[Varicella--mild or dangerous disease].

OBJECTIVE: The aim was an analysis of causes of varicella complications in children hospitalized in the Department if Infectious Diseases In Childhood of Medical University in Warsaw. METHODS AND MAIN OBSERVATIONS: 105 children from 15 day to 15 years of age was observed. 58.1% were male. The median age was 5,8 years. RESULTS AND CONCLUSIONS: Bacterial skin infections and neurological complications were the most frequent complications. Sepsis was diagnosed in 3 children. Vaccination program against varicella can reduce the risk of very severe complications.

The mode of spinal motor neurons degeneration in a model of slow glutamate excitotoxicity in vitro.

The defective glial and/or neuronal glutamate transport may, in chronic neurotoxicity, contribute to several neurodegenerative diseases including amyotrophic lateral sclerosis (ALS)--a progressive neurodegenerative disorder of lower and upper motor neurons (MNs). To determine the detailed ultrastructural characteristics of excitotoxic motor neurons neurodegeneration we used a model of slow excitotoxicity in vitro based on selective inhibition of glutamate uptake. The study was performed on organotypic cultures of the rat lumbar spinal cord subjected to various concentrations of glutamate uptake blockers: threohydroxyaspartate (THA) and L-trans-pyrrolidine-2, 4-dicarboxylate (PDC). The chronic inhibition of glutamate transport resulted in a dose-dependent slow neurodegeneration of spinal MNs consisting of necrotic, apoptotic and autophagic mode of cell death. There were some MNs that shared certain characteristics of a different type of cell injury. The results showed that a different mode of cell death in excitotoxic MNs degeneration may coexist resulting in apoptosis-necrosis and apoptosis-autophagocytosis continuum.

Expression of apoptosis-related proteins in model of anoxia in vitro.

There has been growing evidence that different modes of cell death exist, among them the apoptosis is thought to be an important mechanism of nerve cell loss implicated in various pathological states. A number of proteins mediated with apoptotic process have been identified, including p53, BAX, BCL-2 and BCL-X. We examined the expression of proteins related to programmed cell death in hippocampal neurons in vitro, exposed to pure anoxia or pretreated with apoptosis modulating agents: zinc and zinc chelator - TPEN. The results evidenced the noticeable differences in the expression of pro- and anti-apoptotic proteins in particular experiments. In the cultures exposed to pure anoxia, a significant increase of p53 and BAX immunoreactivity, associated with the decreased level of BCL-2 and BCL-X immunopositive cells was observed, related to the activation of apoptotic process. Hippocampal cultures pretreated with ZnCl2 before anoxia showed decreased immunoreactivity for p53 and BAX, connected with BCL-2 overexpression, whereas the cultures exposed to zinc chelating agent - TPEN or TPEN connected with anoxia showed significant increase of immunorectivity for p53 and BAX. This strong immunoreactivity of proapototic proteins (p53 and BAX) in hippocampal cultures exposed to anoxia or/and TPEN correlated with previous ultrastructural evidences of anoxia- and TPEN-induced apoptosis, while the overexpression of anti-apoptotic protein (BCL-2 and BCL-X) in zinc-pretreated cultures evidenced the protective ability of this metal against apoptosis in model of anoxia in vitro.

Effect of rutin on total antioxidant status of rats exposed to cigarette smoke.

Exposure to tobacco smoke impairs the antioxidant defense mechanisms. In female Wistar rats fed on regular rodent chow and supplemented with a flavonoids rutin, Trolox Equivalent Antioxidant Capacity (TEAC) was measured as an ABTS-radical cation reduction power in plasma, lungs, liver, brain and kidneys. Exposure to smoke reduced the TEAC values in the liver, brain and kidneys and enhanced antioxidant potential in lungs in comparison to control animals. In plasma no change of TEAC value was observed. Supplementation with rutin increased antioxidant status of plasma, but TEAC was reduced in kidneys, brain and liver of smoke-exposed animals when compared to the matched controls. In lung no change in TEAC was found. The results suggest a complex pattern of influence of tobacco smoke on blood and tissue antioxidant mechanisms. The enrichment of diet with non-nutrient antioxidant rutin did not result in direct improvement of tissue TEAC with the exception of blood plasma.

