Suspension of the eyelid to the check ligament of the superior fornix for congenital blepharoptosis.

A new surgical procedure for the treatment of all types of congenital blepharoptosis is described: suspension of the eyelid to the check ligament of the superior fornix. This is a dynamic suspension technique by which the check ligament, which is an extension of Tenon's capsule and normally inserts into the superior conjunctival fornix, is brought forward and sutured to the tarsus, which raises the eyelid. This technique does not sacrifice or add any tissue and is simple to repeat if necessary. Sixty-two patients were operated on using the technique and followed up for a mean of 23 months (range 3 months to 9.6 years). In a group of patients not operated on before for ptosis, 50 eyelids were raised with 74% normalisation, 22% improvement, and one eyelid each that showed only slight change or overcorrection. In a group of patients with 27 eyelids operated on before using other techniques, 67% of the eyelids were normalised, 30% were improved, and only one eyelid showed no change. In conclusion, this new technique has proved to be quite successful in raising the level of the upper eyelid in congenital blepharoptosis, with results at least comparable to those of most other techniques. The advantages with the check ligament over other techniques are the minimal trauma of the surgery, its simplicity, and its repeatability.

Leiomyosarcoma of the esophagus metastatic to the eyelid: a clinicopathologic report.

PURPOSE: To report a case of gastrointestinal sarcoma with leiomyosarcomatous differentiation metastatic to the eyelid. METHODS: Clinical data including a comprehensive ophthalmologic examination, histologic findings, a bone scan, and hospital records were reviewed. RESULTS: A 56-year-old woman with a history of esophageal cancer had a rapidly growing lesion on her right upper eyelid that was initially treated as a chalazion. A biopsy specimen of the lesion was consistent with a gastrointestinal sarcoma with leiomyosarcomatous differentiation. The esophageal tumor was reclassified after histopathologic evaluation of the eyelid specimen. Bony metastasis developed soon after excision of the eyelid lesion. The patient died 3 months after proper diagnosis of the eyelid lesion. CONCLUSIONS: This is a rare case of a metastatic leiomyosarcoma of the eyelid. To our knowledge, only two other cases of eyelid leiomyosarcoma have been described in the literature. The importance of correct histopathologic diagnosis of eyelid lesions is underscored in this case report.

The cynomolgus monkey eyelid as an anatomic model for oculoplastic surgery.

PURPOSE: To investigate the Cynomolgus monkey eyelid as an experimental model for oculoplastic surgery METHODS: Eyelid and periocular tissue were removed from Cynomolgus monkeys being euthanized. After fixation, the macroscopic and microscopic characteristics of the Cynomolgus monkey eyelid were studied. RESULTS: Macroscopic and microscopic characteristics of the Cynomolgus monkey eyelids were described. The Cynomolgus monkey eyelid bears resemblance to the human eyelid in its compartmentalization and complexity. CONCLUSIONS: The Cynomolgus monkey eyelid is a suitable experimental research model. Its compartmentalization resembles that of the human eyelid both microscopically and macroscopically.

Effect of unilateral blepharoptosis repair on contralateral eyelid position.

PURPOSE: This study evaluates the effect of unilateral blepharoptosis repair on contralateral eyelid position and assesses the relation between preoperative eyelid height interdependence, consistent with Hering law, and surgical outcome. METHODS: The medical records of 54 patients (21 men, 33 women; mean age, 65 years) who underwent external levator advancement for unilateral aponeurotic blepharoptosis were reviewed for preoperative and postoperative margin reflex distance (MRD) of the nonoperated eye. To assess the relation between preoperative Hering dependence (mechanical elevation of the ptotic eyelid causing a decrease in contralateral eyelid height) and postoperative eyelid position, the change in MRD of the nonoperated eye was compared between subjects who on preoperative evaluation did (n=18) and did not (n=36) demonstrate eyelid height interdependence, using the 2-sample t test. RESULTS: After unilateral blepharoptosis repair, the mean (+/- SD) change in contralateral MRD was -0.2 +/- 0.8 mm. There was no significant difference in contralateral MRD change in subjects with and without preoperative Hering dependence (-0.3 +/- 0.8 mm versus -0.2 +/- 0.9 mm, respectively, p=0.78). Seventeen percent (9 of 54) of patients had a contralateral MRD decrease of more than 1 mm. Three patients (5.6%) required contralateral blepharoptosis repair within 1 year of initial surgery. CONCLUSIONS: After levator advancement for unilateral blepharoptosis, roughly 17% of patients will have a decrease in contralateral eyelid height of more than 1 mm, with 5% of patients requiring surgical repair during the first postoperative year. The degree of change in contralateral eyelid height cannot be reliably predicted by preoperative assessment of Hering dependence.

