Albrecht Ludwig Berblinger--inventor of the spring prosthesis and hang-glider (1811).

Albrecht Ludwig Berblinger (1770-1829), known as the "Flying Tailor of Ulm", started with flight experiments in Ulm, Germany, in the early 19th century. He gained experience in downhill gliding with a maneuverable airworthy semi-rigid hang-glider and then attempted to cross the Danube River at Ulm's Eagle's Bastion on the 31st of May 1811. The tricky local winds caused him to crash and he was rescued by fishermen, making him the first survivor of a water immersion accident of a heavier-than-air manned "flight machine". Though he failed in his attempt to be the first man to fly, Berblinger can be regarded as one of the significant aviation pioneers who applied the "heavier than air" principle and paved the way for the more effective glide-flights of Otto Lilienthal (1891) and the Wright Brothers (1902). Less known are Berblinger's significant contributions to the construction of artificial limbs for medical use, as well as the spring-application in aviation. His invention of a special mechanical joint was also used for the juncture of the wings of his "flying machine". Because of his worthwhile contributions to medicine and flight, in 1993 the German Academy of Aviation Medicine named an annual award for young scientists in the field of aerospace medicine in his honor.

[Vertebral prostheses in surgical treatment of metastatic disease].

Metastasis disease is often located at the spine, especially with solid tumors where frequency can reach 80%. Indications for metastasis operational approach are threatening or existing neurologic deficit, static dislocations or even unbearable pain. Operation treatment may be extensive, with radical metastasis removal or even partial one with decompressive laminectomy and tumor reduction and spine stabilization. Using various kinds of alenthesis, the authors have used their own vertebra artificial limb in 10 cases, which enables spine stabilization after radical vertebra removal. They report their experience in the essay.

Computer use in the medical specialties.

This article summarizes new and innovative uses of the computer in various medical applications. It highlights the use of this technology in the fields of Cardiology, Radiology, Rehabilitative Medicine, Laboratory Medicine, and so on. It also discusses future uses in aiding the handicapped patient to ambulate, the deaf to hear, and the blind to see.

[A rehabilitation unit for geriatric amputees (author's transl)].

Elderly leg amputees are patients with the most somatic, emotional and social problems requiring rehabilitation assistance. Expensive artificial limbs often cannot be used because many detailed questions are left to chance. In order to coordinate the rehabilitation of these patients in an optimal manner and utilise the invested means efficiently, a specialised rehabilitation centre was organised at the Medizinisch-Geriatrische Klinik I of the Felix-Platter-Spital Basle and its mode of organisation was intentionally designed to be simple and flexible. The rehabilitation team consists of staff members of the Orthopedic and Geriatric Department of the hospital. From 1974-1978, 59 geriatric amputees aged 60-90 years were cared for and 75% could later be discharged to their own homes. In spite of the advanced age and various illnesses of these patients, the results of the rehabilitation unit are encouraging. However, prime importance should be given to finding a suitable milieu after dismissal.

[Transgenicular amputation: an alternative to thigh amputation?].

Since 1976 we attempted to avoid an above-knee (AK) amputation in favor of a knee disarticulation. From 1976 to 1979 83 patients with an average age of 72 years could be amputated through the knee and 55 patients with average age of 73 years had to be submitted to an AK-amputation. As compared to the AK-amputation the transgenicular (TG) resection has the following advantages: 1) Lower postoperative mortality (TG 10%, AK 33%). --2) Higher prosthetic fitting rate (TG 77%, AK 61%). --3) Significantly improved rehabilitation of the patient (walking with artificial limb: TG 67%, AK 20%). --The results of a TG-amputation may be impaired by disturbances of wound healing with subsequent necrosis and/or infection. Among our patients every forth amputation stump following TG-resection had to be reamputated at above-knee level.

[Mycetoma: attempt at intra-arterial management].

