Nuclear DNA analysis of insulinomas and gastrinomas.

The potential for malignancy of an islet cell tumor of the pancreas is difficult to cytologically judge when one evaluates only the primary lesion, because a malignant condition is usually determined by the presence of regional or distant metastases. Nuclear DNA cytometric measurements have proved helpful both in the evaluation of the malignant potential of other endocrine and nonendocrine lesions and in the determination of the "aggressiveness" of these tumors. Thirty-six islet cell tumors or their metastases from 25 patients were studied. Eleven patients had insulinomas and typical insulinoma syndromes, and 14 others had gastrinomas with the Zollinger-Ellison syndrome. Tissue from each tumor was stained by the Feulgen technique, and nuclear DNA cytometry was performed by means of the microTICAS system designed by the Cytopathology Laboratory of the University of Chicago. Ploidy measurements of insulinomas, taken alone, did not discriminate well between benign and malignant states. However, the single malignant insulinoma could be clearly recognized, for it was one of only two lesions in that group with 5N-exceeding rate (5N-ER) values of 1% or greater. (5N-ER is defined as the percentage of aneuploid nuclei with nuclear DNA content greater than 5N.) On the other hand, seven of eight malignant gastrinomas had ploidy values of 2.5N or greater (our definition of an aneuploid state) and/or had 5N-ER values of 1% or greater, while five of six benign gastrinomas had ploidy values of less than 2.5N and had 5N-ER values of 0%. In addition, the two most aggressive tumors had the highest ploidy and 5N-ER values. Nuclear DNA cytometric studies appear to offer promise as an aid in the evaluation of pancreatic islet cell tumors, particularly gastrinomas.

Intrarectal ultrasonography in the preoperative staging of rectal cancer.

Intrarectal ultrasonography is a new imaging method used in the preoperative staging of rectal cancer. Twenty five patients with proven rectal cancer were submitted to preoperative intrarectal ultrasonography; 24 were resected. Real time scanner with a linear probe of 5 MHz was employed. Five sonographic structures corresponding to the five histological layers were evidenced. Ultrasound T staging was compared to pathology, using UICC classification. In all but two cases pathology corresponded to ultrasound T staging (accuracy 92%). Perirectal nodal involvement was also investigated. It is concluded that intrarectal ultrasonography will play a definite role in the preoperative staging as well as in programming treatment of rectal carcinoma.

Cross-over axillary by-pass. Treatment of subclavian or innominate artery obstructive lesions.

A 13-year experience consisting on 16 consecutive patients submitted to axillary-to-contralateral artery by-pass in the treatment of obstructive lesions of the subclavian or innominate artery is reported. Nine patients complained of typical subclavian steal syndrome. Extended patency with very satisfactory functional result was achieved in twelve patients. An extensive survey is made of the results of such procedure, as reported in the literature. The theoretical advantages in comparison with other kinds of extrathoracic procedures aiming at the same goal are briefly outlined.

Prophylaxis and treatment of inflammatory anorectal complications in leukemia.

Forty leukemic patients with inflammatory anorectal complications were examined. Twenty two were affected by acute lymphatic leukemia, 10 by chronic lymphatic leukemia, 6 by acute myelocytic leukemia and 2 by non H lymphoma and chronic myelocytic leukemia, respectively. In all cases surgery was indicated not only to treat the anorectal complication, but mainly to resume the antiblastic chemotherapy discontinued because of the risk of sepsis and to prevent the failure of bone marrow transplantation in patients with chronic myelocytic leukemia. The underlying malignant disease and the altered platelet, white blood cell and neutrophil levels were shown to be the major factors conditioning the surgical treatment. In 2 cases, acute recurrence of the underlying disease and the development of a graft verus host disease have been the cause of death. It is concluded that in patients eligible for bone marrow transplantation or undergoing radio and/or chemotherapy, local and general antinfective prophylaxis is of paramount importance to decrease the risk of inflammatory anorectal complications.

Correlation between lowered PgE2 levels and alkaline reflux gastritis in patients subjected to Billroth II. Pylorogastrectomy: possible improvement with sulglycotide.

In patients subjected to pylorogastrectomy with anterior gastroenterostomy (Billroth's operation II) the remaining gastric mucosa is exposed to the damaging action of bile with consequent reduction of locally generated prostaglandins. The purpose of the present study was to explore the correlation between PgE2 levels in such postgastrectomy patients before and after treatment with sulglycotide. Twelve patients with clinical, endoscopic and histological evidence of alkaline reflux gastritis were treated with sulglycotide in daily doses of 1600 mg for 30 days. At termination there was a definite clinical, endoscopic and histological improvement with significant (p less than 0.001) increase of PgE2 levels.

Modifications of transcutaneous oxygen tension in lower limb peripheral arterial occlusive disease patients treated with spinal cord stimulation.

The current noninvasive techniques for investigating the response to epidural spinal cord stimulation in peripheral arterial occlusive disease patients are far from optimal. Transcutaneous oxygen tension measurements in these patients treated with spinal cord stimulation demonstrated a statistically significant increase during the stimulation and 30 min after the pacemaker has been switched off, suggesting the effectiveness of the spinal cord stimulation on cutaneous oxygenation. The transcutaneous oxygen tension measurements would represent in the future a reliable parameter to predict the effect and to modulate the pacemaker setting in peripheral arterial occlusive disease patients.

Comparative results of three different techniques for colonic anastomosis in the dog.

Three different techniques of colonic anastomosis were evaluated in the dog. Left hemicolectomy and two layer anastomosis were performed in ten dogs, one layer anastomosis in ten dogs and sleeve anastomosis in ten dogs. In each group, five dogs were sacrificed on the second postoperative day, and the anastomotic specimen was studied by a bursting test. The remaining five dogs were sacrificed seven days after operation, and the specimen was studied by hydroxyproline dosage and by microscopy. The bursting test showed a significant difference between sleeve anastamosis and other techniques. The results of hydroxyproline dosage showed no significant differences between the different anastamotic techniques.
