HIV infection in Ukraine (1987-96).

In Ukraine, the number of reported HIV infections increased extremely rapidly during the second half of the 1990s, from less than 50 per year until 1994 to more than 12,000 in 1996. The increase was initially observed and was particularly striking in the regions along the Black Sea. The majority of reported HIV infections were diagnosed in injecting drug users. The extend of HIV spread through sexual transmission is more difficult to assess because of the strong social stigma attached to homosexuality and the lack of information on sexual behaviour in general. The reported number of syphilis cases have also dramatically increased, from 3,000 cases in 1990 to nearly 80,000 cases in 1996. In this paper, we describe the surveillance systems for, and epidemiologic data on HIV infections, AIDS, and other STD in Ukraine from 1987 to 1996. We review the contributions of different vulnerable groups and we also discuss the factors influencing the past spread and the potential for future spread of HIV infection and make recommendations for surveillance, research and prevention.

Ukraine in the context of new European migrations.

The author examines changes in migration from the Ukraine, with a focus on recent trends. He concludes that "like other Soviet successor states, the Ukraine is faced by multiple international migration-related dilemmas and opportunities. However, apocalyptic predictions forecasting mass emigration appear unwarranted."

[Some aspects of the problem of competence, restricted competence in legislation of Ukraine].

Analized in the article are clauses of the Civil code, Civil legal code of Ukraine that contain information concerning the concepts "competence", "restricted competence". It is shown that the laws in force make no provision for restriction of competence in respect of those subjects with non-psychotic mental disturbances. At the same time, certain clauses of the Civil code of Ukraine do make provision indirectly for restriction of competence of persons, those with non-psychotic mental disorders included, under certain conditions.

Strontium fallout from Chernobyl and perinatal mortality in Ukraine and Belarus.

Perinatal mortality rates in the regions of Ukraine and Belarus surrounding the Chernobyl site increased in 1987, the year following the Chernobyl accident. The same year, increases of perinatal mortality were also observed in Germany and Poland, and the effect can be associated with the caesium burden in pregnant women. After 1989, there is an unexpected second rise of perinatal mortality in Belarus and Ukraine. This increase is shown to correlate with the strontium content in pregnant women. The findings parallel an increase of perinatal mortality in Germany following the atmospheric bomb tests in the 1950's and 1960's. While the effect from caesium is essentially limited to 1987, the effect from strontium persists until the end of the study period in 1998. The cumulative effect from strontium around Chernobyl outweighs the effect from caesium by at least a factor of 10. This is contrary to the assertion that the caesium content in the Chernobyl fallout was more than 10-times greater than the strontium content. Thus, the dose factor presently used seems to severely underestimate the effect of strontium on perinatal mortality.

[Possibilities and prospects of the use of DNA analysis in the diagnosis and prevention of inherited disease in the Ukraine].

The results from molecular genetic analysis of some mutations in the 10th and 11th exons of cystic fibrosis transmembrane regulator (CFTR) gene as well as of deletions in the 8, 17, 19, 43, 50, 60th exons of dystrophin gene in 61 CF-families and 21 DMD-families from different Ukraine regions are presented. It was shown that delta F508 frequency of CF-patients was 59.2%, the frequencies of S5491, G551D and K533X were about 1%. The frequency of delta F508-carriers analysed among 365 healthy donors from different regions of Ukraine was 1:40. The analyzed deletions of dystrophin gene were revealed only among 6 DMD-patients. The associations of analyzed mutations of CFTR gene and DMD-gene with RELP's in 4 loci of chromosome 7 and 2 loci of X-chromosome, respectively, were found. The results of prenatal diagnosis of cystic fibrosis and DMD are presented.

[Cholera prevalence worldwide and in Ukraine].

In the course of the 7th pandemic cholera morbidity has been registered in 163 countries of the world. 5 periods in the development of the pandemic hav been established. The pandemic process has a pronounced tendency to growth. The spread and dynamics of cholera morbidity have their specific features on each continent. In Asia the epidemic process is manifested as permanent morbidity. Africa determines the total morbidity level in the 7th pandemic. In America both the import of cholera infection and large local outbreaks due to the formation of the secondary foci are registered. In Europe the infection is mainly brought from different territories, and in a number of cases an epidemic spread of this infection occurs. The paths of the penetration of cholera to Europe and Ukraine are, probably, identical. In Ukraine 3 pandemic periods have been established, corresponding to the periods of pandemic spread.

[Retrospective analysis of the effect of Ukraine and USA research interrelations on the development of sport biochemistry in the XX century].

This review highlights some of the important developments in Sport Biochemistry and relates these endeavors to the current understanding of muscle biochemistry and function. Attempts have been made to establish some scientific links involved in the evolution of such research, in particular the influences shared by scientists working in some premier institutions in Ukraine and the USA in the 20th century. Research initiated in the United States by Sutherland lead to further Ukrainian investigations that aided in the understanding of the role cAMP and cAMP-dependent protein kinase in muscle metabolism during exercise. Topics covered include Palladin's pioneering research on muscle creatine in Ukraine at the beginning of the 20th century, Russian program on creatine supplements in the 1970s-1980s and its implementation at Olympic games.

Legislation for Ukraine's public health: the current situation and paths to further development.

The paper contains the findings of an analytical study of the current national health care legislation of Ukraine and identification of priority areas in its further development. One of the key objectives of the above study was to identify the compliance of the national healthcare legal framework with the approaches to health policy formation, which are set out in the documents of global and European regional international organizations, and to assess whether the national legislation includes that spectrum of functions that are to be covered by the health care legislation. The analysis showed that Ukraine's national healthcare legislation is rather strong and well developed. Though the national healthcare legislation is mostly in line with international approaches to the state health care policy formulation, the issue of enforcing already adopted laws and by-laws and ensuring their compliance by all legally established bodies, including state authorities and self-governments, citizens, NGOs, etc, is problematic.

[Immunoprophylaxis of poliomyelitis in Ukraine with the use of inactivated vaccine].

In connection with the cessation of the circulation of "wild" poliovirus on the territory of the European region, including Ukraine, the strategy of the vaccinal prophylaxis of poliomyelitis is reviewed. Its main aims are the creation of a high level of the specific protection of the population, the prophylaxis of vaccine-associated paralytic poliomyelitis and a decrease in the intensity of the circulation vaccine polioviruses. These aims may be achieved only by the inclusion of vaccinations with inactivated poliomyelitis vaccine (IPV) into the immunization schedule. IPV "Imovax Polio" produced by the firm "Aventis Pasteur" (France) has been shown to have low reactogenicity and high effectiveness, especially with respect to type 3 poliovirus, under the conditions of Ukraine. On the basis of our studies all children, starting from the age of 3 months, are recommended to be vaccinated first with two injections of IPV, followed by further immunization with oral vaccine.
