Endocrinological and histological aspects of the process of polycystic ovary formation in the rat treated with testosterone propionate.

To elucidate the process underlying polycystic ovary (PCO) syndrome, we tried to produce experimental PCO by sterilizing rats with androgen and investigated the formation of PCO endocrinologically and histologically. Wistar-Imamichi strain female rats were given an injection of testosterone propionate 1.25 mg on 5 days of age. On 5, 10, 20, 40, 60, 80, 100, 130, 160, 200 days after the injection the ovaries were obtained. Ovarian wedge resection was also performed at 100 and 200 day-rats. Serum gonadotropins, prolactin and steroids were measured with RIA and compared with ovarian pathology. Serum prolactin was increased drastically 40 days after the injection and maintained at a constant level higher than control. Through wedge resection, the serum LH level returned to normal, but FSH slightly increased. Serum estradiol decreased more markedly than estrone did. The 40 day-ovaries showed polycystic features, lack of corpus luteum, hyperthecosis, luteinization of stroma, while no thickening of tunica albuginea. In conclusion, the rats treated with testosterone propionate had many endocrinological and histological aspects similar to those of human PCO syndrome.

Clinical and endocrinological features of statistical clusters of women with climacteric symptoms.

Clinical and endocrinological features of women with climacteric symptoms, who were clustered by means of factor analysis and cluster analysis, were evaluated as to serum concentrations of estradiol, progesterone, follicle-stimulating hormone and luteinizing hormone, and also as to mean ages as well as a rate of premenopausal women or that of postmenopausal women against all women in each one of the clusters. Comparison of these parameters among these clusters indicated, with statistical significance, that vasomotor symptoms, neurotic symptoms, and depressive state of the climacteric women are endocrinologically characterized by a depressed level of serum estrogen, relatively high level of serum estrogen, and an elevation of serum gonadotropin, particularly of luteinizing hormone, respectively.

An endocrinological study of persistent pubertal macromastia.

We performed an endocrinological study on three males with persistent pubertal macromastia. The blood testosterone (T) level in these patients was either elevated or at the upper normal limit, estrone (E1) and estradiol (E2) levels were high, and T/E1 and T/E2 ratios were decreased. Persistence of low T/E1 and T/E2 ratios was thought to be the cause of the macromastia. Results of therapy using drugs (cyproterone acetate, tamoxifen citrate, danazol, and clomiphene citrate) were unsatisfactory. Persistent pubertal macromastia is an extreme condition of physiological pubertal gynecomastia, a transient phenomenon in more than half of pubertal males. It is a pathological condition characterized by a great degree of breast enlargement and abnormal endocrinological findings, and its clinical course and treatment are different from those of physiological pubertal gynecomastia.

Endocrinological responses during suckling in Hatano high- and low-avoidance rats.

Hatano high-avoidance (HAA) and low-avoidance (LAA) animals were originally selected from Sprague-Dawley rats for good and poor active avoidance learning in a shuttle box. We studied the endocrinological profile in lactating rats to determine the effect of suckling during mid-lactation in HAA and LAA rats. The pups were separated from their mother rats 6 h before the onset of suckling and blood samples were drawn from unanaesthetized mother rats via a jugular cannula at 0, 5 and 15 min after the suckling stimulus and then 15, 45 and 105 min after pups were removed. Plasma concentrations of oxytocin in HAA rats were significantly higher than in LAA rats during the suckling period. Plasma concentrations of prolactin and ACTH in HAA rats were significantly higher than in LAA rats during the suckling period, and at 15 min and 45 min after the pups were removed. However, there were no strain differences in circulating corticosterone between the two lines, indicating that the response of the hypothalamo-pituitary axis to the suckling stimulus was greater in HAA rats than in LAA rats, whereas the ACTH-induced adrenal response of corticosterone release was higher in LAA rats than in HAA rats. Since dopamine from the median eminence inhibits prolactin secretion from the lactotrophs of the anterior pituitary, and tuberoinfundibular dopaminergic neurones are partially regulated by the level of circulating prolactin, we evaluated the activity of tyrosine hydroxylase (TH), the rate-limiting enzyme in dopamine biosynthesis. TH, measured by the accumulation of 3,4-dihydroxyphenylalanine, was significantly higher in HAA rats than in LAA rats before the suckling stimulus. After the suckling stimulus, TH activity in HAA rats was significantly lower than before suckling, whereas TH activity in LAA rats was not changed. These findings clearly demonstrated that apparent differences between the two Hatano lines exist in endocrinological profiles during suckling. These strain differences probably originate from neurotransmitter changes, such as dopamine.

