Does habitual, vigorous optokinetic stimulation alter optokinetic nystagmus and sensitivity to circularvection?

Previous studies have shown that experience with optokinetic stimulation can alter a subject's sensitivity to illusions such as circularvection (CV). The aim of the present experiment was to compare optokinetic nystagmus (OKN), optokinetic afternystagmus (OKAN), and sensitivity to CV between 2 groups of sportspeople: 1) squash players (n = 16), who regularly experience vigorous optokinetic stimulation while engaging in their sporting activity, and 2) weightlifters (n = 16), whose sport does not involve the same degree of optokinetic stimulation as squash, but who nevertheless have to achieve a high degree of physical skill. OKN, OKAN (frequency, slow phase velocity, and timeconstant), and latency to CV (Stage 2 and Stage 3) were measured using electro-oculographic recording inside an optokinetic drum. Contrary to predictions, there were no significant differences in OKN, OKAN, or latency to CV between the 2 groups. These results suggest that 1) the practice effects that alter the sensitivity to CV may decay relatively quickly, and 2) differences in recreational sporting activities between subjects may not be a significant confounding factor in visual-vestibular interaction experiments.

[Acute cardiovascular complications of vigorous physical exertion by untrained persons].

Physical exertion has paradoxical effects: regular physical activity offers protection against cardiovascular diseases, but, on the other hand, it has been shown that strenuous exercise can provoke coronary heart disease and sudden death. The risks of vigorous exertion apply particularly to untrained individuals and persons with a sedentary lifestyle. Physical exertion can cause a myocardial infarction or sudden death in people who were considered perfectly healthy before this event. In most cases, the mechanism of this is probably a rupture at the site of a vulnerable atherosclerotic plaque, with local activation of coagulation. Disturbances in cardiac rhythm may also be a cause of these complications.

Detection of a vigorous HIV-1-specific cytotoxic T lymphocyte response in cerebrospinal fluid from infected persons with AIDS dementia complex.

AIDS dementia complex is a common neurologic disorder in later stages of HIV-1 infection. Because virus-specific CTL have been shown to contribute to neurologic disease in certain viral illnesses, we examined the cerebrospinal fluid of HIV-1-infected persons with various stages of AIDS dementia complex for the presence of HIV-1-specific CTL. In five of six subjects studied, HIV-1-specific CTL were identified in the cerebrospinal fluid. These CTL were directed at epitopes within the gag, reverse transcriptase, envelope, and nef proteins and restricted by HLA class I Ag. In four of these subjects, virus-specific CTL were detected in higher numbers in the cerebrospinal fluid compared to the peripheral blood, suggesting a specific recruitment to or local induction within the nervous system. These studies demonstrate the presence of a vigorous and broadly directed CTL response to HIV-1 in the central nervous system of infected persons with AIDS dementia complex, and provide immunologic evidence of localized intrathecal infection. Although HIV-1-specific CTL may serve to inhibit viral replication in the central nervous system, the presence of a persistent CTL response in the central nervous system may also contribute to the neurologic disorders characteristic of HIV-1 infection.

Methicillin-resistant Staphylococcus aureus: need for constant surveillance, stringent control and vigorous treatment measures.

Staphylococcus aureus has shown a disconcerting propensity to develop resistance to antimicrobial agents and an increasing proportion of these have become resistant to methicillin as well. Regular surveillance should be carried out in every hospital and stringent infection control measures and vigorous treatment should be employed to prevent the nosocomial spread of MRSA. A total of 1056 MRSA were isolated from various specimens received in bacteriology division of SKIMS Srinagar out of which 46.32% were from inpatients and 31.87% from outpatients. None of these strains showed resistance to vancomycin and a good proportion were sensitive to amikacin (80.75%) gentamicin (69%) or ciprofloxacin (63.25%).

[Effects of transgenes insertion on pollen vigor and hybrid seed set of rice].

