The effect of surgical gowns made with barrier cloth on bacterial dispersal.

A dispersal chamber (body box) technique has been used to compare bacterial dispersal from the skin of subjects carrying out a stepping test under controlled conditions while wearing four differing garment systems namely: basic underwear, cotton 'blues' (standard pyjama style jacket and trousers for men or dress for women), ankle socks, boots for men and shoes for women, mask and theatre hat; the basic set covered with a cotton gown; the basic set covered by a gown with a front made from GORE-TEX fabric in which an expanded polytetrafluoroethylene membrane is sandwiched between layers of woven or knitted polyester; the basic set covered with a fully enclosed suit of the same fabric. A slit sampler was used to measure the number of bacteria liberated in a downward current of air. Six subjects (three female and three male) were studied. Males liberated more bacteria. Covering the 'blues' with a cotton gown increased the bacterial count; a gown of the new material reduced the increase by 50%, and the suit cut the dispersal to virtually zero. Preliminary work suggests that GORE-TEX garments survive laundering better than cotton, and may be cost-effective, but are not yet as comfortable. Research is presently in progress to improve this aspect.

Occurrence of some organic UV filters in wastewater, in surface waters, and in fish from Swiss Lakes.

Organic UV filters are used in personal care products such as sunscreen products, and in cosmetics, beauty creams, skin lotions, lipsticks, hair sprays, hair dyes, shampoos, and so forth. The compounds enter the aquatic environmentfrom showering, wash-off, washing (laundering), and so forth via wastewater treatment plants (WWTPs) ("indirect inputs") and from recreational activities such as swimming and bathing in lakes and rivers ("direct inputs"). In this study, we investigated the occurrence of four important organic UV filter compounds (benzophenone-3, BP-3; 4-methylbenzylidene camphor, 4-MBC; ethylhexyl methoxy cinnamate, EHMC; octocrylene, OC) in wastewater, and in water and fish from various Swiss lakes, using gas chromatographic/mass spectrometric analyses. All four UV filters were present in untreated wastewater (WWTP influent) with a maximum concentration of 19 microg L(-1) for EHMC. The data indicate a seasonal variation with influent loads higher in the warmer season (June 2002) than in the colder one (April 2002). The influent loads were in the order EHMC > 4-MBC approximately BP-3 > OC. The concentrations in treated wastewater (WWTP effluent) were considerably lower, indicating substantial elimination in the plants. 4-MBC was usually the most prevalent compound (maximum concentration, 2.7 microg L(-1)), followed by BP-3, EHMC, and OC. UV filters were also detected in Swiss midland lakes and a river (Limmat) receiving inputs from WWTPs and recreational activities. However, all concentrations were low (<2-35 ng L(-1)); no UV filters (<2 ng L(-1)) were detected in a remote mountain lake. Data from passive sampling using semipermeable membrane devices (SPMDs) supported the presence of these UV filters in the lakes and the river and suggested some potential for accumulation of these compounds in biota. SPMD-derived water concentrations increased in the order Greifensee < Zürichsee < Hüttnersee. This order is reversed from that observed for methyl triclosan, used as a chemical marker for WWTP-derived lipophilic contaminants in the lakes. This indicated inputs of UV filters from sources other than WWTPs to the lakes during summer, for example, inputs from recreational activities. Fish (white fish, Coregonus sp.; roach, Rutilus rutilus; perch, Perca fluviatilis) from these lakes contained low but detectable concentrations of UV filters, in particular, 4-MBC (up to 166 ng g(-1) on a lipid basis). 4-MBC concentrations relative to methyl triclosan were lower in fish than in SPMDs exposed in the same lakes, suggesting that 4-MBC is less bioaccumulated than expected or metabolized in fish. The lipid-based bioconcentration factor (BCF(L)) estimated from the fish (roach) data and SPMD-derived water concentrations was about 1-2.3 x 10(4) and thus approximately 1 order of magnitude lower than expected from its Kow value.

