[A case of isolated tricuspid regurgitation caused by a dilated tricuspid annulus treated by ring annuloplasty].

Isolated tricuspid regurgitation is rare. A 64-year-old man was admitted to our hospital with a complaining of shortness of breath and edema of lower extremities. Chest X-ray showed remarkable cardiomegaly (cardiothoracic ratio, 73%). Echocardiogram demonstrated severe tricuspid regurgitation and dilated right atrium. At operation, the tricuspid annulus was severely dilated about 7 cm in diameter, and all leaflets of tricuspid valve were intact. Prosthetic ring annuloplasty with a 32 mm Carpentier-Edwards ring was performed. Postoperative course was uneventful. Four weeks after operation he was discharged from the hospital and he is now leading a normal life. Careful follow up for long time is essential in this case.

[Evaluation of the relationship between deep venous thrombosis and pulmonary embolism with radionuclide techniques].

OBJECTIVE: The purpose of this study was to evaluate the relation between deep venous thrombosis (DVT) and pulmonary thromboembolism (PTE) by radionuclide imaging. METHODS: One hundred forty patients with PTE from September 1997 to March 2001 at this institution was confirmed by clinical manifestation, pulmonary perfusion (PPI)/ventilation scan (PPV) and deep venous radionuclide venography (RNV), which were performed in all patients. There were 79 males and 61 females, with an average age of 39 +/- 18 years. Twenty-six cases underwent pulmonary angiography; 11 underwent X Ray venography of lower extremities (XRV); 18 underwent impedence plethymography (IPG); and 36 underwent lower limb echocardiography (UCG). RESULTS: Of the 140 patients with PTE, 120 (85.7%) had lower limb venous pathological changes. Among them, 94 patients had risk factors for DVT. The agreement rates of RNV with XRV, UCG and IPG were 90.9%, 72.2% and 80.0%, respectively. CONCLUSIONS: The results indicated that DVT was highly prevalent in patients with acute pulmonary embolism. Thrombosis was a primary pathogenic factor for PTE, and thrombi were mostly from proximal veins. (99m)TC-MAA radionuclide imaging was a useful method for noninvasive detection of DVT and PTE.

Localized argyria 20-years after embedding of acupuncture needles.

We report a 66-year-old woman with localized argyria caused by embedding of acupuncture needles. Ten years after she had received acupuncture, she noticed two asymptomatic bluish macules on her right arm. A biopsied specimen from the macule revealed many brownish-black granules mainly located around the sweat glands and the blood vessels in the dermis. The X-ray examination of the extremities revealed numerous needle-like fragments around her extremities. "Embedding of needles" induces some serious adverse events. We should know the adverse events for the safety and health of patients.

[A case of non-specific interstitial pneumonia in patient with microscopic polyangiitis].

Non-specific interstitial pneumonia developed as an initial manifestation in a patient with microscopic polyangiitis. A 62-year-old man was admitted to our hospital in March 2001, because of fever and intermittent myalgia of lower extremities. Chest X-ray had revealed reticular shadows in the bilateral middle and lower lung fields since 1996. Just before admission, the patient had been diagnosed as having nonspecific interstitial pneumonia (NSIP) from the specimen obtained by video-assisted thoracoscopic surgery (VATS) in another hospital. Physical examination on admission revealed bilateral episcleritis. Laboratory data showed elevated levels of CRP and KL-6, polyclonal gammaglobulinemia, positive rheumatoid factor and myeloperoxidase-antineutrophil cytoplasmic antibody (MPO-ANCA). Sensory and motor nerve conducting velocities were delayed in left peroneal nerve, but not other nerves, suggesting mononeuropathy. Biopsied specimens of the left quadriceps revealed vasculitis of arteioles. In spite of positive proteinuria and hematuria, no pathological lesion was found in the kidney. From all of these findings, the patient was diagnosed as having microscopic polyangiitis (MPA) without renal involvement. Methylprednisolone pulse therapy followed by intravenous cyclophosphamide pulse therapy improved his clinical conditions such as pyrexia, cough, myalgia, episcleritis and respiratory symptoms with decreased titer of serum MPO-ANCA. Thereafter, the dose of prednisolone was successfully tapered to 10 mg/day without clinical relapse. In the present patient who developed demonstrated non-specific interstitial pneumonia as an initial manifestation of MPA, VATS provided useful diagnostic and prognostic information, leading to an appropriate therapeutic choice.

