The process of recovering from anorexia nervosa.

The authors describe the process of recovering from anorexia nervosa as it is revealed by 13 women who have recovered from the illness. Emphasizing the patient's perspective, the paper reviews the perceived causes of the disorder, helpful and harmful therapy-related and life experiences, features hardest to change, the defensive function of anorexia nervosa and residual traits. Generally, the movement toward health entails forming a therapeutic relationship in which the anorexic can identify and express feelings, experience the empathic, nonjudgmental understanding of another person, separate from a pathological family system, resolve hostile dependent attachment to parents, assuage primitive guilt, and engage in the trials of adolescent psychosexual development to enter adulthood with the beginnings of a firm, cohesive sense of self.

Children of gay or lesbian parents.

There are no data to suggest that children who have gay or lesbian parents are different in any aspects of psychological, social, and sexual development from children in heterosexual families. There has been fear that children raised in gay or lesbian households will grow up to be homosexual, develop improper sex-role behavior or sexual conflicts, and may be sexually abused. There has been concern that children raised by gay or lesbian parents will be stigmatized and have conflicts with their peer group, thus threatening their psychological health, self-esteem, and social relationships. These fears and concerns have not been substantiated by research. Pediatricians can facilitate the health care and development of these children by being aware of these and their own attitudes, by educating themselves about special concerns of gay or lesbian parents, and by being a resource and an advocate for children who have homosexual parents.

Development of internal body image from childhood to early adolescence.

The study aimed at providing a psychogenetic characterization of the developmental patterns through which the graphic representations of the inside of the body progress, and at testing the developmental hypothesis of a progressive integration of genital parts into the internal body image. The drawings of the inside of the body by 360 children of both sexes and of age 5 to 10 yr., and by 213 early adolescents, boys and girls of ages 11 to 14 yr., were analyzed on the basis of a series of qualitative and quantitative criteria. The relationship between age and type of drawing corresponds to expectations in children and appears less linear for early adolescents. Further, data support the developmental process of integration of genitals into the body image and show differential aspects in boys and girls.

Family perception of rapists and pedophiles.

Previous research indicates that sex offenders were more likely to have been reared in dysfunctional families and to have identified less with their parents than were individuals in most comparison groups. It is unclear whether such failure to identify may be related to the perceived parenting styles and attitudes of the sex offenders' parents. 16 rapists, 18 pedophiles, 9 general offenders, and 11 college students completed the Children's Report of Parental Behavior Inventory to assess perceived parental communication patterns and attitudes. It was predicted that rapists and pedophiles would perceive their parents as (1) having been more rejecting, (2) as having exerted more control while encouraging less autonomy, and (3) as having provided harsher discipline than would individuals in comparison groups. Results supported the first two predictions.

Case report of a female-to-male transsexual homicide offender.

A 22 year old female-to-male half-Aboriginal transsexual had been exposed to gross neglect and violence, separation and inconsistent cultural supports during childhood. Her mother had also been convicted of homicide in a context of alcohol and violence. Transsexual identification, antisocial behaviours, self mutilation, substance abuse and unmet dependency needs were evident from childhood or early adolescence. The killing was a confrontational peer group stabbing in a brawl under influence of alcohol--the male mode of homicide. This is the first known case in world literature of a female-to-male transsexual guilty of homicide.

Alpha rhythm and the pineal gland.

Alpha rhythm is classically described as a bilateral posterior rhythm of substantially constant frequency in the range of 8-13 Hz which is enhanced by mental relaxation and blocked by attention. Since the full expression of alpha rhythm has been shown to occur coincident with puberty, it is possible that the establishment of alpha rhythm is subject to neuroendocrine influences which govern psychosexual maturation. There is ample evidence to indicate that the pineal gland is implicated in cerebral maturation and psychosexual development. Nocturnal plasma melatonin levels have been shown to decline progressively throughout childhood reaching a nadir at puberty. Since administration of melatonin has been reported to block alpha rhythm, it is proposed that the progressive decline in melatonin secretion during childhood facilitates the maturation of the alpha rhythm. Consequently, the presence of alpha rhythm could be used as a neurophysiological marker for the activity of the pineal gland and disorders associated with absent or delayed maturation of the alpha rhythm such as autism, dyslexia, personality disorders, epilepsy, Tourette's syndrome, and schizophrenia might be related to disturbances of pineal melatonin functions in early life. Moreover, since the EEG patterns associated with cerebral immaturity (i.e., slowing, absence of alpha activity) are more pronounced in the left hemisphere, this hypothesis implies differential influence of the pineal gland on hemispheric maturation potentially accounting for the vulnerability of the left hemisphere to cerebral insults.

Adolescent sexual matricide following repetitive mother-son incest.

A case of a 16-year-old male who committed a sexual matricide following years of mother-son incest is reported. After murdering his mother by strangulation, which itself was sexually arousing, the youngster engaged in both vaginal and anal necrophilia. The homicide occurred while the perpetrator was in a dissociative state and experiencing what has been referred to as a catathymic crisis: the sudden release of emotionally charged psychic conflict and tension, resulting in extreme violence within an interpersonal bond. Discussion of maternal image and maternal sexual conduct in relationship to the psychosexual development of adolescent males offers insight into the motivation in this extremely rare case.

[Psychodynamic theory of childhood psychoses].

A review of the different concepts of childhood psychoses is made. Margaret Mahler's observations of normal and psychotic children concerning their relationship with their mothers during their first years of life are reviewed. Through the discussion of the concepts that Mahler has reached, understanding the dynamics of growth and their alterations, we obtain a clear understanding of the defects in psychological functions observed in the psychoses, including early infantile autism (Kanner) and symbiotic psychosis (Mahler). A correlation of events occurring at different levels of growth: object relations, cognitive growth, psychosexual development, growth of anxiety, etc., is attempted.

