Keratosis palmoplantaris nummularis ("hereditary painful callosities"). Clinical and histopathologic aspects.

Three families are described. A number of the members of these families showed nummular keratoses on the plantar pressure points. The lesions usually appeared when the children began to walk. They progressed slowly and were often accompanied by pain. The hands showed only minor lesions, which frequently followed manual labor. Extrapalmoplantar lesions were rare. The histopathologic findings in our patients were those of focal epidermolytic hyperkeratosis. The classification of this type of dominant hereditary focal palmoplantar keratosis, for which the name keratosis palmoplantaris nummularis is suggested, is discussed.

Immunological abnormalities in oral carcinoma and oral keratosis.

Examination of serum concentrations of IgG, IgA, IgM, IgE, and IgD in oral carcinoma and oral keratosis confirmed a rise in serum IgA concentration in oral carcinoma and revealed a rise in IgE. In oral carcinoma, other serum immunoglobulins were present in normal concentrations. Serum IgA and IgE were not increased in oral keratosis but IgM was reduced, with normal levels of other immunoglobulins. The results may reflect changes in cell-mediated immunity rather than simply changes in humoral immunity.

Two brothers with keratosis follicularis spinulosa decalvans.

Keratosis follicularis spinulosa decalvans is a rare, X-linked disorder affecting both the skin and eyes. There are few reports about this entity. The aim of this report is to describe 2 brothers with progressive scarring alopecia of the scalp, hypotrichosis with follicular prominence of the eyelashes, and extensive keratosis pilaris. The second patient has Down syndrome with palmoplantar keratoderma and partial alopecia of the eyebrows. We also reviewed the literature about this uncommon entity.

Snuff dippers keratosis (snuff-induced leukoplakia).

Patients with chronic snuff-induced keratosis associated with mucosal ulcerations were treated with topically applied vitamin A acid. In each case, after a marked, local inflammatory response initially, topical vitamin A acid showed a definite beneficial effect in promoting healing and involution of the precancerous snuff-induced leukoplakic keratosis. In one patient there was distinct involution of the oral lesions with complete healing and restoration of a normal-appearing mucosa within five weeks after beginning of treatment. In four patients there was healing of superficial ulcerations and regression in the size of the luekoplakic lesions.

Keratosis pilaris atrophicans: case series and review.

Keratosis pilaris atrophicans is a group of three rare and distinct clinical entities representing the scarring types of keratosis pilaris. To date, our complete understanding of these processes and therapeutics are suboptimal. This case series illustrates unique aspects of each entity and offers a review of the literature.

Keratosis pilaris decalvans non-atrophicans.

A patient is described who presented with an eruption of tiny follicular keratotic papules on the limbs and the trunk accompanied by profuse hair loss. Histologically, a diagnosis of keratosis pilaris was made. The eruption cleared spontaneously in 3 months with complete regrowth of hair. Neither atrophy nor scarring remained. This appears to be the first reported case of keratosis pilaris decalvans non-atrophicans.

Trichilemmal differentiation in seborrheic keratosis.

An 88-year-old woman who developed a nodule in a lesion of seborrheic keratosis on the frontal scalp was observed. Histopathologically, the nodule was characterized by lobular acanthosis consisting of glycogen rich clear cells and squamoid cells with squamous eddies and melanin blockade melanocytes. Each lobule was covered with an orthokeratotic horny layer which formed wedge-shaped masses in its central area without formation of an intervening granular layer. The clinical course and histopathological findings suggest that the tumor cells in seborrheic keratosis may tend to a new development of trichilemmal differentiation.

Cardiac abnormalities in familial palmoplantar keratosis.

Cardiac abnormalities were identified in patients with familial palmoplantar keratosis. All of them were descended from families on the Greek island of Naxos. Four families were studied and nine cases of palmoplantar keratosis were identified; seven of them showed symptoms and signs of heart disease. Cardiomegaly on chest x ray and electrocardiographic abnormalities were common findings. Three cases had episodes of ventricular tachycardia and a fourth patient died suddenly. All patients with cardiac signs and symptoms showed echocardiographic enlargement of the right ventricle and a right ventricular band; in three the left ventricle was also affected.