Ozonation of persistent DOC in municipal WWTP effluent for groundwater recharge.

Groundwater recharge is becoming common in areas where the withdrawal of groundwater exceeds its natural recharge. Wastewater treatment plant (WWTP) effluent can be used for this purpose, but persistent organic compounds can only be partly removed during soil passage. This point was confirmed in degradation tests using soil columns in which the DOC of a membrane bioreactor (MBR) permeate could only be reduced by 15% and adsorbable organic iodine (AOI) by 2%. However, ozonation was found to improve biodegradation and at the maximum formation of biodegradable DOC at 2.5 mg O3/mg DOC0, the DOC values of MBR permeate (11-13 mg/L) could be reduced in aerobic degradation batch tests to the DOC of Berlin drinking water (3-5 mg/L). A combination of ozonation at 1.9 g O3/g DOC0 with soil passage could adjust DOC, UVA254, colour (436 nm) and the molecular DOC size distribution to drinking water ranges, but AOI was only reduced from 143 microg/L to 92 microg/L and remained high compared to tap water (2.2 microg/L). The extremely persistent X-ray contrast compound iopromide, which represents part of AOI, was spiked into MBR permeate at a low concentration. Iopromide was reduced by 88% during ozonation, but AOI only decreased by 23% indicating that a transformation, but not a mineralization, of iodinated organic compounds occurs.

[Cervical spinal cord injury caused by a broken acupuncture needle: a case report].

Spinal cord injury is a rare but considerable complication of acupuncture. A case with cervical spinal cord injury caused by a broken acupuncture needle was reported and sixteen previously reported cases including our case were reviewed. A 49-year-old woman was treated by herself with acupuncture on the nuchal region for occipitalgia, and the needle was accidentally broken during the treatment. Six hours later she noticed pain and numbness in the right upper and lower extremities. Neurological examination revealed slight impairment of temperature, pain and touch sensation on the right extremities. Plain X-ray film and CT scan showed a broken needle in the interspinous ligament between C1/C2 vertebrae, the tip of the needle appearing to be in the spinal canal. The needle was removed surgically nineteen days after the accident. Intraoperative fluoroscopic monitoring with injection of dye enabled the needle to be found without difficulty. Postoperative course was uneventful, and her pain and sensory impairment gradually disappeared. Once the diagnosis for cervical spinal cord injury by an acupuncture needle is made, the needle should be removed surgically as soon as possible, especially prior to the development of motor symptoms. This is because movement of the needle in the spinal cord is considered to be a main possible cause of the development and progression of symptoms.

[Changes in the locomotor system in inflammatory intestinal diseases. III. Clinical aspects of seronegative spondyloarthritis].

This investigation is a follow up study of our previous study (Sborn. lék., 90, 1988, 140-146). In the same group of 119 patients with inflammatory intestinal diseases (Crohn's disease - 37, ulcerative colitis - 50, celiac sprue - 32) we followed further affection of locomotor system, peripheral arthritides. Inflammatory manifestations in peripheral articulations were manifest in 11 (9.2%) patients. Among common features of enteropathic arthritides belong: asymmetrical mono-oligoarthritis, in 67% of cases in lower extremities, without X-ray structural manifestations - erosive changes in the whole course, without functional deficit, inflammatory nature of the exudate, absence of rheumatoid nodes, RF negativity, episodic course. On the contrary in 3 (27%) patients the course was chronic. Incidence, relapse, activation of arthritis depends in 18% on activity and in 17% on intestinal complications of the underlying disease. Surgical treatment of small and/or large intestines lead in 60% of cases to remission of peripheral arthritis. In 1 patient (with celiac sprue) synoviorthesis with radioactive coloid 90Y was performed.

[Acute epidural hematoma of the posterior fossa caused by forehead impact].

A rare case of acute epidural hematoma of the posterior fossa caused by forehead impact is reported. This 36-year-old man fell from a truck and hit his face. He was conscious and was brought to our center 30 minutes after the injury. On admission, a contused wound of the right forehead was noticed. He was restless and had severe pain in the neck and upper extremities. Skull X-ray showed a linear fracture of the frontal bone and computed tomography (CT) scan was normal. He continued to be restless and sudden respiratory arrest and pupillary dilation occurred 10 hours after the admission. A CT scan revealed a lenticular high density area in the left posterior fossa which extended to the supratentorial region. The 4th ventricle was compressed and displaced to the right and also the quadrigeminal and ambient cisterns were not visualized at all. Immediate surgery disclosed a 30 g epidural hematoma of the left posterior fossa and the supratentorium and the clot was completely evacuated. The source of bleeding could not be identified. Opening of the dura revealed contusion in the occipital lobe. He died on the 17th postoperative day. The possible mechanism in the production of the posterior fossa hematoma in this case is discussed.