[Perinatal complications in newborns of mothers with gestational diabetes].

OBJECTIVE: To determine how metabolic monitoring of women with gestational diabetes influences the morbidity of newborns. DESIGN: Retrospective study. PATIENTS: 352 newborns of mothers with gestational diabetes. 295 mothers suffered from diabetes class G1 (G1DM) and 57 from diabetes class G2 (G2DM). MATERIAL AND METHODS: Glycated haemoglobin levels measured at the moment of the diagnosis of gestational diabetes and shortly before delivery were higher in women with diabetes type G2, but the difference was statistically significant only at the first measurement (G1 6.1+/-1,5% vs. G2 6.7+/- 1.6%, p<0.01). Mean gestational age at the time of delivery in the whole group was 38 +/- 2 hbd and there was not any difference between newborns of mothers with G1DM and G2DM. Mean birth weight in newborns of G1DM mothers was 3390 +/- 620 g and in newborns ofG2DM mothers it was 3330 +/- 720 g (NS). RESULTS: 31 (8.8%) infants were large for gestational age (LGA) and 20 (5.7%) infants were small for gestational age (SGA). There was not any difference between newborns of mothers with diabetes class G1 and G2 in the incidence of LGA and SGA. Neither there was any difference between these two groups in the incidence of metabolic disorders (hypoglycemia, hypocalcaemia. hypomagnesaemia. hyperbilirubinemia) and other pathologic conditions (respiratory distress syndrome (RDS), polycythemia, hypertrophic cardiomyopathy and perinatal infections). The incidence of congenital malformations was similar in both groups. LGA newborns were statistically more often newborns of mothers with HbA1c level exceeding the normal value (HbA1c>6.3%). This correlation was observed both in the first measurement of HbA l c and the measurement performed shortly before delivery. In LGA newborns we observed hypoglycemia, polycythemia and hypertrophic cardiomyopathy more often. Perinatal infections appeared statistically more often in the SGA newborns. 31.25% infants from the study group were born before the end of the 37th week of gestation. These newborns more often suffered from RDS, hypoglycemia, hyperbilirubinemia and perinatal infections. CONCLUSION: There was not any significant difference in neonatal outcome in newborns of mothers with gestational diabetes class G1 and G2. This might have resulted from the fact that there was neither any statistically significant difference between these two groups in HbA1c level measured shortly before delivery. The incidence of metabolic disorders in newborns was correlated with LGA, SGA and prematurity, but there was not any difference in the occurrence of metabolic disorders between newborns of mothers with G1DM and G2DM.

[Carbohydrate and lipid metabolism disorders in obese children and adolescents].

UNLABELLED: THE AIM of the study was to assess the incidence of: 1. insulin resistance, hyperinsulinemia, impaired glucose tolerance and dyslipoproteinemia in serum of obese children according to the type of obesity; 2. family history of obesity, hypertension and type 2 diabetes mellitus. MATERIAL AND METHODS: 177 obese children (M/F 80/97) aged 10-17.5 yrs and 38 normostenic children (control group) were screened for carbohydrate and lipid metabolism disorders. BMI (> or =97 c) and WHR (M/F> or =0.9/0.85 -- abdominal adiposity) were estimated. Glycemia (mmol/l) and insulinemia (microIU/ml) at 0, 30, 60, 90, 120 minutes of OGTT were measured. Fasting C-peptide (ng/ml), HbAlc (%). total cholesterol, LDL-Ch, HDL-Ch, triglycerides (mmol/l), I/G ratio (microIU/ml/mg%) and ChT/HDL-Ch atherogenic index were also investigated. RESULTS: Abdominal adiposity was present in 56.5%. fasting hyperinsulinemia (>20.0 microIU/ml) in 32.2%, insulin resistance (I/G>0.30) in 73.7%; in the group of patients with hyperinsulinemia 66.7% had abdominal adiposity. The glycemia ill OGTT was: normal in 57.0% -- group I; 26.6% did not suit the criteria of normal nor impaired glucose tolerance according to American Diabetes Association (ADA) -- group Ia. The impaired glucose tolerance (IGT) was found in 16.4% -- group II, with fasting hyperinsulinemia in 58.6% and I/G ratio > or =0.30 in 41.3%, HbAlc > 6.2% in 34.5%. Atherogenic lipoproteins levels prevailed (p<0.05) in group Ia and II. The incidence of family history of obesity, hypertension and diabetes mellitus type 2 was higher (statistically not significant, p>0.05) in group Ia and II and significantly higher (p<0.05) in all groups of obese children compared to the control group. CONCLUSION: The results of our study prove that obesity in children is a risk factor for early diabetes mellitus type 2 and atherosclerosis.

