[Productivity and practice profiles of general practitioners in Tirana, Albania].

BACKGROUND: Albania, as with all Central and Eastern European countries whose health systems were highly centralized, has undertaken a number of reforms aiming to transform, among many items, the financing and delivery of primary health care services. OBJECTIVE: This study assesses the practice activities of general practitioners working in the region of Tirana, over a period of 12 months. METHODS: Production is measured by the number of monthly visits carried out by the practitioner, and practice profiles are determined by referral rates for specialist care and prescription rates per visit. Multi-level regression analyses, taking into account the hierarchical structure of the data, were performed to identify the factors associated with productivity and profiles of practice. RESULTS: Results show large urban-rural variations with respect to practice conditions, characteristics of practitioners, productivity, and profiles of practice. Productivity was weak in the city of Tirana (an average of 277 monthly visits), 18% of patients were referred to specialists, and 66% received prescriptions. In rural areas, productivity was weaker (an average of 179 monthly visits), referral rates were lower (11%), and the prescription rate was 74%. In urban and rural areas, productivity and profiles of practice were related to the characteristics of both the client and the health centre and to the type of practice. CONCLUSION: There are only a few available epidemiological studies documenting the ongoing health transition and the concomitant increase in demand for primary health care services; therefore, we are unable to (causally) link the reported low productivity of general practitioners with population needs. Physician productivity and patient care is better for certain groups and in health care settings where a wide range of services and sophisticated medical technologies are available. The capacity to efficiently plan for medical manpower is limited - this may be attributed to deficiencies of the patient registration system on the lists of physicians who are paid on the basis of capitation. Additional studies examining utilization of health services, and satisfaction of patients and providers, is needed in order to provide sound recommendations for improving Albania's health care system.

The magnitude of child injuries in Bangladesh: a major child health problem.

In recent times, many developing countries including Bangladesh not only have to cope with infectious diseases and malnutrition but also with new health problems, such as asthma, cancer and accidents. The emergence of chronic diseases and injuries has not been seen as an important health issue to date. The work presented here has the objectives of conceptualizing the dynamic changes in child mortality within the framework of the health transition, to provide a basis for projection of future mortality and disability in children in Bangladesh. This paper reviews a number of reports and published articles related to the causes of child deaths in Bangladesh. These include: 1) Year books of Bangladesh Bureau of Statistics; 2) UNICEF reports; 3) Reports of International Centre for Diarrhoeal Disease and Research, Bangladesh; and 4) Reports of Institute of Child and Mother Health. Bangladesh clearly has been progressing along its epidemiological transition. At the current stage, chronic diseases and injuries have overtaken infectious diseases as leading causes of child death. Injury has been identified as a major cause of child death in Bangladesh, and is emerging as the leading cause of child mortality, similar to what is occurring in other developing countries. For these countries, in the advancing stages of their health transition, more research aimed at understanding the dynamic change of child health priorities is urgently needed for appropriate policy and planning.

The development and testing of a food portion photograph book for use in an African population.

OBJECTIVE: To develop and test a book of photographs of commonly eaten foods to be used as a visual aid in the description of portion sizes in a large cross-sectional study on the health profile of Africans in transition (THUSA - Transition, Health and Urbanization in South Africa). SETTING: Clinics in the North West Province of South Africa. PARTICIPANTS: One hundred and sixty-nine adult African volunteers DESIGN: Data on commonly eaten foods, preparation methods, recipes and portion sizes were collected in a pilot study. Colour photographs, taken of foods prepared by the researchers and measured into three or four portion sizes, were enlarged and put together in a book. The food portion photograph book (FPPB) was tested by presenting subjects with a portion of real food and asking them to estimate the portion size by matching it with one of the portion photographs. MAIN RESULTS: Of 2959 portions tested, 68% were accurately estimated (estimated weight within 10% of actual weight). Clearly defined solid foods were better estimated than foods that had an amorphous appearance. Accuracy of estimation was not affected by gender, age or education. CONCLUSION: The FPPB was accepted as a useful and convenient visual aid to estimate portion sizes in the cross-sectional dietary intake study.

Self-image and self-esteem in African-American preteen girls: implications for mental health.