Eyelid rejuvenation: a marriage of old and new.

PURPOSE OF REVIEW: Lower lid anatomy and the subtleties of this region have been studied extensively in the past. The variations that are found in the lower eyelid can lead to complications if the surgeon does not have a complete understanding. Lower lid blepharoplasty techniques have progressed over the past from simple skin excisions to midface and lower eyelid repositioning. With these modifications, our understanding of the anatomy and the function of the lower eyelid improve. This review highlights the findings of lower eyelid anatomy and rejuvenating techniques reported over the past year. RECENT FINDINGS: Over this past year anatomic studies have been performed focusing on the aging eye and midface in both Asians and non-Asians. Controversial work has been presented using an injectable (phosphatidylcholine) to ablate fat in the lower eyelids. A myriad of articles have discussed various options for combined midface and lower eyelid rejuvenation. The age-old argument of transconjunctival vs skin muscle techniques has been presented. More recent debates over fat volumetric preservation continue. Finally, a highlight of this year was a retrospective study looking at a conservative approach to blepharoplasty. SUMMARY: This has been a year of confirmation of old techniques and anatomic findings and a year of increasing complexity in midface lower eyelid rejuvenation. What is clear is that no one technique is ideal for every patient. A basic understanding of lower eyelid anatomy, preoperative evaluation, and a grasp of the multiple techniques will allow us to formulate a treatment algorithm that can be safe and effective for our patients.

Comparison of the efficacy of hard palate grafts with acellular human dermis grafts in lower eyelid surgery.

BACKGROUND: The Madame Butterfly procedure of lower eyelid reconstruction addresses lower eyelid retraction after blepharoplasty caused by middle lamellar tethering. After scar lysis and subperiosteal midface lift, the surgical procedure uses a lower eyelid spacer graft to prevent further postoperative contraction of the middle and posterior lamellae. Different graft materials have been used, including autogenous hard palate and AlloDerm. METHODS: The preoperative and postoperative photographs of consecutive patients who had undergone the Madame Butterfly procedure were analyzed retrospectively. Thirty-five patients undergoing AlloDerm grafting were compared with 25 patients undergoing hard palate grafting. Their lower eyelid heights were digitally measured and analyzed statistically for differences between the two groups. RESULTS: No statistically significant difference was found in the amount of eyelid elevation between the AlloDerm and hard palate groups, although a trend was seen that hard palate grafts resulted in both better elevation (p = 0.121) and a lower failure rate (p = 0.092). Female patients in both groups were found to experience significantly greater eyelid elevation than male patients (p = 0.018). CONCLUSIONS: Both AlloDerm and hard palate were found to be successful grafting materials for lower eyelid elevation in patients with postblepharoplasty lower eyelid retraction. Each has advantages and disadvantages, and surgeons should tailor their choices according to each patient.

The validity of a digital eyelid tonometer (TGDc-01) and its comparison with Goldmann applanation tonometry - a pilot study.