[Possibility of biological indication of radiation injury from the blood levels of deoxynucleosides and deoxynucleotides in combined radiation-mechanical injuries].

A study was made of the dependence of a total content of deoxynucleosides and deoxynucleotides in blood serum and blood clot leukocytes upon radiation dose and time lapsed after radiation- and radiation-mechanical affection. It was established that after radiation-mechanical affection this dependence was different from that observed after the effect of radiation alone. From the analysis of the data obtained it was inferred that deoxynucleosides and deoxynucleotides could be used for biological indication of radiation damage in conditions of radiation-mechanical affection.

[Current status of the diagnosis and surgical treatment of the postthrombophlebitic syndrome of the lower extremities and pelvis].

The clinical, phlebographic, and hemodynamic signs of the postthrombophlebitic syndrome were studied. In 99.3% of patients the disease was found to be attended with edema of the extremities, in 100%--with the pain syndrome, in 64.2%--with pigmentation, in 59.7%--with tissue induration, and in 53.6% of patients--with trophic ulcers. A study of microcirculation revealed a significant increase of the time of tissue resorption. The venous pressure proved to be increased; the degree of its normalization is an objective test for the success of the operation. The volume blood flow in the affected extremity was 1.8 times that in the normal extremity. Contrast X-ray examination of the veins showed phlebographic signs of the disease. Reconstructive operations on the deep veins were performed in 109 patients; 85.4% had good remote results. Linton's operation was performed on 29 patients, Cockett's operation--on 17, and Felder's operation--on 8 patients; good remote results were noted in 75.7% of cases. For determining the tactics of the treatment and its prognostication, a classification of the postthrombophlebitic syndrome is suggested.

[Dysplasia epiphysialis hemimelica (author's transl)].

The first report of dysplasia epiphysialis hemimelica in the spanish literature is presented. The case has peculiar clinical features as the early presentation (two months) and to involve lower and upper extremities. On X-ray examination exostosis and calcium deposits were shown on the most affected epiphysis. Microscopically an osteochondroma, anomalous vascular channels, non hitherto described, were detected, as well as multivacuolated cells into them although ultrastructural and histochemical studies were carried on the precise nature of these cells could not be established. The possible pathogenetic significance of these findings is discussed.

[Efficiency and efficacy of radiologic diagnosis in skeletal trauma in childhood and adolescence].

In a retrospective analysis, 2,006 X-ray examinations of the skeleton obtained after trauma in 1,386 children were examined; 354 fractures were diagnosed. X-ray films of the skull were requested in 596 (29.7%); fractures were diagnosed in only 13 patients (2.2%). The radiological result changed the medical management in none of these cases. On the other hand, X-ray after trauma in the extremities is indispensable, except for the knee region. Examinations of the lower arm showed a fracture in 54.9%, X-ray of the shoulder 47.7%.

[Postlaminectomy kyphosis of the cervical spine complicating spinal cord tumor in the foramen magnum].

Postlaminectomy kyphosis is rarely encountered as a sequela of decompression of the upper cervical spine, especially in adults. We present a case of disabling cervical kyphosis which developed after laminectomy for excision of a foramen magnum tumor and was treated successfully by occipito-vertebral fusion. A sixty-one-year old female was treated by laminectomy of the first through to the third cervical spine for a foramen magnum meningioma. One year later, the laminectomy was extended downward to include the fourth through to the seventh cervical spine because of neurological deterioration. Seven years after the first laminectomy, she was referred to our spinal service because of severe neck pain, numbness and muscle weakness of both upper extremities. Lateral X-ray of her cervical spine presented a severe kyphosis. Roentgenogram two months after the first laminectomy already showed slight deformity and, timewise, it corresponded with the time when the patient experienced neurological deterioration. Subsequent X-rays showed progression of kyphosis. After we confirmed a lessening of the pain and improvement of the neurological symptoms during an axial traction using a halo-vest, posterior occipito-cervico-thoracic fusion was performed using Luque segmental spinal instrumentation and autogenous bone graft. After surgery the pain decreased and neurological symptoms improved. It is important to be aware of the complication of kyphotic deformity after laminectomy and once it has developed, it is recommended to stabilize the progression by early spinal fusion.

