A comparison of maladaptive schemata in treatment-seeking obese adults and normal-weight control subjects.

OBJECTIVES: The aims of this study were to determine whether treatment-seeking obese adults display a greater severity of maladaptive schemata than normal-weight adults and to investigate the possible correlates of maladaptive schemata among obese individuals. METHODS: The sample included 52 obese adults participating in a weight loss treatment and 39 normal-weight adults. Participants in the obese and normal-weight control groups completed standardized self-report questionnaires designed to assess attitudes and behaviors regarding eating and weight (Questionnaire on Eating and Weight Patterns-Revised and Binge Eating Scale), maladaptive schemata (Young Schema Questionnaire-Short Version), mood disturbance (Profile of Mood States-Adolescents) and socially desirable responding (Balanced Inventory of Desirable Responding). RESULTS: The obese patients reported a significantly greater severity of maladaptive schemata (after controlling for demographic variables and binge eating disorder status) than the normal-weight control subjects. In addition, within the obese group, there were significant positive correlations between the severity of maladaptive schema scores and both mood disturbance and problem eating scores. CONCLUSION: The present findings suggest that obesity may be associated with a higher severity of maladaptive schemata, at least among those obese individuals who have sought treatment. Possible etiological and treatment implications of the findings are discussed.

Blood donor selection and screening: strategies to reduce recipient risk.

Various measures are taken to ensure the safety of the blood supply. Donor selection begins with education of the public about transfusion-transmissible diseases. Potential donors must answer a questionnaire designed to identify specific risk factors for these infections. The questionnaire is the only line of protection against certain infections for which no testing is performed, such as malaria, babesiosis, leishmaniasis, and Chagas disease. All donations are tested for the presence of antibodies to HIV-1 and -2, HCV, HTLV and syphilis, the hepatitis B surface antigen (HbsAg), the p24 antigen (HIV), and also for HIV and HCV nucleic acids. The introduction of new and improved screening tests for transfusion-transmissible diseases has led to remarkable improvement in the safety of the blood supply, with substantial shortening of the window period for HIV, HCV, and HBV infections. The current challenge of the industry is to reduce even further the small but significant risk of bacterial contamination of platelet components. Finally, some safety measures are purely precautionary, such as the deferral of donors who have traveled to certain countries affected by the bovine spongiform encephalopathy (BSE).

Thirty years of experience living with a continent ileostomy: bad restrooms--not my reservoir--decide my life.

OBJECTIVE: This study describes long-term adjustment of people with a continent ileostomy. SUBJECTS AND METHODS: Subjects were 68 people (25 males and 43 females) who underwent an operation for ulcerative colitis with a continent ileostomy (Kock pouch). The median number of years subjects had lived with a continent reservoir was 31 (range 29-36) at follow-up and the median subject age was 60 (40-89). Subjects completed a 36-item questionnaire designed to assess adjustment to ostomy surgery. Subjects ranked the questionnaire using a 6-point Likert scale; a response alternative "not relevant" was available. Content analysis was conducted on subjects' responses to an open-ended question covering aspects of their quality of life. RESULTS: High median adjustment ratings were found for all 36 statements; the maximum median rating of 6 appeared on 28 items. Eight items with the lowest median ratings were in the following domains: embarrassing situations, activity, body image, sexuality, and good care. Five items on medical care and 3 items on sexuality were most frequently considered "not relevant" by respondents. The most frequently mentioned quality-of-life domains were family, health, friends, and employment. Content analysis indicated that respondents were self-reliant, although they indicated that they experienced considerable impediments to bowel evacuation outside of the home. CONCLUSIONS: In the long-term, people with a continent ileostomy have good self-care. The quality and availability of public restrooms, however, reduces their daily activities away from home.

Recognition of cigarette brand names and logos by primary schoolchildren in Ankara, Turkey.

