Pharmacologic treatment of intimal hyperplasia after metallic stent placement in the peripheral arteries. An experimental study.

RATIONALE AND OBJECTIVES: To evaluate the efficacy of oral administration of cilostazol, an antithrombotic agent, for the prevention of thrombotic occlusion and intimal hyperplasia after stenting. METHODS: Single-bodied Z-stents were placed in the iliac arteries of 23 dogs. Before stenting, an embolizing coil was introduced into the right femoral artery to reduce blood flow in the right iliac artery. Eleven dogs were given cilostazol orally, and the other 12 were unmedicated as a control group. The dogs were killed at 4, 13, and 24 weeks. RESULTS: Intraluminal narrowing due to thrombus was observed in 25% of dogs in the control group but in none of the dogs in the cilostazol group. The thickness of the neointima was significantly thinner in the cilostazol group than in the control group at 24 weeks on the noncoiled side (P < 0.05), and at 4 and 24 weeks on the coiled side (P < 0.01). CONCLUSIONS: These results suggest that oral administration of cilostazol is an effective method of preventing thrombotic occlusion and intimal hyperplasia after stenting.

[A study on identification method of coxsackie virus A16 and enterovirus 71].

The simple and rapid identification method of coxsackie virus A16 (CA16) and enterovirus 71 (Ev71), the main cause of hand foot and mouth disease, was described in this report. This method was consists of three steps, those were virus isolation, amplification by RT-PCR, and digestion by restriction enzyme Taq I and EcoT22 I. In 1990, many virus strains were isolated in Vero cell line. But after 1994 the number of isolated viruses in Caco-2 cell line increased instead of isolated in Vero cell line. Concerning to isolation of CA16, in 1998, MRC-5 cell line was also used and it's sensitivity was same as Caco-2 cell line. Cytopathic effects were first observed in MRC-5 cell line among these three lines. RNAs of CA1-10, poliovirus 1-3, echovirus 1-7, 9, 11, 14, 16, 17, 18, 24, 25, 27, 30, Ev71, and isolated viruses were extracted by using QIAamp viral RNA kit (QIAGEN). Then two series of reverse transcription using two down stream primers (E31 and E33) were performed. In PCR, the same upper stream primer (primer 2) was used. CA6, CA16 and Ev71 were the only viruses those were not amplified by RT-PCR using primer 2/E31 but amplified by RT-PCR using primer 2/E33. After PCR, PCR products of isolated viruses using primer 2/E33 were digested by Taq I and Eco T22 I. All of Ev71 products were not digested but all of CA16 products were digested. The band pattern of PCR products (CA16) digested by Taq I were divided into three groups. And Eco T22 I digestion pattern is only one. These results were in accord with Taq I and EcoT22 I digestion sites on sequences of CA16 and Ev71. This method should be useful for the rapid identification of CA16 and Ev71.

Diagnosis of cornual obstruction by transcervical fallopian tube cannulation.

Interstitial fallopian tube obstruction (IFTO) occurs in 15% of hysterosalpingograms (HSG) performed for infertility. Conventional HSG or laparoscopy may not differentiate cornual spasm or other temporary cause from true obstruction. We used transcervical cannulation of the proximal oviduct with a 3-F Teflon catheter and flexible guidewire 0.018 inch (0.043 cm) in diameter under hysteroscopic or fluoroscopic guidance to evaluate IFTO in 28 patients. Fluoroscopic catheterization techniques with selective salpingography demonstrated patency in 84% of obstructed tubes. Hysteroscopic cannulation with direct visualization by laparoscopy or laparotomy was successful in 92%. In one patient, perforation of the isthmus occurred without sequelae. Transcervical coaxial cannulation of the proximal oviduct is an effective method for evaluating cornual obstruction.

[Glomerular endothelial cell culture and its application].

Glomerular endothelial cells (GEN) have recently been cloned, cultivated and characterized. They have as same biological characteristics as expressed in endothelial cells derived from different sources. Because of the difficulties of cloning of GEN, we first transfected plasmid DNA containing large T antigen of SV-40 (NH-GEN1). Culture method of GEN and NH-GEN1 and its application are discussed.

[Usefulness of superparamagnetic iron oxide particle (AMI-25) enhanced MR imaging for the diagnosis of liver tumors: comparison with contrast enhanced CT].