Digital x-ray imaging: a diagnostic aid for the clinician and a technological instrument for the researcher.

Digital x-ray imaging is a relatively new technology which may have potential for both the oral health care practitioner and the oral researcher. The ability to acquire low x-ray dose images is extremely important to patient care. Additionally, the ability to manipulate these images provides valuable clinical and research information which can enhance overall patient care.

[The coexistence of pulmonary tuberculosis and thymoma a case report].

A 33-year-old male was admitted with complaints of cough, dysphagia, and swelling of face and upper extremities. Chest X-ray and CT scan revealed a large mediastinal mass and infiltrates in the right upper lobe. Percutaneous biopsy proved the mediastinal tumor as thymoma with cellular atypia. After irradiation, the tumor was surgically removed. Caseous epitheloid granulomas were found in the dissected mediastinal lymph nodes. AFB (Acid fast bacillus) stain of the patient's gastric fluid was positive for Mycobacterium. The coexistence of these two diseases was incidental, however, this case suggested that clinicians should perform careful evaluation of lung parenchyma as well as mediastinum on chest X-ray to identify occult diseases including pulmonary tuberculosis in patients with mediastinal mass lesion.

The physical nature of life.

Life evolved from the primeval world of physics. Sensory systems inform animals of the natural environment, enabling them to conduct responsively. The discovery of weak, DC bioelectric fields in the vicinity of aquatic organisms and the role they play in guiding sharks and rays to their prey have led to the recognition of fundamental, hitherto less well known, physical aspects of sensory biology. The inferred cybernetic algorithm of electric-field orientation in sharks and rays is highly effective and extremely robust. In orienting to the weak DC electric fields of ocean currents and to the earth's magnetic field, sharks and rays unwittingly practise the motional-electric principles that Einstein had in mind when he introduced the special theory of relativity. At the sense-organ, receptor-membrane, and ion-channel levels, the elasmobranch ampullae of Lorenzini operate on the basis of graded positive feedback driven by negative conductance, supposedly employing voltage-sensitive ion channels as the active, excitable elements. The electric sense of sharks and rays presents an exquisite implementation of the very biophysical principles that also govern the graded, much richer than all-or-none, integrative brain processes of animal and man.

Bilateral multiple cervical root avulsions without skeletal or ligamentous damage resulting from blast injury: case report.

OBJECTIVE AND IMPORTANCE: We describe a unique case of multiple bilateral cervical root injuries without ligamentous or bony injury secondary to a sandblast accident. CLINICAL PRESENTATION: A 19-year-old man sustained a sandblast injury to his face, neck, chest, and upper extremities, with immediate loss of motor and sensory function occurring in both of his upper extremities. Cervical spine x-rays, computed tomography, and magnetic resonance imaging demonstrated no fracture, soft tissue abnormality, or malalignment. The restriction of deficits to the patient's upper extremities suggested a central cervical spinal cord injury, bilateral brachial injuries, or a conversion disorder. INTERVENTION: Cervical computed tomographic myelography revealed multiple bilateral nerve root injuries. CONCLUSION: This case report is unique in the literature in that it describes a patient with multiple cervical nerve root injuries secondary to sandblast injury without ligamentous or bony injury. Although magnetic resonance imaging remains the diagnostic modality of choice in patients with acute spinal cord injury, it is deficient in demonstrating cervical root injury in the acute setting. In this setting, computed tomographic myelography is superior.

Unstable superheavy relic particles as a source of neutrinos responsible for ultrahigh-energy cosmic rays

Decays of superheavy relic particles may produce extremely energetic neutrinos. Their annihilations on the relic neutrinos can be the origin of the cosmic rays with energies beyond the Greisen-Zatsepin-Kuzmin cutoff. The redshift acts as a cosmological filter selecting the sources at some particular value z(e)+/-deltaz, for which the present neutrino energy is close to the Z pole of the annihilation cross section. We predict no directional correlation of the ultrahigh-energy cosmic rays with the galactic halo. At the same time, there can be some directional correlations in the data, reflecting the distribution of matter at redshift z = z(e)+/-deltaz. Both of these features are manifest in the existing data. Our scenario is consistent with the neutrino mass reported by super-Kamiokande and requires no lepton asymmetry or clustering of the background neutrinos.