In this paper, the effects of transgenes insertion on the outcross potentiality of rice varieties were assessed by pollen vigor and hybrid seed set. The in vitro pollen germination rates of five transgenic rice lines transformed respectively with bar, crylAb, BADH and Xa21 gene were investigated, and compared with their relative receptor rice varieties. The results showed that there existed a significant difference in in vitro pollen germination rates between receptor rice varieties, but no significant difference was found between transgenic rice varieties and their relative receptors. The in vitro pollen germination rate for transgenic rice varieties varied from 0.416 to 0.584, similar to that of their relative receptors (0.400-0.574). Investigation on the hybrid seed set of 26 hand-crosses showed that the inserted bar or crylAb gene had a significant effect on the hybrid seed set of receptors, while the effect of Xa21 gene was smaller. The hybrid seed set rate of non-transgenic rice in crosses with transgenic rice (pollen donor) was from 0.056 to 0.413, not different from that in crosses with their relative receptors (0.052-0.417). It's suggested that transgenes insertion had little effect on the outcross potentiality of rice varieties.

Comparison of fluorescence polarization immunoassay, enzyme immunoassay, and thin-layer chromatography for urine cannabinoid screening. Effects of analyte adsorption and vigorous mixing of specimen on detectability.

Four commercial assays for the screening of cannabinoids in urine were compared. Urine specimens from 93 selected subjects were run by fluorescence polarization immunoassay on the Abbott TDx; by enzyme multiplied immunoassay with two Syva EMIT assays; and by thin-layer chromatography with the TOXI-LAB system (Marion Laboratories). The TDx cannabinoid threshold can be set anywhere from 25 to 150 micrograms per L. Twenty-five micrograms per L was chosen for this study. The thresholds for EMIT are fixed at 20 micrograms per L for one assay and 100 micrograms per L for the other. The detection limit for TOXI-LAB, according to the manufacturer, can be anywhere from 5 to 50 micrograms/L, depending on the specimen. Urines, positive by at least one method, were further analyzed by gas chromatography with mass spectrometry (GC/MS), The detection limit for the GC/MS method was 10 micrograms per L. The results showed a few false negatives and unconfirmable positives; in general, correlation was considered acceptable. Dose-response curves comparing TDx and EMIT gave paralell results, with comparable cross-reactivity for the major metabolite, 11-nor-delta-9-tetrahydrocannabinol-9-carboxylic acid (delta-9-THC-COOH). A dose-response study of TOXI-LAB using delta-9-THC-COOH also gave acceptable results. Adsorption to glass was investigated using spiked urine; a 27 percent reduction in concentration was caused by this phenomenon. Foaming of spiked urine caused by vigorous mixing resulted in a reversible 89 percent apparent reduction in concentration.

[Etiopathogenetic hypothesis on the course of esophageal dyskinesias regarding particularly the transitional forms between esophageal spasm, vigorous achalasia and decompensated achalasia].

The aetiology of oesophageal motor disturbances (OMD) including achalasia is unknown. Many causes have been hypothesised including: autoimmune inflammatory lesions, mechanical obstruction, degeneration of the dorsal nucleus of the vagus etc. to explain the anatomopathological lesions of Auerbach's plexus. On the basis of the manometric studies carried out so far on motor disturbances of the oesophagus, it is considered that dyskinesia develop into decompensated achalasia. The disturbance would appear to begin as an oesophageal spasm, turn into vigorous achalasia and finally reach the point of decompensated achalasia characterised by hypotonic, synchronous waves and the absence of relaxation of the lower oesophageal sphincter. A personal series of 76 cases of OMD chosen from among 420 monometries carried out from 1980 to 1987 showed that the duration of the wave, namely the length of time the abnormal pressure is maintained is as important as the pressure of the peristaltic wave and the lower oesophageal sphincter. In fact, wave pressure is generally greater than the systolic pressure of the patient and hence, during contraction, the oesophageal walls are ischaemic and this ischaemia might be the cause of the anatomopathological lesions of Auerbach's plexus observed in decompensated achalasia.

Affective beneficence of vigorous physical activity.

