae is a typical example. To eliminate these difficulties in assessing eye irritants the relationship between corneal swelling, which can be determined using an objective technique, and the reading of other symptoms was investigated. The in vivo eye irritation assay was carried out on 34 substances according to the protocol laid down in Annex V, part B of Directive 79/831/EEC of the European Community on the classification, labelling and packaging of dangerous substances. Results were obtained for erythema, oedema, corneal opacity and corneal swelling. Erythema, oedema and opacity were evaluated according to the interpretation rules laid down in Annex VI, part IID of the EEC Directive 83/467/EEC. Corneal swelling was determined by the ultrasonic pachometer technique. Good correlations were found between the mean percentage corneal swelling after 24, 48 and 72 hr and the mean corneal opacity (r = 0.94) and erythema scores (r = 0.93) after the same observation times. The corneal swelling scores measured after 24 hr using the ultrasonic pachometer are comparable with those reported in the literature, which were obtained by optical pachometry. The good correlation with the other effects scored and the good repeatability of the results opens the possibility of introducing an objective and sensitive method into the ocular irritation assay and so reducing intra- and interlaboratory variations.

Congenital insensitivity to pain with anhidrosis: ocular and systemic manifestations.

PURPOSE: To report the incidence and severity of the ophthalmologic manifestations in patients with congenital insensitivity to pain with anhidrosis. METHODS: Fifteen Bedouin children with congenital insensitivity to pain with anhidrosis underwent complete ocular examination, including refraction and assessment of corneal sensation, and a detailed neurologic examination, including measurement of median nerve motor and sensory conduction. Patients with corneal ulcers were treated appropriately. RESULTS: In the 15 children (eight girls and seven boys, with a mean age of 3.75 +/- 2.67 years; range, 9 months to 9 years), corneal sensation was absent in both eyes. Corneal opacities were present in 10 children, five of whom had bilateral corneal opacities. Corneal ulcers were found in seven children, two of whom had bilateral ulcers, and in three children the ulcers recurred. The corneal ulcers were characterized by very poor healing. The surgical procedures included four lateral tarsorrhaphies, two corneal patch grafts, and one penetrating keratoplasty. All the patients had self-inflicted injuries varying from skin ulcers, burns, and bone fractures to autoamputations of fingertips and tongues. Many patients showed delayed healing and repair of bone and skin injuries. All patients had attacks of hyperpyrexia, moderate mental retardation, and hypotonicity with absent superficial sensation to light touch. Results of median nerve motor and sensory conduction studies were within normal limits. CONCLUSIONS: The patients with congenital insensitivity to pain and anhidrosis and absent corneal sensation showed a marked tendency to develop corneal ulcers that healed poorly. Congenital insensitivity to pain and anhidrosis, although rare, should be considered in the differential diagnosis of neurotrophic keratitis.

Combined surgery with deep lamellar keratoplasty.

The opportunity to perform two or more ophthalmic operations simultaneously has increased in recent years. In keratoplasty too, the simultaneous operation consisting of cataract extraction and IOL insertion (triple procedure) has become common. Penetrating keratoplasty (PKP) and planned extracapsular cataract extraction (PECCE) or vitrectomy is today often done simultaneously. However, phacoemulsification and aspiration (PEA) or 3 ports vitrectomy has to be performed on patients with corneal opacity due to some corneal diseases. In such cases, we performed the PEA or vitrectomy coupled with deep lamellar keratoplasty (DLK), successfully applied to closed eye surgery with corneal opacity. Simultaneous surgery using DLK, free of postoperative endothelial type of rejection, and PEA, enabling cataract extraction by a small incision, is a surgical technique making use of the advantage of the two types of operation. We performed the DLK + PEA + IOL on 17 cases. In the other hand, simultaneous surgery consisting of DLK and vitrectomy may be taken into consideration. The use of DLK will make it possible to perform surgery on urgent vitreoretinal diseases with corneal opacity and cataract. We performed the DLK + 3 ports vitrectomy on 3 cases. The current study describes the advantages and disadvantages of these types of operation.

A Japanese family with Grayson-Wilbrandt variant of Reis-Bücklers' corneal dystrophy.

A 50-year-old man (the proband) experienced recurring attacks of ocular irritation and had had bilateral ring-shaped anterior corneal opacity since adolescence. Corneal sensation was normal. The proband's 15-year-old daughter also had recurring episodes of ocular irritation and bilateral fleck-like anterior corneal opacities. The proband's deceased father and brother reportedly had similar corneal lesions from adolescence. We believe that the corneal findings in this family may be similar to those found in patients with the Grayson-Wilbrandt variant of Reis-Bücklers' corneal dystrophy.

Effect of ketorolac on herpes simplex virus type one ocular infection in rabbits.

Corticosteroids can exacerbate viral ocular infections. Ketorolac tromethamine is an effective nonsteroidal anti-inflammatory agent that may be a useful substitute for corticosteroids following ocular surgery. In this study, rabbits ocularly infected with herpes simplex virus type 1 (HSV-1) were treated topically four times daily with 0.5 percent ketorolac or 0.1 percent dexamethasone for 7 days after infection. Severity of the infection was determined by scoring corneal opacity and HSV-1 corneal ulcerations with the Draize scale as well as iritis and conjunctivitis. Ten days after treatment ended both the corneal opacity scores (1.5 out of 4) and HSV-1 corneal ulcerations (0.3 to 0.7 out of 4) were similar for ketorolac and the vehicle, indicating no exacerbation of the infection, whereas with dexamethasone these scores were increased (3.6/4 and 3.4/4, respectively). Furthermore, both iritis scores (0.5/2) and conjunctivitis scores (1.3 to 1.4/10) were also similar for ketorolac and the vehicle, while dexamethasone increased both iritis (1.8/2) and conjunctivitis (4.3/10) compared to vehicle. Thus, ketorolac appears to be an anti-inflammatory agent that does not worsen viral ocular infection.
