In 1984, ictus patients were hospitalized at the neurological department of the Regional Hospital, Ustí n. Labem, Czechoslovakia. This group included 70% hypertensive patients and 30% normotensives. The diagnosis was determined through direct blood pressure measuring in 71% of them, 26% were diagnosed with the aid of anamnesis and clinical records, and 2.5% were determined on the basis of ECG, eyeground changes or echocardiography. Only 56% of the followed-up patients were found to have signs of hypertrophy and left ventricular overload. The hypertensive patients, both living and dead, were found to have the frequency of clinically manifest ischaemic heart disease significantly higher than the normotensives. The five-year follow-up of hypertensive ictus patients showed a 38.5% death rate, but only 21.7% in normotensives, the average age of both groups being the same. Insufficient follow-up care of hypertensive ictus patients was found in 26% cases.
On the basis of its pharmacologic action Deanol (dimethyl aminoethanol) was hypothesized to be of benefit in the Gilles de la Tourette Syndrome. In one case report the addition of Deanol to perphenazine did not result in an improvement of uncontrollable movements or involuntary speech utterances. Gilles de la Tourette Syndrome is a condition combining organic and psychogenic features existing in the interface between two etiologies. Classically the disease begins in childhood and is characterized by the appearance of sudden involuntary movements, involuntary speech utterances frequently consisting of curse words (coprolalia), and imitative phenomena such as echolalia and echopraxia. Neurotic symptomatology such as anxiety and obsessive thinking have also been reported. This condition is regarded neuropharmacologically as a dopaminergic state that responds to drugs with antidopaminergic activity e.g. the phenothiazines and butyrophenones. Deanol (dimethyl aminoethanol) is a putative cholinergic agonist and has reported effectiveness in conditions where there is a predominance of dopaminergic versus cholinergic activity, e.g. levodopa-induced dyskinesias, neuroleptic induced tardive dyskinesia, and Huntington's chorea. Because of its effectiveness in dopaminergic states it was hypothesized that Deanol could also be of benefit in the Gilles de la Tourette Syndrome.

Four cases of cerebral thrombophlebitis, 3 of which occurred during post-partum and 1 during the first trimester of pregnancy, are reported. The symptoms were misleading, being predominantly psychiatric, with secondary appearance of an epileptic attack or motor deficit. The value of cerebral angiography is stressed in the literature.
Depression is a medical illness related to an interplay of genetic, biochemical and environmental/experiential factors. Careful diagnosis, using contemporary psychiatric criteria, simplifies treatment by the family physician. Awareness of these criteria may lead to improved selection of candidates for drug treatment and early intervention in recurrent illness. The new nosology in psychiatry avoids much confusion and should take some of the guesswork out of psychiatric treatment in the family physician's office.
The authors report the case of a 24 year old man with no previous disease who presented with a severe autonomic neuropathy. This included major gastrointestinal dysfunction characterised by decreased peristalsis without distension and paralysis of the gall bladder, and orthostatic hypotension with a normal cardiac tachycardia reflex. There was an associated sensory neuropathy affecting heat sensitivity without motor dysfunction and an increased CSF protein content. The proprioceptive nerve fibre conduction was decreased but another nerve conduction was normal initially. The mesenteric plexuses examined during sigmoidectomy performed for peritonitis due to multiple bowel perforations caused by fecoliths, showed no significant changes. Peripheral nerve biopsy revealed massive rarefaction of myelinated fibres which were of small diameter, and of the unmyelinated fibres, mainly due to axonal degeneration. Only two similar cases with incomplete recovery were found in the literature. In our case, a complete recovery was observed. The cause of the condition is unknown.

Seven cases presenting with the features of Byler's disease (BD) are reported. The diagnosis of BD can only be made after exclusion of other causes of cholestasis. It seems likely that BD is a syndrome and that more accurate biological and histological investigations will allow to differentiate several entities within this affection. At the present, liver transplantation appears to be the only rational treatment.
Acute pseudo-obstruction of the colon involves acute colic distension without mechanical obstruction or stercoroma in a previously healthy colon. Our study is of 13 patients, all of whom presented a pre-existing extradigestive disorder, for which 12 were taking medication. Nine patients were treated by mechanical ventilation, and five of these had previously presented meteorism. Colic dilatation was maximal in the cecum, the diameter of which measured 9 to 14.5 cm. Two patients were treated by decompression colonoscopy, which completely cured meteorism in one case. Two patients treated by digestive aspiration died due to extra-abdominal causes. Operations were carried out on 6 patients: once for peritonitis due to cecal perforation five times for deterioration of meteorism. Five patients died due to postoperative complications. These observations show that the diameter of the cecum should be monitored daily. When it exceeds 9 cm, decompression colonoscopy must be performed. Surgery should only be envisaged when there are setbacks, due to the seriousness of the operation and the possibility of postoperative complications.