Vigorous exercise is associated with a sensation of well-being, and this subjective state has been objectively quantified with psychometric, cardiovascular, and neurophysiological data. Reductions in state anxiety have been demonstrated to follow acute physical activity, and this response persists for 2-5 h. Chronic physical activity has been associated with reductions in anxiety and depression, as well as increases in self-esteem. This research has been limited to designs of a correlational nature, and the issue of causality vs mere association has not been resolved or addressed. Three hypotheses based upon distraction, monoamine metabolism, and endorphin release are discussed in this paper. Investigators have traditionally attempted to illustrate the mechanism involved in improved mood following exercise by testing one of these or related hypotheses, but it is likely that advances will not be made in this area until these hypotheses are examined in a multiple or synergistic manner. It is concluded that each of the hypotheses reviewed remains tenable.

[The effect of vigorous exercise training on cholesterol metabolism in rats].

Twenty-eight male Wistar rats, aged 7 weeks, were subjected to a vigorous exercise regimen of running for 5 weeks and 35 rats were selected as their controls. After 5 weeks both exercised and control rats were injected 2 microCi of 14C-mevalonate per 100 g body weight into peritoneal cavities and sequentially sacrificed at 20, 40, 60, 80, 100, 120, and 180 min after the injections in each subgroup. The exercised rats showed a significantly lower level of serum total cholesterol than the control rats. The activity of HMG-CoA reductase in liver microsome was significantly higher in the exercised rats than in the control rats. No differences existed in intestinal HMG-CoA reductase activity between exercise and control groups. The incorporation of mevalonate into liver cholesterol and serum cholesterol (especially HDL fraction) in the exercised rats were significantly higher compared with these of the control. Furthermore, the exercised rats showed a higher rate of cholesterol synthesis activity in liver. From these results it was concluded that the hepatic lipoprotein cholesterol production was elevated by exercise compared with that of control.

Eosinophilic esophagitis in a patient with vigorous achalasia.

A patient with vigorous achalasia is presented who had marked smooth muscle hypertrophy and eosinophilic infiltration of the esophagus identical to that seen in patients with eosinophilic gastroenteritis. Eosinophilic infiltration of the esophagus probably represents a variant of the eosinophilic gastroenteritis syndrome and may predispose to an esophageal motor disorder.

Body cooling in human males by cold-water immersion after vigorous exercise.

Five male subjects were immersed to neck level in a whole-body water calorimeter (water temperature 19 degrees C) on two occasions. One immersion was preceded by 30 min of exercise on a treadmill at 80% of the subjects' maximum heart rate, while the other was preceded by no exercise (control). Ventilation, oxygen consumption, hand-grip strength, and heat loss (measured by calorimetry) results showed no significant differences between resting and exercise trials. Minute ventilation and oxygen consumption increased during the immersion but the magnitude of the increase varied among subjects. There was a significant decrease is isometric hand-grip strength after 30 min of immersion. Rectal temperatures fell faster (0.031 degree C +/- 0.004 degree C/min) for exercised subjects than for controls (0.019 degree C +/- 0.005 degree C/min) between 10 and 45 min of immersion (P less than 0.01). It appears that vigorous preimmersion exercise may shorten survival time in cold water due to an increase in cooling rate.

Knee muscular moment, tendon tension force and EMG during a vigorous movement in man.

With injuries to the components of the extensor apparatus of the knee as a background, it is interesting to investigate the magnitude of forces acting on these components, i.e. m. quadriceps femoris, the quadriceps tendon, patella, lig. patellae and tuberositas tibiae, during a vigorous but physiological movement. By means of the dynamic laws of mechanics the muscular moment of force with respect to the bilateral knee axis during kicking was calculated in 6 normal subjects. It was found that the maximum extending muscular moment in the knee occurs very early in the movement, when the initial flexion changes into extension, and thus long before the ball is hit. The peak of quadriceps EMG activity coincides with maximum moment. The EMG peak of the antagonistically acting hamstrings comes later, nearer to when the ball is struck. The greatest extending muscular moment obtained during the swing phase of kicking was surprisingly high, 260 Nm, corresponding to a tension force in the patellar tendon of 5200 N or about 7 times body weight. These values are discussed in relation to tendon strength.

