Fos, RVLM-projecting neurons, and spinally projecting neurons in the PVN following hypertonic saline infusion.

Hypertonic saline (HTS; 1.7 M) infused intravenously into conscious rats increases the production of Fos, a marker of cell activation, in the hypothalamic paraventricular nucleus (PVN). The parvocellular PVN contains subpopulations of neurons. However, which subpopulations are activated by HTS is unknown. We determined whether PVN neurons that innervate the rostral ventrolateral medulla (RVLM) or the spinal cord (important autonomic sites) expressed Fos following HTS. Experiments were performed 24-96 h after chronic implantation of an intravenous cannula. HTS significantly increased the number of Fos-positive cells. In the parvocellular PVN, the maximum number of Fos-positive cells occurred rostral of the anterior-posterior level at which the number of neurons that projected to the medulla or spinal cord peaked. Compared with controls, HTS did not significantly increase the number of double-labeled neurons. These findings demonstrate that an elevation in plasma osmolality activates PVN neurons but not the subgroups of PVN neurons with projections to the RVLM or to the spinal cord.

The Pentalfa project. 3: participant evaluations of the interactivity of the educational videoconferences and the effectiveness of project promotion.

A distance continuing medical education programme was established at the Katholieke Universiteit Leuven using multipoint videoconferencing via ISDN lines at 384 kbit/s. During each academic year, 24-26 sessions were organized. The interactivity afforded by the videoconferencing was investigated during the first three years of the project. More than 60% of the participants rated the interactivity as good or very good. No differences were found between the sites or over the three years of the project, but female participants were more positive (66% making ratings of good or very good) in this regard than male participants (57%). Also, the effectiveness of the various types of publicity used to promote the programme was analysed. A brochure was the main source of information (62% of participants heard of the programme by this means), followed by direct mailings (27%), posters (15%), personal contact (14%) and the programme's Website (1%). The proportions of participants who heard of the programme through these different means varied with age and gender.

The Maine electronic document delivery project: a cooperative project of Maine hospital libraries and the NN/LM New England Region.

The article discusses an Ariel document delivery project with seven Maine hospital libraries, the University of Massachusetts Medical School Library (UMass), and the New England Regional office of the National Network of Libraries of Medicine. Funding was awarded to six network members to purchase equipment or Ariel software. UMass served as a document provider. During the test, libraries received documents from UMass via Ariel or via the Web as a PDF document. This form of document delivery was faster than standard service with better quality of delivered articles. The column describes the project and outlines possible future steps.

The Ottawa County project: a report of a tuberculosis screening project in a small mining community.

Following a retrospective review of tuberculosis cases reported from Ottawa County, Oklahoma, from 1969 through 1973, a selective tuberculosis screening project was implemented. Screening of a "target group" of the population, 519 former miners, greater than or equal to 50 years of age, resulted in the discovery of abnormal chest X-rays in 182; (103 with silicosis, 36 with silicotuberculosis, 12 with inactive tuberculosis, and 31 with other abnormalities). Eighty-five of these persons had positive tuberculin skin tests. Preventive therapy was recommended for 50, and 36 completed the prescribed course of treatment. Eight new bacteriologically confirmed cases of tuberculosis were found and treated. A large number of persons (1,904) residing in the same area who were not part of the target group were also screened for tuberculosis. This group contained a large number of positive tuberculin reactors but very few were candidates for isoniazid preventive therapy. Thirteen persons in this group had abnormal chest X-rays consistent with inactive tuberculosis but 12 had been identified and given preventive therapy before the project began. These data suggest that selective approaches to screening for tuberculosis in a community which are based on an in-depth retrospective review of the tuberculosis case register can be highly successful.

WHO Multinational Project for Childhood Diabetes. WHO Diamond Project Group.

Insulin-dependent diabetes mellitus (IDDM) is one of the most important chronic diseases of children worldwide. IDDM leads to an 8- to 10-fold excess risk of mortality in developed countries, whereas in developing countries, most cases die within a few years. A 60-fold international gradient in IDDM incidence has been reported, and epidemic periods have been identified. A new World Health Organization program, Multinational Project for Childhood Diabetes (Diabetes Mondiale or DIAMOND), has been developed to investigate and characterize global incidence, mortality, and health care. Over 10 yr (1990-1999), this study will collect population-based data concerning IDDM in greater than 90 centers in 50 countries worldwide. The goals of DIAMOND are to collect standard information on incidence, risk factors, and mortality associated with IDDM; evaluate the efficiency and effectiveness of health care and the economics of diabetes; and establish national and international training programs in diabetes epidemiology. It is hoped that the DIAMOND project will be instrumental for the prevention of this serious disease and its sequelae.

