Early development of selenium-induced cataract: slit lamp evaluation.

The purpose of our research was to document early lenticular changes preceding cataract formation in rats receiving an elevated dose of selenium. The following stages were observed after selenium injection: Stage 1 (13-24 hr post-injection), formation of posterior equatorial subcapsular cataract (PESC); Stage 2 (24-72 hr), decline of the PESC, development of a prominent 'washer' shaped change in the refraction of the cortex, and first appearance of swollen fibers around the nucleus; Stage 3 (three to five days), appearance of bilateral dense central nuclear cataracts and further development of perinuclear swollen fibers; Stage 4 (five to 10 days), some nuclear cataracts became more opaque and/or angular. The results are consistent with the hypothesis that the initial site of action of selenium in nuclear cataract formation is not in the lens nucleus. Rather, selenium causes early changes outside the nucleus, which are followed by nuclear cataracts.

Slit-lamp visualization of aqueous flow through an inferior iridectomy.

PURPOSE: To demonstrate how an inferior iridectomy prevents a pupillary block in aphakic patients with a silicone oil tamponade. DESIGN: Observational case series. METHODS: Photographs of two patients who had undergone vitrectomy, lensectomy, membrane peeling, silicone oil tamponade, and inferior iridectomy for a complex retinal detachment were evaluated. RESULTS: In the thin layer of erythrocytes covering the endothelium, we observed an arc clear of erythrocytes anterior to the inferior iridectomy. CONCLUSIONS: With silicone oil blocking the pupil, aqueous humor was channeled through the inferior iridectomy with sufficient force to sweep the endothelium anterior to the iridectomy clean of erythrocytes, thus visualizing the mechanism that prevents a pupillary block glaucoma.

Development of a Scheimpflug slit lamp camera system for quantitative densitometric analysis.

We have developed a computerised video system for accurate quantification of lens opacities from Scheimpflug images. Using a modified Zeiss Scheimpflug camera and Macintosh computer, the system software can warn of improper exposure and can provide immediate results of cataract progression on individual patients. These results are obtained from a novel technique which automatically divides the lens into anatomical regions and computes regional densities in the standard optical density unit. Because of the system's speed, the operator can properly capture and analyse a cataract in seconds, compare results with past patient records and give feedback to the patient on the results of the analysis. The system provides the appropriate data integrity and formats for documenting and monitoring lens opacities. Such a system will be useful in longitudinal studies of cataracts (i.e. for natural history and anti-cataract drug trials).

[Evaluating biological tolerance of PMMA, heparin-modified PMMA and hydrogel intraocular lenses using slit lamp microscopy].

In order to find a difference in the biocompatibility of different IOL-materials we examined the lens-surface of poly(methylmetacrylate)-(PMMA-), heparin surface-modified PMMA-(HSM-PMMA-) and hydrogel-IOLs in a prospective study by specular microscopy. Documentation consisted of cellular reaction, precipitates on the lens-surface and irregularities of the IOL-material. Early postoperative on HSM-PMMA and hydrogel-IOLs there were fewer fibroblast-like cells as compared to PMMA-IOLs. In the later postoperative period foreign body giant cells were seen on PMMA-IOLs in 26.6%, on HSM-PMMA and hydrogel-IOLs in less than 10% of the cases. In cases with increased postoperative inflammation or contact between uveal tissue and lens-surface we found more cells and in a higher percentage. Concerning precipitates on the surface and material irregularities we also found lens-specific differences. Clinical results were satisfactory for all lens-types.