A 28 year old African student had acquired a mycetoma of his left foot after an injury in 1975. As the surgical and antimycotic therapy had no effect, he was brought to Düsseldorf for the amputation of his foot. Madurella mycetomi was isolated from the skin, pus and bone. Before the amputation was made a last attempt to treat him with the antimycotic agent miconasol nitrate, administered intraarterially through the arteria tibialis posterior, which had been shown i arteriography to perfuse the whole foot. On the 23rd day of therapy pain and fever indicated bacterial infection. The fungus could still be isolated from the bone tissue. The therapy was terminated on the 30th day. The catheter was blocked and amputation of the food became inevitable. The patient received an artificial limb and is able to move freely. In similar cases of mycotic bone infection an attempt with intraarterial infusion of an antimycotic agent should be made immediately.

Would people with a disability in the highlands benefit from a community-based rehabilitation program?

Community-based rehabilitation for those people with a physical disability is a policy promoted by the World Health Organization. In order to assess whether such a program might be useful to the disabled people of the highlands of Papua New Guinea a survey was conducted which first identified those people with a severe physical disability and then investigated the degree that they were handicapped in terms of their social and physical environment. The survey was performed in the Tari area of the Southern Highlands Province in conjunction with the Papua New Guinea Institute of Medical Research (PNGIMR). By using the PNGIMR demographic surveillance system it was possible to calculate a prevalence level of physical disability (including walking disability, deafness and blindness) of 46 per 10,000 (0.46%). A significant physical disability was identified in 114 people, of whom 54 were given personal interviews to establish their quality of life. The survey found the social and economic situation of disabled people to be generally good although there is a lack of basic material aids for handicapped people. The disabled people and their families do not need extra knowledge about how to care for their disabilities and a formal community-based rehabilitation program would be inappropriate, but they would find mobility aids such as wheelchairs and artificial limbs of enormous help. The regular health care services are widely used although both people with a disability and health service staff are not always aware of the types of treatment which can usefully be provided.(ABSTRACT TRUNCATED AT 250 WORDS)

[Extremity gangrene caused by rare co-infection of multiple bacteria: a case report].

UNLABELLED: Because of progressive infectious necrosis on right hand and forearm for 3 months and the necrosis on left mid-finger for 2 months, a girl patient, Yang Xiaoxia was admitted on 15th, Nov. 1994. Systemic intoxicative symptom was slight and the necrotic tissue presented black scar. After 3 months treatment by application of multiple antibiotics, local dressing changes and an amputation of right froearm, the necrosis could not be stopped but keeping deteriorating. Through experts' consultation, systemic surpportive therapy, wide-spectrum antibiotics cilastatin sodium and complete debridement were adopted. One week later, the fresh surface of granulation tissue was seen, and skin grafting was performed. The wound healed 3 months later. After further rehabilitation, the left thumb, index and fifth finger showed normal function. A mechanical motional artificial limb was installed on right forearm and discharged. The deep necrotic tissue of left mid-finger was examined. Results of culture from several hospital presented 2 kinds of aerobic bacteria and 10 kinds of the anaerobic, of which, the names of 3 kinds are still unknown. Pathological examination showed chronic pyogenic necrotic infection, spreading from skin to deep muscles and interosseous membrane. And infiltration of eosinophilic granulocytes was found in the infectious region. CONCLUSION: This patients is progressive gangrene of the extremity caused by rare co-infection of multiple bacteria. The characteristics were slight systemic intoxicative symptom, local progressive necrosis and formation of black scar. By means of systemic surpportive therapy, application of wide-spectrum antibiotics cilastatin sodium, complete debridement and skin grafting, the patient was cured.

[Total aortic arch replacement for distal aortic arch aneurysms].

Between January 1991 to April 1997, 13 consecutive patients underwent the total aortic arch replacement for distal aortic arch aneurysms. The operation was performed through the median sternotomy and the cardiopulmonary bypass was instituted with the cannulation of ascending aorta for arterial perfusion and of right atrium for venous return. Then the aortic arch was totally replaced with four limbed artificial graft with the aid of selective cerebral perfusion and open distal anastomosis of the distal end of the aneurysm. The mortality was 16% (2 patients) and these cases had ruptured aneurysm and had deep shock preoperatively. There were no severe cerebral complication. Total aortic arch replacement would be the acceptable procedure for the distal aortic arch aneurysm and antegrade ascending aortic perfusion was useful method to avoid the thromboembolic attack due to the retrograde femoral perfusion.