The purpose of this study was to clarify the usefulness of SPIO-enhanced MRI in the diagnosis of liver tumors in comparison with contrast-enhanced CT. The subjects were forty patients with 154 nodules in the liver. We compared SPIO-enhanced MRI with contrast-enhanced CT in terms of tumor-liver contrast and detectability of liver tumors. In terms of tumor liver contrast, SPIO-enhanced MRI was equal or superior to contrast-enhanced CT in 82% of cases. In the detectability of liver tumor, SPIO-enhanced MRI detected more tumors than contrast-enhanced CT, especially small tumors. Tumors undetected by SPIO-enhanced MRI that were detected by contrast-enhanced CT and/or plain MRI were adenomatous hyperplasia and inflammatory pseudotumor according to fine needle biopsy. There were no severe complications of SPIO-enhanced MRI. In conclusion, SPIO-enhanced MRI will be more useful than contrast-enhanced CT in the diagnosis of liver tumors. SPIO-enhanced MRI may be a promising diagnostic method for the detection of hepatic tumors, especially small ones.

Continuous production of NADP by immobilized Achromobacter aceris cells.

Several microorganisms having higher nicotinamide adenine dinucleotide kinase (NAD kinase, EC 2.7.1.23) activity were immobilized into polyacrylamide gel lattices. The enzyme activity field by immobilization was highest in Achromobacter aceris AKU 0120. By the incubation of the immobilized A. aceris cells at pH 4.0, the NAD kinase activity increased and the adenosine triphosphate (ATP)-degradation activity disappeared completely. Enzymatic properties of the immobilized A. aceris cells were investigated and compared with those of intact cells. The optimal pH and the optimal temperature of immobilized cells were the same as those of intact cells. Immobilized cell NAD kinase was more stable than that of intact cells. The operational half-life of immobilized cells was 20 days when the substrate solution was passed through a column packed with immobilized cells at a flow rate which gives a space velocity (SV) of 0.1 hr-1 at 37 degrees C. On the other hand, the half-life of the intact cells was only 6 hr.

A new ultrasonic apparatus for ophthalmology using manuel compound scanning.

A new ultrasonic equipment for ophthalmological diagnosis has been developed. Manual compound scanning using water immersion made acurate representation of cross-section of the orbital structure possible. A large, focused transducer brought resolution and sensitivity. Gray scale display of the ultrasonogram increased information concerning intensity of echoes. Ultrasonograms of retinoblastoma, vitreous hemorrhage, melanocytoma, optic disc coloboma and unilateral exophthalmos are presented.

Phage particle-mediated gene transfer of recombinant cosmids to cultured mammalian cells.

An efficient procedure for the introduction of recombinant cosmids into cultured mammalian cells consists of the following steps. Cosmids were packaged, in vitro, into lambda phage particles and transduced into Escherichia coli hosts lysogenized with thermo-inducible lambda c Its phage. The introduced cosmids were repackaged into phage particles in the thermo-induced hosts. The efficiency of such in vivo cosmid packaging was further improved by construction of pTC vectors that carried three cohesive end sites (cos) of phage lambda, arrayed in tandem. Two types of cosmids, in almost equal numbers (i.e., cosmids with one cos and cosmids with two cos), were obtained from a cosmid library constructed with pTC vectors. The efficiency of packaging in vivo of cosmids with two cos, was found to be 7-20 times higher than that of corresponding cosmids with only one cos. Use of a high-copy-number derivative of pTCl further improved the phage yield by 20- to 30-fold. The packaged cosmids, which carried the thymidine kinase-encoding gene of herpes simplex virus type 1 as a selective marker, were introduced into mouse Ltk- cells with an efficiency of 10(-5), by the phage transfer method [Ishiura et al., Mol. Cell. Biol. 2 (1982) 607-616].

Simultaneous determination of a new inhibitor of acyl CoA:cholesterol acyltransferase, YM17E, and five metabolites using high-performance liquid chromatography with electrochemical detection.

We describe a reversed-phase high-performance liquid chromatographic method for the determination in plasma of YM17E (I), an inhibitor of acyl CoA:cholesterol acyltransferase, and its five metabolites using electrochemical detection. This method enables simultaneous quantification of I and five active metabolites. The plasma sample is extracted by a one-step solid-phase extraction using a SepPak C18 cartridge, with high recovery and reproducibility of the analytes. The method is sensitive and the limits of determination are 0.5 ng/ml for I and 1 ng/ml for metabolites M1, M2-a, M2-b, M3 and M4. This method is applicable to rat, dog and human plasma, and is useful for pharmacokinetic studies.

The development of infants' circadian rest-activity rhythm and mothers' rhythm.

