[Teaching the nursing process at undergraduate nursing programs in the State of Sao Paulo].

The goal of this study was to describe the teaching of the nursing process at undergraduate nursing programs in the state of São Paulo. The sample was formed by 247 faculty members. With respect to the study methodology, authors elaborated a questionnaire that was sent to 899 faculty members from 32 public and private nursing schools in the state of São Paulo. Results showed regarding the teaching of the nursing process that, from the total of 147 faculty members who taught the content, 66 (44.9%) were from 19 private schools and 81 (55.1%) from 8 public schools.

Integrating the nursing process into a nursing quality improvement program.

In 1991, the Joint Commission's nursing services standards scoring guidelines went into effect. Nancy Claflin discusses the changes in the nursing process needed to meet these revised standards under the rubrics of assessment, planning, intervention, and evaluation. She then presents an extensive implementation plan for integrating this revised nursing process into a nursing QI program. The focus of this article is both on the principles of quality improvement and the exigencies of aspects of care, standards of nursing practice, and indicators.

Evolution of the nursing process.

The nursing process, long considered the basic intellectual tool for nursing, has continued to evolve through use by advanced practice nurses (APNs). Traditionally, the nursing process has been comprehensive in guiding the assessment and intervention phases but has left unspecified the cognitive processes involved in clinical decision making. In an effort to compensate for that weakness, nurses in advanced practice have typically integrated medical diagnostic reasoning into the nursing process. The work of Benner and colleagues (1984, 1996) in explicating the unique reasoning process used by APNs has helped further our understanding of clinical decision making in advanced practice. The evolved nursing process, as described in this article, includes the array of clinical decisions that must be made during a patient encounter, including assessment decisions, diagnosis, choice of interventions, and identification of outcomes. This article reviews the nursing and medical literature related to both clinical decision making and diagnostic reasoning, and applies that literature to the evolution of the nursing process.

A restructured nursing process model.

Nursing process has guided clinical nursing practice for more than 25 years. During this time, many changes have occurred in healthcare practices, delivery systems, and nursing. The cumulative effects of such changes show a need to restructure the basic nursing process model. The author presents a three-level differentiated nursing process model.
