[Evaluation of bone marrow by opposed phase T1-weighted images and enhanced MR imaging].

We investigated bone marrow in a control group, cases of aplastic anemia and post-irradiation patients by examining T1-weighted (T1WI), short TI inversion recovery (STIR), opposed phase T1WI (op-T1WI) and Gd-DTPA enhanced op-T1WI images obtained by 0.5T MRI. Bone marrow was classified into four types based on MR findings. Normal marrow showed low intensity on op-T1WI and STIR images without enhancement (I). Fatty marrow, which showed high intensity on T1WI and op-T1WI images was observed in aplastic anemia and post-irradiation patients (II). Hematopoietic marrow (III) showed low intensity on op-T1WI and enhanced, while active hematopoietic marrow (IV) revealed high intensity on both STIR and op-T1WI images and was enhanced following Gd-DTPA infusion. Aplastic anemia of moderate grade included types II, III and IV. Enhanced MR was needed to differentiate between types I and III since both types showed low intensity on op-T1WI images. Furthermore, type IV was considered as hyperplastic compared with type III. Enhanced MR and op-T1WI images were useful in evaluating hematopoiesis of bone marrow.

Human rapid acquisition with relaxation enhancement imaging at 8 T without specific absorption rate violation.

A standard fast imaging sequence, rapid acquisition with relaxation enhancement (RARE), has been applied to human magnetic resonance at 8 T. RARE is known for its speed, good contrast and high RF power content. Highly T2 weighted images, the hallmark of RARE imaging, were acquired from the human brain. It is demonstrated that while T2 values may be reduced at 8 T, high quality RARE images could still be acquired at this field strength. Most importantly however, it is demonstrated that RARE images could be acquired without violating specific absorption rate (SAR) guidelines. Since it is well known that T2 weighted images are of significant value in clinical diagnosis, the implementation of RARE at this field strength will provide ultra high field MRI (UHFMRI) with a valuable imaging protocol at this field strength without exceeding SAR limitations.

Enhanced image mammography.

A blurred mass subtraction technique has been developed for mammography that will enhance small object contrast and visibility throughout the breast area. The procedure is easy to implement and requires no additional exposure. Perception of low-contrast objects is improved by eliminating extreme light and dark image areas. Contrast of structures within certain parts of the breast is increased by compression into the high-contrast part of the film characteristic curve. Detail visibility is also increased by the edge enhancement produced by this process. This paper describes the enhancement process and gives an analysis of its capabilities and limitations.

[3-D imaging with a mobile surgical image enhancement equipment (ISO-C-3D). Initial examples of fracture diagnosis of peripheral joints in comparison with spiral CT and conventional radiography].

To analyze a prototype mobile C-arm image amplifier (Iso-C3D) in the evaluation of different joint fractures of the upper and lower limbs using multiplanar reconstruction (MPR). Different cadaver joint specimens (n = 14) were examined with a prototype (Iso-C3D, Siemens AG, Erlangen, Germany), plain radiographs (CR), and spiral computed tomography (CT). The motorized C-arm provides fluoroscopic images during a 190 degrees orbital rotation computing a 119-mm data cube. These three-dimensional data sets yielded MPR images, which were evaluated by two readers for the detection and extent of fracture lines. Initial data show that fractures were easily depicted and correctly classified on MPR images with the Iso-C3D and that there was no difference between the Iso-C and CT in the detection of fractures. The preliminary results suggest a remarkably good detection of joint fractures with the Iso-C3D. Studies in an adequate number of patients are required to elucidate whether these promising results can also be obtained in a clinical setting.

MR imaging enhancement with superparamagnetic iron oxide in chronic liver disease: influence of liver dysfunction and parenchymal pathology.

PURPOSE: To analyze the influence of liver dysfunction and parenchymal pathology on the accumulation of superparamagnetic iron oxide (SPIO). METHODS: We evaluated MR images of 13 patients having small hepatic neoplasms before and after administration of SPIO (10 micronol/kg). Biopsy and laboratory data confirmed the presence of severe cirrhosis in two patients, mild cirrhosis in four, chronic hepatitis in five, and normal livers in two. Degrees of liver dysfunction or liver parenchymal pathology were correlated with reductions in signal intensity of the liver and spleen after administration of SPIO. Signal intensity reduction was evaluated using a 1.5 Tesla MR unit. RESULTS: Response to SPIO of the liver and spleen did not correlate with liver parenchymal pathology, although reductions in signal intensity of the liver were somewhat small in severely cirrhotic livers. There were slight correlations between signal intensity alterations of the liver and laboratory data such as the indocyanine green retention rate (correlation coefficient 0. 47), albumin (0.36), total bilirubin (0.36), and serum glutamic oxaloacetic transaminase (GOT) (0.46). Signal intensity reduction of spleen did not correlate with liver function tests except for serum GOT. In patients with cirrhosis, heterogeneous structures were detected in the nontumorous portions of the liver. However, these did not prevent the diagnosis of small hepatomas. CONCLUSION: The uptake of SPIO showed some correlation with liver function but not with chronic liver parenchymal pathology. SPIO provided sufficient contrast between tumor and surrounding liver parenchyma among patients with chronic liver disease.

Image enhancement by digital-analog filtration. II. Tests of filters.

The function of a device for frequency filtration of roentgenograms with digital filters has been tested with the use of different filters. The image was altered in the expected way, which means that the device is suitable for investigating the possibilities of different filters to improve the diagnostic ability. The results of the tests indicate that improvement should be achieved by low-pass filtration.

[Enhanced imaging diagnosis of liver carcinomas. Technical progress is not a substitute for clinical knowledge].

Cholangiocellular carcinomas (CCCs) of the peripheral type (not Klatskin tumors) appear sonographically as solid space-occupying lesions. No typical sonomorphological signs are known. Important hint for the clinician is the knowledge of earlier or accompanying diseases in which CCCs occur more often (primary sclerosing cholangitis, ulcerative colitis, intrahepatic gallstones, parasitic diseases of the bile ducts, choledochal cysts, Caroli's syndrome). Typical Color-Doppler signs are also missing. Definitive diagnostic is left to biopsy. Hepatocellular carcinomas (HCCs) mostly occur in cirrhotic livers. Every newly detected space-occupying lesion in a cirrhotic liver is suspected of HCC until otherwise proven, even when AFP-levels are normal. AFP levels constantly higher than 400 ng/ml make HCC probable, AFP levels of 2000 ng/ml ore more proof HCC. Gray-scale imaging is not uniform and not guiding. Using Color-Doppler nearly all HCCs show markedly hyperperfusion compared to the surrounding liver tissue, vessels are arranged in a chaotic shape. Echo-enhanced Color-Doppler always shows a distinct early arterial hyperperfusion.