PURPOSE: To determine the intra- and inter-examiner repeatability of a new eyelid tonometer, and its agreement with Goldmann applanation tonometry (GAT). MATERIALS AND METHODS: Forty normal subjects were recruited and their intraocular pressure was measured by two examiners using the eyelid tonometer. Examiner 1 carried out the first set of measurement, followed by examiner 2, and then examiner 1 again. Finally, a third examiner was responsible for GAT measurement. Five readings were obtained from the eyelid tonometer by each examiner each time and were masked, and three readings were obtained from GAT. The mean readings from the eyelid tonometer and GAT were used for analysis. RESULTS: The intra-examiner repeatability for the eyelid tonometer was better (around +/-5 mmHg 95% of the cases) than the inter-examiner repeatability (around +/-8 mmHg 95% of the cases). The mean difference between the eyelid tonometer and GAT was small from examiner 1 (<0.5 mmHg). However, the limits of agreement were greater than the clinically acceptable level of +/-3 mmHg. Examiner 2 gave an even greater difference from GAT. CONCLUSIONS: Similar to other recent studies, we have found that this eyelid tonometer does not provide good agreement with GAT. Different examiners could position the instrument differently and this eyelid tonometer is suggested for screening purposes only.

Eyelid dermatitis: an evaluation of 150 patients.

This study was performed to evaluate eyelid dermatitis in a group of patients of our Allergy Unit between January 1990 and April 1991. Among the 1158 patients seen during this period, 150 had eyelid dermatitis: 135 females and 15 males, with a mean age of 35 years. Of the 150 patients with eyelid dermatitis, 54 had eczema localized to the eyelids only, 49 to the eyelids and face, 19 to the eyelids and hands, and 28 to the eyelids and other sites. 98 patients (65.3%) were diagnosed as having allergic contact dermatitis, 25 (16.6%) irritant contact dermatitis, 21 (14%) atopic dermatitis, and 6 (4%) seborrhoeic dermatitis. Patch test reactions to nickel sulphate, Kathon CG and fragrance-mix occurred more frequently in patients with eyelid dermatitis than in those without.

Basal cell carcinoma of the eyelids and solar ultraviolet radiation exposure.

AIMS: To compare the distribution of eyelid basal cell carcinoma (BCC) with the relative ultraviolet radiation (UVR) exposure to different sites on the eyelids. METHODS: The location of BCC on the eyelids was allocated to one of seven regions. The UVR exposure was recorded with a polymer film attached to the eyelids at seven sites in a manikin and in human subjects. RESULTS: Localisation of the 329 tumours was mainly on the lower eyelids (225 tumours), and the medial canthal regions (87 tumours). There was no association between UVR doses at the seven sites of the eyelids and the location of BCCs. The UVR exposure was similar on the upper and lower eyelids, while the number of tumours on the lower eyelids outnumbered the upper lids by a factor of 13 (17 upper, 225 lower) CONCLUSION: UVR exposure only partially explains the aetiology of periorbital BCC.

Kinematic analysis of eyelid movements in patients recovering from unilateral facial nerve palsy.

We assessed eyelid function by subjective clinical examination and quantitative means in patients recovering from facial nerve palsy. Electromagnetic search coil techniques were used to record the concurrent movements of the two eyelids to study alterations in blink main sequence (peak velocity versus amplitude) relationships and interocular differences in eyelid kinematics. After onset of unilateral palsy, the paresis of eyelid closure showed varying degrees of recovery. Adaptive increases in blink main sequence slope contributed to maximizing closure of the paretic eyelid. However, blink adaptation mechanisms must operate bilaterally, as there also was evidence of altered main sequence slope in the nonparetic eyelid. In general, main sequence slope was inversely related to the level of eyelid paresis. The highest indices of blink adaptation were in those patients with moderate paresis, and main sequence slope was decreased in those patients with increasing degrees of recovery. The assessment of eyelid function with search coil techniques provides a sensitive means of monitoring disease and treatment course. Data also aid understanding of adaptive gain control in the neural control of blink in health and disease.

HB-EGF promotes epithelial cell migration in eyelid development.