[Cemento-ossifying fibroma--case report and review of the literature].

Authors report two cases of benign, osteogenic neoplasms of the maxilla in children which defined as cemento-ossifying fibroma and juvenile cemento-ossifying fibroma. Difficulty in diagnosis of the tumor is indicated. The importance of clinical data as well as radiology and histopathology examination for a proper diagnosis is indicated. Review of literature is presented.

[Subtelomeric aberration as a cause of severe somatic and psychomotor retardation in a child with dysmorphic features and CNS defects].

Subtelomeric chromosome rearrangements are a significant cause of severe somatic and psychomotor retardation. Their incidence is estimated as 2.1/10000 liveborns and about 30-50% of the reported aberrations were familial. They result in a wide spectrum of clinical signs and most of reported cases are non-repeatable. Clinical verification of mentally retarded patients suspected of subtelomeric rearrangements requires a special molecular-cytogenetic method: subtelomeric fluorescence in situ hybridisation -- FISH. We present a case of submicroscopic subtelomeric aberration of chromosome 13 - der(13)t(X; 13) in a severely mentally retarded child with congenital defects and dysmorphic features. Detailed clinical characteristics of the proband as well as an attempt of a phenotype- genotype correlation of the identified chromosomal aberration were performed.

Uptake of radiolabelled endomorphins by experimental mammary adenocarcinoma.

BACKGROUND: The aim of this study was to examine the accumulation of endomorphin-1 (Tyr-Pro-Trp-Phe-NH2) and endomorphin- 2 (Tyr-Pro-Phe-Phe-NH2) labelled with radioiodine in tumour-bearing C3H/Bi mice. MATERIAL AND METHODS: Mice C3H/Bi bearing transplantable mammary adenocarcinoma were used as animal models to study the interaction between micro-opioid receptors and endomorphin-1 and 2. The expression of the micro-opioid receptor in the tumours was confirmed by cross-linking assay and by RT- PCR technique. RESULTS: The endomorphins showed relatively high tumour accumulation - about 5.2% of dose/g tissue for endomorphin-1 and about 3.8% for endomorphin-2. The ratio of tumour to muscle for endomorphin-2 reached the highest value (12.7) six hours after injection. For endomorhin-1 this ratio was the highest (7.5) three hours after injection. The cross-linking assay of [125I]-labelled peptides with membranes, isolated from the mouse adenocarcinoma, followed by electrophoresis and autoradiography revealed the presence of a radioactive complex with molecular weight of about 65 kDa. This complex was detectable by polyclonal antibodies raised against the N-terminal end of a micro-opioid receptor. The expression of gene encoding micro-opioid receptor on mouse mammary adenocarcinoma was further confirmed by RT-PCR technique. The binding studies with membranes of mouse mammary adenocarcinoma cells have shown significantly higher Bmax values for endomorphin-1 and endomorphin- 2 (806 and 671, respectively) than for morphiceptin (131), a well-known specific micro-opioid receptor ligand. CONCLUSIONS: Endomorphin-1 and 2 have shown a high affinity to the m-opioid receptor present in mouse mammary adenocarcinoma. However, endomorphin-2 showed more promising characteristics in biodistribution studies.

