Spontaneous fracture of the sternum in a child being treated in a Boston brace for kyphoscoliosis. A case report and review of the literature.

We report a child who suffered from short stature of unknown aetiology and rigid thoracolumbar kyphoscoliosis and developed a symptomatic spontaneous fracture of the sternum while being treated in a Boston brace. Modification of the brace was followed by uneventful healing of the sternal fracture. To our knowledge no mention has been previously made of this extremely rare complication, but the spine surgeon who treats complex spinal deformities in early childhood with braces should be aware of this potential complication, particularly if they are associated with rigid rotational kyphosis.

Persistent sternum synchondroses on bone scintigraphy.

This report describes the case history of two patients who showed a hyperactive midsternal area on bone scintigraphy due to a persistent sternal synchondrosis. Whereas this anatomical variant is diagnosed infrequently, midsternal hotspots can sometimes be related to arthrotic degeneration of these extra joint spaces. In the differential diagnosis of midsternal hotspots on bone scintigraphy, persistent sternum synchondroses should be considered.

Surgical reconstruction for absence of sternum and pericardium in a newborn.

We report the surgical management of a 3 day old baby with absent sternum and pericardium with impending rupture of the skin cover. The repair described is innovative and may be used in place of primary repair by bringing the ribs together. Review of embryology and various modalities of closure are provided.

Video-assisted mediastinoscopic resection of a thymic cyst by sternum lifting.

We performed mediastinoscopic surgery on two patients with a thymic cyst using a new instrument by sternum-lifting with tape. The merits of this technique are that a resection of the thymus and cyst can be achieved without a midline sternotomy, and under bilateral ventilation, and in cases with pleural adhesion. This technique can be applicable to most benign thymic lesions including thymectomy for myasthenia gravis and to selected cases with a malignant thymic tumor.

Chondromyxoid fibroma of the sternum.

We report the fourth case of chondromyxoid fibroma of the sternum described in the literature. The clinical, radiographic, pathologic and treatment aspects of this case are presented, and the literature on chondromyxoid fibroma in this unusual location is reviewed.

Increased kyphosis by a combination of fractures of the sternum and thoracic spine.

A 67-year-old patient suffered a fall and sustained injuries to the thorax, combined with fractures of the ribs, sternum and thoracic spine. Her pre-existing kyphosis (caused by osteoporotic vertebral collapse) increased in severity. There was a clear relationship between a sternal fracture, deterioration of thoracic kyphosis and a fracture of the thoracic spine.

Video-assisted thoracoscopic resection of a giant anterior mediastinal tumor (lipoma) using an original sternum-lifting technique.

We report here the complete thoracoscopic resection of a giant lipoma (18x15x5 cm, 560 g) in a 33-year-old man using an original sternum-lifting technique. The patient had an uncomplicated postoperative course and was discharged on the seventh postoperative day. This is the largest reported anterior mediastinal tumor that was completely resected by video-assisted thoracic surgery without median sternotomy or thoracotomy. This original method may be useful from the standpoint of minimal access, rapid recovery, less pain, and good cosmetic results for an anterior mediastinal giant tumor.

The role of sternum in osteomyocutaneous reconstruction of major mandibular defects.

Replacement of skeletal support after extensive intraoral resection demands the use of a vascularized bone graft. This can be provided by microvascular free transfer using the ilium or metatarsal bones or by using muscle as a vascular carrier, employing the rib, the spine of the scapula, or the clavicle. Over a 5 year period, 27 patients underwent mandibular reconstruction with a vascularized segment of the outer table of sternum transposed together with a skin island on the pectoralis major muscle. Loss of bone occurred in two patients. A 93 percent survival rate was achieved. An earlier, smaller series of six patients using rib for reconstruction resulted in an unacceptably high complication rate. Success in survival of the bone graft, however, is not enough to procure a satisfactory result. Specific aspects of surgical technique are described that create an alveolar ridge with a sulcus on either side that can accept a denture; a diaphragm to replace the floor of the mouth and eliminate dead space; and an esthetically acceptable appearance, with definition of the jawline and a properly supported lower lip.

Solitary plasmacytoma of the sternum: surgical resection with long-term follow-up.

Two patients with solitary plasmacytoma of the sternum were treated with primary surgical resection. Good long-term results were obtained. There is no long-term documentation that primary radiotherapy alone is the treatment of choice for solitary plasmacytomas. An individualized approach is warranted, especially when there is an isolated bulky lesion involving the chest wall.

Transesophageal echocardiography in the postoperative child with an open sternum.

We report the use of transesophageal echocardiography in 3 children whose sternums remained open for several days postoperatively. In these patients transesophageal echocardiography provided critical information when transthoracic echocardiography was ineffective due to limited acoustic windows. This report points out the application of transesophageal echocardiography in children with chest wall distortion.

A one-stage correction of mandibular defects using a split sternum pectoralis major osteo-musculocutaneous transfer.

This paper describes a technique of mandibular reconstruction using the sternal skin and the outer table of the sternum transferred as a compound osteo-musculocutaneous flap based on the pectoralis major muscle. The technique has been used for immediate reconstruction at the time of cancer ablation but it can equally well be used as a delayed reconstructive procedure.

Type X collagen synthesis by cultured chondrocytes derived from the permanent cartilaginous region of chick embryo sternum.

In the developing chick embryo sternum, type X collagen is synthesized by chondrocytes from the cephalic region (presumptive mineralization zone) but not by chondrocytes from the caudal region (permanent cartilaginous zone) (Gibson et al., 1984, J. Cell Biol. 99, 208-216). To distinguish between two possibilities, the presence of a nonpermissive microenvironment in the permanent cartilage or the intrinsic inability of caudal chondrocytes to become hypertrophic, type X-producing cells, we have isolated chondrocytes from the caudal third of stage 44 chick embryo sterna and grown them in suspension on agarose-coated dishes. We have found that in these conditions chondrocytes from the caudal zone differentiate to hypertrophic chondrocytes and synthesize large amount of type X collagen, as revealed by the electrophoretic pattern of labeled proteins made in vitro and by slot blot analysis of mRNAs with specific cDNA probes.

Solitary plasmacytoma of the sternum: diagnosis by computed tomography.

We report a large solitary plasmacytoma arising in the sternum of a 62-year-old man. On computed tomography the tumor demonstrated a multilobular appearance associated with bone destruction and expansion and absence of intrinsic calcification. Computed tomography was helpful in determining the size and extent of the tumor and in suggesting the correct diagnosis.

Correlation of mid-sternum and mid-spine positions in 82 patients irradiated for breast cancer.

The validity of designing anterior internal mammary node (IMN) radiation therapy fields based on vertebral body landmarks was tested by reviewing computer tomography (CT) images in 82 patients treated for breast cancer. The horizontal distance between the geometric center of the sternum and the vertebral body was 0-5, 6-10 and > 10 mm in approximately 30%, 25%, and 45% of patients, respectively. This suggests that using the projection of the vertebral bodies to design an anterior internal mammary node field is frequently inaccurate and may lead to inadequate coverage of these lymph nodes.