Stereoisomer composition of the chiral UV filter 4-methylbenzylidene camphor in environmental samples.

4-Methylbenzylidene camphor (4-MBC) is an important organic UV filter used in many personal care products such as sunscreens and cosmetics. After use, 4-MBC may enter the aquatic environment due to its release from skin during recreational activities (swimming, bathing) and from personal hygiene measures (washing, laundering of cloths) via wastewater treatment plants (WWTPs). In fact, 4-MBC has been detected in wastewater, in surface waters, and even in fish. 4-MBC can exist as distinct cis-(Z)- and trans-(E)-isomers, both of which are chiral. Despite the fact that stereoisomers often show a different biological behavior, the stereochemistry of 4-MBC has hardly ever been considered in environmental or biological studies. In this study, enantioselective gas chromatography-mass spectrometry (GC-MS) was used to determine the stereoisomer composition of 4-MBC. For stereoisomer assignment, the pure enantiomers of (E)-4-MBC were synthesized from (+)- and (-)-camphor. The photochemical isomerization (sunlight) of these (E)-isomers to the corresponding (Z)-isomers eventually allowed the configurational assignment of all four stereoisomers of 4-MBC. In a technical material and in a major brand sun lotion, 4-MBC was shown to consist entirely (>99%) of (E)-isomers and to be racemic (R/S, 1.00 +/- 0.02). Wastewater showed the presence of both (E)- and (Z)-4-MBC with a clear excess of (E)-isomers (E > Z). Untreated wastewater showed a nearly racemic composition (R/S= 0.95-1.09), suggesting that most if not all commercial 4-MBC is racemic. Treated wastewater indicated some excess of (R)- or (S)-stereoisomers (R/S, 0.89-1.17), likely as a result of some enantioselective (bio)degradation in WWTPs. Residues of 4-MBC in lakes and in a river with inputs from WWTPs and/or recreational activities consisted mainly of (E)-4-MBC and, with exception of one lake (Greifensee), showed a small enantiomer excess (R/S, 1.04-1.16). In Greifensee, 4-MBC showed a higher enantiomer excess (R/S, 1.70-1.83), probably as a result of more extensive biotic degradation in this lake. The analysis of 4-MBC in a small number of fish from these lakes indicated residues with nearly racemic compositions or a moderate enantiomer excess (R/S, approximately 1.0-1.2) in roach (Rutilus rutilus), whereas in perch (Perca fluviatilis) a much higher enantiomer excess (R/S, approximately 5) was observed. The data indicate that the stereoisomer composition of 4-MBC in environmental samples is not only a function of initial product composition but is also modified by enantioselective processes in lakes and biota (fish).

Child dermal sediment loads following play in a tide flat.

Dermal contact with sediment is sometimes identified as a pathway of concern in risk assessments. Dermal exposure to sediment is poorly characterized and exposure assessors may rely on default soil adherence values. The purpose of this study was to obtain sediment adherence data for a genuine exposure scenario, child play in a tide flat. This study reports direct measurements of sediment loadings on five body parts (face, forearms, hands, lower legs and feet) after play in a tide flat. Each of nine subjects participated in two timed sessions and pre- and post-activity sediment loading data were collected. Geometric mean (geometric standard deviation) dermal loadings (mg/cm(2)) on the face, forearm, hands, lower legs and feet for the combined sessions were 0.04 (2.9), 0.17 (3.1), 0.49 (8.2), 0.70 (3.6) and 21 (1.9), respectively. Participants' parents completed questionnaires regarding their child's typical activity patterns during tide flat play, exposure frequency and duration, clothing choices, bathing practices and clothes laundering. Data presented in this paper supplement very limited prior adherence data for sediment contact scenarios. Results will be useful to risk assessors considering exposure scenarios involving child activities at a coastal shoreline or tide flat.

Exposure to wheat allergen and fungal alpha-amylase in the homes of bakers.

