Addressing diversity in mental health care: a review of guidance documents.

Discriminated against and marginalised groups of people within our communities continue to be over represented within mental health services and frequently their particular needs are not met. Challenging discrimination and working towards more equitable services are a vital part of anyone's role working within mental health services. This paper provides a review of guidance documents on diversity issues with a particular focus on ethnicity, gender, sexuality, learning disability, spirituality, homelessness and age. The review concludes with a summary of the range of approaches that are currently being advocated for translating guidelines and recommendations into improved and equitable mental health services that meet the needs of service users from a wide range of diverse groups.

Providing accessible health care for adolescents with sexually transmitted disease.

Provision of sexually transmitted disease (STD) care for sexually active adolescents has been neglected in developing countries, although this is changing. Available evidence indicates that STDs are a serious problem among adolescents (10-19 years), especially in rural areas where services are limited for any age group. Curative care is hampered by the inadequacy of the syndromic approach for identifying adolescents with asymptomatic infections, especially Chlamydia trachomatis. There is an urgent need to asses STD interventions for adolescents in controlled studies, with numbers and follow-up sufficient to monitor changes in STD markers. Many programmes report increased uptake of condoms by youth but have been unable to demonstrate its effect on STD/HIV rates. It is unlikely that any one approach to adolescent STD services will be feasible and hence the importance of understanding the benefits and limitations of each approach.

Oral health promotion and health education programmes for Nigeria--policy guidelines.

Nigeria like other developing countries is presently faced with the arduous problem of coping with scarce resources to control existing and increasing oral disease levels. The World Health Organization has emphasized the importance of oral health promotion for initiating successful, effective, preventive oral health programmes. At present however, Nigeria is without formal oral health promotion and health education policies or programmes. In the "National Policy and Strategy to Achieve Health for All Nigerians" (Federal Ministry of Health 1986), no specific mention was made of oral health promotion or oral health education. The present paper therefore proposes definitive policy guidelines that will help in the development of coherent oral health promotion programmes for the country. The approach proposed tackles causes common to a number of chronic diseases and incorporates oral health into general health strategies.

Preventing suicidal behaviour in a general hospital psychiatric service: priorities for programming.

PURPOSE: General hospital psychiatric services are able to provide leadership and coordinate the development of suicide prevention programs for individuals serviced in general hospital settings. We completed this literature review to suggest priorities for programming. METHODS: Our procedure was to update the review by Gunnell and Frankel that guided priorities for Health of the Nation, the national suicide prevention strategy in the UK. We completed a search, using the terms suicide prevention and control, of all English-language research and clinical trials conducted between January 1, 1994, and May 1, 2004. RESULTS: We identified 82 papers. Of these, 48 were excluded and the remaining 34 were grouped by secondary care setting categories. We found no articles on screening tools for predicting risk of suicide, 16 articles on interventions for individuals with suicidal behaviour, 14 articles on the treatment of major psychiatric disorders, 1 article and 1 published abstract on discharge from hospital, and 2 articles on reducing access to means. CONCLUSIONS: Based on a review of each category, we make several program and policy recommendations, including regularly updating clinical assessment skills, using guidelines for assessment of patients following a suicide attempt, assessing the risk of suicide 24 to 48 hours before discharge from hospital, and incorporating education about reducing access to means into routine psychiatric care.
