Natural and structured baselines in the treatment of aggression following brain injury.

This single-case study describes the importance of presenting relevant baseline conditions in planning and evaluating treatment for aggression in a severely brain injured man 1 year after injury. An artificially low natural baseline resulted from staff reluctance to deliver instructions and corrective feedback needed for rehabilitation of self-care skill because these were frequently followed by aggression. A subsequent structured baseline presented these antecedents at the higher rates that were necessary for progress in the patient's rehabilitation. This resulted in an increase in aggressive behaviour, but also gave a more accurate representation of what his behaviour would be like under effective rehabilitation conditions. Intervention was based on data from the structured baseline, and included providing clear expectations, social reinforcement, and decelerative procedures. After reduction of aggression to zero, regular staff were reintroduced and presented instruction and corrective feedback as required. No further aggressive behaviour was noted, and self-care improved so that only minimal assistance was needed. Six months following reintroduction of regular staff, both baseline conditions were replicated. No aggressive behaviour was observed during either, suggesting that maintenance of gains could not be attributed to an artificially reduced rate caused by staff avoiding trigger antecedents.

Homicidal somnambulism: a case report.

A case of a homicide and an attempted homicide during presumed sleepwalking is reported in which somnambulism was the legal defense and led to an acquittal. Other possible explanations including complex partial seizures, dissociative state, rapid eye movement sleep behavior disorder and volitional waking behavior are discussed. The evidence supporting the probability that this act occurred during an episode of somnambulism and sleep-related confusional arousal is reviewed and weighed. This evidence includes personal and family history of somnambulism and related disorders; neurological, psychiatric and psychological assessments; presence of possible precipitating factors; and polysomnographic data.

Pre- and postnatal growth retardation--severe mental retardation--acral limb deficiencies with poorly keratinized nails. Another example of a distinct syndrome of inherited intrauterine dwarfism?

In this report, we further delineate an autosomal recessive syndrome with prenatal growth retardation, severe postnatal growth failure, profound mental and motor retardation, characteristic face and acral limb deficiencies with poorly keratinized nails and hypoplastic distal phalanges.

Prevalence and presentation of depressive illness in a primary health care setting in Kenya.

Using a two-stage screening procedure, ICD-9 diagnostic criteria, and the Hamilton Rating Scale for Depression, the authors diagnosed depressive disorders in 81 (9.2%) of 881 patients in a primary care setting in Kenya. All depressed patients had somatic symptoms, and all of the 27 depressed patients assessed with the Hamilton scale scored higher than 2 on the work and activities item. These findings contradict the earlier reports that Africans do not admit to being depressed. Nearly one-third of the depressed patients were moderately or severely ill and would have benefited from psychiatric assessment and treatment.

Recovery from macular phototoxicity after corneal triple procedure.

PURPOSE: Visual recovery from macular phototoxicity in 2 cases after prolonged exposure to operating microscope light from uncomplicated corneal triple-procedure surgery. Recovery is discussed in the context of repair and regeneration. METHODS: Retrospective case reports. RESULTS: Immediately postoperatively, both patients reported positive scotomata and were found to have macular retinal pigment epithelial depigmentation. In 1 case, the fovea was involved. By 6 to 12 months, the scotomata had disappeared despite large areas of retinal pigment epithelial hyperpigmentation remaining. CONCLUSION: Recovery from macular phototoxicity occurs, although the mechanism remains unclear. Positive scotomata in these cases resolved over several months. The time scale of recovery was consistent with the time required for cellular replacement and possible differentiation from neural progenitor cells.

Tension pneumoorbitus.

Posttraumatic tension orbitus developed in a young man causing subconjunctival emphysema and proptosis. Intraocular pressures were monitored as a means of indirectly measuring intraorbital pressure. The patient was serially examined for evidence of compressive optic neuropathy. Because of the possibility of orbital pressure increasing several hours from the time of injury, we recommend monitoring of these patients through intraocular pressure measurement and evaluation of the optic nerve for evidence of compression. Our findings, however, do support previous documentation that the intraocular pressure rise following orbital trauma with orbital emphysema is usually not of sufficient severity or duration to result in visual compromise.

Congenital rubella after previous maternal immunity.

Two mothers who had asymptomatic rubella infection in pregnancy gave birth to severely affected infants. In both, the presence of preexisting antibody was well documented, although it could not be established whether it was the result of vaccine or natural infection.

The interaction of Src and RACK1 is enhanced by activation of protein kinase C and tyrosine phosphorylation of RACK1.