BACKGROUND: Few data are available on exposure to occupational allergens in dwellings occupied by inhabitants with occupational exposure to allergens. In small bakeries working and living often takes place in the same building. It is possible that allergens from the bakery can be transported into the homes of the bakers, via the clothes or shoes of the baker. OBJECTIVE: The aims of this study were to investigate exposure to occupational allergens, wheat and fungal alpha-amylase in the homes of bakers, and evaluate potential determinants of exposure. Sensitization in family members to occupational allergens was investigated in a small preliminary survey. METHODS: Floor dust samples were collected in the homes of 34 bakers. Levels of wheat and fungal alpha-amylase allergens were determined in an extract of the dust samples. Blood samples were collected from bakers and their family members to determine the prevalence of sensitization to occupational allergens. RESULTS: The concentration of wheat and alpha-amylase allergens ranged from 38.9 to 172.4 microgeq/m(2) (GM), to 10.5-76.7 ngeq/m(2) (GM). Higher levels of dust and allergens were measured when the house could be reached directly through the bakery, and in houses with textile floor covers. Higher concentrations were also measured when bakers brought their work clothes and shoes into the house and when textiles from the bakery were laundered at home. Some family members appeared to be sensitized to wheat flour and alpha-amylase, but it cannot be excluded that they became sensitized because of their incidental presence in the bakery. CONCLUSIONS: Occupational allergens can be found in house dust from the homes of bakers and levels are associated with hygienic behaviour and distance to the bakery.

A questionnaire study of road pavers' and roofers' work-related skin symptoms and bitumen exposure.

BACKGROUND: Skin contact with the components in asphalt and bitumen can lead to irritant and allergic contact dermatitis, but few data are available in the dermatologic literature on the skin symptoms caused by work with bitumen. In addition, working methods have changed markedly during recent decades. METHODS: A questionnaire was delivered to 50 roofers and 101 road pavers. The questionnaire dealt with skin symptoms, symptoms caused by solvent products, the use of personal protection, smoking, eating habits, work conditions, changing and cleaning of overalls, etc. RESULTS: Forty-eight per cent of the road pavers and 58% of the roofers responded to the questionnaire. Relatively high percentages of work-induced skin irritation were reported by both the roofers (44%) and the road pavers (31%); 22% of the road pavers had dermatitis often or sometimes compared with 15% of the roofers. The hands, arms, face, and lower extremities were the most common sites affected. In addition to bitumen products, the road pavers considered amine adhesion-improving agents for paving and naphtha and solvents used in machine repairs, to be the main causes of their skin symptoms. The skin problems of the roofers were caused by man-made mineral fibers, cutback bitumen, and hot bitumen burns. CONCLUSIONS: Roofers endure greater exposure to chemicals than road pavers. Certain components, e.g. polycyclic aromatic hydrocarbons in bitumen, can be absorbed through the skin. Skin exposure should be lowered by keeping the tools, working clothes, shoes, and gloves clean. Overalls and gloves are recommended to be changed at least once a week. Water laundering is not sufficient in dissolving bitumen from overalls and underwear. Cleaning the skin with solvents or naphtha is not recommended, as they are skin irritants.

Long-term sick leave and its risk factors during pregnancy among Danish hospital employees.

AIM: The authors sought to describe risk indicators of long-term sick leave during pregnancy among hospital employees. METHODS: A register-based study was undertaken of 4,852 female hospital employees aged 20-45 years from the second largest hospital in Denmark during 1995-99 based on job titles, working time, sick leave, and births combined with a survey among a total of 773 women who had been pregnant during their employment (response rate 85%). RESULTS: Altogether 236 (31%) were on sick leave for at least 10% of their scheduled work time during their latest pregnancy and 169 (22%) had been absent at least 20% of the time. The pregnant women had an average sickness absence of 6.1 days per month, non-pregnant women 0.95 days per month. Sick leave was more frequent in late than in early gestation. Women employed as nursing aides or hospital orderlies, launderers, and nurses had more sick leave days than other hospital employees. Part-time work, previous sickness absence not related to pregnancy, and previous chronic back pain were risk factors for long-term sick leave as were much walking or standing, long working days, high work level, little practical support from supervisors and colleagues, low job control, much lifting and night or shift work. Sick leave was unrelated to family size, support from the family and number of working years. CONCLUSION: Long-term sick leave during pregnancy was frequent and to some extent predictable. Efforts should be made to organize work for pregnant women in a manner that optimizes their health and well-being.