OBJECTIVE: To assess the smoking behaviour of primary schoolchildren and their ability to recognise brand names and logos of widely advertised cigarettes, compared with other commercial products intended for children. DESIGN: Cross-sectional survey in classroom settings using a questionnaire designed to measure attitudes towards smoking and the recognition of brand names and logos for 16 food, beverage, cigarette, and toothpaste products. SETTING: Ankara, Turkey. SUBJECTS: 1093 children (54.6% boys, 44.4% girls) aged 7-13 years (mean = 10, SD = 1), from grades 2-5. The student sample was taken from three primary schools--one school in each of three residential districts representing high, middle, and low income populations. MAIN OUTCOME MEASURES: Prevalence of ever-smoking, recognition of brand names and logos. RESULTS: Prevalence of ever-smoking was 11.7% overall (13.9% among boys and 9.1% among girls; p < 0.05). Children aged eight years or less had a higher prevalence of ever-smoking (19.6%) than older children (p < 0.002). Ever-smoking prevalence did not differ significantly across the three school districts. Ever-smoking prevalence was higher among children with at least one parent who smoked (15.3%) than among those whose parents did not (4.8%) (p < 0.001). Brand recognition rates ranged from 58.1% for Chee-tos (a food product) to 95.2% for Samsun (a Turkish cigarette brand). Recognition rates for cigarette brand names and logos were 95.2% and 80.8%, respectively, for Samsun; 84.0% and 90.5%, respectively, for Camel; and 92.1% and 69.5%, respectively, for Marlboro. The Camel logo and the Samsun and Marlboro brand names were the most highly recognised of all product logos and brand names tested. CONCLUSIONS: The high recognition of cigarette brand names and logos is most likely the result of tobacco advertising and promotion. Our results indicate the need to implement comprehensive tobacco control measures in Turkey.

Psychopathological profile of tinnitus sufferers: evidence concerning the relationship between tinnitus features and impact on life.

This study involving 281 French tinnitus sufferers sought to investigate possible correlations between psychopathological profile and scores obtained from three tinnitus questionnaires. The patients all completed a French version of the Mini-Mult--a shortened Minnesota Multiphasic Personality Inventory--and French translations of three questionnaires designed especially for the clinical assessment of tinnitus: Tinnitus Reaction Questionnaire (TRQ), Subjective Tinnitus Severity Scale (STSS) and Tinnitus Handicap Questionnaire (THQ). Significant correlations were found (p < 0.0001) between scores on various Mini-Mult scales and total or factor 1 THQ and total TRQ scores. No significant correlation was found between the STSS and any Mini-Mult score.

Critical comparison of nine different self-administered questionnaires for the evaluation of disability caused by low back pain.

AIM: The aim of this study was to critically compare 9 self-administered questionnaires designed to evaluate disability caused by back pain. METHODS: The questionnaires were analyzed considering and comparing their psychometric characteristics (reliability, validity, responsiveness, minimal clinically relevant difference), together with other practical and technical aspects (number of items, number and kind of domains, scaling of items, scoring, time to complete, validated translations, etc.). Data were obtained from scientific literature. RESULTS: Only 3 out of 9 analyzed instruments (i.e., the Oswestry Low Back Pain Disability Questionnaire, the Quebec Back Pain Disability Scale, and the Roland-Morris Low Back Pain Disability Questionnaire) appear fully validated from a psychometric standpoint. CONCLUSIONS: On the basis of psychometric evaluations as well as feasibility considerations, the authors suggest using either the Roland-Morris or Oswestry questionnaire as the best assessment of the level of disability caused by back pain.

[A comparative study of two methods of personality assessment, the TCI and the NVM (Dutch shortened version of the MMPI)].

BACKGROUND: Various types of reliable instruments are available for measuring personality dimensions. One of these instruments is the NVM, which is a Dutch shortened version of the Minnesota Multiphasic Personality Inventory, but its use is no longer permitted. AIM: To find out whether the Temperament and Character Inventory (TCI) can serve as an alternative to the NVM. METHOD: A study has compared the NVM questionnaire to the TCI, which is a personality questionnaire designed from a more psychiatric perspective. The two types of questionnaire were completed by 91 psychiatric (out)patients. Correlation analyses were performed. RESULTS: Statistically significant correlations were found between the various scales of the NVM and the TCI. Unique combinations of the TCI scales were found to predict the various NVM scales. CONCLUSION: It is worth considering the TCI as an alternative to the NVM.