Current research suggests that pubertal development is occurring earlier in African-American preteen girls in response to familial contextual factors, which may make them vulnerable to low self-image and self-esteem dissatisfaction. This lowering in self-image and self-esteem may contribute to the early initiation of sexual behaviors, putting these girls at risk for pregnancy and sexually transmitted diseases. These potential risks place these girls in need of prepubertal health promotion, yet preadolescents are not frequently a focus of nursing care delivery except when summer camp and back-to-school physicals are performed. This article presents an in-depth overview of selected literature on self-esteem, discusses findings on self-image and self-esteem from a pilot study on pubertal influences on accelerated sexual behavior, and proposes health promotion strategies for pre- and peripubertal girls to promote positive mental health outcomes. More focused attention is needed on health promotion targeting the developmental transition health needs of prepubertal girls. Targeted health promotion activities may foster healthier pre- and peripubertal girls' perceptions of the meaning of their pubertal physical changes and stronger self-image and self-esteem. The goal of these health promotion activities should be to foster continuity of positive self-image and self-esteem among preteen girls, which is essential to prevent initiation of premature-for-age risk of problem behavior, such as early coitus.

The transition in practice: health professionals and patients.

The transition to CFC-free pressurised metered dose inhalers (pMDIs) raises issues for health professionals and patients. Health professionals need to be aware of any differences between old and reformulated products. The currently available reformulated salbutamol is equivalent to the old product so no dosage adjustment is implicated in the changeover. Available information on the new reformulated steroids suggests significantly smaller doses may be required for some reformulations as a result of improved drug deposition. This should not pose a problem if health professionals follow current asthma guidelines and titrate treatment against effect. Patients must be well prepared for transition, be aware of key facts about the new formulation and be aware of the reason their physician recommends change. The likelihood of a seamless transition to the new inhalers will be enhanced if health professionals coordinate the transition in a planned way in each district.

[Multidimensional impacts of transitional mid-life healthcare].

Mid-life transition is a passage from young adulthood to old age, which can be expressed from transition concept--five attributes and four types of transitions. Mid-life changes that occur over several years, usually between the ages of 40 and 60. Common "disconnectedness" issues during this period such as menopause will occur during mid-life. Individuals become increasingly aware of the special nature of physiological and psychological changes that take place, and express a complex combination of feelings and response patterns in light of these changes. Middle-aged adults often face multidimensional issues related to developmental transition, changes in family structure related to organization transitions, life events related to situational transitions, and perimenopausal changes related to health transitions. Nurses who provide care to middle-aged clients need to consider transitional issues from a multidimensional and holistic perspective.

Changing mortality and morbidity rates and the health status and life expectancy of the older population.

This paper demonstrates the consequences of changes in mortality and health transition rates for changes in both health status life expectancy and the prevalence of health problems in the older population. A five-state multistate life table for the mid-1980s provides the baseline for estimating the effect of differing mortality and morbidity schedules. Results show that improving mortality alone implies increases in both the years and the proportion of dependent life; improving morbidity alone reduces both the years and the proportion of dependent life. Improving mortality alone leads to a higher prevalence of dependent individuals in the life table population; improving morbidity alone leads to a lower percentage of individuals with problems in functioning.

Mortality from cardiovascular diseases in Bavi District, Vietnam.

AIMS: Like other developing countries, Vietnam is undergoing epidemiological transition, which is characterized by many changes in terms of morbidity and mortality patterns. The fact that cardiovascular diseases (CVD) are leading causes of death in hospitals in the whole country was ascertained from annual statistics. However, the magnitude of the burden of mortality from CVD at the community level remained unknown. The aims of the study were to outline CVD mortality during health transition in a rural community in the north of Vietnam and to examine associations between CVD mortality and the socioeconomic status of the population. METHODS: Within an established demographic surveillance system (DSS), verbal autopsy (VA) was used to identify the burden of mortality from CVD. Cohort analysis was used in this study to measure associations between CVD mortality and socioeconomic determinants. RESULTS: CVD emerged as a leading cause of death in the study area, accounting for more than infectious and parasitic diseases combined. CVD killed many people among the most economically productive age group, both men and women, in all socioeconomic groups. Occupational status was shown to be significantly associated with CVD mortality. DISCUSSION: Already at this point in the epidemiological transition, there is evidence of a substantial burden of CVD in rural Vietnam. Although this study was not able to show trends, the current situation is a cause for concern in health policy and planning. Verbal autopsy methods and CVD risk factor evaluations will form important parts of future research agendas.