Endocrinological assessment of toxic effects on the male reproductive system in rats treated with 5-fluorouracil for 2 or 4 weeks.

Endocrinological assessment of male reproductive toxicity was carried out in SD-Slc male rats treated with 5-FU (0, 20, 30 mg/kg/day) orally for 2-week or 4-week term. Serum hormone levels including GnRH, FSH, LH, prolactin, total and free testosterone, inhibin B, pro-alpha C, and activin A were determined as well as histopathological examination of the reproductive organs. The 5-FU treated groups showed histopathological changes in the testis such as degeneration of seminiferous epithelium. An obvious decrease in serum testosterone level was observed with a reduced organ weight of the seminal vesicle and prostate. However, no significant changes were noted in serum LH or FSH levels, nor in the morphological examination of the Leydig cells. Decreased serum levels were noted in activin A and prolactin. An increased serum level was noted in GnRH and pro-alpha C whose synthesis is regulated by FSH. Serum inhibin B levels showed a tendency toward decreasing with morphological change (vacuolation) in Sertoli cells. These results indicated that male reproductive toxicity induced by 5-FU would be augmented by decreased serum prolactin and testosterone levels as well as a decreased function of Sertoli cell, in addition to the direct cytotoxic effects on germ cells. It is suggested that these endocrinological changes related to male reproductive toxicities can be detected even in the 2-week-treated study.

Williams syndrome associated with chronic renal failure and various endocrinological abnormalities.

A 31-year-old man who had been under regular hemodialysis for 6 months was diagnosed as Williams syndrome (WS) by fluorescence in situ hybridization (FISH) chromosomal analysis. The association of WS and chronic renal failure (CRF) is only rarely encountered. Endocrinological examinations revealed hypergonadotropic hypogonadism. Prolonged and exaggerated responses of adrenocorticotropin (ACTH) to insulin-induced hypoglycemia and corticotropin releasing hormone (CRH) were also noted. While most of the endocrinological abnormalities observed in this patient could be attributed to altered endocrine circumstances in CRF, some findings stand in contrast. Furthermore, the testicular biopsy specimen showed severe hypospermatogenesis. Endocrine disorders observed in this patient may be at least in part, responsible for various clinical features underlying WS.

[Comorbidity of posttraumatic stress disorder and mild closed head injury in war veterans: endocrinological and psychological profiles].

AIM: To determine the degree of psychological and endocrinological changes in war veterans with the diagnosis of Chronic Posttraumatic Stress Disorder (PTSD) regarding presence/absence of comorbid mild closed head injury (mCHI) caused by explosive devices. METHODS: Two groups of PTSD inpatients, with (n = 37), and without (n = 86) sustained blast trauma followed by mCHI were formed during the psychiatric treatment. Participants were interviewed by experienced clinicians who used the PTSD Interview (PTSD-I). In addition, patients completed the Symptom Checklist-90-Revised (SCL-90-R). Serum levels of ten hormones were assessed: triiodothyronine, thyroxine, thyrotropin-stimulating hormone, prolactin, luteinizing hormone, follicle-stimulating hormone, and insulin, by radioimmunoassays and hydrocortisone, growth hormone and testosterone by fluoroimmunoassays. RESULTS: Veterans with comorbid mCHI and PTSD showed significantly higher level of amnesia for traumatic event as well as of somatization on the SCL-90-R. Significant differences of hormone levels were not found. CONCLUSION: The results didn't support the hypothesis on specific PTSD subgroup characterized by history of mCHI and consecutive postconcussion syndrome. The absence of differences in levels of hormones indicated the dominant role of psychogenic trauma in the etiology of hormone disbalance in chronic PTSD. Amnesia for traumatic event in war veterans with comorbid PTSD and mCHI was easily explained by neurogenic peritraumatic amnesia due to the blast trauma, but it did not affect either quality of intensity or posttraumatic symptoms as well as endocrinological parameters.