Heparin-binding EGF-like growth factor (HB-EGF) is a member of the EGF family of growth factors that binds to and activates the EGF receptor (EGFR) and ERBB4. Here, we show that HB-EGF-EGFR signaling is involved in eyelid development. HB-EGF expression is restricted to the tip of the leading edge of the migrating epithelium during eyelid closure in late gestation mouse embryos. Both HB-EGF null (HB(del/del)) and secretion-deficient (HB(uc/uc)) mutant embryos exhibited delayed eyelid closure, owing to slower leading edge extension and reduced actin bundle formation in migrating epithelial cells. No changes in cell proliferation were observed in these embryos. In addition, activation of EGFR and ERK was decreased in HB(del/del) eyelids. Crosses between HB(del/del) mice and waved 2 mice, a hypomorphic EGFR mutant strain, indicate that HB-EGF and EGFR interact genetically in eyelid closure. Together with our data showing that embryos treated with an EGFR-specific kinase inhibitor phenocopy HB(del/del) embryos, these data indicate that EGFR mediates HB-EGF-dependent eyelid closure. Finally, analysis of eyelid closure in TGFalpha-null mice and in HB-EGF and TGFalpha double null mice revealed that HB-EGF and TGFalpha contribute equally to and function synergistically in this process. These results indicate that soluble HB-EGF secreted from the tip of the leading edge activates the EGFR and ERK pathway, and that synergy with TGFalpha is required for leading edge extension in epithelial sheet migration during eyelid closure.

Electrical stimulation as a therapeutic option to improve eyelid function in chronic facial nerve disorders.

PURPOSE: To establish whether it is possible to improve orbicularis oculi muscle function in the eyelids of patients with a chronic seventh cranial nerve palsy by using transcutaneous electrical stimulation to the point at which electrical stimulation induces a functional blink. METHODS: Ten subjects (one woman, nine men) aged 36 to 76 with chronic, moderate to severe facial nerve palsy were recruited into the study. Voluntary and spontaneous eyelid movements were assessed, using an optical measuring system, before, during, and after a 3-month treatment period. Voluntary and spontaneous lid velocities were also measured and compared with eyelid kinematic data in normal subjects (12 women, 18 men; age range, 22-56 years). RESULTS: Therapeutic electrical stimulation applied over 3 months produced improvement in eyelid movement (>2 mm) in 8 of 10 patients during voluntary eyelid closure. However, there was no significant improvement recorded in spontaneous blink amplitudes or peak downward-phase velocity of the upper eyelid. This regimen of stimulation failed to recover function well enough that a functional blink could be induced in the paretic eyelid by electrical stimulation. CONCLUSIONS: Electrical stimulation using transcutaneous electrical nerve stimulators units can improve voluntary eye closure, apparently because of a reduction in stiffness of eyelid mechanics, rather than an improvement of muscle function. Investigation of alternative stimulation regimens is warranted.

Gene transfer of a soluble receptor of VEGF inhibits the growth of experimental eyelid malignant melanoma.