[Changes of the concentration of lipid peroxidation products and thiol groups in rat brains with long-term intoxication with methanol].

The aim of this paper was to define the influence of long-term methanol intoxication on the changes of end products of lipid peroxidation, reacting with thiobarbituric acid (TBA-reactive products) and free SH-group concentration in rat brains. The experiment was conducted on male Lewis rats. The experimental groups received a 1-molar methanol solution and the control group received tap water. The animals were killed after 4, 8 and 12 weeks of intoxication and their brains were collected for further examination, which encompassed measurement of the concentration of TBA-reactive products and free SH-groups. The revealed changes of TBA-reactive products and free SH-group concentration were not statistically significant.

[Understanding health by adolescents with diabetes mellitus type 1 assessed by own criteria].

BACKGROUND: The word "health" has different meanings according to gender, age, social and health status as well as value system which points out life goals. OBJECTIVES: To determine the criteria of understanding "health" by adolescents with diabetes type 1 compared to the healthy peers. MATERIAL AND METHODS: Study group comprised 66 adolescents (34 girls and 32 boys) 14-18 years old (mean 15.9+.1.3 years) suffering from diabetes mellitus for more than 2 years (mean 4.3+/-3.4 years). Control group of 60 healthy adolescents (30 girls and 30 toys) was matched for age and gender. The List of Health's Criteria by Juczyński including 24 items of positive descriptions of physical, psychological and social dimensions of health was used. RESULTS: Important differences as well as some similarities between two groups were found. Adolescents with diabetes mellitus understood health in physical and psychological aspects (as proper eating habits, enjoying life, having efficient all parts of the body). Less important value they attribute to processes connected with: being responsible, solving own problems, controlling emotions and impulses. CONCLUSIONS: Diabetic adolescents understand the concept of "being healthy" in a different way than their healthy peers. Criteria the diabetic youngsters choose result probably from their health status and demonstrate the difficulties these children face while following the therapeutic recommendations. Problems understanding, psychological support as well as increased medical information about the disease may improve their quality of their life.

The use of recombinant human activated protein C (rhAPC) in the treatment of severe sepsis in immunosuppressed patients in the course of hematological diseases.

BACKGROUND: Treatment of hemopoietic system neoplasias involves severe complications associated with immunosuppression. We present two cases of treating severe sepsis utilizing recombinant human activated protein C (rhAPC) in the course of bilateral pneumonia in patients with hairy cell leukemia (HCL) and T-cell acute lymphoblastic leukemia (ALL). RhAPC, limited the coagulation cascade by inactivating FVa and FVIIIa, directly and indirectly limiting systemic inflammatory response syndrome (SIRS), and improved the fibrinolysis process. These actions break the pathomechanism of sepsis and improve survival. CASE REPORT: Besides intensive, multidirectional treatment of severe sepsis, Xigris (activated drotrecogin alfa) was administered on the second day in both cases. The infusion continued, as recommended, for 96 hours without complications. During treatment the patients' general condition and respiratory efficiency improved, allowing respirator weaning on days 5 and 8 of therapy. These cases of severe sepsis and immunosuppression indicate a high therapeutic efficacy of drotrecogin alfa (activated). Treatment outcome was uncertain because of the patients' hematological condition, so rapid restoration of respiratory efficiency and no disease progression after discontinuing treatment was a great success, possibly due to implementing Xigris at a relatively early stage of sepsis and the intensive therapy conducted according to Surviving Sepsis Campaign guidelines. CONCLUSIONS: Patient survival in severe sepsis directly depends on early diagnosis and institution of treatment. 1. These cases confirm the effectiveness of drotrecogin alfa in severe sepsis as part of multidirectional therapy. 2. Microorganisms causing atypical pneumonia should be considered in diagnosing infections in patients treated with cytostatic agents.

[Oxidative stress parameter analysis in rat brains during long-term ethanol intoxication].