[Feminizing genital plasty in patients with intersexuality].

Feminizing genitoplasty (FGP) involves modification of a hypertrophic clitoris, vaginoplasty, and modelling of the labia. FGP is performed during childhood and usually in one stage. In 1996-1999 30 patients with intersex were operated on. 25 girls with female pseudohermaphroditism caused by congenital adrenal hyperplasia (CAH), 3 patients with mixed gonadal dysgenesis (MGD), 1 patient with male pseudohermaphroditism and one female patient with true hermaphroditism (HFV). Seventeen patients were between the ages of 3 and 6 years, 13 patients (all female with CAH) were 13 to 21 years old. One-stage FGP, i.e. partial resection of the clitoris and vaginoplasty (VP) was performed in 11 patients. A two-stage operation, i.e. partial resection of the clitoris and subsequent vaginoplasty, was done in 3 patients (all with VHN with a high-orifice vagina). Vaginoplasty after excision of the clitoris was made in 13 patients (all with CAH, 5 of them with a high-orifice vagina). Three female patients (2 VHN, 1 HFV) had partial resection of the clitoris and wait to have VP. Fourteen patients with partial resection of the clitoris healed p.p. Healing p.s. was recorded in 5/27 patients with vaginoplasty (all CAH). Three of them were older, menstruating and obese. In 4/14 patients after partial resection of the clitoris both modelling and fixation of the glans clitoridis was necessary. Urinary incontinence was not recorded. Stricture of the vaginal orifice was found in one 6-year-old patient with CAH after reconstruction of a high-orifice vagina. Thirteen older patients have a normal vaginal orifice, two of them have sexual intercourse. FGP can be performed already at toddler age, the cosmetic effect is favourable, the glans clitoridis is preserved and the psychosexual development of patients is not impaired. FGP can be used also in adults, severely virilized patients.

[Intersex: which gender should be chosen?].

A newborn infant with ambiguous genitalia is a medical emergency, and the choice of gender must take into account both the chromosomal and the gonadal sex, the hormonal milieu during fetal life, surgical aspects, the anatomy of the internal genitalia, as well as the prospects for future fertility, normal psychosexual development, and sexual function as an adult. Counselling requires paediatric endocrine, surgical, and psychological expertise, but the lack of knowledge of the long-term consequences of an intersex condition hampers rational treatment. It has long been customary to assign the child a female gender, whereas recent research points to a choice of a gender compatible with the chromosomal sex, if at all possible. This article reviews our knowledge in this field.

[Adolescent arsonists--psychodynamics, family dynamics and attempt at framing a typology based on 40 expert opinions].

Psychiatric, psychological and medical data of 40 adolescent firesetters are obtained from a retrospective analysis of expert opinions which have been drawn up by the Department of Child and Adolescent Psychiatry in Tübingen over a period of 16 years (1981-1997). This analysis mainly focusses on the patients' physical and psychosexual development, their social integration and competence and their family dynamics. It further concentrates on the psychic situation at the time of the offense, the reasons and motives given for firesetting (by the patient and by the expert) and specific psychological test results. According to the results three different characteristic groups can be found: 1st a group of offenders who have set fires to their parents' house, 2nd firesetters being member of the firebrigade, and 3rd an unspecific inhomogeneous group of firesetters.

[Psychodynamics of ideological democracy].

In order to understand "ideological democracy" and its consecuences for therapy it is important to study the ideology of group members from the psychodynamic standpoint. Ideology is understood as a system of rules to interpret reality, although from a theoretical point of view, we contend reality itself is free from ideology. Ideology is thus defined as a cognitive system, stemming from emotional matrices of the psychosexual development of personality. The understanding of how the members of the Therapeutic Community attain "ideological democracy" is, to our mind, essential in order to be able to attain therapeutic goals. After a brief introduction the paper deals with ideology and its relationship with different personality structures: hysteric, phobic, psychopatic, depressive, paranoid and schizoid. Several common mechanisms for building ideology are set forth. The final conclusion is that such an analysis helps to ascertain the best strategies for community therapy.

[Pedophrenia].

Among childhood psychoses which are so polymorphous and so liable to modifications because of the development of the personality, a rather special place can be given to a form which we intend to call "Pedophrenia". It corresponds roughly to hebephrenia, but occurs before adolescence, sometimes even as early as the first months of life, as it can be a continuation of a precocious autism. Furthermore, it also involves the classic discordances, rites, stereotypies, etc... Because of the age, the communication processes are expecially affected, malleability is greater than in hebephrenia, but the prognosis remains serious.

Adolescent scoliosis patients. Personality patterns and effects of corrective surgery.

Personality patterns of 84 adolescent scoliosis patients were assessed and an evaluation made of certain psychological effects of corrective surgery. Results indicate the following: Scoliosis has fewer psychologically debilitating effects on younger adolescents (those under 16) than it does on older adolescents; scoliotic boys show comparatively better general personality integration than girls; scoliotic girls are less disturbed in psychosexual development than are boys. Postoperatively, boys can be expected to show less overt incapacitation than girls; psychologically, boys will require a longer period of recuperation than will girls; boys can be expected to present fewer immediate management problems than girls; the internalization of a sense of "difference" from the normal in adolescent scoliotics increases in direct relation to age. Consequently, the eariler surgical correction can be undertaken the better. Important differences exist between the psychological "set" of male and female adolescent patients. For boys, self-acceptance is at the core. Postoperative surgical care should thus focus on reinforcing characteristics of personal adequacy, namely, the ability to compete, and fulfill reasonable goals. For girls, acceptance by others is paramount. Postoperative care is best directed toward strengthening feelings of attractiveness (of any personal characteristic), worth, and, above all, future interpersonal possibilities.