Transplanted neuroblasts differentiate appropriately into projection neurons with correct neurotransmitter and receptor phenotype in neocortex undergoing targeted projection neuron degeneration.

Reconstruction of complex neocortical and other CNS circuitry may be possible via transplantation of appropriate neural precursors, guided by cellular and molecular controls. Although cellular repopulation and complex circuitry repair may make possible new avenues of treatment for degenerative, developmental, or acquired CNS diseases, functional integration may depend critically on specificity of neuronal synaptic integration and appropriate neurotransmitter/receptor phenotype. The current study investigated neurotransmitter and receptor phenotypes of newly incorporated neurons after transplantation in regions of targeted neuronal degeneration of cortical callosal projection neurons (CPNs). Donor neuroblasts were compared to the population of normal endogenous CPNs in their expression of appropriate neurotransmitters (glutamate, aspartate, and GABA) and receptors (kainate-R, AMPA-R, NMDA-R. and GABA-R), and the time course over which this phenotype developed after transplantation. Transplanted immature neuroblasts from embryonic day 17 (E17) primary somatosensory (S1) cortex migrated to cortical layers undergoing degeneration, differentiated to a mature CPN phenotype, and received synaptic input from other neurons. In addition, 23.1 +/- 13.6% of the donor-derived neurons extended appropriate long-distance callosal projections to the contralateral S1 cortex. The percentage of donor-derived neurons expressing appropriate neurotransmitters and receptors showed a steady increase with time, reaching numbers equivalent to adult endogenous CPNs by 4-16 weeks after transplantation. These results suggest that previously demonstrated changes in gene expression induced by synchronous apoptotic degeneration of adult CPNs create a cellular and molecular environment that is both permissive and instructive for the specific and appropriate maturation of transplanted neuroblasts. These experiments demonstrate, for the first time, that newly repopulating neurons can undergo directed differentiation with high fidelity of their neurotransmitter and receptor phenotype, toward reconstruction of complex CNS circuitry.

[The influence of recognition of a visual stimulus on evoked potentials of the projection and non-projection areas of the cerebral cortex].

Evoked potentials (EP) were recorded in the projection and non-projection areas of the cerebral cortex in juveniles in response to exposures of structured visual stimuli with subthreshold and supraliminal durations. The data obtained have shown that recognition of the presented stimulus is attended with intensification of the EP late complex. This effect is most pronouned in the central and frontal parts of the cortex. The Nv component with a 240 to 300 msec latency has a more regular connection with recognition as compared with other components.

[Theories and methodologies of engineering designs on sustainable agricultural land consolidation project--a case study of Xuemeiyang land consolidation project in Changtai County, Fujian Province].

The concept and characteristics of engineering designs on sustainable agricultural land consolidation project were discussed in this paper. Principles, basic methods and procedures of engineering designs on agricultural land consolidation project were put forward, which were successfully adopted for designing agricultural land consolidation in Xuemeiyang region of Changtai County, including diversity designs of sustainable land use, engineering designs of soil improvement, roads, ditches, and drains for protecting existent animal environments, and design of ecological shelter-forests in farmland. Moreover, from sustainable economic, ecological and social points, the results of these engineering designs were evaluated based on fouteen important indexes. After carrying out these engineeringdesigns, the eco-environments and agricultural production conditions were significantly improved, and the farm income was increased in planned regions.

Quality care improvement program in a community-based participatory research project: example of Project DIRECT.