In order to know when and how infants obtain their circadian sleep-wake rhythm, infants' developing circadian rest-activity rhythm and mothers' circadian rhythm in the postpartum period were examined using actigraph monitoring. The subjects were 11 primipara and their infants. Actigraphic recordings for the infants and their mothers were made over 3-5 continuous days during the 3rd, 6th, 9th and 12th weeks after birth. A 24-h peak on a mean autocorrelogram of the infants' movements was detected at the 3rd week. The infants' circadian rest-activity rhythm already existed in the 3rd week. The amplitude of this 24-h peak gradually increased from the 6th to 12th week. This may be useful as an index of the development of infants' circadian rest-activity rhythm. An 11-h peak was also observed at the 3rd week. This 11-h peak was thought to be a semi-circadian rhythm. Regarding the mothers, the amplitude of the 24-h peak on the mean autocorrelogram at the 3rd week was the smallest of all other weeks, and it became larger from the 3rd to 12th week. This meant that the mothers' circadian rhythm at the 3rd week was influenced by their interrupted sleep at night to take care of their infants. The mother-infant synchronization is probably the 1st factor in the entrainment of infants' circadian sleep-wake rhythm. In this study, we also propose a novel method for compensating for missing data in autocorrelogram analysis.

Customized mold brachytherapy for oral carcinomas through use of high-dose-rate remote afterloading apparatus.

OBJECTIVE: The aim of this study was to investigate the role of the combined use of customized molds and recently available remote afterloading brachytherapy apparatus with more flexible catheters in the treatment of superficial oral carcinomas. STUDY DESIGN: Four patients with oral squamous cell carcinoma who were treated through use of this combined technique were analyzed retrospectively. The molds were made from transparent acrylic resin through use of a dental technique. The combined approach was applied as a boost therapy after external irradiation. RESULTS: The 4 patients had had no recurrence of tumor or radiation injury by the end of the follow-up period. CONCLUSIONS: The combined technique could be an excellent method of treating superficial carcinomas of the oral cavity.

Radiofrequency catheter ablation for intra-atrial reentrant tachycardia after surgery of atrial septal defect: use of isopotential mapping (QMS system) to demonstrate bidirectional complete block.

A 51 year-old Japanese man who had undergone surgical correction of an atrial septal defect at the age of 18 years old was referred to our institute for evaluation of his atrial arrhythmia. The conventional electrophysiological study was combined with a new technique utilizing an isopotential and isochronal mapping system (QMS) to visualize the electrical signals recorded with a 64-electrode basket catheter. Using this system, an intra-atrial reentrant tachycardia (IART) was demonstrated. The isopotential map recorded with the QMS (QMS-isoP) rapidly revealed a clockwise global reentrant circuit in the mid free wall of the right atrium and a narrowest activation isthmus between the lower end of the atriotomy scar and the inferior vena cava (IVC). After confirming entrainment with concealed fusion at the lower end of the atriotomy scar, radiofrequency energy was delivered linearly from this site to the IVC by slowly dragging the catheter. The elimination of the IART was defined by the QMS-isoP which demonstrated bidirectional block during pacing from both sides of the ablated linear lesion. The conventional technique of entrainment with concealed fusion combined with the QMS-isoP may result in a highly sophisticated method for identifying global reentrant circuits and for defining bidirectional block after eliminating the IART.

Modification of histidine residues in proteins by reaction with 4-hydroxynonenal.

We find that histidine residues in proteins are major targets for reaction with the lipid peroxidation product 4-hydroxynon-2-enal (HNE). Reaction of insulin (which contains no sulfhydryl groups) with HNE leads to the generation of HNE-protein adducts, which are converted to radioactive derivatives upon subsequent treatment with NaB[3H]H4. Amino acid analysis of the modified protein showed that the HNE treatment leads to the selective loss of histidine residues and the stiochiometric formation of 3H-labeled amino acid derivatives. The same labeled products were detected in acid hydrolysates of polyhistidine and N-acetylhistidine after their reactions with HNE and NaB[3H]H4. The reaction of N-acetylhistidine with HNE led to the production of two compounds. Upon acid hydrolysis, both derivatives yielded stoichiometric amounts of histidine. However, after reduction with NaBH4, acid hydrolysis led to a mixture of amino acid derivatives [presumably, isomeric forms of N pi (N tau)-1,4-dihydroxynonanylhistidine] that were indistinguishable from those obtained from insulin and polyhistidine after similar treatment. Although other possibilities are not excluded, it is suggested that the modification of histidine residues in proteins by HNE involves a Michael-type addition of the imidazole nitrogen atom of histidine to the alpha, beta-unsaturated bond of HNE, followed by secondary reaction involving the aldehyde group with the C-4 hydroxyl group of HNE. The reaction of histidine residues with HNE provides the basis for methods by which the contributions of HNE in the modification of proteins can be determined.