The aim of this research was to define the influence of long-term ethanol intoxication on the changes in end products of lipid peroxidation, reacting with thiobarbituric acid (TBA-reactive products) and free SH-groups concentrations in rat brains. The experiment was conducted on male Lewis rats. The experimental groups received a 1-molar ethanol solution and the control group received tap water. The animals were killed after 4, 8 and 12 weeks of intoxication and their brains were collected for further examination, encompassing measurement of the concentration of TBA-reactive products and free SH-groups. A statistically significant increase in the concentration of TBA-reactive products in 4th week of intoxication and decrease in the concentration of free SH-groups in 8th and 12th week of intoxication--compared to the control group--was noticed.

[The evolution of epidemiology and clinical characteristics of laryngeal and hypopharyngeal carcinoma in Poland from 1991 to 2001].

On the base of retrospective analysis of 12,888 cases of carcinoma of larynx and hypopharynx, diagnosed in 19 ENT Departments in Poland from 1991 to 2001, the assessment of basic epidemiological data, including the localization of tumor and stage of local and clinical advancement of the disease at the time of diagnosis has been conducted. In analyzed period of 11 years the trends to change of the mentioned above parameters has been examined. The significant increase of female patients in this period was observed, with average proportion M:F = 8:1. The glottis localization of carcinoma dominated (47.6%), followed by supraglottis (40.8%) and pyriform fossa (7.8%), with significant increase of pyriform fossa tumors in the analyzed period of 11 years. In the majority of cases the carcinoma of larynx and hypopharynx was diagnosed in the advanced stage (T3 + T4) of local disease, with the highest percentage in localization within the pyriform fossa (81.0%), and the lowest percentage in glottis tumors (45.6%). The regional lymph nodes metastases has been diagnosed in 46.7% of the analyzed group, with the highest percentage in tumors localized in pyriform fossa (82.9%), and the lowest percentage in tumors of glottis localization (33.1%). In the 11 years time the significant drop down of N0 cases and tendency to increase of N2 and N3 in the supraglottis localization of tumor. The distant metastases in the analyzed group at the time of diagnosis has been registered in 2.0%, with the highest percentage in posterior pharyngeal wall (7.6%) and pyriform fossa (7.4%). The authors postulate the renewal of prospective study on epidemiology, clinical characteristics and treatment results of larynx and hypopharynx carcinoma in Poland.

[Effect of drugs on the diagnostic laboratory tests results].

The results of clinical laboratory determinations depend on physiological, environmental and pharmacological factors. Thousand pharmacological factors used daily, are known to interfere with routine laboratory tests by pharmaco-toxicological and physico-chemical reactions. This confusing interference may cause the wrong diagnoses but also incorrect treatment, unnecessary tests and further costs. Even if we would be able to recognize the most important effects of drugs, we will not be able to remember all of the drugs adverse reactions; therefore use of computer databases could be helpful for interpreting doubtful laboratory test results.

[Acute, idiopathic, bilateral uveitis with periphlebitis--case report].

The purpose of this paper is to present the case of a 35 years old healthy male suffering from acute, bilateral uveitis with periphlebitis which despite broad ophthalmic and internal diagnosis remained of unknown origin. Despite anti-inflammatory treatment with acyclovir, ciprofloxacin and Encorton remission of inflammation was not achieved. Due to the failure of steroid treatment the decision of immunosuppressive therapy with Rapamycine and Endoxan was made. New therapeutic regimen caused the remission of inflammation and preserved patient's useful visual acuity.

Psychosocial functioning of the people with delusional disorders under forensic psychiatric observation in the ward of forensic psychiatry in 1998-2002.

In the literature on the subject frequency and kind of deeds committed by people with mental illness in relation to individual clinical diagnoses are analyzed mainly, whereas little attention is paid to the issue of their psychosocial functioning. Intellectual functioning of the people manifesting delusional disorders in the course of committing attributed deeds, was on an average level. The patients' social functioning was on a low level.