Pulsed estrogen therapy improves postmenopausal quality of life: a 2-year placebo-controlled study.

OBJECTIVE: To investigate the effect of pulsed estrogen therapy S21400 (intranasal 17 beta-estradiol) on different quality of life (QoL) dimensions in early postmenopausal women treated with S21400 150 microg per day, S21400 300 microg per day, or placebo in a double blind, randomized, controlled 2-year study. STUDY DESIGN: QoL was assessed based on the validated Women's Health Questionnaire designed for peri- and post-menopausal women. Three hundred and thirty-five healthy, early postmenopausal Danish women, 53 years of age in average, who completed one questionnaire at baseline and one under study treatment were included in the analysis set. All analyses were performed on an intention-to-treat basis. RESULTS: QoL improved significantly in both S21400 groups compared to placebo in the dimensions 'memory/concentration', 'vasomotor symptoms', 'sleep problems' and 'sexual behavior' (difference in mean change scores being respectively +7.9, +28.3, +9.9 and +10.8%, p < 0.001, between the S21400 300 microg and placebo group). There were no significant differences between actively treated groups and placebo in the dimensions 'anxiety/depressed mood' and 'well-being'. CONCLUSION: Pulsed estradiol therapy had a pronounced effect not only on vasomotor symptoms but also a significant and clinically relevant improvement in several other QoL dimensions.

Changes in sex behaviour after menopause: effects of tibolone.

In order to analyse both sexual desire after menopause and its response to treatment with tibolone, a randomized placebo/tibolone trial has been designed with 28 postmenopausal patients. They were asked to answer a questionnaire designed by us to obtain quantitative measurements to describe changes in sexual desire. After selection, the patients were randomly assigned to two groups as follows: 14 in the group treated with 2.5 mg/day of tibolone and 14 in the placebo group treated with 500 mg/day of calcium. The patients were monitored after 3, 6 and 12 months. Before joining this study, they had signed a written consent. It was observed that the sexual desire after menopause underwent a significant fall, with decline in arousability and intercourse. The comparative results show that the patients treated with tibolone experience an improvement after the third month of treatment and this improvement was maintained until the end of treatment. We conclude that the questionnaire proposed here is a useful non-parametric method to diagnose a patient's sexuality status at the baseline assessment and a valuable tool for monitoring various therapies. Furthermore, tibolone proved to be effective in managing reduced sexual desire which appears in postmenopausal patients.

A regional food-frequency questionnaire for the US Mississippi Delta.

OBJECTIVE: To describe food sources of nutrient intake for white and African American adults in the Lower Mississippi Delta (LMD), and their use in the development of a regional food-frequency questionnaire (FFQ) based on an earlier version of the National Cancer Institute's Health Habits and History Questionnaire. DESIGN: We ranked food sources of energy, macronutrients, vitamins and minerals, and examined portion size distributions for 842 white and 857 African American residents aged 19 years and older, using 24-hour dietary intake recall data from a telephone survey of 36 LMD counties. These values were used to develop a regional FFQ, which was then field-tested with 100 subjects and revised to improve interpretability. SETTING: The LMD region of the USA. SUBJECTS: White and African American adult residents of the LMD. RESULTS: LMD African Americans obtained more of their energy and nutrient intakes from poultry, processed meat, salty snacks, fruit drinks, pork and cornbread; and less from milk, alcohol, legumes, salad dressing, butter/margarine and sweetened tea than did white residents. Regional foods not on nationally used FFQs included grits, turnip greens, okra, ham hocks, chitterlings, crawfish, catfish, cracklings, jambalaya, potato logs, chicken and dumplings, and sweet potato pie. Based on responses during field-testing, the questionnaire was also designed to add four portion sizes for each food item, presented as questions, rather than in grid format. CONCLUSIONS: Regional food use patterns differ from national patterns and furthermore differ between African American and white adults in the LMD. The resulting Delta NIRI FFQ for Adults should contribute to improved assessment of usual intake for use in studies of diet and health in this region.

Attitudes towards and beliefs about nutrition and health among a random sample of adults in the Republic of Ireland and Northern Ireland.

