[Intra-arterial regional chemotherapy in stomatological cancers using implantable chambers. 15-month experience].

The authors report their experience after 15 months of use of an implantable chamber for regional intra-arterial chemotherapy in stomatological cancer. This technique rekindles interest in intra-arterial chemotherapy via a retrograde temporal route, by simplifying the technique. The remarkable local tolerance of implantable chambers makes them preferable to external catheters and suitable for long term intra-arterial chemotherapy.

[The effect of the combined prevention of stomatologic diseases on the quality of the treatment of dental caries and the stability of fillings].

The state of teeth filled with silver amalgam, silidont, 'evicrol' was clinically assessed in 280 schoolchildren which had been earlier subjects to dental caries prevention. The quality of dental care was assessed by 5 scores developed by the Central Stomatological Institute. One to two years after a combined preventive intervention, the state of filling was found unsatisfactory in 11.0 to 11.3% of amalgam-filled teeth, in 15.8-22.7% of silidont- and 4.3-10.8% of 'evicrol'-filled teeth. The respective values were 18.8-26.3% ; 37.9-39.1%; 14.9-23.9% in control groups. The combined prevention of caries was conductive to improved treatment quality which was accounted for by a 1.5 to 2-fold reduction in the rate of disorders in marginal contact with filling material and secondary caries. The electrometric diagnostic technique was used to evaluate the marginal permeability of filling and to diagnose the caries relapses. The technique allowed detection of the initial stages in the state of filling and concealed secondary caries.

[The possible development of non-academic professions in stomatology].

The dental care of the population may be considerably inproved by the further development of non-academic careers in stomatology. Past experience has shown that, in the majority of cases, the dental care of the children is jeopardized if the ratio of dentists to population is low, and the need for care is great. Therefore, the employment of specialized nurses with appropriate duties is of particular importance in paedodontics. The possibilities and variants are discussed in the light of international trends.

[Preventive therapy in stomatological and maxillofacial surgery. Clinical considerations on the administration of cephaloridine, using a new technic (short-term prophylaxis)].

The "Short term prophylaxis" technique was used in 100 patients who had undergone Stomatologic surgery and Maxillo-facial surgery for post-operative prophylaxis of surgical infections according to analogous researches conducted in other surgical branches by various authors. The results obtained can be summarized as following: 1) a marked reduction in administered antibiotics (cefaloridina); 2) a marked reduction of post-operative fever; 3) a maintainment of average febrile levels at 37,8 degrees-38 degrees. The above tecnique thus allowed a more rapid mobilization of patients who had undergone surgical intervention, a reduction of the recovery period and the therapeutic costs.

[New drugs, new therapeutic regimens, and new strategies for chemotherapy of stomatological cancers (author's transl)].

Two new drugs have been proved to be effective for isolated or recurrent stomatological tumors : detorubicine (limited because of cardiotoxicity), and CDDP (limited because of renal and neurological toxicity). Drugs used mainly as adjuvant treatment concurrently with BCG, as immunotherapy, are bestatine (without clinical or biological toxicity) and levamisole (rare cases of agranulocytosis). Randomized trials of chemo- and immunotherapy now underway are the only means of assessing long-term efficacy of adjuvant treatments.

[Use of a computer for selecting persons to undergo cytological study in prophylactic stomatological cancer examinations].

5,320 workers of various professions in Leningrad were involved in a computerized primary screening for further detailed oncological and stomatological examination. On the basis of computer data, 55% of those involved in the questionnaire survey became candidates for such examination. Curettage was performed in cases of pathological changes in the mucosa of lip and oral cavity. Various diseases of the lip, oral cavity and throat (precancerous lesions--3.7, cancer--0.17%) were detected in 22.6%. No precancerous lesions or cancer were identified in the control group of 2,321 subjects. The results point to the effectiveness of computer and cytological examination application in mass oncostomatological screenings.

Pharmacologic management of stomatologic problems.

A wide variety of diseases of the soft tissues of the oral cavity occur. Some of the common and significant problems have been presented with recommendations for their management. Diagnosis and patient evaluation including adequate laboratory studies when required, are the keys to a more successful outcome of treatment. Antibiotics and short-term anti-inflammatory steroids benefit selected cases. In others, treatment may require a team approach in concert with the physician. Dentists should be alert to causes of the oral lesions, especially in cases of xerostomia, candidiasis, and nutritional deficiencies. This article should form a basis for therapy of stomatologic problems.

[A model study on air contamination at the stomatologic work place by use of the turbine].

Certain dental measures, e. g., the application of a turbine, an ultra-sonic hand-piece or a spray, produce aqueous, polydisperse aerosols which escape from the patient's oral cavity during treatment and contaminate the room air with micro-organisms from the oral microbial flora. An aerobacteriometric model study at a stomatological work place showed that the germ content of the air was significantly higher on days when the turbine was used than on days when it was not used.

[Clinical use of BAS-O, a bioactive glass ceramic, for filling cystic cavities in stomatology].

The bioactive glass-ceramic material BAS-0 and results of clinical testing of this material in stomatology are described. The granules of bioactive glass-ceramic BAS-0 were chosen for implantation into cyst cavities in jaw-bones. Radiological evaluations were carried out after 3, 6 and 12 months. Altogether 37 radiographs of 23 patients were evaluated. The symptoms of ossification were proved in 28 cases (75.7%), 5 radiographs (13.5%) were without visible apposition of the bone and in 4 cases the loss of the BAS-0 granules from operation wound was observed (10.8%). The authors proved very good healing properties and tolerance of BAS-0 material, no significant changes in biochemical and haematology examinations were observed.