The effects of regular moderate to vigorous physical activity on student outcomes: a review.

Studies on the effects of regular (three to five times per week) periods of moderate to vigorous physical activity (MVPA) on the health, academic performance, attitudes and classroom behaviour of children at school are reviewed. Conclusions from the literature provide a basis for enhancement of school-based physical education programs that are currently considered to lack sufficient intensity, duration and frequency to benefit the children involved. The review not only confirms the value of and need for inclusion of MVPA in physical education programs but also shows the feasibility of doing so, to the advantage of both students and teachers.

Family planning in Viet Nam: a vigorous approach.

Viet Nam, a country in transition, is vigorously implementing its population and family planning policies and plans, despite the fact that increased government expenditure for family planning only amounts to some US $0.15 per capita, far short of the minimum $0.60 required. An increased range of family planning methods will be offered to the population through a "cafeteria" approach. Genital tract infections will be reduced, as well as the incidence of abortion. Research continues on injectable and implanted contraceptives, and the use of quinacrine nonsurgical female sterilization. In the long term, a mix of imported and locally-produced contraceptives is envisaged to achieve greater sustainability. Family planning will be closely linked to economic development and poverty alleviation, with mass organizations developing family planning-linked credit, savings and income generating schemes. Pilot experience has indicated value if integrated approach linking family planning, nutrition, and parasite control. The health manpower and health services systems are being overhauled, with plans to appoint family planning focal points at commune level, linked to mobile district teams and grassroots motivators. Medical equipment, contraceptives and essential drugs for family planning and reproductive health services are in short supply at an estimated 60% of commune health stations. Both the public sector and the growing private sector will need to face the challenge of providing the population with high-quality reproductive health services and family planning options suited to individual needs and preferences.

Comparison of upper gastrointestinal toxicity of rofecoxib and naproxen in patients with rheumatoid arthritis. VIGOR Study Group.

BACKGROUND: Each year, clinical upper gastrointestinal events occur in 2 to 4 percent of patients who are taking nonselective nonsteroidal antiinflammatory drugs (NSAIDs). We assessed whether rofecoxib, a selective inhibitor of cyclooxygenase-2, would be associated with a lower incidence of clinically important upper gastrointestinal events than is the nonselective NSAID naproxen among patients with rheumatoid arthritis. METHODS: We randomly assigned 8076 patients who were at least 50 years of age (or at least 40 years of age and receiving long-term glucocorticoid therapy) and who had rheumatoid arthritis to receive either 50 mg of rofecoxib daily or 500 mg of naproxen twice daily. The primary end point was confirmed clinical upper gastrointestinal events (gastroduodenal perforation or obstruction, upper gastrointestinal bleeding, and symptomatic gastroduodenal ulcers). RESULTS: Rofecoxib and naproxen had similar efficacy against rheumatoid arthritis. During a median follow-up of 9.0 months, 2.1 confirmed gastrointestinal events per 100 patient-years occurred with rofecoxib, as compared with 4.5 per 100 patient-years with naproxen (relative risk, 0.5; 95 percent confidence interval, 0.3 to 0.6; P<0.001). The respective rates of complicated confirmed events (perforation, obstruction, and severe upper gastrointestinal bleeding) were 0.6 per 100 patient-years and 1.4 per 100 patient-years (relative risk, 0.4; 95 percent confidence interval, 0.2 to 0.8; P=0.005). The incidence of myocardial infarction was lower among patients in the naproxen group than among those in the rofecoxib group (0.1 percent vs. 0.4 percent; relative risk, 0.2; 95 percent confidence interval, 0.1 to 0.7); the overall mortality rate and the rate of death from cardiovascular causes were similar in the two groups. CONCLUSIONS: In patients with rheumatoid arthritis, treatment with rofecoxib, a selective inhibitor of cyclooxygenase-2, is associated with significantly fewer clinically important upper gastrointestinal events than treatment with naproxen, a nonselective inhibitor.