Long-term endocrinological follow-up evaluation in 115 patients who underwent transsphenoidal surgery for acromegaly.

OBJECT: The results of surgical therapy for acromegaly were assessed using carefully conducted endocrinological testing in 115 patients who underwent transsphenoidal surgery from 1981 to 1995. METHODS: Ninety-nine of the 115 patients could be contacted for follow-up review; in 57 of the patients an endocrinological assessment was performed by the authors. Biochemical cure was strictly defined as a normalization of the insulin-like growth factor(IGF)-I level (obtained in 100 patients) and/or a basal or glucose-suppressed growth hormone (GH) level of 2 ng/ml or less. The mean length of follow up from transsphenoidal surgery to laboratory testing was 5.4 years with a maximum of 15.7 years. After transsphenoidal surgery alone, 61% of the patients achieved a biochemical remission; the remission rate was 88% for patients with microadenomas and 53% for those with macroadenomas. Tumor size and preoperative GH level tended to correlate negatively with outcome of surgery. Early postoperative GH level tended to correlate with long-term outcome; in cases in which the early postoperative GH level was lower than 3 ng/ml, the chance of long-term remission was 89%. Thirty-two patients received postoperative radiotherapy: in 10 (31%) of these patients the disease is currently in remission after surgery and radiotherapy only and in three others the disease is in remission with the addition of medical therapy. The overall complication rate was 6.9% with no cerebrospinal fluid leaks, meningitis, permanent diabetes insipidus, or new hypopituitarism. The overall recurrence rate was low at 5.4%. CONCLUSIONS: This series shows, based on IFGF-I measurements and strict GH suppression criteria to define remission, that transsphenoidal surgery provides an excellent chance for long-term cure in patients with microadenomas. Surgery alone is successful in most patients with noninvasive macroadenomas; however, most patients with invasive macroadenomas will require adjunctive therapy. Recurrences are uncommon when biochemical remission is clearly documented postoperatively.

Pterional surgery of meningiomas of the tuberculum sellae and planum sphenoidale: surgical results with special consideration of ophthalmological and endocrinological outcomes.

OBJECT: The authors reviewed 47 cases of suprasellar meningiomas with special attention to ophthalmological and endocrinological outcomes. METHODS: All patients underwent surgery performed via a unilateral pterional approach between January 1983 and January 1998. Ophthalmological and endocrinological examinations were performed before the operation as well as 1 week and 3 months postoperatively. A special scoring system was adopted to quantify the extent of ophthalmological disturbances. Complete tumor resection was possible in all but one patient. There were no fatalities and the rate of visual improvement was 80%. The best prognoses were found in patients younger than 50 years and in patients in whom the duration of symptoms was less than 1 year. Before surgery, tumor-related endocrine disturbances were present in only three women who suffered from secondary hypogonadism; two of these patients recovered after surgery. Postoperatively, no patient needed replacement therapy for pituitary dysfunction. The overall tumor recurrence rate was 2.1% (one of 47 cases). For patients in whom long-term (> 5 years) follow-up data were available, the recurrence rate was 4.2% (one of 24 cases). CONCLUSIONS: In this series, complete resection of suprasellar meningiomas was possible through a unilateral pterional craniotomy and was associated with a low morbidity rate and no deaths.

[Three cases of Klinefelter syndrome in childhood--endocrinological and gonadal changes in puberty].

Three children at the ages of 4, 10 and 12 years, with external genital malformation, were diagnosed to be with the Klinefelter syndrome by chromosome analysis. To clarify the pubertal changes in this syndrome, all of them were studied for physical and endocrinological examinations and two underwent testicular biopsy. Before puberty any remarkable abnormality were not observed in the hypothalamus-pituitary-gonadal axis and in the physical status. After the onset of puberty they started showing increases of the basal levels of plasma FSH and LH with over-response to LH-RH stimulation test. During the period of this study the levels of plasma testosterone were in the normal range and increased gradually with age. One boy showed a transient high level of plasma testosterone at early puberty. The reactions of plasma testosterone to HCG stimulation of all cases showed the normal pattern. The histological examination of the testis revealed that the number of spermatogonia was reduced in both cases compared with that of normal boys reported by Mancini et al. These findings indicate that most endocrinological and histological abnormalities in adult Klinefelter syndrome occur after the onset of puberty. These changes may be induced at puberty by hypergonadotrophic condition which result from slightly impaired testicular function which is present before puberty.