PURPOSE: To determine the effect of adenovirus-mediated gene transfer of a soluble receptor of vascular endothelial growth factor (VEGF) on the growth of experimental eyelid malignant melanoma. METHODS: An adenovirus vector encoding a soluble VEGF receptor/flt-1 (Adflt-ExR) was constructed. The bovine retinal endothelial cells (ECs) were incubated in a culture medium of 293E1 cells infected by means of an adenovirus vector or uninfected (control), which contained human recombinant VEGF, and the [3H]thymidine uptake was tested. The experimental eyelid malignant melanoma was induced by the injection of B16 melanoma cells (4 x 10(6) cells) into the right upper eyelid of BALB/c nu/nu mice, and the size of the tumor was recorded for 3 weeks after tumor cell injection. The effect of Adflt-ExR was examined in three ways. Model 1: B16 cells were infected by Adflt-ExR beforehand (at a multiplicity of infection [MOI] of 10) and injected into the eyelid. Model 2: Adflt-ExR was injected into pre-established B16 cell-induced eyelid malignant melanoma. Model 3: Adflt-ExR was injected into the femoral muscle of mice before B16 cell injection into the eyelid, and the remote effect was evaluated. An adenovirus vector bearing the LacZ gene (AdLacZ) or phosphate-buffered saline was used as a control. The amount of VEGF and the flt-ExR protein was measured by sandwich enzyme-linked immunosorbent assay (ELISA). Vascularization was evaluated by counting the number and the size of the vessels. RESULTS: The supernatant of Adflt-ExR-transfected cells clearly inhibited VEGF-induced bovine retinal EC proliferation in vitro. In models 1 and 2, the tumor growth in Adflt-ExR-treated mice was significantly lower than that of controls (P < 0.05). In model 3, no significant difference was found (P = 0.14). The molar ratio of VEGF/flt-ExR protein was clearly low in the tumors of Adflt-ExR-treated mice in models 1 and 2 (P < 0.01) but not in model 3 (P > 0.05). In vessel density, the tumors in Adflt-ExR-treated mice had fewer vessels than tumors in control animals in models 1 and 2 (P < 0.05). CONCLUSIONS: Adenovirus-mediated gene transfer of a soluble form of VEGF receptor (flt-1) gene inhibited the growth of the experimental eyelid malignant melanoma. This method may be useful as an antiangiogenic therapy for eyelid malignant melanoma.

"Madame Butterfly" procedure: combined cheek and lateral canthal suspension procedure for post-blepharoplasty, "round eye," and lower eyelid retraction.

The most common complication of lower eyelid blepharoplasty is eyelid retraction ("scleral show") of the lateral one-third of the lower eyelid, with associated rounding and inferior displacement of the lateral canthus. A procedure in which the middle lamellar cicatrix is lysed, the lateral canthus reconstructed, and the cheek elevated to provide additional vertical anterior lamellar skin is described. Skin grafting is often aesthetically poor in the lower eyelid and is unacceptable in the upper eyelid. This new procedure may be used to elevate the lower eyelid rather than lower the upper eyelid in cases of post-blepharoplasty lagophthalmos with exposure keratopathy. Thus, the procedure offers cosmetic and functional repair, without the use of skin grafts, for the patient who would normally need upper and lower eyelid skin grafting.

The development of the eyelids. Part I. External features.

The sequence of developmental events leading to the formation of the eyelids is described in staged human embryos. By the end of the fourth week the optic vesicle lies close to the surface ectoderm. The surface ectoderm overlying the optic vesicle, in response to this contact, has thickened to form the lense placode (Stage 13). A few days later (about 32 days, Stage 14) the lens placode is indented by the lens pit. A day or two later (about 33 days, Stage 15) the lens pit is closed: however, the lens vesicle and optic cup lie close to the surface ectoderm and appear to press against the surface. Prior to the development of the eyelids, one small sulcus or groove forms above the eye (eyelid groove) and another below it (stage 16, 37 days). As these grooves deepen, in Stages 17--19, eyelid folds develop, first below, and then above, the eye. In Stages 19--22 the eyelid folds develop into the eyelids and cover more of the eye as the palpebral fissure takes shape. The upper and the lower eyelids meet at the outer canthus in Stage 19. The inner canthus is established a few days later in Stage 20. Closure of the eyelids is complete in Stage 23.

Physiology of the lower eyelid retractors: tight linkage of the anterior capsulopalpebral fascia demonstrated using dynamic ultrafine surface coil MRI.