OBJECTIVES: For effective healthy eating promotion, it is necessary to understand the attitudes towards and beliefs about nutrition of the general public. The objective of this study was to provide data on attitudes towards eating a healthy diet and the perceived need to alter eating habits from a random sample of adults in the Republic of Ireland and in Northern Ireland, using a self-administered questionnaire. DESIGN: Cross-sectional survey using a self-administered attitudinal questionnaire on beliefs and attitudes to healthy eating. SETTING: The survey was carried out between October 1997 and October 1999 in the Republic of Ireland and in Northern Ireland. SUBJECTS: A randomly selected sample of 1256 adults from the Republic of Ireland and Northern Ireland completed the attitudinal questionnaire. RESULTS: A majority of subjects (62%) perceived that they make conscious efforts to eat a healthy diet either most of the time or quite often, while just over half (52%) agreed that they do not need to make changes to their diet as it is healthy enough. Subjects most likely to make conscious efforts to try to eat a healthy diet were females, older subjects (51-64 years) and those with the highest intakes of fruit and vegetables and lowest quartile of fat (% food energy). When self-assessed adequacy of fruit and vegetables was examined, two-thirds of the total sample felt they ate too little fruit while just one-third felt they ate too little vegetables. CONCLUSION: Results of the present study, in general, revealed good agreement between attitude and behaviour with respect to healthy eating. This suggests that people appear to be reasonably accurate at evaluating their own diet in terms of how healthy it is. In terms of the two food groups examined in this study, some optimistic bias was evident for vegetables but not for fruit. It may be useful therefore to assess attitudes and beliefs about healthy eating by way of examining attitudes to such food groups individually.

Physical therapists in burn care. Role and staffing patterns.

The purpose of this study was to identify the role and the staffing patterns of physical therapists in 42 major burn care facilities in the United States. A questionnaire designed by the investigators was used to collect the data. Completed questionnaires were returned from 34 (81%) of the burn care facilities, almost all (91%) of which employed physical therapists. Physical therapy was provided six or seven days a week in 74 percent of the facilities and five days a week in 26 percent of the facilities. The work roles of physical therapists and occupational therapists were compared, and significant differences are reported. The primary treatment provided by the physical therapists was exercise (90%), and the primary treatment by occupational therapists was splinting (96%). The most frequently used methods of contracture control and methods of physical therapy follow-up care are reported, as well as the respondents' perceptions about adequacy of physical therapy coverage and turnover of therapists in burn care.

Exercise preferences among a population-based sample of non-Hodgkin's lymphoma survivors.

In the present study, we examined the exercise preferences of a population-based sample of non-Hodgkin's lymphoma (NHL) survivors. A secondary purpose was to explore the association between various demographic, medical, and exercise behaviour variables and elicited exercise preferences. Using a retrospective survey design, 431 NHL survivors residing in Alberta, Canada completed a mailed questionnaire designed to assess exercise preferences, past exercise behaviour, and various demographic variables. Overall, 77% of participants preferred or maybe preferred to receive exercise counselling at some point after their NHL diagnosis. An overwhelming majority indicated that they would possibly be interested (81%) and able (85%) to participate in an exercise programme designed for NHL survivors. The majority of participants (55%) listed walking as their preferred choice of exercise. Logistic regression analyses indicated that NHL survivors' exercise preferences were influenced by body mass index (BMI), exercise behaviour, and gender. Eliciting exercise preferences from the population in question yields important information for cancer care professionals designing exercise programmes for NHL survivors. Furthermore, tailoring exercise programmes to the preferences of NHL survivors may be one method to potentially enhance exercise adherence in this population both inside and outside of clinical trials.

The prevalence of sexual assault in a genitourinary medicine clinic: service implications.