Detection and normalization of biases present in spotted cDNA microarray data: a composite method addressing dye, intensity-dependent, spatially-dependent, and print-order biases.

Microarrays are often used to identify target genes that trigger specific diseases, to elucidate the mechanisms of drug effects, and to check SNPs. However, data from microarray experiments are well known to contain biases resulting from the experimental protocols. Therefore, in order to elucidate biological knowledge from the data, systematic biases arising from their protocols must be removed prior to any data analysis. To remove these biases, many normalization methods are used by researchers. However, not all biases are eliminated from the microarray data because not all types of errors from experimental protocols are known. In this paper, we report an effective way of removing various types of biases by treating each microarray dataset independently to detect biases present in the dataset. After the biases contained in each dataset were identified, a combination of normalization methods specifically made for each dataset was applied to remove biases one at a time.

Quantitative computed tomographic evaluation of femoral bone mineral content in renal osteodystrophy compared with radial photon absorptiometry.

The computed tomography (CT) numbers of cortical bone at the level of 20 cm (CT20) and of spongiosa in the lateral condyle at the level of 2 cm (CT02) from the distal end of the femur were obtained by a quantitative CT method and compared with the bone mineral density of mostly cortical bone within the radius (BMD) by photon absorptiometry. The study included 47 patients with chronic renal failure not dialyzed or induced to regular hemodialysis within 4 weeks of the study (group 1), 28 patients on regular hemodialysis for more than one month (group 2), and ten healthy volunteers (group 3). The measures of bone mineral content (BMC), namely CT20, CT02, and BMD, were compared in terms of their abilities to distinguish members in the various groups. For group 1 and group 3, the greatest variation in BMC was in the difference in CT02, which was primarily a measurement of the BMC of spongiosa. For groups 1 and 2, the greatest variation was in the difference in BMD, which was primarily a measurement of the BMC of cortex. The reproducibility of CT02 was estimated as almost equal to the difference in CT02 values at intervals of 10 months' duration of hemodialysis. The results indicated that CT02 was a useful measurement for evaluating the progress in the early stage of the renal osteodystrophy, and it is recommended that the bone mineral measurement with this QCT method should be performed once or twice a year.

Comparative evaluation of standard dilution method and commercial kit for frozen plate antifungal susceptibility testing of yeasts using 200 clinical isolates.

A comparative evaluation of standard microdilution methods and a commercial kit for frozen plate antifungal susceptibility testing of yeasts was performed using amphotericin B, flucytosine, fluconazole, miconazole, and itraconazole on 200 yeast isolates. The isolates included 100 strains of Candida albicans, eight of C. tropicalis, twelve of C. parapsilosis, eight of C. glabrata, five of Cryptococcus neoformans, thirteen of Trichosporon asahii, and 54 other strains of seven other species of ascomycotic yeasts. Microdilution testing was performed according to the standard method for antifungal susceptibility testing published by the Japanese Society for Medical Mycology (JSMM), which are a modification of the method developed by the National Committee for Clinical Laboratory Standards (NCCLS) M27-P. The commercial kit was prepared according to the manufacturer's instructions. The degree of agreement within +/-1 dilution for 200 clinical isolates against five antifungal agents was excellent with values for amphotericin B, flucytosine, fluconazole, miconazole, and itraconazole of 100%, 99.0%, 97.5%, 97.0%, and 97.0%, respectively. Overall, the frozen plate antifungal susceptibility testing kit provided convenient and reproducible results comparable to those obtained with the JSMM standard method.

Two case reports of successful prevention of overdosing in borderline patients.

Overdosing often occurs among patients with borderline personality disorder (BPD) and confronts physicians with the dilemma of whether to prescribe psychotropic agents for them or not. Prescribing psychotropic agents is often unavoidable in the real world, because psychotherapy alone is sometimes insufficient to control their labile emotions. We report two cases of BPD in which overdosing was successfully prevented by changing the dosage form from tablets to a powder and adding a bitter taste to make it difficult to swallow a large dose at one time. Prevention of overdosing by BPD patients definitely requires a comprehensive treatment approach in terms of its social, biological, psychological, and psychopathological aspects. Although no perfect method of preventing overdosing is currently available, our method provides an option when psychotropic agents are prescribed for BPD patients in clinical practice.