A continuous quality care improvement program (CQIP) was built into Project DIRECT (Diabetes Interventions Reaching and Educating Communities Together) to improve providers' patterns of diabetes care and patients' glycemic control. Project DIRECT consisted of a comprehensive program aimed at reducing the burden of diabetes in the vulnerable high-risk African-American population of southeast Raleigh, NC. Forty-seven providers caring for this target population of adult diabetes patients were included in this quasi-experimental study. At the initial session, providers learned about the CQIP components, completed a planning worksheet, and chose a CQIP coordinator. Educational events included continuing education in practices and through conferences by experts, and guideline distribution. Follow-up was accomplished through phone calls and visits. Effectiveness was measured by a change in prevalence of selected patterns of care abstracted from 1,006 medical charts. Appropriate statistical methods were used to account for the cluster design and repeated measures. At the fourth follow-up year, approximately 40% of providers still participated in the program. Among the providers who stayed in the program for the whole study period, most selected quality care patterns showed significant upward trends. Glycemic control indicators did not change, however, despite an increased number of hemoglobin A1c tests per year. A diabetes CQI program can be effectively implemented in a community setting. Improved performance measures were not associated with improved outcomes. These results suggest that a patient-centered component should reinforce the provider-centered component.

Claustro-neocortical projections in the rhesus monkey (projections to area 6).

The distribution of claustral neurons projecting to area 6 on the convexity and medial wall of the hemisphere was studied in four macaque monkeys. In all cases, HRP-positive neurons were found almost throughout the antero-posterior extent of the claustrum in its dorsal half. Following HRP injections in area 6 on the medial wall of the hemisphere most of the labelled neurons were found in the dorsal and dorsolateral portions of the claustrum. After injections in area 6 on the hemispheral convexity, labelled neurons predominated in about the middle third of the vertical extent of the claustrum. Perikaryal polymorphism and considerable differences in perikaryal size are features characteristic of claustral neurons projecting to area 6. There is a preponderance of labelled neurons whose perikarya are noted for their multipolar or pyramidal (triangular) shape. Oval and spindle-shaped neurons were found less frequently. The size of the perikarya ranged from 11 to 26 microns while most of the labelled neurons had perikarya ranging form 14 to 18 microns in size.

The Beta-Blocker Pooling Project (BBPP): subgroup findings from randomized trials in post infarction patients. The Beta-Blocker Pooling Project Research Group.

The objective of the Beta-Blocker Pooling Project (BBPP) was to collect and analyse data from the major long-term secondary prevention trials in order to determine whether there are subsets of post-infarction patients who benefit to a greater or lesser extent from beta-blocker therapy than the average patient population. One-year all-cause mortality data from nine trials involving 13,679 patients were obtained. Overall, mortality was 24% lower in the beta-blocker group compared to the placebo group. However, there was heterogeneity among the results of the trials, which tested seven different beta-blockers. Subgroups with high placebo group mortality (e.g. patients with a history of previous myocardial infarction (MI), angina pectoris, mechanical or electrical complications, and digitalis usage) seemed to benefit particularly from beta-blocker treatment. These findings were consistent in the nine trials. Patients in the lower risk subgroups also appeared to benefit from beta-blockers, but this benefit was smaller in absolute terms and inconsistent across the trials. There was no evidence that treatment outcome was related to gender, baseline level of heart rate or blood pressure, or time of initiation of treatment after hospital admission. In conclusion, the Pooling Project indicates that high risk MI patients, without contraindications to beta-blockers, are the prime candidates for long-term therapy, but the lower risk patients may also receive some benefit.

[Myocardial infarction and complications. Longitudinal observation of a population of 280,000 women and men--Project POL-MONICA Krakow. I. Genesis and objectives of the WHO MONICA Project].

The WHO MONICA Project is a multicentre collaborative study coordinated by The World Health Organization. The aim is to measure trends in cardiovascular disease mortality and morbidity and to assess the extent, to which these trends are related to changes in risk factor levels and in medical care measured at the same time in defined communities in different countries. There are 38 participating research centres from 21 countries from 4 continents, including two Polish centres: POL-MONICA Warszawa and POL-MONICA Kraków. Population studied in POL-MONICA Warszawa Project consisted of residents of two districts of Warsaw capital. The interest of the research of the POL-MONICA Kraków focuses on problems of the myocardial infarction in semi-rural population and the population observed consisted of men and women at age 25-64 years residents of one Polish province-Tarnobrzeg Voivodship.

Molecular epidemiology of insulin-dependent diabetes mellitus: WHO DiaMond Project. WHO DiaMond Molecular Epidemiology Sub-Project Group.