Although the histologic anatomy of the lower eyelid retractors is well defined, the physiology of the lower retractors has been determined only by interference based on anatomic and clinical findings. In this study, in five normal subjects we investigated the physiology of the lower eyelid retractors utilizing dynamic high resolution magnetic resonance imaging (MRI) with a custom designed surface coil. Measurements of the excursion of the cornea, lower eyelid margin, and anterior edge of the inferior oblique muscle were made from scans taken in upgaze and downgaze. We found that the corneal movement was substantially greater than the movement of the eyelid margin, a finding that can readily be made clinically. A more important result was that the movement of the eyelid margin and the movement of the inferior oblique margin were similar in all cases. Thus, the length of the anterior capsulopalpebral fascia between the tarsus and inferior oblique muscle remains constant in downgaze, and the source of the stretch in the lower eyelid retractors lies in the posterior capsulopalpebral system, at the capsulopalpebral head. High resolution eyelid MR has great potential to allow investigation of essential aspects of normal and pathologic eyelid anatomy and physiology.

[Optical coherence tomography versus ultrasound biomicroscopy of conjunctival and eyelid lesions].

BACKGROUND: The value of ultrasound biomicroscopy in the diagnosis of conjunctival and eyelid lesions is still unknown. METHOD: For the examination of conjunctival and eyelid lesions, we used an anterior segment OCT (4optics, Lübeck) and an ultrasound biomicroscope (Humphrey, Zeiss, Oberkochen) with high frequency transducer (30 MHz). Included in this study were all patients with conjunctival and eyelid lesions, which were consecutively operated on between December 2002 and January 2003. Histological examination of the excised tissue was performed. RESULTS: 38 tumours of 35 patients, aged 8 - 93 years, were examined. 13 tumours were conjunctival lesions and 25 tumours were eyelid lesions. Histological examination displayed the presence of a pterygium (8/38), a seborrhoic keratosis (7/38), a cyst of the eyelid (5/38), a basal cell carcinoma of the eyelid (4/38), a compound naevus of the conjunctiva (4/38), a chalazion (3/38), a primary acquired melanosis (1/38), an actinic keratosis (1/38), a naevus (1/38), a cavernous haemangioma (1/38), a melanoma in situ (1/38), a foreign body (1/38), and an epidermoid cyst (1/38). Using both ultrasound biomicroscopy and optical coherence tomography (OCT) we were able to demonstrate a cystic tumour in the five patients (13 %) with a cyst of the eyelid. With OCT and with ultrasoundbiomicroscopy in all patients with pterygium a slit was found and in patients with compound naevus very small cystic structures were seen, but with OCT the imaging was more reliable. In patients with solid tumours the definite diagnosis could not be differentiated by ultrasound biomicroscopy or OCT alone. Using OCT, assessment of the margins of the tumours (particularly in depth) was impossible or uncertain. CONCLUSION: Compared to ultrasound biomicroscopy OCT is able to show very small cystic structures more distinctly. For assessing the margins of the tumour ultrasound biomicroscopy is the better tool.

Laser blepharoplasty for making double eyelids in Asians.

The double-eyelid operation is a cosmetic procedure performed primarily on Asians who have no supratarsal folds. The goal in Oriental blepharoplasty is to ensure a stable double-eyelid fold with predictable and long-lasting results. Candidates for this procedure desire attractive eyelids with a natural-looking fold. Today, the CO2 laser is used as a surgical tool by plastic surgeons, and its use in cosmetic surgery has recently been expanded to blepharoplasties. We used the high-power CO2 laser (UltraPulse; Coherent, Santa Clara, Calif.) in the double-eyelid operation. Between September of 1995 and September of 1999, a total of 241 patients underwent laser double-eyelid operations at Korea University Medical Center and Dr. Choi's Aesthetic Clinic, with an average follow-up of 18 months. By using the CO2 laser, we could create a stable double-eyelid fold with long-lasting results. The rate of fold release was 2 percent, which was less than the rate using the conventional scalpel incision method. The CO2 laser contributed to a reduction in pain or discomfort after the operation; therefore, patient comfort and early recovery could be enhanced after double-eyelid surgery. In the healing of the incision line, it showed mild redness for 3 weeks, but long-term follow-up showed the scar produced by the laser appeared to be equal in quality compared with the conventional method. Laser blepharoplasty is a useful and effective method to create a stable and attractive double eyelid for Asian patients.