Keratosis punctata of the palmar creases: case report and prevalence study in Caucasians.

Keratosis punctata of the palmar creases (KPPC) is a variant of keratosis punctata of palms and soles in which lesions are confined to palmar creases. We present a patient with KPPC and study the prevalence of the disease in Caucasians. The survey included 1,001 white patients, that were examined for palmar lesions. We could not find any case that fulfilled the diagnostic criteria for KPPC. We conclude that KPPC is a rare disease among Caucasians but rather common in the black race.

Keratosis palmoplantaris varians of Wachters.

We report a case of hereditary palmoplantar keratosis (HPPK) causing a progressive reduction of the prehension capacity of the fingers due to the presence of hyperkeratotic lesions which had appeared approximately 25 years earlier. These lesions, also involving the soles, appeared yellowish in color, linear or round in shape, symmetrical and often confluent, developed prevalently at the pressure points displaying a non-transgrediens pattern. The histological examination, clinical picture and careful analysis of the literature enabled us to define this form as 'keratosis palmoplantaris varians of Wachters'. As a contribution to a conclusive HPPK classification we discuss the differential diagnosis of this disorder most commonly identified through nummular-linear keratoses also known as Siemens' syndrome.

[Surgical treatment of excessive hyperplasia symptom of skin keratosis at the limb ends].

OBJECTIVE: We introduce the experience of treating 10 cases with excessive hyperplasia of skin keratosis at the limb end. There were 9 cases of palmar and plantar keratosis and 1 case of epidermodysplasia verruciformis. METHOD: All cases received the operation of lesion excision and skin grafting except 2 cases who received skin flap for defect covering after lesion excision because of deep tissue exposure and postradiation ulcer. RESULT: The operative effects were satisfactory after 8 years of follow-up. No recurrence was found in all cases and all patients resumed normal activity. CONCLUSION: The effect of the skin flap is better than skin graft at the plantar area. Because of lack of fibrous septum in the subcutaneous tissue, the flap, often moving during walking, is liable to trauma and ulceration. The operation area must be well protected.

Benign lymphangioendothelioma manifested clinically as actinic keratosis.

Benign lymphangioendothelioma is an acquired lymphangiectatic lesion that must be recognized and differentiated from angiosarcoma, early Kaposi's sarcoma, and lymphangioma circumscriptum. We report the case of a 68-year-old woman with the clinical presentation of a possible actinic keratosis and the typical histologic findings of benign lymphangioendothelioma and an overlying actinic keratosis.

[A case of keratosis follicularis spinulosa decalvans (author's transl)].

The case of a 2-year-old boy with keratosis follicularis spinulosa decalvans is described. On of his sisters had keratosis follicularis of the upper arms, forearms, thighs and legs as well as blepharonconjunctivitis chronica catarrhalis bilateralis and was considered as forme fruste of the anomaly. His mother had sparse eyebrows. The mode of inheritance and the Lyon hypothesis are discussed.

47 patients in 14 families with the rare genodermatosis keratosis punctata palmoplantaris Buschke-Fischer-Brauer.

We summarize the clinical data of 47 patients with the rare genodermatosis keratosis punctata palmoplantaris Buschke-Fischer-Brauer. The pedigrees of 14 German families were studied. In three families there was only one member affected, two or more affected members were found in the other families. These family pedigrees were consistent with autosomal dominant inheritance. Variable expression of the disease was noted in members within one family. Over pressure points punctate keratoses coalesced into hyperkeratotic plaques. There was palmoplantar hyperhidrosis in 3 families associated with keratosis. Continuous systemic retinoid treatment can clear symptoms. Future genetic classification on a molecular basis may reveal the existence of more than one entity of this clinically heterogeneous genodermatosis.