The WHO DiaMond Molecular IDDM Epidemiology Sub-Project is testing the hypothesis that population variation in the frequency of high-risk HLA-DQ alleles is a primary determinant of the global patterns of IDDM incidence. Data are currently available for 16 populations, and reveal significant variations in the frequencies of HLA-DQA1 and DQB1 alleles among the case and the control groups. However, DQA1 x Arg-(52) and DQB1 x non-Asp-57 (ND) were consistent and independent markers of IDDM susceptibility in all populations, except Japan. Individuals who carried only DQA1 x R and DQB1 x ND alleles had an IDDM risk similar to that observed for first degree relatives of affected individuals (3%-5%). Such information is essential for the development of clinical strategies or disease prevention approaches for the general population or individuals at high-risk. Thus, the DiaMond Molecular Epidemiology Sub-Project provides an excellent model that can be followed to assess the impact of new genetic discoveries on medicine and public health practice for diabetes and other chronic diseases.

[Evaluation of a project of primary prevention of smoking: the Barcelona PASE project].

The results of the short-term evaluation of a program to prevent the use of tobacco and other substances among schoolchildren of sixth and seventh grade are presented. Schoolchildren who participated in the project show significant differences when compared to a reference group through a matched analysis in the regular use of tobacco and having bought tobacco, but not in the experimentation nor in the intention to smoke. The results suggest the program is useful to prevent the onset of smoking, although longer term evaluation data are needed. The results and their implications for a comprehensive smoking prevention approach in schools are discussed.

Pathologic findings from the National Surgical Adjuvant Breast Project (Protocol 4). Discriminants for 15-year survival. National Surgical Adjuvant Breast and Bowel Project Investigators.

Twenty-one pathologic and five clinical features of Stage I and II invasive breast cancers from 620 patients enrolled in National Surgical Adjuvant Breast Project Protocol B-04 were analyzed to determine their predictive value for 15-year survival. Ten pathologic features had a statistically significant univariate prognostic relationship with long-term survival. These were analyzed further using a Cox regression model that found only the number of nodal metastases (0, 1-3, 4-9, or 10+), tumor size (< or = 2.0 cm versus 2.1-4 and 4.1 + cm), and the presence or absence of nipple involvement to be significant independent prognostic discriminants. Combinations of these three characteristics modestly increased their individual prognostic value. Differences in the findings in this study from those observed in the same patient population at 5 and 10 postoperative years and their relationship to other markers detected by ancillary pathologic techniques briefly are discussed.

Pathologic findings from the National Surgical Adjuvant Breast Project (NSABP) Protocol B-17. Intraductal carcinoma (ductal carcinoma in situ). The National Surgical Adjuvant Breast and Bowel Project Collaborating Investigators.

BACKGROUND: Controversy exists concerning the natural history of ductal carcinoma in situ (DCIS) of the breast, including its pathologic expression and treatment. This controversy has been fostered largely by the retrospective nature and limited sample sizes of extant studies. METHOD: Resolution of some of these issues was attempted by analyzing the pathologic features of 573 examples of DCIS obtained from a larger cohort of 790 women with DCIS enrolled in Protocol B-17 of the National Surgical Adjuvant Breast Project. This prospective randomized clinical trial was performed to assess the efficacy of local breast irradiation to reduce the incidence of second ipsilateral breast tumors (IBT) after lumpectomy. RESULTS: Tumor and patient characteristics, including significantly less IBT for those treated by lumpectomy and irradiation than lumpectomy alone, were almost identical for the subset comprising this analysis and the total B-17 cohort reported previously. The presence of moderate/marked comedo necrosis, which was evaluated as an independent parameter rather than as a specific histologic type of DCIS and uncertain/involved lumpectomy margins were the only statistically significant independent predictors of IBT for patients treated by lumpectomy as well as irradiation. The latter markedly reduced the annual hazard rates for the IBT associated with these indicators. CONCLUSIONS: Although not an endpoint of this study, the authors' findings suggest that the beneficial effect of irradiation in reducing IBT after lumpectomy for DCIS occurs with small (< 1.0 cm.) and larger lesions. Moderate/marked comedo necrosis and uncertain/involved lumpectomy margins represent independent predictors of IBT.