RACK1 is an intracellular receptor for the serine/ threonine protein kinase C. Previously, we demonstrated that RACK1 also interacts with the Src protein-tyrosine kinase. RACK1, via its association with these protein kinases, may play a key role in signal transduction. To further characterize the Src-RACK1 interaction and to analyze mechanisms by which cross-talk occurs between the two RACK1-linked signaling kinases, we identified sites on Src and RACK1 that mediate their binding, and factors that regulate their interaction. We found that the interaction of Src and RACK1 is mediated, in part, by the SH2 domain of Src and by phosphotyrosines in the sixth WD repeat of RACK1, and is enhanced by serum or platelet-derived growth factor stimulation, protein kinase C activation, and tyrosine phosphorylation of RACK1. To the best of our knowledge, this is the first report of tyrosine phosphorylation of a member of the WD repeat family of proteins. We think that tyrosine phosphorylation of these proteins is an important mechanism of signal transduction in cells.

Primary tethered cord syndrome: diagnosis and treatment of an insidious defect.

Failure to recognize the signs and symptoms of tethered cord syndrome in patients with spina bifida occulta can result in tragic consequences. Of patients with tethered spinal cord, 35% have bowel and/or bladder dysfunction. Scoliosis, foot or leg length discrepancies, pes cavus, and varus or valgus deformities also can occur. Early assessment and intervention by the neuroscience nurse who is familiar with these clinical signs can help prevent significant deformities and irreversible neurological deficits.

Social class differences in health and care in the year before death.

STUDY OBJECTIVE: The aim was to identify any social class differences in health and care in the year before death emerging from a wider study of life before death. DESIGN AND SETTING: Data were collected at interviews, mainly of close relatives, with those who knew most about the people's lives in the year before they died. The sample was a random sample of adult (15 and over) deaths in 10 areas of England in 1987. SUBJECTS: Information was obtained about 639 persons, 80% of the initial sample of 800 deaths. MAIN RESULTS: While middle class people die at an older age, the symptoms and physical restrictions reported for middle class and working class people were similar, and middle class people were reported to have a better quality of life before death. More working class people were felt to be in financial need. CONCLUSIONS: Money and class contribute to the quality of life before death as well as postponing death.

Intraoperative cytology as an elective surgical procedure. Analysis of 57 cases.

Fifty-seven consecutive cases submitted by surgeons for intraoperative cytology were reviewed. In 47 cases there was a concomitant biopsy from the same anatomic site; among them, the cytologic and surgical diagnoses agreed in 44 (94%). The three discordant cases (6%) represented false-negative frozen sections. In the remaining 10 cases the cytologic and surgical biopsy sites differed in 5, 4 were considered inadequate for cytologic interpretation, and 1 did not have a corresponding surgical biopsy. Attitudes toward the use of intraoperative cytology were assessed with questionnaires sent to surgeons from all specialties represented at the University of California, Davis, Medical Center. Fifty-three percent of the respondents reported use of this procedure. At our institution thoracic surgeons and otorhinolaryngologists were more apt than others to use intraoperative cytology as an adjunct to frozen section analysis. Factors influencing their selection of intraoperative cytology are discussed.

Quantitative measurement of negligible trypsin inhibitor activity and nutrient analysis of guar meal fractions.

A complete nutrient characterization of three possible products of guar bean processing does not apparently exist in the literature. Guar meal is a high-protein byproduct produced during extraction of galactomannan gum from the guar bean. During the extraction process, two fractions are produced (germ and hull). Germ and hull fractions are usually combined to form the marketed product, guar meal. Analyses characterized the nutrient, trypsin inhibitor, and galactomannan gum content of the three guar meal byproducts to determine which fraction is more valuable as an ingredient in poultry diets. Analyses indicated that the germ fraction is most appropriate for inclusion in poultry diets. Trypsin inhibitor activity previously reported as an antigrowth factor associated with guar meal was negligible and not considered to be a significant factor limiting its use in poultry feeds.

Dynamic neuro-immune regulation of psychiatric risk loci in human neurons.

The prenatal environment influences neurodevelopment and subsequent clinical trajectories for psychiatric outcomes in childhood and adolescence. Yet it remains unclear if the impact of maternal and fetal immune activation varies with distinct polygenic risk profiles. Therefore, here we catalogue genotype and environment (GxE) interactions, contrasting allele-specific regulatory activity between inflammatory contexts. We report a cue-specific neuronal massively parallel reporter assay (MPRA) of 220 loci from genome-wide association study (GWAS) linked to ten brain traits/disorders, empirically dissecting the impact of interleukin-6 (IL-6) and interferon-alpha (IFNα) on transcriptional activity. Of 1,469 active candidate regulatory risk elements (MPRA-active CRSs) across three conditions, we identify 316 with dynamic variant-specific effects (MPRA-QTLs) in human induced pluripotent stem cell (hiPSC)-derived glutamatergic neurons. Broadly, across hundreds of variants, neuronal immune-mediated regulatory activity is driven by differences in transcription factor binding and chromatin accessibility, the gene targets of which show pleiotropic enrichments for brain, metabolic, and immune disorders. Dynamic genetic regulation mediates immune effects, informing our understanding of mechanisms governing pleiotropy and variable penetrance. Understanding neurodevelopmental GxE interactions will inform mental health trajectories and resolve mechanisms mediating prenatal risk.