Bacterial contamination of fabric stethoscope covers: the velveteen rabbit of health care?

To determine if fabric stethoscope covers pose an infection control problem, we studied how they are cared for by our healthcare workers and performed microbiological investigations on 22 covers collected over a 3-week period. Our study suggests that fabric stethoscope covers represent a potential infection control problem because they are used for prolonged periods, are infrequently laundered, and are contaminated with bacteria.

Guidelines for controlling urinary incontinence without drugs or catheters.

Catheterization of the elderly incontinent patient is frequently unnecessary. It is also often unsuccessful, as most patients have unstable bladders and therefore leakage continues around the tube. Simple measures, like toileting regimens and restricting fluids in the evening, are often effective alone. Incontinence garments restore a patient's self-respect, reduce laundering and should be used more frequently whilst other methods, such as drugs, are being tried.

Acceptability and compliance with wearing energy-shunting hip protectors: a 6-month prospective follow-up in a Finnish nursing home.

OBJECTIVES: To assess the acceptability and compliance with use of an energy-shunting hip protector in institutionalized elderly people. DESIGN AND SETTING: A 6 month prospective follow-up in a Finnish nursing home. PARTICIPANTS: 19 ambulatory nursing home residents with a high risk of hip fracture. MAIN OUTCOME MEASURES: The proportion of the residents who were willing to use the device, the number of hours of wearing the protector and the attitudes of the study subjects and the caregivers towards the appearance, comfort, fit, efficacy and laundering of the protector. RESULTS: 12 of the 19 ambulatory residents (63%) agreed to use the protector. During the study period, these subjects wore the protector on average for more than 90% of their active days, i.e. the days they were mobile. Two subjects wore the protectors at night time; the rest only during waking hours. Mean wearing time during waking hours exceeded 90%. CONCLUSION: External hip joint protectors are a feasible strategy to prevent hip fractures in institutionalized elderly people. The attitude, education and motivation of the staff may be a factor in achieving good user compliance. Further community-based studies on acceptability and compliance in wearing external hip joint protectors are needed for verification of benefits to the general population of older people.

Reduction of urinary 1-hydroxypyrene excretion in coke-oven workers exposed to polycyclic aromatic hydrocarbons due to improved hygienic skin protective measures.

The effect of hygienic skin protective measures on the internal exposure to polycyclic aromatic hydrocarbons (PAHs) was studied in 13 coke-oven workers. The study took place over 2 consecutive weeks. In 1 week the subjects worked under the normal circumstances, in the other week extra hygienic skin protective measures were instituted: laundered working clothes and a new pair of gloves before each 8-h work shift, and the washing both of the hands and of the face before each break. Biological monitoring was undertaken to measure the effect of the extra hygienic measures on the urinary 1-hydroxypyrene excretion, which is a measure of the internal PAH exposure. The increase of the urinary 1-hydroxypyrene concentration over the 4-day workweek was on average 37% lower when extra hygienic measures were taken, being 1.3 instead of 2.3 mumole 1-hydroxypyrene per mole creatinine (P = 0.03, N = 13). This study demonstrates that simple hygienic skin protective measures result in a significant reduction of the internal PAH exposure.

Occupation and bladder cancer: a population-based, case-control study in Iowa.