OBJECTIVE: To determine the prevalence of sexual assault amongst patients attending a department of genitourinary medicine and to investigate the circumstances of the assault. SETTING AND SUBJECTS: The Department of Genitourinary Medicine (GUM), St. Mary's Hospital, London. All patients attending the walk-in GUM Clinic over a fixed time period were invited to take part in the study. METHODS: Patients were asked to complete an anonymous questionnaire designed by the investigators. RESULTS: 351 (209 women, 142 male) usable questionnaires were returned of 370 distributed. Seventy five persons (21.4%) reported sexual assault: 59 (28.2%) women and 16 (11.3%) men. The incident occurred more than three months before presentation in the majority of persons studied (93%). Twenty one per cent reported the assault to the police, and 34% attended for medical screening. For those who did attend for medical screening, 61% informed their doctor of the assault. The main reason for non-reporting to police and medical staff was "a wish to forget". Counselling was sought by 24% after the sexual assault. CONCLUSIONS: A past history of sexual assault is common amongst GUM clinic attenders, more often disclosed amongst women. Reporting both to police and medical services is low. Clinicians need to be sensitive to the fact that a significant proportion of patients attending GUM services may have been sexually assaulted either in the present or the past and the impact that this may have on health care usage. GUM clinics maybe ideally placed to provide medical and psychological support to these individuals.

A comparison of four measures of self-control skills.

This study compares the psychometric characteristics of four questionnaires designed to assess self-control skills: the Self-Control Questionnaire, the Frequency of Self-Reinforcement Questionnaire, the Cognitive Self-Management Test, and the Lifestyle Approaches Inventory. Content validity was judged to be fairly comparable by three raters in Study 1. In Study 2, convergent and divergent validity support was obtained for all four questionnaires when administered to 369 multiethnic college students, but the relative degree of support varied across constructs. Hence, selection of a self-control instrument may be guided by the target behavior of interest.

Quality of life in ALS is maintained as physical function declines.

OBJECTIVES: To study patients with ALS to determine how physical function, quality of life (QOL), and spirituality or religiousness change over time, and what relationship these changes have to one another. METHODS: Sixty patients with ALS were studied prospectively. They were assessed at baseline, 3 months, and 6 months, using questionnaires designed to measure general quality of life (McGill Quality of Life questionnaire), religiosity (Idler Index of Religiosity), ALS-specific health-related quality of life (SIP/ALS-19), and ALS-specific function (ALS functional rating scale). RESULTS: A two-way repeated measures multivariate analysis of variance revealed that both the passage of time and the specific QOL scales used were factors in predicting patient quality of life (F[1, 59]= 9.87, p < 0.003 and F[3, 177]= 16.90, p < 0.001) Despite a progressive decline in physical function as measured by the ALS-specific function score, the general QOL and religiosity scores changed little. In contrast, the ALS-specific health-related QOL score declined in parallel with the ALS-specific function score. CONCLUSIONS: QOL in patients with ALS appears to be independent of physical function, which agrees with a previous cross-sectional study. The ALS-specific health-related QOL score is primarily a measure of physical function. QOL instruments that assess spiritual, religious, and psychological factors produce different results than those obtained using measures of physical function alone.

Predicting depressive symptoms with a new measure of shame: The Experience of Shame Scale.

OBJECTIVE: To investigate whether previous findings from interview studies of a prospective relationship between shame and psychopathology (e.g. Andrews, 1995) could be replicated using questionnaires. DESIGN: A total of 163 university students participated in a longitudinal questionnaire study. METHOD: The Experience of Shame Scale (ESS), a questionnaire based on a previous interview measure, and an established shame scale (TOSCA), were considered in their relation to depressive symptoms assessed at two time points 11 weeks apart. RESULTS: Both scales made significant independent contributions to depressive symptoms at time 1. However, only the ESS predicted additional significant variance in time 2 symptoms when time 1 symptoms were controlled. CONCLUSIONS: It was concluded that the reason for the differential performance of the two scales was that the ESS, like the shame interview, assesses specific areas of shame related to self and performance, whereas the TOSCA assesses general shame and may therefore be more prone to mood-state effects.

Vividness of visual and haptic imagery of movement.

This study investigated the relationships between visual and haptic imagery of movement. A total of 338 subjects, all university students aged between 18 and 26 years, completed the Vividness of Movement Imagery Questionnaire, which evaluates capacity for visual imaging, and a modification of this questionnaire, the Vividness of Haptic Movement Imagery Questionnaire, designed to evaluate capacity for haptic imaging. Scores on the two questionnaires were significantly correlated .60.