[Analysis of clinical manifestations and endocrinological aspects of patients having PADAM-like symptoms].

PURPOSE: To analyze clinical manifestations and endocrinological aspects of the patients who visited our hospital to be examined for partial androgen deficiency in the aging male (PADAM). MATERIALS AND METHODS: Two hundred forty-three patients were evaluated. History taking and questionnaires were employed for analysis of their symptoms. Serum total testosterone (T), free-T and other hormones were measured for evaluation of the androgen deficiency. RESULTS: The chief complains of the patients were categorized as psychological symptoms, somatovegatative symptoms, and sexual symptoms, accounting for 51%, 36%, and 13%, respectively. A depressed mood, hot flashes and sweating and erectile dysfunction were dominant in these symptom categories, respectively. The prevalence of the psychological symptoms and somatovegatative symptoms was relatively high in the 30-50-year-old and 60-70-year-old groups, respectively. The free-T, but not total-T, was significantly decreased with aging as measured by the RIA method. Twenty one percent of the patients showed a lower total-T level than the normal range (2.7-10.7 ng/ml). The percentage of those with a total-T level lower than 2.0 ng/dl, which is the criterion for T-replacement recommended by the American Association of Clinical Endocrinologists guidelines, was 9%. Eighty two percent and 30% of the patients had lower free-T levels than the normal range (15.2-43.5 pg/ml) and that of men in their twenties in the Canadian data (9.3-26.5 pg/ml). Depending on the criteria of androgen deficiency, not all patients had low total-T and free-T levels. CONCLUSION: Patients suspected of having PADAM present various clinical symptoms and endocrinological aspects.

[Validity of scintigraphic and endocrinological methods in classifying autonomous adenomas of the thyroid (author's transl)].

The validity of various scintigraphic and endocrinological methods was assessed in order to classify confirmed autonomous adenomas (AA) and to differentiate AA from euthyroid and hyperthyroid goitres. Methods used included: Measurement of 131J-uptake 6, 24 and 48 hours after 131J-application, before and after suppression by T3, thyroid scanning, count-rate measurement in the AA and the paranodular tissue, and determination of T4. All patients received 131J-therapy (40,000 rads). In 8 out of 30 scintigraphically compensated autonomous adenomas (CAA) there was no TSH response to TRH. 12 cases showed a subnormal TSH response and 10 patients had a normal TSH response. T4-values were increased in only 2 cases. Out of 73 decompensated autonomous adenomas (DAA), 66 cases (90%) had no TSH response and 7 cases showed only a subnormal response. In 48% of DAA T4-levels turned out to be in the hyperthyroid range. Both, scintigraphic and endocrinological tests were found to agree in most cases of DAA. However, the diagnosis of CAA could only be proved on the basis of a scintigraphically confirmed suppressive effect of T3 on the paranodular tissue. Thus, in the individual case, separation of CAA from DAA turned out to be impossible using the results of TRH-test alone.

A statement for health care professionals on type 2 diabetes mellitus in Hong Kong. Diabetes division, Hong Kong Society for Endocrinology, Metabolism, and Reproduction.

OBJECTIVE: To issue a statement for health care professionals on type 2 diabetes mellitus in Hong Kong. PARTICIPANTS: The Diabetes Division was established under the auspices of the Hong Kong Society for Endocrinology, Metabolism, and Reproduction. The Division consists of medical and paediatric specialists, as well as health educators, including nurses, podiatrists, and dietitians who have a particular interest in diabetes. EVIDENCE: The statement was based on evidence from the available scientific literature on diabetes management from Hong Kong and overseas. CONSENSUS PROCESS: The draft statement was prepared on 16 February 2000 by a working group of diabetologists working in the public and private sectors. It was presented to the Council of the Diabetes Division on 7 March 2000 and approved by the Council of the Hong Kong Society for Endocrinology, Metabolism, and Reproduction on 11 March 2000. CONCLUSIONS: Type 2 diabetes mellitus affects people of all ages and is a massive public health problem. The criteria used to diagnose diabetes mellitus have recently been revised. Physicians require increased vigilance to screen for glucose intolerance in individuals who have risk factors for type 2 diabetes mellitus. There are now recommended cost-effective procedures for the optimal management of type 2 diabetes mellitus, which emphasise regular monitoring, the control of both diabetes and associated risk factors, and self-management. To address this public health problem, concerted efforts by health care professionals and public bodies are needed to increase levels of awareness and improve the standard of care.