While considerable efforts have been made to investigate the role of occupation and industry in the risk of bladder cancer, many reported associations have not been consistent, and strong evidence of increased risk is apparent for few occupational groups. To further examine the issue, a large, population-based, case-control study was conducted in the state of Iowa among both men and women. A total of 1452 incident bladder cancer cases and 2434 controls were included in the study. Occupational history was collected from respondents for each job held for 5 years or longer since age 16. Among men, excess risk was observed for industries including plumbing, heating, and air conditioning (odds ratio [OR], = 2.2; 95% confidence interval [CI], 1.0 to 5.0); rubber and plastic products (OR = 3.1; 95% CI, 1.2 to 8.5), motor vehicle parts and supplies (OR = 4.5; 95% CI, 1.2 to 16.5), and occupations including supervisors for transportation and material moving (OR = 6.5; 95% CI, 1.4 to 29.9), material-moving-equipment operators (OR = 1.9; 95% CI, 1.0 to 3.6), automobile mechanics (OR = 1.6; 95% CI, 1.0 to 2.6), painters (OR = 2.7; 95% CI, 1.0 to 7.7), and metal- and plastic-working machine operators (OR = 2.0; 95% CI, 1.1 to 3.4). Among women, significant excess risk was observed for secondary school teachers and record clerks. Housekeepers and butlers and workers in laundering and dry cleaning were also at increased risk. In conclusion, these results suggest that occupational exposures may play a significant role in the risk of bladder cancer.

Dose response relationships in allergic contact dermatitis: glutaraldehyde-containing liquid fabric softener.

Glutaraldehyde (GTA) is an effective preservative in some consumer products. Because it is a weak cutaneous allergen at certain concentrations, studies were conducted to assess any potential hazard of adding a low level (550 ppm) of GTA to an experimental liquid fabric softener. Allergic contact dermatitis (delayed hypersensitivity) was not elicited in challenge tests of 16 GTA-sensitive subjects or in repeated insult patch tests of 706 normal human male and female subjects. The subjects were exposed (1) to the experiment formulation, (2) to the formulation with 10 times the normal level of GTA, and (3) to fabrics laundered with the normal use level or with 10 times the normal use level of these experimental formulations.

Dementia and occupational exposure to organic solvents.

OBJECTIVES: To investigate whether occupational exposure to organic solvents increases the risk of dementia. METHODS: Cases of dementia were identified from the computed tomography records of eight neuroradiology centres in England and Wales, and were compared with two sets of controls investigated at the same centres. The first set of controls were patients with brain cancer and the second set were patients with other disorders that were not chronically disabling. Lifetime occupational histories were obtained through a postal questionnaire completed by the subjects or their next of kin. Associations between dementia and occupation were examined by logistic regression. RESULTS: Usable questionnaires were returned for 204 (61%) of the cases, 225 (51%) of the controls with brain cancer, and 441 (61%) of the other controls. The findings with each of the two sets of controls were similar. In comparison with all controls combined, cases had less often worked ever as a painter or printer (odds ratio (OR) 0.6, 95% confidence interval (95% CI) 0.3 to 1.2), and were less likely to have worked for > 1 year as a printer, painter, or launderer or dry cleaner (OR 0.6, 95% CI 0.3 to 1.4). CONCLUSIONS: These findings provide no support for the hypothesis that occupational exposure to solvents is a cause of dementia. An excess risk in subsets of workers with extremes of exposure cannot be discounted, but the data indicate that any influence of exposure to solvents on the overall incidence of severe dementia in the general population of England and Wales is small.

Occupation and small bowel adenocarcinoma: a European case-control study.

OBJECTIVES: Because of the rarity of small bowel adenocarcinoma (SBA), little is known about the aetiology of this disease. This study aimed to identify occupational clustering of cases SBA as a systematic approach to new hypotheses on the aetiology of this disease. METHODS: A European multicentre case-control study was conducted in 1995-7, inclusive. Incident cases aged 35-69 years with SBA (n=168) were recruited before acceptance by a pathologist. Altogether 107 cases and 3915 controls were accepted, of which 79 cases, 579 colon cancer controls, and 2070 population controls were interviewed. RESULTS: The strongest industrial risk factors for SBA taking account of 10 years' exposure lag were dry cleaning, manufacture of workwear, mixed farming (women), and manufacture of motor vehicles (men). A significantly increased risk of SBA (odds ratio (OR) and 95% confidence interval (95% CI)) was found among men employed as building caretakers, OR 6.7 (1.7 to 26.0) and women employed as housekeepers, OR 2.2 (1.1 to 4.9); general farm labourers, OR 4.7 (1.8 to 12.2); dockers, OR 2.9 (1.0 to 8.2); dry cleaners or launderers, OR 4.1 (1.2 to 13.6); and textile workers (sewers or embroiderers), OR 2.6 (1.0 to 6.8). For the last four groups, together with welders OR 2.7 (1.1 to 6.6) (men) an exposure-response pattern was found when calculating the ORs for jobs held 1-5 years and >5 years, with never having held the job as reference. The ORs (95% CIs) for 1-5 years and >5 years were 4.3 (0.4 to 44.0) and 3.5 (0.9 to 13.7), 3.0 (0.3 to 26.2) and 4.3 (0.9 to 21.2), 4.6 (0.4 to 48.1) and 11.0 (2.0 to 60.4), 1.3 (0.2 to 11.0) and 5.8 (2.0 to 17.2), and 2.8 (0.3 to 23.8) and 4.6 (1.3 to 16.6), respectively, for each of these occupations. Among welders, people performing semiautomatic arc welding (MIG/MAG) were identified as a high risk group (OR 5.0 (1.3 to 19.6)). CONCLUSIONS: This explorative study suggests an increased occurrence of SBA in certain occupations, which needs further evaluation.

Stomach cancer and occupation in Sweden: 1971-89.

OBJECTIVES: To examine the relation between gastric cancer and occupation among men and women gainfully employed in 1970 in Sweden for the period 1971-89 and, more specifically, to evaluate whether any excess of incidence of gastric cancer had also occurred among the subcohort of people reporting the same occupation in 1960 and 1970. METHODS: In both sexes and cohorts, relative risks adjusted for age, period of diagnosis, and geographical risk area were computed for occupational codes specified at one, two, or three level (occupational sector, occupational group, and occupation, respectively). Relative risks were calculated with all other occupations as reference and then, to take socioeconomic status into account, solely other occupations within the same occupational sector were used. RESULTS: Among men, occupations with increased risk included miners and quarrymen, construction and metal processing workers, supporting the possible causative role of dusty environments in stomach cancer. In men, the results also provide support for increased risks among electrical and mechanical engineers, fishermen, petrol station workers, motor vehicle drivers, butchers and meat preparers, dockers, freight handlers, launderers and dry cleaners. Furthermore, it is worth noting interesting results for women, whose occupational risks have been studied less. Excess risks were found for practical nurses, cashiers, bank employees, engineering and electronic industry workers, food industry, housekeeping and cleaning workers. Due to the many occupations studied, several significant associations may be expected by chance. CONCLUSIONS: The study is explorative but provides support for the relations suggested previously between occupational exposure to dusty environments and stomach cancer, together with some new high risk occupations which should be further studied.

Recent changes in Danish law on drugs and drug offences.

The article recounts changes in Danish Drug Law and Enforcement since the beginning of the 1990s and relates them to general trends in Danish criminal policy during the period. In addition to the implementation of EU directives, e.g. on money laundering and growth hormones, legislation has been passed to curb conspicuous dealing of drugs in the streets of Copenhagen. This part of the legislation is seen as a reaction to public fears and reactions to visible aliens dealing in drugs in a conspicuous way, albeit in minor quantities. The legal changes imply a considerable rise in penalties for repeated dealings in minor quantities and easier access to deportation of aliens. The latter has been criticized as potential violation of the human rights of aliens. This and other recent changes in criminal law and related legislation is seen as an indication of politicians' concerns with voters' anxieties, possibly at the edge of moral panics.