Clinical and endocrinological features of adrenocorticotropic hormone- independent bilateral macronodular adrenocortical hyperplasia.

PURPOSE: We report clinical findings in 5 patients with adrenocorticotropic hormone independent bilateral macronodular adrenocortical hyperplasia. MATERIALS AND METHODS: In 4 males and 1 female 32 to 61 years old (median age 50) we evaluated clinical symptoms, endocrinological and radiological characteristics, treatment modality and postoperative clinical course. RESULTS: All cases presented with some features of Cushing's syndrome. Endocrinological examination revealed autonomous adrenal cortisol production with suppressed adrenocorticotropic hormone and a loss in the diurnal circadian rhythm of plasma cortisol. Abdominal computerized tomography showed bilateral enlargement of the adrenal glands with multiple nodules. 131Iodine labeled adosterol scintigraphy demonstrated remarkable bilateral uptake by the adrenal glands. The pituitary gland appeared normal on magnetic resonance imaging. Open unilateral complete adrenalectomy and contralateral partial adrenalectomy were performed in patient 1, open bilateral complete adrenalectomy was done in patients 2 and 3, and 2 and 1-stage laparoscopic bilateral complete adrenalectomy was performed in patients 4 and 5. Single removed adrenal glands weighed 32 to 108 gm. (median 60). The histological diagnosis was macronodular adrenocortical hyperplasia in all cases. Postoperative followup was 3 to 90 months. Clinical symptoms of Cushing's syndrome disappeared or improved after surgery in all cases. CONCLUSIONS: Although adrenocorticotropic hormone independent bilateral macronodular adrenocortical hyperplasia is a rare form of Cushing's syndrome, physicians are advised to consider it when diagnosing and treating cases of Cushing's syndrome with enlarged bilateral adrenal glands. Bilateral complete adrenalectomy is currently recommended as the treatment of choice.

Endocrinology of calcium metabolism in amphibians, with emphasis on the evolution of hypercalcemic regulation in tetrapods.

Amphibians are the most diverse group as regards the endocrinology of Ca homeostasis because in them only "true" parathyroid glands made first appearance but its involvement varies greatly according to their phyletic affinities and habitats. Presence of ultimobranchial gland, parathyroid and renal 25-hydroxyvitamin-D-1-hydroxylase enzyme in anurans represents their tetrapod pattern of Ca regulation. Among "higher" urodeles, both parathyroid and pituitary are involved in hypercalcemic regulation whereas in "lower" urodeles (some of them are "aparathyroid" too), pituitary alone appears to discharge this function. The latter situation is analogous to the fish. Apodans do possess ultimobranchial and parathyroid glands but the problem of their Ca metabolism has not yet been explored. This review deals exhaustively with the endocrinology of Ca metabolism in amphibians. Also, the evolution of hypercalcemic regulation in tetrapods will be discussed in the light of recent developments.

[The endocrinology of the human placenta].

The human placenta performs various, important functions essential for the maintenance of pregnancy and development of the fetus. Its secretory diversity surpasses any of the other endocrine organs. The placenta is provided with precursors of hormones by the mother as well as by the fetus. It synthesizes and secretes steroid and protein hormones, growth factors, cytokines. The paper is the review of the present knowledge of steroid hormones (progesterone, estrogens), in particular their synthesis and functions in fetoplacental unit. The protein hormones (chorionic gonadotropin, placental lactogen) are also discussed. The last findings concerning placental gonadotropin-releasing hormone, corticotropin-releasing hormone, leptin, activin, inhibin, follistatin and urocortin are presented. The endocrinology of the placenta is considered to be a progressive field of science. The performed studies elucidate the role of the newly identified hormones in the placenta. It is believed, that research on endocrinology of the placenta contributes to the development of perinatology and, moreover, they improve the prenatal care.
